1.A Case of Angioma Serpiginosum.

Angioma serpiginosum is a rare acquired vascular lesion that usually presents in the first two decades of life with a predilection for females. A typical lesion is characterized by deeply red nonpalpable puncta that are grouped closely together in macular or netlike patterns on the lower extremities and buttocks. Histopathologically, thin-walled dilated capillaries are seen in some of the dermal papillae and the superficial reticular dermis without extravasation of red blood cells. We report a case of angioma serpiginosum developed on the left chest and left upper extremity in a 18-year-old man.

3.Evaluation of Muscular Atrophy and Fatty Infiltration Using Time-zero Magnetic Resonance Imaging as Baseline Data, After Rotator Cuff Repair

BACKGROUND: This study evaluated postoperative changes in the supraspinatus from time-zero to 6 months, using magnetic resonance imaging (MRI). We hypothesized that restoration of the musculotendinous unit of the rotator cuff by tendon repair immediately improves the rotator cuff muscle status, and maintains it months after surgery. METHODS: Totally, 76 patients (29 men, 47 women) with rotator cuff tears involving the supraspinatus tendon who underwent arthroscopic rotator cuff repairs were examined. MRI evaluation showed complete repair with intact integrity of the torn tendon at both time-zero and at 6 months follow-up. All patients underwent standardized MRI at our institution preoperatively, at 1 or 2 days postoperative, and at 6 months after surgery. Supraspinatus muscular (SSP) atrophy (Thomazeau grade) and fatty infiltrations (Goutallier stage) were evaluated by MRI. The cross-sectional area of SSP in the fossa was also measured. RESULTS: As determined by MRI, the cross-sectional area of SSP significantly decreased 11.41% from time-zero (immediate repair) to 6 months post-surgery, whereas the Goutallier stage and Thomazeau grade showed no significant changes (p<0.01). Furthermore, compared to the preoperative MRI, the postoperative MRI at 6 months showed a no statistically significant increase of 8.03% in the cross-sectional area. In addition, morphological improvements were observed in patients with high grade Goutallier and Thomazeau at time-zero, whereas morphology of patients with low grade factors were almost similar to before surgery. CONCLUSIONS: Our results indicate that cross-sectional area of the initial repair appears to decrease after a few months postoperatively, possibly due to medial retraction or strained muscle.

5.Change of Inducible Nitric Oxide Synthase Expression by Ultraviolet B Irradiation on the Skin of a Rat.

BACKGROUND: Nitric oxide synthase (NOS) is known to mediate ultraviolet B (UVB)-induced skin inflammation However, there is still ambiguity as to which NOS isotype mediates the process in vivo. Furthermore, contradictory results have been reported on which cell types respond to UVB irradiation in vitro. OBJECTIVE: This study was performed to evaluate the change of inducible NOS (iNOS) expression in vivo as a result of UVB radiation on the skin of a rat. METHOD: To examine the time-course change in iNOS expression in the rat skin, the rats were exposed to 400 ml/cm2 of UVB radiation, and skin samples were taken at various time intervals up to 48 h. iNOS expression on the skin of a rat was evaluated by both Western blot analysis and immunohistochemical staining. RESULTS: From Western blot analysis, UVB irradiation induced inducible NOS (iNOS) expression in the epidermis at 12-48 h postirradiation with a peak expression at 24 h. Immunohistochemical staining revealed that UVB-induced iNOS expression was localized to the epidermis and infiltrating inflammatory cells in the upper dermis of the rat. CONCLUSION: iNOS was induced by UVB irradiation on the skin of a rat, mainly in the epidermis. Therefore, iNOS is supposed to be one of the major mediators with regard to inducing an inflammatory response in UVB-irradiated rat skin in vivo.

Lead has been widely used in industry and contained in various household products. However, allergic reaction to lead is known to be extremely rare. A 23-year-old woman who had been working in an electronics company for 1 year presented with pruritic erythematous macules and papules on both arms and hands of 1 month duration. She was diagnosed as a contact dermatitis due to lead by patch test using lead nitrite(0.01% aq.). The cutaneous eruption subsided with treatment and has not recurred by changing the working place for 6 months. We report a case of contact dermatitis due to lead.

8.A study of skin barrier function in atopic eczema using the patch test and transepidermal water loss.

