2.A Case of Contact Urticaria from a Latex Glove Occurred to an Operationg Room Nurse.

The prevalence of latex allergies has been on the increase along with the greater use of rubber products in daily life, medical, dental and occupational settings. Allergic reactions to latex can take two clinical forms of either contact dermatitis or immediate hypersensitivity reactions, which are provoked by the natural latex proteins or chemical additives used in the manufacturing process, respectively. A 25-year-old female, an operating room nurse, complained of recurrent pruritic erythematous wheals on both hands after the wearing of latex rubber gloves. The prick test and the usage Mlit't to the latex gloves were positive, and the RAST was class 3. To the best of our knowledge, is the first case report of contact urticaria from latex rubber gloves in Korean dermatologic literature.

3.A Case of Similar Pattern of Hepatotoxicity after Propylthiouracil and Methimazole.

A 46-year-old male patient developed jaundice after 23 days' treatment of propylthiouracil (PTU) for Graves' disease. Serum alkaline phosphatase was elevated markedly with moderate increase in serum aminotransferase, and the peak level of total bilirubin was 7.3 mg/dL. After withdrawal of PTU, serum aminotransferase and bilirubin began to decrease simultaneously, and completely normalized. One week after the discharge from hospital, he received radioiodine treatment for Graves' disease, but he showed aggravation of hyperthyroidism and ophthalmopathy. So we prescribed methimazole inevitably 16 days' after the radiodiodine treatment. He developed jaundice again after 69 days' treatment of methimazole, but the pattern of hepatotoxicity was slightly different from that of the previous PTU-nduced hepatotoxicity. Serum aminotransferase increased slightly and quickly normalized after discontinuance of methimazole, but serum total bilirubin increased continuously and reached to 24.6 mg/dL. Eosinophilia was prominent and the result of liver biopsy was compatible with cholestatic jaundice. The patient was treated with prednisolone and recovered from jaundice rapidly.

4.Experimental atudy on pulmonary embolism caused by lipiodol and absolute ethanol.

Pulmonary embolism of lipiodol or ethanol may occur inadvertently during the procedure of diagnostic or therapeutic embolization. We studied the nature of pulmonary damage, and its relation to the amount of lipiodol and ethanol embolized into the lung. Various doses of embolic agents(lipiodol 0.4cc/kg, 0.08cc/kg, absolute ethanol 0.4cc/kg, 0.03cc/kg and normal saline 0.4cc/kg)were injected intravenously through femoral vein in 50 rats(10 rats in each group). Pulmonary pathologic changes were analyzed on the 3rd and 28th days subsequent to injection. On the 3rd day, three of 10 lipiodol cases and all(10) ethanol cases showed multifocal pulmonary hemorrhages. Accompanying inflammatory cell infiltration was found only in lipiodol cases. On the 28th day, one of 10 lipiodol cases and four of 10 ethanol cases showed non-specific pneumopathy suggesting mild focal pulmonary fibrosis. The degree of pulmonary damage appeared to depend on the amount of injected embolic agents although their safety doses could not be clarified exactly. Therefore, we suggest careful injection of embolic agents, especially ethanol, during the embolization procedure to prevent possible pulmonary damage by inadvertent pulmonary embolism.

5.The Incidence of Unexpected Antibodies in Transfusion Candidates.

BACKGROUND: Some investigators have done several studies on the frequency and distribution of unexpected antibodies in Korea. But more studies are necessary to estimate the precise frequency of unexpected antibodies. METHOD: In order to determine the frequency of unexpected antibodies, we analyzed the results from 23,735 antibody-screening tests performed for transfusion candidates at Yonsei University Severance Hospital during a recent two-year period (March 1998~ Feb 2000). Screening and identification of unexpected antibodies were carried out using a gel agglutination technique with DiaMed-ID system (DiaMed, Murten, Switzerland). RESLUT: Out of all 23,735 serum samples, positive results were obtained from 109 sera (0.46%). Clinically significant antibodies were found from 0.29% of all the population. Antibodies that were detected most frequently were anti-E+c (33 sera), followed by anti-E (19 sera) and anti-Lea (12 sera). CONCLUSION: We found that Rh antibodies, clinically significant unexpected antibodies, are detected frequently in Korea, too. So we consider that unexpected antibody screening test must be included in pre-transfusion test in Korea and this information is reassuring in assessing the risk associated with blood transfusion in many hospitals of Korea.

6.A Survey of Blood Component Use in Relation to International Statistical Classification of Diseases (ICD)-10.

