1.Clinical Study of Osteitis Condensans Ilii

Osteitis condensans ilii is one of the rare disease which represents an alteration in the normal bony architecture of that portion of the ilium alongside the sacro-iliac joint: namely condensation without involvement of joint space or adjacent sacrum. We analyzed clinically 9 cases of osteitis condensans ilii who were treated at the Department of Orthopedic Surgery, Medial college of Yonsei University. The results were as follows :l. All patients were women and oldest patient was 47, the youngest 23 years of age. 2. The pain occured following delivery in 3 cases and in one case ofter trauma. There was urinary tract infection in one case. 3. All patients complained low back pain and the pain extended to buttock in 2 cases whereas the radiating pain to lower extremity in 4 cases. 4. In 4 cases was a positive straight leg raising test elicited. 5. In 3 cases the blood sedimentation rate was increased above normal. 6. All patient succeed in relieving pain with conssrvative treatment. In 2 instances steroid was used. 7. There were combined diseases in 4 cases : herniated nucleus pulposus in 2 cases, rheumatoid arthritis in 1 case, osteoarthritis in 1 case.

2.Prednisolone Influence on Avascular Osteonecrosis after Renal Transplantation

Avascular osteonecrosis has frequently occured in patients who have undergone transplantation of a kidney and generally has been considered to be a complication of the use of corticosteroids. But there are controversies regarding their dosage and duration of use in its influence on prevalence of avascular osteonecrosis. We analyzed the cases of our patients who underwent transplantation of a kidney and who we were able to follow up more than 8 months. l. Of a total of 300 patients, osteonecrosis developed in 24 joints of 12 patients. 2. Osteonecrosis was more frequently found in group of patients who were administered with larger doses of steroid. 3. Rejections of the transplanted kidney occured more frequently in those groups with osteonecrosis. 4. Most of rejections occured within 3 months after renal transplantation. 5. It took 4–26 months (average 12.3 months) the osteonecrosis could be found. 6. There were no differences in blood concentration of calcium phosphate and creatinine between osteonecrosis and non-osteonecrosis group. 7. Replacement arthroplasties were done in 10 joints without significant complication.

3.A Study of Treatment Realities and Recognition of Male Osteoporosis.

BACKGROUND: Osteoporosis has been thought of as a disease of women, but the incidence of male osteoporosis is increasing and is expected to keep rising due to alcohol abuse, smoking and increased life expectancy for men. This study was done to evaluate how many men recognized the significance of osteoporosis and its management. METHODS: There were 138 male subjects who underwent Bone Mineral Density (DEXA) from January 2000 to August 2003 at Sung Nam Central Hospital. We reviewed their BMD results and formed questionnaires which were constructed to evaluate men's recognition and treatment realities of osteoporosis. Telephone interviews and mailing services were utilized. The respondents were 96 (69.6%) and analysis was done. RESULTS: The average age of the 138 men was 55. Among them, there were 55 (39.9%) and 68 (49.3%) men who had been diagnosed with osteopenia and osteoporosis, respectively. Information on osteoporosis was obtained mainly from mass media like TV and newspaper (57.3% of total answers). The reason for undergoing test for osteoporosis was related to the annual screening tests (40.6% of total answers). Most of these people were likely to ignore the significance of osteoporosis, because only 26 percent answered they should receive proper treatment in case of osteoporosis although they did not have any symptoms at the time of the diagnosis. Among these osteoporotic patients, 76.7 percent did not seek any medical care and the most common reason for refusal to treatment was no existing symptoms for osteoporosis. Among the total, 67.4 percent of the patients said there had been no change of life behavior after the diagnosis. CONCLUSION: We found that men had relatively more knowledge of osteoporosis than we expected, but they knew little about the need for treatment and possible severe complications like major fractures that may occur. Also, we found the diagnosis of osteoporosis has little effect on behavioral changes. In conclusion, family physicians should take more interest in the education of patients to increase compliance for its treatment.

4.Egg detection rates of Enterobius vermicularis in children.

The cello-tape anal swab specimens from 936 primary school children in Seoul, 308 primary school children in a rural area and 92 children in Nanjido, were examined for Enterobius vermicularis eggs. Out of 936 specimens from a primary school in Seoul, E. vermicularis eggs were detected in 114 specimens, the egg detection rate being 12.2 percent. In 308 specimens from a primary school in a rural area, 77 (25.0 percent) were positive for E. vermicularis eggs. Among 92 specimens from Nanjido children in Seoul, egg positive rate of E. vermicularis was 28.3 percent. The egg detection rate of primary school children in Seoul was significantly lower than that of primary school children in a rural area and of Nanjido children. The egg detection rate in boys was lower than that of girls. As the age of children increased, the egg detection rate decreased.

