1.Development of elastin layers in the aortic wall of human fetuses.

The presence of elastin layers in the aortic walls of twelve human fetuses was confirmed with scanning electron microscope pictures after hot alkali treatment and histochemical examination. In addition, the number of elastin layers in aortic walls of 5 different segments were compared in fetuses of varying ages. Aldehyde fuchsin stained slides of elastin ascending aortas showed a range between 27 and 55 layers of elastin in fetuses of 8 weeks to 32 weeks. However, in the lower abdominal aortas, elastin layers decreased from 28 to only 3 layers for fetuses of the same age. Furthermore, as elastin layers decreased from ascending aorta to abdominal aorta with the progression of fetal life, similar changes in the elastin lamellae were observed. These results suggest that while aortas grow rapidly in length, the medial elastin thickens slowly, perhaps due to slow development of hydrodynamic forces and pressures. Also the adventitial elastin appears to lose out gradually along the length from ascending aorta to abdominal aorta.

2.Morphogenic development of the pancreas in the staged human embryo.

The Carnegie stage is widely applied in the field of human embryology, and it is more logical to analyze the embryos by this stage than CR length or menstrual age. In this study, the early development of the pancreas is studied by tissue observation and reconstruction using serial sections of 33 human embryo ranging from Carnegie stages 11 to 23. The dorsal pancreas develops from the dorsal wall of the duodenum in stage 12, and the ventral pancreas from the proximal part of the cystic primordium in stage 13 or 14 as a single epithelial thickening, but in one case, as a bilateral thickening which contains some isolated spaces. The rotation of the ventral pancreas starts in stage 15, and completes in stage 17. Surrounding connective tissue differentiates in stage 18.

4.Treatment of Old Achilles Tendon Rupture using Modified Flexor Hallucis Longus Tendon Transfer.

PURPOSE: The purpose of this study was to evaluate the clinical results of the old Achilles tendon rupture treated with modified flexor hallucis longus (FHL) tendon transfer. MATERIALS AND METHODS: Seventeen patients with old Achilles tendon rupture treated with modified FHL tendon transfer between March 2004 and February 2008 were enrolled in this study. Technically FHL was pass through the distal portion of the ruptured tendon instead of the drilled hole made on the calcaneus. The mean age of the patients was 37 years (range, 22~67 years), mean follow-up period was 28 months (range, 12~30 months). Patients' subjective satisfaction, calf circumferential diameter, range of motion of ankle and AOFAS ankle-hind foot score and Arner-Lidholm score was evaluated. RESULTS: The average gap between the ruptured tendon was 52 mm (range, 47~56 mm). The AOFAS score improved from 47 pre-operatively to 91 points at the last follow-up. Sixteen patients were satisfied with the result free from discomfort, a patient had mild discomfort who had DM. fourteen patients had decreased range of motion less than 5 degrees while 2 patients had more than 7 degrees decrease compared to the intact side but had no discomfort in daily activities. Nine patients had less than 1 cm calf circumferential diameter difference and 7 patients had 1 to 3 cm diameter difference compared to the intact side. One who had more than 3 cm diameter difference had deteriorated muscle strength. CONCLUSION: Modified FHL tendon transfer can be a useful technique for the treatment of old Achilles tendon rupture when the gap is with large gap placed too proximal.

6.Osteochondroma of the Proximal Phalanx of the Fourth Toe(A Case Report).

Osteochondroma is the most common benign bone tumor which commonly occurs in the metaphysis of the long bones such as proximal humerus, tibia, and distal femur. It is rarely found in bones of the foot. Although they are benign lesions, when they occur in the foot, they are typically identified earlier than other regions because of the low proliferation of subcutaneous tissue in the region and may cause symptoms. We experienced a rare case of osteochondroma in a 60-year-old male which cause pain and swelling of the fourth toe.

7.The Role of Serum Pepsinogen in Detection of Gastric Cancer.

