1.Effect of Benzoic Acid Containing Foods on the Urinary Hippuric Acid Concentration in Workers Exposed to Toluene.

This Study was performed to investigate the effect of foods containing benzoic acid on the urinary hippuric acid concentration in the toluene exposed workers. The urinary hippuric acid concentration were measured at 0, 30, 60, 90, 120, and 150 minute after intake of soft drink and bread containing benzoic .acid: in volunteers,: who did not have been exposed toluene occupationally. After that urinary hippuric: acids also were measured at the end of shift in toluene exposed workers. And also the concentration of toluene in air were analysed! by gas chromatography. So the relationships between urinary hippuric acid concentration and age, work duration, concentration of toluene in air and intake of benzoic acid containing food were observed. The results were as follows: 1. The urinary hippuric acid concentration reached the highest level at 30 minute after intake of soft drink with bread at a level of 1.41g/g creatinine and soft drink; alone at a level of 1.04g/g creatinine which contained benzoic acid among unexposed toluene, and then gradually decreased. 2. In toluene exposed group no significant effect of general; characteristics siich; as gender, age, work duration, drinking and smoking on urinary: hippuric acid concentration was showed in toluene exposed group. 3. The mean value of urinary hippuric acid concentration of group taking benzoic acid containing foods was significantly higher than that of group not taking. 4. Multiple regression analysis showed that toluene concentration in .air and benzoic acid containing foods were together significantly influence on urinary ; hippuric acid concentration. And the function between variables is Y = 0.014Tpluene +0.489Benzoic acid +0.745 and the contribution rates were 31%. In conclusion, it was suggested that the information on intake of benzoic acid containing foods should be considered in measurement of urinary hippuric acid for the more accurate toluene exposure evaluation.

3.A Case of a del(8p)/dup(8q) Recombinant Chromosome.

A male baby with intrauterine growth retardation had a short neck, small hands and feet, hypospadia, both grade I hydronephrosis, type II atrial septal defect, and moderate valvular pulmonary stenosis. The routine chromosome and banding analyses revealed a 46,XY,rec(8)del(8)(p21)dup(8) (q24.1)inv(8)(p21q24.1)pat chromosome constitution. His mother has normal chromosomes, but the father had 46,XY,inv(8)(p21q24.1). Also his uncle had an inv(8) chromosome constitution. We used lymphocytes and examined 40 mitotic cells. All mitotic cells showed deletion of 8p21-->pter and duplication of 8q24.1-->qter. Because 8p21 involves secretion of macrophage and lymphocyte against cancer cells, long-term follow-up for cancer will be needed.

BACKGROUND: In recent years, the incidence of extended-spectrum beta-lactamase (ESBL) producing Klebsiella has been steadily increased, and the newer species K. planticola and K terrigena, formerly regarded as nonpathogen, have been reported with astonishing frequency from human infectious processes by some investigators. The aim of this study is to elucidate the isolation rate and antimicrobial susceptibility of recent clinical Klebsiella isolates. METHOD: For the clinical Klebsiella isolates during the period of June 1999 to May 2000, isolation frequency of Klebsiella species by specimen, departments, age, and sex were analyzed. And antimicrobial susceptibilities were also analyzed. RESULT: Isolation rate of Klebsiella in order of decreasing frequency were K. pneumoniae (74:7%), K. oxytoca (12.1%), K. ozaenae(1.7%), K. planticola(1.0%), K. terngena(0.9%), and K, ornithinolytica (0.7%), respectively. K. rhinoscleromatis was not isolated. Compared with outpatients, increase of resistance rates of inpatients's Klebsiella isolates were 10% in ciprofloxacin, 15% in cefoperazone/sulbactam, and the others were ranged from 24% to 31%. Isolation rate of ESBL producing K. pneumoniae by double disk (DD) synergy test was 41%, and detection rates by antimicrobial agents were as follows: cefotaxime (95%), aztreonam (58%), and ceftriaxone (37%). Antimicrobial susceptibility rate with the exception of ampicillin and imipenem decreased from the range of 81%-96% on admission day to 29-62% after one week on admission. CONCLUSION: The isolation rates of K. planticola and K. terrigena were less than 1%. The proportion of ESBL producing K. pneumoniae was 41 %. And the vast majority of multidrug resistant Klebsiella including ESBL producing strains are acquired by hospitalization.

6.The Effect of Alfacalcidol in the Treatment of Idiopathic Myelofibrosis in Children.

Idiopathic myelofibrosis (IMF), which is characterized by marrow fibrosis, leukoerythroblastic anemia, teardrop poikilocytosis and splenomegaly due to extrumedullary hematopoiesis, has known to have no form of therapy. On the ground of the possibility of reversing collagen deposion in IMF using 1, 25dihydroxycholecalciferol [1, 25(OH)2D3], we report here our observations of 5 patients (M:F=1:4) with IMF before and after treatment with 0.5 microgram/day of alfacalcidol, precursor of 1, 25(OH)2D3. In 3 fo 5 patients the hemoglobin rose and in 4 of 5 the platelet count increased. Follow-up marrow examination revealed that marrow trephine reticulin fibrosis decreased according as the amelioration of clinical and laboratory findings. But these did not persist except one patient in spite of the sustained use of alfacalcidol. Our results suggest that alfacalcidol may have a therapeutic role in some patients with IMF. More extensive studies will be clarify the action of alfacalcidol in IMF.