5.A Comparison of Misoprostol and Dinoprostone Vaginal Tablet for Labor Induction at Full Term Pregnancy.

Our purpose was to compare the safety and efficacy of intravaginal misoprostol versus dinoprostone vaginal tablet for induction of labor at term pregnancies. Two hundred three patients with indications for induction of labor at term were randomly assigned to receive either intravaginal misoprostol or dinoprostone vaginal tablet. Fifty micrograms of misoprostol was placed in the posterior vaginal fornix every six hours, with a potential maximum of four doses. 3 milligrams of dinoprostone vaginal tablet was placed in the posterior vaginal fornix every eight hours, with a potential maximum of three doses. Further medication was withheld with the occurrence of spontaneous rupture of membrane, entry into adequate contraction pattern(twenty second sustained with two or more frequent uterine contraction in 10 minutes), nonreassuring FHR tracing, or delivery. Artificial membrane rupture with both study protocol was done at the discretion of the attending physician. After membrane rupture, in the cases of failure of active labor or arrest of dilation, oxytocin was administerated. Among those evaluated, 100 received misoprostol and 102 received dinoprostone. The average interval from start of induction to vaginal delivery was shorter in the misoprostol group(784.7 +/- 389.3 min) than in the dinoprostone group(988.3 +/- 369.5 min)(p<0.01). There was no significant difference in change of Bishop score between the two groups. No statistically significant differences were noted between two groups in case of need for oxytocin and oxytocin total dose, but significant difference was noted between two groups in case of indication for oxytocin augmentation. There were no significant differences in the routes of delivery. Intravaginal administration of misoprostol appears to be as effective as dinoprostone vaginal tablet for labor induction at full term pregnancies. Complications associated with prostaglndin administration were not statistically different between the two treatment groups.

6.Safety and Feasibility of a Laparoscopic Colorectal Cancer Resection in Elderly Patients.

PURPOSE: The aim of this study is to assess the effects of age on the short-term outcomes of a laparoscopic resection of colorectal cancer in elderly (> or =75 years old), as compared with younger (<75 years old), patients. METHODS: A retrospective analysis of patients who underwent laparoscopic surgery for colorectal cancer between January 2007 and December 2009 was performed. There were two groups: age <75 years old (group A) and age > or =75 years old (group B). The perioperative outcomes between group A and group B were compared. RESULTS: The study included 824 patients in group A and 92 patients in group B. The body mass index (BMI) and the American Society of Anesthesiologists (ASA) score were significantly different between group B and group A (BMI: 22.5 vs. 23.5, P = 0.002; ASA score: 1.88 vs. 1.48, P = 0.001). Mean operating times were similar between the groups (325.4 minutes vs. 351.6 minutes, P = 0.07). We observed a higher overall complication rate in group B than in group A (12.0% vs. 6.2%, P = 0.047), but the number of severe complications of Accordion Severity Classification > or =3 (those that required an invasive procedure) was not significantly different between the two groups (6.5% vs. 3.4%, P = 0.142). There was no significant difference in the length of hospital stay (13.0 days vs. 12.0 days, P = 0.053). CONCLUSION: Although the elderly patients had a significantly higher overall postoperative complication rate, no significant difference was seen in either the number of severe complications of Accordion Severity Classification > or =3 or in the length of hospital stay. A laparoscopic colorectal cancer resection in elderly patients, especially those aged 75 years or older, is safe and feasible.

7.Optimal timing of surgery after neoadjuvant chemoradiation therapy in locally advanced rectal cancer.

PURPOSE: The optimal time between neoadjuvant chemoradiotherapy (CRT) and surgery for rectal cancer has been debated. This study evaluated the influence of this interval on oncological outcomes. METHODS: We compared postoperative complications, pathological downstaging, disease recurrence, and survival in patients with locally advanced rectal cancer who underwent surgical resection <8 weeks (group A, n = 105) to those who had surgery > or =8 weeks (group B, n = 48) after neoadjuvant CRT. RESULTS: Of 153 patients, 117 (76.5%) were male and 36 (23.5%) were female. Mean age was 57.8 years (range, 28 to 79 years). There was no difference in the rate of sphincter preserving surgery between the two groups (group A, 82.7% vs. group B, 77.6%; P = 0.509). The longer interval group had decreased postoperative complications, although statistical significance was not reached (group A, 28.8% vs. group B, 14.3%; P = 0.068). A total of 111 (group A, 75 [71.4%] and group B, 36 [75%]) patients were downstaged and 26 (group A, 17 [16.2%] and group B, 9 [18%]) achieved pathological complete response (pCR). There was no significant difference in the pCR rate (P = 0.817). The longer interval group experienced significant improvement in the nodal (N) downstaging rate (group A, 46.7% vs. group B, 66.7%; P = 0.024). The local recurrence (P = 0.279), distant recurrence (P = 0.427), disease-free survival (P = 0.967), and overall survival (P = 0.825) rates were not significantly different. CONCLUSION: It is worth delaying surgical resection for 8 weeks or more after completion of CRT as it is safe and is associated with higher nodal downstaging rates.