PURPOSE: This study was done to determine the usefulness of serum pepsinogen (PG) levels as a screening method for gastric cancer, and to assess the relationships between serum PG and clinicopathologic factors of gastric adenocarcinoma. MATERIALS AND METHODS: Serum PG concentrations were measured in 94 subjects who were classified into (a) a control group (50 subjects) without abnormal endoscopic finding on a health checkup, or (b) a gastric cancer group (44 subjects) who had surgery at Daegu Catholic University Hospital between Nov. 2008 and May 2009. Receiver operator characteristic curves were utilized to select the most suitable test. Using different cutoff points, sensitivity and specificity were calculated. We compared preoperative serum PG levels with several clinicopathologic findings for patients with gastric adenocarcinoma. RESULTS: The Serum PG I:II ratio was the most useful as a screening test. The sensitivity and specificity of PG screening for gastric cancer were, respectively, 81.8% and 82%. The cut off point correlated with the type of intestinal cancer (Lauren classification; P=0.003), tumor stage (P=0.001), and gastric adenocarcinoma with peritumoral chronic atrophic gastritis (P=0.036). CONCLUSION: Serum PG levels were found to be a potentially useful screening test and to correlate with clinicopathologic factors in gastric cancer patients. But, in order to use serum PG found in a health checkup for gastric cancer as a clinical application a large scale study is recommended.

8.Pharmacokinetic Changes of 5-Fluorouracil Absorption After a Subtotal Gastrectomy.

5-fluorouracil(5-FU) and its derivatives have been prescribed preoperatively in Korea for oral administration to patients with gastric cancer as an adjuvant therapy. Since the conditions of the patients after gastrectomy may be different and thereby influence the absorption of 5-fluorouracil, we were interested in the pharmacokinetic changes of 5-fluorouracil absorption after gastric resection. Fourteen of 40 gastric cancer patients, who underwent radical subtotal gastrectomy with D2 lymphnode dissection for gastric cancer, were selected at Ewha Womans University Mok-Dong Hospital from May 1996 to December 1996. The plasma concentrations of 5-fluorouracil were determined before oral administration of 5-FU(500 mg/M2) at 15-min intervals for two hours, and then at 30-min intervals for two hours after oral administration during the pre- and the post-operative periods. High pressure liquid chromatography was used to determine the plasma concentrations of 5-FU. The pharmacokinetic indices included the area under the curve (AUC), the peak concentration (Cmax), and the time to peak concentration (Tmax). The mean postoperative AUC for 5-FU was 25.8+/-14.8% of the mean preoperative AUC, which was a significant decrease in the amount of 5-FU absorption postoperatively. The mean postoperative Cmax was 33.4+/-30.2% of the mean preoperative Cmax, which also meant a significant decrease of the peak concentration postoperatively. The Tmax of most patients were delayed postoperatively, but there was no significant change statistically. In conclusion, we found that the amount of absorbed 5-FU, as calculated by the AUC and the Cmax, significantly decreased after a subtotal gastrectomy by as much as 75 percent compared to the preoperation. Hence, it may be necessary to increase the dose of 5-FU after a subtotal gastrectomy by more than 75% of the preoperative dose. The subtotal gastrectomy did not have a significant effect on the absorptive velocity of 5-FU.

10.Neutron Therapy of Unresectable and Recurrent Rectal Cancer.

Total of 53 patients of unresectable and recurrent rectal cancer treated with neutron beam during the period from Oct. 1987 to Apr. 1992 were analyzed. Dose fractionation for the neutron only group was 1.5 Gy per fraction, 3 fraction per week, 21 Gy/41/2 wks for 42 patients out of 53(76%). Neutron only but modified fractionation schedule (10% more or less of total dose) was applied for 9 patients, and mixed team (neutron boost) was for 4 patients. Complete tumor response was obtained in 40 patients(76% response rate). Local control rate was 28 out of 53(53%). Statistically significant better prognostic factors for local control were age below 49 years old (15/22, 68%) than above 50 years old (13/31, 42%), male (20/32, 63%) than female(8/21, 38%), tumor size less than 5 cm and non-metastatic(16/24, 67%) than size more than 5 cm or metastatic(12/29, 4l%). Major complication had developed in 7 patients (13%). Two year overall survival rate by Kaplan-Meier method was 30%, but it was rised to, 47% when the tumor was less than 5cm non-metastatic.
