4.A Case of Retroperitoneal Teratoma in an Adult.

The teratoma is made up of a variety of parenchymal cell types representative of more than one germ layer. Retroperitoneal teratoma is an uncommon neoplasm which occurs in only 10% of all retroperitoneal primary neoplasm. Primary retroperitoneal teratoma has been rarely seen in adults and 3 cases has been reported in Korea. Recently we experienced a case of retroperitoneal teratoma, which was suspected by clinical investigation and confirmed by exploration and microscopic examination. CT scan is very useful in differential diagnosis of this rare tumor. We report a case of retroperitoneal teratoma in 22 year old young woman.

5.Reactive Oxygen Species Generated by 17beta-estradiol Play a Role in the Up-regulation of GPX4 Protein in MCF-7 Breast Cancer Cells.

PURPOSE: Estrogen is known to act as both a growth factor and a survival factor for breast cancer. The responsible molecular mechanisms remain, however, to be fully elucidated. We hypothesize that the effect of estrogen relates to its ability to induce the cellular antioxidant defense enzymes. METHODS: In the presence study, we examined the ability of 17beta-estradiol (E2) to regulate the level of phospholipid hydroperoxide glutathione peroxidase (GPX4) protein, which is an anti-oxidative enzyme that can directly reduce both phospholipids and cholesterol-hydroperoxides located in the cell membranes and lipoproteins. RESULTS: E2 elicited a dose- and time-dependent increase in the GPX4 expression in the MCF-7 breast cancer cells, and this up-regulation was blocked by the free radical scavenger N-acetylcysteine (NAC). Additionally, we confirmed that E2 triggered a rapid and transient increase in the intracellular reactive oxygen species (ROS) levels, and this E2-induced increase in the ROS levels was inhibited by pretreatment with NAC. Moreover, such ROS inducers as TGF-beta, TNF-alpha and insulin induced an increase in the level of GPX4 protein. However, estrogen receptor (ER)alpha knockdown by transfection with ERalpha-siRNA did not significantly change the GPX4 protein level that was induced by E2. Furthermore, pre-incubation with the ER antagonist ICI 182,780 did not inhibit E2-mediated GPX4 induction. Conversely, pretreatment of cells with LY294002, a pharmacological inhibitor of phosphatidylinositol 3-kinase inhibitor, suppressed the E2-augmented GPX4 expression. CONCLUSION: Collectively, our data show that E2 may partly provide a survival advantage through the regulation of cellular oxidative homeostasis in MCF-7 breast cancer cells.

6.The Physical Development and Dietary Intake for Korean Children and Adolescents Body Composition and Obesity Prevalence.

The aim of this study was to evaluate the body composition difference and obesity prevalence in grade 5 to 8 by gender. A total of 1,333 students, 707 boys and 631 girls, were recruited from one elementary and one middle school in Seoul, Korea. Anthropometric measurements including height, weight, lean body mass, and body fat were assessed. Height and weight were increased by grade, but Body Mass Index (BMI) by grade showed no significant difference among boys. Lean body mass had growth spurt between 7 and 8 grade among boys compared to gradual increase among girls. Overweight and obesity prevalence were higher among elementary schoolers than middle schoolers. Overweight (BMI for age >85 th percentile) prevalence was about 44% among 5 th grade boys compared 29% among same grade girls. These results provided that there were prominent difference regarding body growth and obesity prevalence in gender and grade. Further studies for children and adolescents should be considered their body composition change.

7.The Physical Development and Dietary Intake for Korean Children and Adolescents: Food and Nutrient Intake.

The aim of this study was to evaluate the food and nutrient intakes in 5th to 8th grade school boys and girls. A total of 1,333 students were recruited from 1 elementary and 1 middle school, both located in Seoul. The data on food and nutrients intake were collected by 3-day food record. The food intake was calculated as the percentage of total energy intake according to 21 food subgroups. The consumption of noodles, and meat's products (including ham and sausage) groups were higher in 7th and 8th grade than 5th and 6th grade. On the other hand consumption of vegetables and milk and its products groups were lower in 7th and 8th grade. As results, calcium, riboflavin, and vitamin C intakes were lower in 7th and 8th grade students. Dietary patterns by cluster analysis resulted in 77% of subjects with traditional pattern based rice and kimchi, and 23% with modified pattern. All students regardless of grades showed high fat intake, above 20 percent energy intake from fat, and high cholesterol intakes. In conclusion, there was significant difference between 5th and 6th grade primary school students and 7th and 8th junior school students regarding food and nutrient intakes. It should be necessary to provide proper nutrient education and monitoring in order to establish good dietary profile.

8.Isolated Microorganisms and Antimicrobial Resistance of the Deep Neck Infection: A Retrospective Review of 76 Cases.

BACKGROUND AND OBJECTIVES: The incidence of deep neck infection has decreased after the introduction of antibiotics and improvement of oral hygiene, but they still may be lethal especially when life-threatening complications occur. The purposes of this study are to find out common organisms, rates of antimicrobial resistance in the deep neck infection. SUBJECTS AND METHOD: We have conducted a retrospective study of 76 patients with the diagnosis of deep neck infection that had been seen between January 1998 and December 2003. RESULTS: The most common pathogens found were Streptococcus viridans (34%), followed by Klebsiella pneumoniae (19.4%), Streptococcus Group F (11.9%), beta-hemolytic streptococci (11.9%) and Peptostreptococcus (7.5%). Staphylococcus species was not found. Of the aerobes found, 74.1% were sensitive to all antibiotics. All Streptococci, with one exception, were not resistant to any antibiotics. All Klebsiella pneumoniae were resistant to ampicillin. Klebsiella pneumoniae were cultured in 50% of DM patients. CONCLUSION: The most common pathogens were Streptococcus viridans (34%). Of these aerobes, 74.1% were sensitive to all antibiotics. All isolated microorganisms were sensitive to ampicillin/sulbactam (Unasyn(R)) used before the results of bacterial cultures.

9.The Factors Influencing Postoperative Aspiration after Supraglottic Laryngectomy.

BACKGROUND AND OBJECTIVES: Supraglottic laryngectomy is a well established technique for the treatment of appropriate carcinoma of supraglottic larynx. The purpose of this study was to determine factors that may contribute to postoperative aspiration related to deglutition problem following supraglottic laryngectomy. MATERIALS AND METHOD: A retrospective analysis of medical records of 19 patients who consecutively underwent supraglottic laryngectomy was undertaken. Contribution of the following factors was investigated: age, T stage, pack-years of smoking, and parameters of pulmonary function test. The authors used a computerized acoustic analysis program (CSL-MDVP) to measure fundamental frequency, jitter, shimmer, and noise to harmonics ratio. RESULTS: Eleven patients had no problem, 5 patients had clinically insignificant problem, and 3 patients had moderate problem in postoperative 6 months. A greater number of pack-years of smoking and decreasing FEV1/FVC were significantly correlated with poor outcome with regards to postoperative aspiration and deglutition problem. No statistically significant difference was seen between patients with supraglottic laryngectomy and the control group. CONCLUSION: This study showed that less than 60% FEV1/FVC signifies a greater risk for postoperative aspiration.