8.Therapeutic Approach of Wrist Ganglion Using Electroacupuncture: Two Case Reports.

A ganglion cyst is a relatively common benign tumor on the wrist. Conservative and surgical approaches have been used for its treatment. Various conservative treatment methods have been suggested such as reassurance, aspiration, sclerosant injection, and direct compression. But, there is no acceptable treatment of choice yet because each suggested method has a relatively high recurrence rate. We want to report two cases in which the size of the wrist ganglion was decreased by using electroacupuncture. One patient presented with a chronic ganglion for six years and the other patient presented with a recently occurred acute ganglion. We applied electroacupuncture for 20 minutes once a week for eight weeks to both of them. Afterwards, the size of the wrist ganglion diminished in the follow-up sonography and the accompanying pain was also relieved. Herein we report both cases along with a review of the relevant literature.

9.The Efficacy of Radial Optic Neurotomy for Central Retinal Vein Occlusion.

PURPOSE: Central retinal vein occlusion (CRVO) often produces significant and permanent loss of vision in the affected eye, however there is no known effective treatment of CRVO. By restoration of retinal venous circulation via radial optic neurotomy (RON), we want to know the efficacy and complication of radial optic neurotomy were evaluated. METHODS: Ten eyes of 10 consecutive patients diagnosed as CRVO in Chonbuk national university hospital with best visual acuity 0.1 or worse underwent RON after a standard three-port vitrectomy as Opremcak's study. The average duration of the CRVO prior to RON was 2.4 months with a range of 1-9 months. RESULTS: The average follow-up was 5 months with a range of 1-12 months. Equal or better postoperative visual acuity, rapid resolution of intraretinal hemorrhage and restoration of normal retinal vasculature were noted in 8 of the 10 (80%) patients. 5 of the 10 (50%) patients had a final visual acuity of 0.1 or better. two patients achieved 0.3 final visual acuity, one patient achieved 0.6 final visual acuity. CONCLUSIONS: Compared with others without any treatment, RON may be a beneficial surgical procedure for severe CRVO and 0.1 or worse visual acuity. More cases and longer follow-up are needed to establish the indication, efficacy, and safety of RON in CRVO

10.The Utility of Ultrasonography for the Diagnosis of Developmental Dysplasia of Hip Joint in Congenital Muscular Torticollis.

OBJECTIVE: To determine whether a routine ultrasonography (US) is necessary for diagnosis of developmental dysplasia of hip (DDH), presenting with congenital muscular torticollis (CMT). METHODS: Cases of 133 patients (81 males, 52 females) diagnosed as CMT were reviewed, retrospectively. We reviewed the medical charts and diagnostic examination. We also assessed the coincidence of CMT and DDH, and investigated the clinical features of CMT related to DDH. RESULTS: Twenty (15.0%) patients out of 133 CMT patients were diagnosed as having DDH by US. Of whom, 8 patients were radiographically positive and 4 patients were both clinically and radiographically positive. Nine patients were treated with a harness and 1 of them needed closed reduction and casting. Out of 9 patients treated with a harness, only 4 were clinically positive. The difference and ratio of the sternocleidomastoid (SCM) muscle thickness between the normal and abnormal side was significantly greater in DDH patients (p=0.014). Further, receiver operating characteristic analysis showed when the SCM ratio is greater than 2.08 and the SCM difference is greater than 6.1 mm, the efficiency of US for the diagnosis of the DDH was found to be the best (p<0.05). CONCLUSION: To evaluate DDH, physical examination showed low sensitivity and radiologic study has limitation for the child before 4 to 6 months of age. Therefore, we recommend that hip is screened by US for the diagnosis of DDH associated with CMT when physical examination is positive or CMT patients with large SCM difference and high SCM ratio.
