Verrucous carcinoma is an unusual, non-metastasizing, distinct variant of squamous cell carcinoma composed of four subtypes according to the site of occurrence: oral type, anogenital type, plantar type, and other cutaneous sites. Oral type verrucous carcinoma usually shows slow progression with a low incidence of metastases. Treatment of verrcous carcinoma is challenging; multiple medical and surgical therapies are often attempted, with limited success. We reported on 2 cases of verrucous carcinoma of the lip treated with topical imiquimod and debulking therapy.

2.A Case of a Cutaneous Metastasis from an Endocrine Pancreatic Carcinoma.

Pancreatic endocrine tumors (PET) are rare neoplasms of the pancreas and account for less than 5% of all primary pancreatic malignancies. Included in this group are insulinomas, gastrinomas, glucagonomas and somatostatinomas. Collectively, these neoplasms are classified as functional PETs. When a PET is not associated with a clinical syndrome due to hormone oversecretion, it is referred to as a non-functional PET. Non-functionalPETs are pancreatic tumors with endocrine differentiation but lack a clinical syndrome of hormone hypersecretion. Although a pancreatic carcinoma shows aggressive biological behavior, a cutaneous metastasis from a pancreas carcinoma is rare. We report a case of a case of a cutaneous metastasis from an endocrine pancreatic carcinoma in a 50-year-old female that clinically manifested as a painful firm nodule on the back.

3.A Case of Clear Cell Syringoma in an Adult Male.

Clear cell syringoma has two distinct features from typical syringomas: histological predominance of clear cells and a correlation with diabetes mellitus. Otherwise it is similar to ordinary syringomas in age, sex, site, and clinical manifestations. We here report a 49-year-old male with clear cell syringoma accompanied with diabetes mellitus. Histological examination revealed numerous, variable-sized, well-defined tumor nests composed largely of clear cells that stained positive with PAS (Periodic Acid Schiff) before diastase digestion and negative after diastase digestion.

4.Cutaneous Focal Mucinosis with an Uneven Surface.

Cutaneous focal mucinosis is a rare condition that manifests with asymptomatic small white to flesh-colored papules on the face, neck, trunk and extremities. Johnson and Helwig first described it in 1961 and it is histologically characterized by localized accumulation of mucin in the upper and mid-dermis with scattered spindle-shaped fibroblasts. We report here on a case of cutaneous focal mucinosis that presented as a nodule with several small papules on it on the face.

5.Significance of overnight D/P sodium test in continuous ambulatory peritoneal dialysis.

BACKGROUND: Standard PET (peritoneal equilibration test) is time consuming and it cannot reflect the water removal accurately because it measures solute transport by diffusion only. Thus, it is important to find parameters that can be used to predict both the diffusive permeability of peritoneal membrane and the peritoneal fluid removal. Sodium removal in CAPD is strongly related to the ultrafiltration and the recent study has suggested that dialysate sodium concentration at 240 minutes could reflect both peritoneal fluid removal and peritoneal transport characteristics. In this study, we investigated whether a single dialysate sodium test after overnight dwell could provide the same information as the standard PET and could reflect ultrafiltration failure. METHODS: A 9-hour overnight dwell study with 2L of 3.86% glucose dialysis solution was carried out in 48 clinically stable CAPD patients. All patients underwent a PET test with 2L of 2.27% glucose solution in the morning after an overnight dwell study. According to ultrafiltration (UF) failure (net UF<100 mL/4 hour on 2.27% glucose solution), patients were divided into two groups (group I, UF failure group; group II, non UF failure group). D/P(Na540) and D(Na540) at the end of the 9-hour dwell were compared between two groups RESULTS: D/P(Na540) was significantly correlated with D/Pcreatinine (r=0.551, p<0.001) and drained volume (r=-0.536, p<0.001) at 240 minutes on PET. There was also a significant correlation between D/P(Na540) and drained volume (r=-0.555, p<0.001) at 540 minutes after overnight dwell. D(Na540) was significantly correlated with D/Pcreatinine (r=0.448, p<0.01) at 240 minutes on PET and also weakly correlated with drain volume at 240 minutes (r=-0.37, p<0.01). There was a significant difference in D/P(Na540) between the group I and group II (p<0.05), but not in D(Na540). CONCLUSION: Overnight 9-hour dwell study with 3.86% glucose solution is a simple procedure and easy to perform on outpatient basis. D/P(Na540) is comparable with PET results and may discriminate the different transport groups. D/P(Na540) may also be used as an indicator of ultrafiltration failure.

6.A Case of Argyria Following Colloidal Silver Ingestion.

