1.Standardization of the Korean Version of the Patient Health Questionnaire-4 (PHQ-4)

Objective:
The Patient Health Questionnaire-4 (PHQ-4) has been used for screening owing to ease of use and brevity.In this study, we developed the Korean version of the PHQ-4 and tested its validity.
Methods:
One hundred sixteen new adult outpatients at the Department of Psychiatry of the Korea University Ansan Hospital participated in the study. We simultaneously administered other depression/anxiety scales: the Hamilton Rating Scale for Depression, the Hamilton Anxiety Scale, the Beck Depression Inventory, and the Beck Anxiety Inventory.
Results:
The mean PHQ-4 score was 6.52 (standard deviation = 3.45). Cronbach’s α was 0.792, and the intraclass correlation coefficient of test and 2-week interval retest was 0.827 (p < 0.01). The Pearson correlation coefficients between the PHQ-4 total score and other depression/anxiety scales were all over 0.6. Confirmatory factorial analysis showed acceptable convergent validity and reliability but questionable discriminant validity for some model fit values.
Conclusion
The Korean version of the PHQ-4 has sufficient internal consistency, test-retest reliability, and construct validity, but its two-factor structure showed incompleteness. However, we suggest that it should be used as a brief screening measure for common psychiatric distress that warrants further detailed assessment, but not to separately assess the severity of depression and anxiety symptoms.
2.One case of endometrial cancer occurrence: Over 10 years after colon cancer in Lynch family.

We have recently experienced an endometrial cancer 12 years after the diagnosis of colon cancer with Lynch syndrome. A 49-year-old Korean woman had a family history of colon cancer. Her mother had colon cancer at 56-year-old, and her brother had colon cancer at 48 years old. The patient received surgery for endometrial cancer at the same hospital 12 years after being treated for colon cancer. Immunohistochemistry showed that her endometrial tissue stained negative for MSH2. A microsatellite instability test was performed and showed the presence of instability high microsatellite instability. An hMLH2 gene mutation was detected at codon 629 codon of exon 12, in which a glutamine was replaced with an arginine (1886A>G [p.Gln629Arg]). To our knowledge, this is the first case of metachronous cancer in a Lynch syndrome family in Korea with a gap of more than ten years between cancer diagnoses.

3.The Change of Lacrimal Gland Volume in Korean Patients with Thyroid-associated Ophthalmopathy.

PURPOSE: To describe the change of lacrimal gland volumes in Korean patients with thyroid-associated ophthalmopathy (TAO) via computed tomography (CT). METHODS: A retrospective review of CT images from 217 TAO patients and 135 control subjects was performed. The TAO patients were diagnosed between May 2005 and May 2014 and had a CT performed on initial presentation (330 orbital CT scans). These images were compared with 270 orbital CT scans from the control group, obtained between May 2013 and May 2014. An open source DICOM viewer was used to calculate the volume of the lacrimal gland. RESULTS: The mean volume of the lacrimal gland in TAO patients was 0.816 cm³ in the right orbit (standard deviation [SD], 0.048) and 0.811 cm3 in the left orbit (SD, 0.051), with no significant difference between right and left (p = 0.192). However, significant differences were observed between TAO patients and healthy individuals (p < 0.001). There was no significant difference between mean lacrimal gland volumes of males (0.812 cm³; SD, 0.037) and females (0.816 cm³; SD, 0.029) (p = 0.513). There was a negative correlation between gland volume and age in TAO patients (Pearson r = -0.479, p = 0.00). The subjective tearing (right: r = 0.244, p = 0.018; left: r = 0.226, p = 0.024), corneal superficial punctate keratopathy (right: r = 0.192, p = 0.040; left: r = 0.206, p = 0.036), and exophthalmometry (right: r = 0.182, p = 0.032; left: r = 0.180, p = 0.046) correlated with lacrimal gland volume. CONCLUSIONS: This study is the first to use CT images to calculate the lacrimal gland volume of Korean TAO patients. In TAO patients, the lacrimal gland volume was notably increased compared to control subjects. The lacrimal gland volume decreased with age, but there was no difference between gender and no difference between left and right. The lacrimal gland volume correlated with subjective tearing, corneal superficial punctate keratopathy and exophthalmometry.