7.Clinical Outcome of Implants Placed in Grafted Maxillary Sinus Using Recombinant Human Bone Morphogenetic Protein-2: A 5-year Follow-Up Study

Purpose:
To investigate the 5-year outcome of dental implants placed in a grafted maxillary sinus using recombinant human bone morphogenetic protein-2 (rhBMP-2).
Materials and Methods:
We retrospectively analyzed 27 implants after maxillary sinus floor augmentation (MSFA) using rhBMP-2 in 16 patients between January 2016 and March 2017. The study evaluated two outcome variables: (1) 5-year cumulative survival and success rate of the implant after functional loading and (2) marginal bone loss (MBL) for implant failure.
Results:
The average residual bone height was 4.78±1.53 mm. The healing period before loading was 8.35±2.34 months. The crown-to-implant ratio was 1.31±0.26. The 5-year cumulative survival and success rate after functional loading were 100% and 96.3%, respectively. The 5-year average MLB was 0.89±0.82 mm.
Conclusion
Placing dental implants with MSFA using rhBMP-2 is a reliable procedure with favorable long-term survival and success rates.
8.Screening Mammography: The Results for Four Years.

PURPOSE: To perform a medical audit of screening mammography for breast cancer and to determine the benefit of this procedure for the early detection of this condition. MATERIALS AND METHODS:We reviewed the results of 43,329 instances of mammography in 36,802 women [18-86 (mean, 46) years old] who underwent the procedure at our health promotion center between January 1995 and December 1998. After reviewing the mammographic reports, we selected ACR BI-RADS assessment cate-gories 0, 4, 5, and then reviewed the follow-up studies and the pathologic results thus obtained. By comparison with the total number of patients diagnosed with breast cancer during the same period, false negative cases were confirmed, and from these data a medical audit was performed. RESULTS: The percentage of women undergoing mammographic examination has increased in recent years (from 64.5% in 1995 to 97.4% in 1998), and cases of breast cancer among those aged 30 to 69 has tended to increase. A total of 1,879 cases were assessed as BI-RADS categories 0, 4, 5, and of these, 155 were recommended for biopsy or surgery. A confirmatory pathologic diagnosis was obtained in 106 cases, and in 43 of these, breast cancer was pathologically proven. The recall rate was 5.1%, and the cancer detection rate was 1.2/1000 population. Positive predictive value 1(PPV1) was found in 2.3% of cases, PPV2 in 27.7%, and PPV3 in 40.6%. The most common mammographic finding was microcalcification (40.5%), and a pathological finding of invasive ductal carcinoma was found in 66.7% of cases. Patients diagnosed with breast cancer totalled 47 (four cases were false negative); the estimated sensitivity was 91.5%, with a specificity of 95.0%. The percentage of minimal cancers found was 48.8, while that of axillary node-positive invasive cancers was 33.3. The total rate of axillary nodal metastasis was 22.0%, and the rate of stages 0 and I was 73.2%. CONCLUSION: When appropriate interpretation and follow-up monitoring of screening mammography is per-formed, the procedure is effective for the early detection of breast cancer.

9.A Decline in Renal Function is Associated With Loss of Bone Mass in Korean Postmenopausal Women With Mild Renal Dysfunction.

This study was conducted to assess the relationship between estimated glomerular filtration rate (eGFR) and bone mineral density (BMD) in Korean postmenopausal women with mild renal dysfunction. A total of 328 postmenopausal women who underwent BMD measurement during health check-up was investigated. BMD was measured in lumbar spine (L1-L4), femoral neck, total proximal femur and femoral trochanteric areas by dual energy radiography absorptiometry and renal function was estimated by eGFR using Cockcroft-Gault equation. Of the 328 subjects, 317 (96.6%) had an eGFR > or =60 mL/min/1.73 m2. By using simple linear regression analysis, age, height, weight and eGFR were significantly associated with BMD for the 4 aforementioned anatomic sites, while serum levels of creatinine and blood urea nitrogen did not influence BMD. When multiple regression analyses were applied, age and body weight still had significant associations with BMD at 4 different anatomic sites (P < 0.001). A significant association of eGFR with BMD remained in the lumbar spine, femoral neck and proximal total femur (P < 0.05) but not in the trochanteric area (P = 0.300). Our study suggests that a decline of renal function is associated with lower BMD in the lumbar spine, femoral neck and total proximal femur areas in Korean menopausal women with mild renal dysfunction.

10.Relationships Between Self-awareness and Clinical Diagnostic Findings of Abnormal Foot Arch Height in Koreans.

OBJECTIVE: To see how people think about their own feet, and evaluate whether there are correlations among self-awareness of the participants and clinical examination findings. METHODS: Adult twins and their families who participated in the Healthy Twin study from May 2008 to April 2010 were recruited. Participants were asked whether they thought their feet were normal, flat, or cavus. The lateral talometatarsal angles were measured on foot X-rays to determine the foot arch height. Using the podoscopic footprints taken with the podobaroscope, the Staheli arch index was also measured. Kappa statistics were used to calculate degree of agreement among the three measurement methods. RESULTS: Self-awareness and radiographic findings were significantly different (Pearson chi-square test, p=0.000) and only slightly agreed (kappa measure of agreement=0.136, p=0.000). Self-awareness and podoscopy results revealed a significant difference (Pearson chi-square test, p=0.000), with only slight agreement (kappa measure of agreement=0.072, p=0.000). CONCLUSION: There is significant disagreement between patients' perception of their feet and actual test results. Many people may have an incorrect assumption about their own foot conditions that may be reflected in improper management. Dissemination of accurate information about foot disorders by foot clinicians would be helpful.