BACKGROUND AND OBJECTIVE: : ?The etiology of atopic dermatitis is still not clear. Atopic dermatitis is known to develop through immunological and non-immunological pathomechanisms. The delayed hyper- sensitivity reaction in atopic dermatitis has been reported as decreasing due to disturbance of cellular immunity. However, in recent days, there is a dispute as to whether disturbed epidermal barrier of atopic dermatitis could elicit a delayed type hypersensitivity reaction by an acceleration in the penetration of allergen. The aim of this study was to determine whether transepidermal water loss (TEWL) increased in atopic dermatitis, and whether the TEWL differed with a presence of delayed type-skin reaction. In addition, the prevalence of positive patch test reaction and TEWL were measured in atopic dermatitis and normal controls. METHODS: We performed the patch test with 24 Korean standard allergens and 5 aeroallergens on 38 atopic dermatitis patients. TEWL was measured using DermaLab (Cortex technology, Denmark) on 38 patients with atopic dermatitis and 40 normal controls. RESULTS: 17 (44.7%) patients with atopic dermatitis had positive reactions on the patch test. The frequency of positive allergens was in the following order: nickel sulfate (18.4%), thimerosal (15.8%), D. pteronyssinus (15.8%), and potassium dichromate (13.1%). TEWL values of both clinically normal skin and eczematous skin in atopic dermatitis patients were higher than those of normal controls. There was no statistically significant difference in TEWL values of normal skin of atopic dermatitis between the positive patch test and negative patch test groups. In positive reaction sites of aeroallergen patch test, the TEWL increased in comparison with both the control site and the positive classical patch test reaction site but it was not statistically significant. CONCLUSION: These results revealed that epidermal barrier is disturbed in atopic patients and suggested that various allergens might easily penetrate the skin to induce the delayed-type hypersensitivity reaction.

9.A Case of Multiple Bowen's Disease with Atypical Psoriasiform Large Plaque Associated with Internal Malignancies.

We report a case of multiple Bowen's disease of probable arsenic origin associated with internal malignancies. A 63-year-old man who had a history of arsenicals injections showed pruritic erythematous 11 8 cm sized psoriasiform plaque on the right flank and multiple keratotic plaques on the trunk and both extremities. He had operation on his bladder due to transitional cell carcinoma. He was found to have a bronchogenic carcinoma by chest X-ray and CT scan. We report this as an interesting case of atypical multiple Bowen's disease manifestating a psoriasiform large plaque associated with internal malignancies of lung and bladder.

10.Developmental Toxicity by Exposure to Bisphenol A Diglycidyl Ether during Gestation and Lactation Period in Sprague-dawley Male Rats.

OBJECTIVES: Bisphenol A diglycidyl ether (BADGE) is the major component in commercial liquid epoxy resins, which are manufactured by co-reacting bisphenol A with epichlorohydrin. This study was performed to show the developmental effects of prenatal and postnatal exposures to BADGE in male rat offspring. METHODS: Mated female rats were divided into four groups, each containing 12 rats. The dosing solutions were prepared by thoroughly mixing BADGE in corn oil at the 0, 375, 1500 and 3000 mg/kg/day concentrations. Mated females were dosed once daily by oral gavage on gestation day (GD) 6 - 20 and postnatal day (PND) 0 - 21. Pregnant female dams were observed general symptoms and body weight. Also, male pups were observed the general symptoms, body weight, developmental parameters (e.g. anogenital distance, pina detachment, incisor eruption, nipple retention, eye opening, testis descent), organ pathologic changes and hormone levels of plasma. RESULTS: Pregnant rats treated with BADGE died at a rate of about 70% in the 1500 mg/kg/day group and all rats treated with 3000 mg/kg/day died. Body weight, for male pups treated with doses of 375 mg/kg/day, was significantly lower than in the control group at PND 42, 56, and 63 (p<0.05). Evaluation of body characteristics including; separation of auricle, eruption of incisor, separation of eyelid, nipple retention, descent of testis, and separation of the prepuce in the BADGE treated group showed no difference in comparisons with the control group. AGD and adjusted AGD (mm/kg) for general developmental items in BADGE 375 mg/kg/day treated pups tended to be longer than in controls, however, these differences were not statistically significant. Relative weights of adrenal gland, lung (p<0.05), brain, epididymis, prostate, and testis (p<0.01) were heavier than in control in measures at PND 9 weeks. There were no significant changes in comparisons of histological findings of these organs. Loss of spermatids was observed in the seminiferous tubule at PND 9 weeks, but no weight changes were observed. The plasma estrogen levels were similar in the control and treatment groups at PND 3, 6 and 9 weeks. The plasma testosterone levels in the control group tended to increase with age. However, in the BADGE 375 mg/kg/day treated male pups it did not tend to increase. CONCLUSIONS: These findings suggest that BADGE is a chemical that has developmental effects consistent with it being an endocrine disruptor.