5.The Effect of Intraoperative Exophthalmometric Values on Enophthalmos Correction in Inferior Orbital Wall Reconstruction.

PURPOSE: To measure the enophthalmos corrective effect after inferior orbital wall reconstruction, we compared preoperative and intraoperative exophthalmometric values with postoperative exophthalmometric values. METHODS: From January 2014 to April 2016, 60 eyes of 60 patients who underwent surgery for inferior orbital wall fracture were included. In Group 1, the exophthalmometric value was measured before surgery, during the operation, and 6 months after surgery using the Naugle exophthalmometer. In Group 2, the value was measured before surgery and 6 months after surgery using the Hertel exophthalmometer. The thickness of implants was determined by preoperative exophthalmometric values and overcorrection of 0.5 mm was performed in Group 1 patients with relatively large fractures. RESULTS: The mean age of the patients was 32.4 years in Group 1 and 34.3 years in Group 2. The mean duration between injury and surgery was 4.2 weeks in Group 1 and 2.3 weeks in Group 2. There was no statistically significant difference between preoperative exophthalmometric values in Group 1 (−1.78 ± 0.31 mm) and Group 2 (−1.81 ± 0.26 mm), but postoperative exophthalmometric values between Group 1 (−0.25 ± 0.78 mm) and Group 2 (−0.53 ± 0.46 mm) were statistically different (p = 0.034). CONCLUSIONS: The exophthalmometric values and wall fracture size are important factors for determining implant thickness of inferior orbital wall reconstruction. Intraoperative measurement of exophthalmometric values should be considered in inferior orbital wall reconstruction for enophthalmos correction.

6.Ciliary Activity in Basal and Activated Status and Its Correlation with Clinical Findings in Chronic Rhinosinusitis.

BACKGROUND AND OBJECTIVES: There are conflicting results about ciliary activity in chronic rhinosinusitis (CRS). And dynamic movements reacting to various stimuli in sinusitis mucosa have been studied rarely. This study was designed to investigate the ciliary activity according to the severity of sinusitis and clinical symptoms. We aimed to identify the dynamic ciliary activity in response to purinergic stimulation in sinusitis. SUBJECTS AND METHOD: Nasal mucosal samples were obtained from 44 CRS and 20 normal subjects. We measured ciliary beat frequency (CBF) in the basal and activated status. For evaluating the correlation of CBF with clinical findings, nasal symptoms, endoscopic findings, CT findings, allergy tests, and olfactory tests were reviewed. RESULTS: No significant differences were found in basal CBF between normal and CRS mucosa. However, the CBF stimulated by ATP in normal and CRS mucosa showed marked differences: both the maximal increase and duration of increased CBF by ATP were significantly reduced in CRS mucosa. The olfactory test showed significant correlation with basal CBF, but the presence of allergy or nasal symptoms did not show significant correlation with the basal CBF. The CT score and presence of nasal polyposis showed negative correlation with the basal CBF (p<0.05). CONCLUSION: The basal CBF of normal and CRS mucosa showed no difference. However, dynamic ciliary activity responding to purinergic stimulation was markedly attenuated in sinusitis mucosa. The severity of sinusitis and presence of polyposis negatively affected CBF.

7.Effects of Nitric Oxide Synthase Inhibitor on Urinary Bladder and Proximal Urethral Function: Comparison between Mature Male and Female Rat.