BACKGROUND: There are few recent studies on transfusion practice and blood use with regard to diagnoses of Korean recipients. We conducted a survey of blood component use in Yonsei University Medical Center in Seoul, Korea. METHODS: A survey was carried out on blood component transfusion from January 2000 to June 2000. Transfused components were listed by broad diagnostic categories formed from the principal diagnoses of the recipients according to the ICD-10. RESULTS: Of all investigated components (67,433 units), 18,623 units of RBCs (27.6%), 10,203 units of FFP (15.1%), 38,547 units of platelets (57.2%), and 60 units of whole blood (0.1%) were transfused. The transfusion rate in relation to sex was 1.7:1 and the majority of all blood units were transfused to patients aged <65 yrs (85.9%). The rate of blood component transfusion was the highest in the department of internal medicine (47,451 units, 70.4%). Of 18,623 RBCs units and 10,203 FFP units, 70.8% and 74.5% were transfused to patients in four diagnostic categories; neoplasms, digestive system diseases, circulatory system diseases, and leukemia or lymphoma. Of 38,547 platelets units, 87.8% were used in patients with neoplasms, leukemia or lymphoma, digestive system diseases and diseases of the blood and blood-forming organs. CONCLUSIONS: This survey provides information on the blood component usage in tertiary-care teaching hospital in Seoul, Korea. It demonstrates the concentration of today's blood utilization among a few diagnostic categories. This information is relevant for quality management of transfusion practice, cost analyses and for planning local and regional blood donation programs.

Deep palmoplantar warts(myrmecia) represent lesions of the DNA virus of the papova group, human papillomavirus, serotype 1(HPV-1). They are occur not only on the palms and soles, but also on the lateral aspects and tips of the fingers and toes. Herein we report a case of deep palmoplantar wart on proximal nail fold of right index finger in a 6 year old male.

8.A Study on Use of Complementary-Alternative Therapy in Middle-aged Women.

PURPOSE: The purpose of this study was to describe the use pattern of alternative therapies in middle aged women. METHOD: The subjects of this study were 530 women from 40 to 64 years of age. The data of this study was patients' records. The data was gathered from April 1st, 2004 to June 30th, 2004. Data was analyzed statistically by using the SPSS/WIN 10.0 program. RESULT: It was shown that 63.5% of the subjects have utilized one or more types of alternative therapy. The most common type of alternative therapy was dietary and nutritional therapy 38.3%, and the place of use was the home 64.1%. The most common reason was a recommendation by friends. The degree of satisfaction after the use of alternative therapy was 95.2%. Among users of alternative therapies, the most frequent responses to each question were as follows; Motive of Use- "for health maintenance and promotion." (62.7%) In the relation ship between general characteristics and utilization of alternative therapy, religion (x2=12.33, p=.02), exercise (x2=8.21, p=004), and health status (x2=14.95, p=.005) showed a significant statistical difference. CONCLUSION: We found that middle aged women used alternative therapies more frequently than other populations. Therefore, it is suggested that medical doctors or nurses verify the true effects or side-effects from the most therapies or nurses verify the true effects or side-effects from the most common complementary or alternative therapies through experiments.

9.A Case of Tethered Cord Syndrome.

Tethered cord syndrome is one of the spinal dysraphism with low-lying conus medullaris frequently associated with spinal lipoma, diastematomyelia and thick filum. The clinical manifestation is variable from no overt symptoms for a long time to back pain, scoliosis, progressive neurological deficit of legs and incontinence. Therefore the diagnosis requires a strong clinical suspicion and aggressive investigation. We experienced a case of 1 day-old male infant who was presented with small soft mass on sacral area. Spinal ultrasonography and spine MRI revealed tethered cord associated with intradural lipoma. We reported a case of tethered cord syndrome with brief review of literatures.

10.Effect of Diabetes Education Program on Glucose Metabolism and Lipid Metabolism, Self-efficacy in NIDDM Patients.

The purpose of this study is to determine the effect of diabetes education program on Glucose Metabolism(blood sugar. HbAlc) and Lipid Metabolism(total cholesterol. triglyceride. low density lipoprotein. high density lipoprotein). Self-efficacy in non-insulin independent diabetes mellitus. The study design was a non equivalent control group pre-test post-test design. Data for the study were collected from March 12 to June 19, 2001. Sixty-two research subjects were assigned to experimental (36) and control (26) groups. The collected data was analyzed using the Chi-Square test. t-test by spsswin program The results are as follows: 1. Experimental group had higher level of glucose metabolism than control group(FBS: t=-3.317. p=.002. HbAlc: t=-4.956. p=.000). 2. Level of lipid metabolism were partly a significant different between experimental group and control group(Triglyceride: t=-2.108. p=.039). 3. Experimental group had higher efficacy score than control group(t=4.651. p=.000). In conclusion. the study supported the effects of diabetes education program to increase metabolism and self-efficacy. Further study with a longitudinal design is suggested to verify the effect of diabetes education program in NIDDM and standardized diabetes education program.
