3.Rethinking the History of Smallpox in the Early Twentieth Century: The SS Korea and Uncertainty Surrounding the Diagnosis of Smallpox

This research explores the case of the 1903 smallpox outbreak on the SS Korea , a transpacific carrier making runs between Southeast Asia, East Asia, Hawaii, and the United States. These regions were connected to a degree that no one had ever imagined through the SS Korea . Honolulu, Hawaii, was one of the most important territories in US maritime history and served as a waypoint between Asia and San Francisco on the mainland. As increasing numbers of people traveled by sea, various microbes were communicated across the Pacific Ocean. International tourists traveling across the ocean to Hawaii and the United States were alerted to infectious diseases, smallpox being one of the most significant of such diseases.
The story of the SS Korea serves as an important lens through which to explore the early twentieth century transpacific world connected through Honolulu. Focusing on the spread of smallpox via international travelers, this research studies aspects of the public health system that were developed to contain smallpox infection on international ships and the application of smallpox vaccination as a method for infectious disease control. More importantly, in bringing attention to the uncertainty surrounding the diagnosis of smallpox, this research argues for the necessity of historians to build a more comprehensive medical historical context for disease control systems that includes the limits of medical science in making diagnoses of infectious diseases, the uncertainties arising from a lack of this component, and the implementation of health policies and preventative medical technologies.
4.Dr. James Smith’s Dream of Eradicating Smallpox and the National Vaccine Institution

This article re-examines from a new perspective the efforts of James Smith (1771–1841), a Maryland doctor, to eradicate smallpox in the United States. As one of the few successful cowpox inoculators at the turn of the nineteenth century, Smith recognized the necessity for a public vaccine institution that could ensure the safe production and continuous preservation and circulation of vaccine matter. Thus, he devoted himself to creating statewide and national vaccine institutions funded by the state and federal governments. He established the National Vaccine Institution (NVI), but despite his efforts, the NVI existed only a short time from 1813 to 1822. Previous studies on Smith have focused on the 1813 Vaccination Act (An Act to Encourage Vaccination) and the NVI, and have evaluated them as failed projects or historically missed opportunities. However, this kind of approach does not justly place the act and institutions within Smith’s larger plan and do not fully discuss the role of the NVI in his system of promoting vaccination in the United States. This article analyzes how he responded to the problems hindering cowpox vaccination, including spurious vaccine, failed vaccination, and low public acceptance of cowpox vaccine. In doing so, this study shows that Smith attempted to establish a universal and systematic vaccination system connecting citizens, government, and medical personnel through the NVI, as well as ensuring a safe and regular supply of vaccine.

6.Effects of irradiation with Cobalt 60 to the development of Ascaris eggs in stool.

This study was carried out in order to find out the valuable measure to kill the parasite eggs in night soil. The fresh eggs of Ascaris put into human stool were irradiated with Cobalt 60 of 200,000 to 1,000,000 rad and cultured in test tubes at 25-30 C, washing with 2 percent formalin solution every 24 hours. The continuous development of those eggs were observed under microscope and the proprotions of developed eggs were compared with those of control groups. The major result can be summarized as follows: In general, The eggs in stool developed poorly than the eggs in saline. If the eggs were irradiated with the larger dose of Cobalt 60, the proportion of developed eggs were reduced subsequently. The eggs irradiated with the dose of 1,000,000 rad in saline developed in the proportion of 15.5 percent, whereas irradiated with 200,000 rad 94 percent developed in 4 weeks. The 44.5 percent of eggs in stool irradiated with 200,000 rad developed after 4 weeks, 30.5 percent with 300,000 rad, 25 percent with 500,000 rad and 3.5 percent with 1,000,000 rad respectively. The effective minimum dose of Cobalt 60 irradiation to kill the Ascaris eggs in stool was estimated 1,000,000 rad. Further examination will be required to observe the infectivity of irradiated Ascaris eggs to animals and to evaluate the effect from the standpoint of sanitary engineerings.

8.A Case of Interstitial Cell Tumor of Testis In Adult.

Interstitial cell tumor of testis is relatively rare, comprising only 0.8 to 1.4 percent of all testicular tumors. A 67-year-old man was admitted to our hospital with rapid growing testicular mass. He had the experience of hydrocele and many times of aspiration since childhood. On physical examination, the testicular mass (left) was hard, adult fist sized and associated with transparent cystic fluid. Under the impression of testicular tumor, orchiectomy was performed. We report this case with review of literature.

9.Changes of serum alkaline phosphatase after enucleation of cysts in the jaws.

This study was to analyze the changes of levels of alkaline phosphatase before and after enucleation of jaw cysts combined with bone grafting, and to evaluate biochemically the effectiveness of the early detection of bone healing and infection as a prognostic marker. Eighteen patients (13 males, 5 females) with cystic lesions of the jaws were divided into two groups. The bone graft group underwent enucleation and bone graft. The control group underwent only enucleation. Both groups were measured levels of ALP before surgery, and plus-minus 4 weeks postoperatively. The more discriminating results were obtained in the bone graft group. The results were as follows : 1. Levels of ALP after enucleation of jaw cysts were decreased in all patients with and without bone graft. 2. The bone graft group showed more marked decrease in variation of levels of ALP than the control group.(p=0.008) This should be considered as a result of increased osteoblastic activity and new bone formation. 3. Such variation could be used as a prognostic marker for bone healing after cyst operation. In the cost/benefit ratio, measurement of ALP activity could be useful as a convenient procedure in routine clinical practice.

10.Histologic Changes of Normal Human Skin in Organ Culture.

Since adult human skin can be grown in chernically defined medium without serum, the skin organ culture has gained a great interest as a method for studies concerning skin biology, pharmacology and toxicology. however, serum supplementation has extensively been used to improve the viahility of tissue culture. This study was undertaken to evaluate the effect of serum on the histologic changes ohserved during the organ culture of the normal human skin. The general architecture of the skin was well maintained for 6 days with or without seru. After then, fetal calf serum or autologous human serum was found to enhance the viability of the epidermis. A confluent layer of necrotic spinous ceils was ovserved earlier and more widespread without serum. The addition of serum had an impressive effect on epibolization. In the absenee of serum, the formation of the epibolus was not only minimal, but also, susceptible to degeneration, and no epibolus remained at 10 days rif incubation. No difference can be found between fetal calf serm and autologous human serum in the formation of the epibolus. There was no favorable effect of serum on the formation of new stratum corneum. The thickness of new straturn corneum increased in parallel with the number of parakeratatic cells, increasing most rapidly between 6 and 8 days of incubation. Parakeratosis was more prominent in the presence of serurn.