PURPOSE: The present study was undertaken to compare the function of the urinary bladder, reflex changes of the urinary bladder, and bladder outlet, during micturition between mature male and female rats. MATERIALS AND METHODS: Mature male(n=6) and female(n=6) Sprague-Dawley rats weighing 250 to 300gm were used. After each rat was anesthetized with urethane(1g/kga simultaneous recording of the intravesical pressure and voided urinary volume during micturition were recorded. And then, the urethra was ligated placing 4-0 silk tie just distal to the bladder neck. Isovolumetric bladder contractions and urethral pressure were recorded independently. RESULTS: The voided volume and intravesical pressure during micturltion showed no significant differences between mature fema1e and male rats. During reflex bladder contraction, isovolumetric bladder contractions and urethral pressure also showed no significant differences between mature female and male rats. Administration of N-nitro-L-arginine(L-NOARG, 15mg/kg), a nitric oxide synthase inhibitor, intravenously in female rats, reversibly decreased the magnitude(42.5%, p<0.05) and duration(35.4%, p<0.05) of reflex urethral relaxation. There was no effect on reflex urethral relaxation in male rats. Administration of L-NOARG 20mg/kgintravenously in male rats reversibly decreased the magnitude(49.8%, p<0.05) and duration(38.7%, p<0.05) of reflex urethral relaxation. CONCLUSIONS: From the results, it appears that the function of the urinary bladder and the reflex change of the urinary bladder and bladder outlet during micturition showed no significant difference between male and female rats of similar weight. Howerever, the dosage of L-NOARG showed difference of the decreasing level of the magnitude and duration of the urethral relaxation.

8.Cyclocryotherapy and Intravitreal Gas Tamponade of a Chronic Cyclodialysis Cleft: Case Report.

PURPOSE: To report a case of chronic hypotony maculopathy caused by traumatic cyclodialysis cleft and treated with 20% sulfur hexafluoride (SF6) gas tamponade with cyclocryotherapy. CASE SUMMARY: A 39-year-old woman with a history of blunt trauma developed a unilateral chronic ocular hypotony in her left eye. She was treated with topical atropine sulphate 1% for 2 months. Three years later, she was referred to our clinic for evaluation and treatment of persistent hypotony. The intraocular pressure (IOP) was 4 mm Hg and the best corrected visual acuity was 0.4. B-scan echography revealed a choroidal effusion and fundus examination showed choroidal detachment and macular folds. Gonioscopy examination revealed cyclodialysis cleft from the direction of 7 o'clock to 11 o'clock. A single bubble of SF6 20% (0.4 cc) was injected into the vitreous cavity and transconjunctival cyclocryotherapy was performed under retrobulbar anesthesia. Six months later, the IOP was 12 mm Hg and the best corrected visual acuity was 1.0. B-scan echograpy and fundus examination showed the disappearance of the choroidal effusion. CONCLUSIONS: Gas tamponade with cyclocryotherapy may be useful in cases of cyclodialysis cleft that failed to respond to medical therapy.

9.Retinitis Pigmentosa Complicated by Vitreous Hemorrhage in a Young Patient: A Case Report.

PURPOSE: To report a case of a young male patient with retinitis pigmentosa (RP) accompanied by vitritis and neovascularization of the optic disk in both eyes who underwent unilateral vitrectomy for the treatment of vitreous hemorrhage in the right eye. CASE SUMMARY: An 8-year-old boy visited our clinic with a complaint of night blindness. Both eyes showed inflammatory cells in the anterior vitreous and neovascularization of the optic disk confirmed by fluorescein angiography. Extensive vitreous hemorrhage developed in his right eye and he underwent unilateral vitrectomy. His final visual acuity was 0.6 in both eyes. CONCLUSIONS: Vitreous hemorrhage may be related to chronic inflammation in the vitreous and is a very rare RP complication. Vitrectomy can be an effective treatment option for RP complicated by vitreous hemorrhage.

10.A Case of Perimenopausal Endometrial Cancer in a Woman with MSH2 Germline Mutation.

Lynch syndrome is a genetic malignancy syndrome affecting the colon, endometrium, and other organs. It is difficult to find a Lynch syndrome patient without any family history of cancer. We have recently examined an endometrial cancer patient with a MSH2 gene mutation without a family history of cancer. A 55-year old Korean woman was admitted to a local clinic for vaginal bleeding. An endometrial biopsy revealed the presence of adenocarcinoma (endometrioid type, grade 1). After surgical staging, no further adjuvant therapy was required. Analysis of the tissue using immunohistochemistry (IHC) showed the endometrium stained negatively for MSH2. Microsatellite instability (MSI) was analyzed for five markers. The patient was scored as unstable. Further, additional gene sequencing revealed one missense mutation in c.23C > T (p.Thr8Met). This is the first case of Lynch syndrome endometrial cancer in Korea in which the patient does not have any family history of cancer.