Argyria is a rare cutaneous discoloration caused by the intake of silver or various compounds containing silver. We report a case of argyria in a 73-year-old male following ingestion of colloidal silver as an alternative medicine over 5 years. He had a diffuse, slate gray discoloration of his face and hands. A biopsy specimen from the face revealed brown-black extracellular granules in the upper dermis and between collagen bundles. We also found silver particles in the mucous of the colon. The ingestion of colloidal silver appears to be increasing among patients using alternative health practices. We report this case to bring people's attention to the problems associated with the ingestion of colloidal silver.

7.A Case of a Compound Nevus That Developed as Papillomatous Melanocytic Nevus.

Papillomatous melanocytic nevus is a clinical type of melanocytic nevus, and this has also been described as keratotic melanocytic nevus or seborrheic wart-like melanocytic nevus. Although the pathogenesis of this phenomenon is not well recognized, some hypotheses do exist, such as it is an abnormality that occurs at some stages in embryonic development. There is also the possibility that dermo-epidermal interactions exist. On our review of the medical literature, there are only four Korean cases of intradermal nevus that have developed as papillomatous melanocytic nevi. We report here on a Korean case of a compound nevus with features of papillomatous melanocytic nevus.

8.A Case of Hidradenoma Papilliferum on the Cheek of an Adult Male.

Hidradenoma papilliferum is a benign, cystic, papillary tumor that occurs almost exclusively in women on the skin of the anogenital region and this is usually a slow-growing cystic dermal mass that can be either asymptomatic or it can ulcerate and bleed, and it represents an adenoma with apocrine differentiation. Nonanogenital (ectopic) hidradenoma papilliferum is rare. We present here a case of hidradenoma papilliferum in a 52-year old male with an erythematous to brownish nodule on the left cheek and the lesion had the typical histopathological findings of hidradenoma papilliferum.

9.Selective Thoracic Fusion with Segmental Pedicle Screw Fixation for the Treatment of Thoracic Idiopathic Scoliosis: More than Five Year's Follow-Up.

STUDY DESIGN: This is a retrospective study. OBJECTIVES: We wanted to evaluate the outcomes of selective thoracic fusion with segmental pedicle screw fixation for treating thoracic idiopathic scoliosis with a minimum 5-year follow-up. SUMMARY OF THE LITERATURE REVIEW: Segmental pedicle screw fixation has been proven to achieve true segmental control and greater correction of scoliosis in both the coronal and sagittal planes. However, there is no long-term study of selective thoracic fusion with segmental pedicle screw fixation for treating thoracic idiopathic scoliosis. MATERIALS AND METHODS: We analyzed 203 thoracic idiopathic scoliosis patients (236 thoracic curves) who underwent selective thoracic fusion with segmental pedicle screw fixation. The mean patient age at the time of operation was 13.8 years (range: 8.9~18 years). RESULTS: The preoperative thoracic curve of 51+/-12 degrees was corrected to 16+/-7 degrees(69% correction with 3% loss of correction) at the most recent follow-up. The non-instrumented lumbar curve of 30+/-10 degrees was corrected to 10+/-8 degrees(66% correction with 5% loss of correction) at the most recent follow-up. The preoperative thoracic kyphosis of 18+/-11 degrees and the lumbar lordosis of 43+/-10 degrees were improved to 23+/-8 degrees and 46+/-9 degrees, respectively, at the most recent follow-up. There was no junctional kyphosis at the most recent follow-up. Coronal decompensation at the most recent follow-up occurred in 10 patients. Postoperative adding-on occurred in 17 patients who were fused two levels short of the neutral vertebra. Of the 2867 thoracic pedicle screws inserted at the thoracic level, 43 screws were found to be malpositioned (1.5%), but they did not cause neurologic complications or adversely affect the long-term results. CONCLUSIONS: Selective thoracic fusion with segmental pedicle screw fixation for treating thoracic idiopathic scoliosis had satisfactory radiographic and clinical outcomes after surgery, and the outcomes were well-maintained for a minimum of 5 years follow-up. It is a safe and effective method for preserving segments of lumbar motion as well as for the restoration and maintenance of both the coronal and sagittal alignments.

10.Pityriasis Rotunda in Healthy Adults.

Pityriasis rotunda is a rare disease characterized by round to oval, sharply defined, hypopigmented or hyperpigmented, scaly patches of variable number and size located trunk and extremity. The etiology is unknown but it has been associated with a variety of systemic illnesses, such as tuberculosis, malnutrition and malignancy. The first case was a 28-year-old male who presented with a 2-year history of multiple round shaped hyperpigmented patches on the trunk. The second-case was a 38-year-old female who presented with a 2-month history of circular, sharply defined multiple patches with ichthyosiform scaling on the trunk. Both patients had no systemic disease or familial history. We experienced 2 cases of pityriasis rotunda on the trunk without underlying systemic illnesses or familial association.
