4.An experimental study about efficacy of drain catheters.

Although percutaneous abscess drainage has become and accepted alternative from of therapy for selected patients with abscess, it is well known that there are several factors in the failure of adequate drainage such as pre-and post- procedural management, technique itself, various features of abscess, and selection and application of catheters. Among these factors, we made an experiment about drain efficacy of commonly used various catheters with different viscosities of water-glycerin solution under the two different pressure gradients. The experimental values of flow rate were lower than than the calculated values. An efficacy of experimental value was 4-14%. Because the inner diameter of fittings and stopcocks was usually smaller than the inner diameter of catheters, these factors also affected the drain efficacy. Finally, we though that it will be very helpful to the treatment of patients as well as to study about the catheter drainage, if the drain efficacy of individual catheters has been notified.

6.Clinical Assessment on Juvenile Diabetes Mellitus.

Eleven cases of juvenile diabetes mellitus who were admitted Seoul National University Hospital from Jan., 1969 to Aug., 1978 were aha1yzed. The mean age of onset was 7 and female to male ratio was 8 to 3 with female preponderance. Four had family history of diabetes, although none had diabetic sibling. Polyuria and polydipsia were noted in all cases as initial symptoms, and symptoms of weight loss (6 cases), disturbances in consciousness (5), generalized weakness (5), anorexia (4) and enuresis (3) were noted in the order of frequency. Leucocytosis was noted in 5 cases, but only 2 among the 4 who had infection revealed leucocytosis, The mean blood glucose level was 320 mg% and serum osmolality was 293 mOsm/L. There was no significant difference in the level of blood sugar and serum osmolality according to the consciousness status. Eight episodes of ketoacidosis occured in 5 cases. Three episodes developed due to discontinuance of insulin and three episodes accompanied infection. The mean blood glucose level was 495 mg% and serum osmolality was 300 mOsm/L in ketoacidosis. The consciousness cleared in average 20 hours and ketonuria disappeared in average 26 hours after the administration of insulin (average 5 units/kg of body weight). At discharge, the average dose of insulin needed was 1.2 U/kg/d. Six patients were followed from 6 months to 6 years. Two developed pulmomary tuberculosis during the period of follow up.

7.A Case of Transient Complete A-V Block.

We experienced a case of transient complete A-V block, which developed probably due to acute myocarditis of viral etiology. The patient was 13 year old boy, and was admitted to our ward with chief complaints of intermittent abdominal pain and chest discomfort. The auscultation of the heart revealed, the 1st heart sounds varied in intensity, and the E.K.G. findings, checked then, showed complete A-V block with atrial rate of 110/min and ventricular rate of 73/min. We recommended absolute bed rest and oral prednisolove(50mg/day). He discharged on 10th hospital day in good condition with normalized E.K.G. finding.

8.Automated bone marrow processing with the blood cell separator.

BACKGROUDNS: Concentration of bone marrow aspirates is an important prerequisite prior to infusion of ABO incompatible allogeneic marrow and prior to cryopreservation and storage of autologous marrow. We describe our experience in processing 39 bone marrow aspirates harvested for autologous and ABO incompatible allogeneic bone marrow transplantation (BMT) using the blood cell separator, and analyzed removal rates of the red blood cells and recovery rates of the mononuclear cells. METHODS: Between September 1992 and July 1998, 39 bone marrow aspirates were processed using CS3000 Plus (38/39) and COBE Spectra (1/39) cell separators. Among 39 bone marrow harvests, 25 cases were for autologous BMT and 14 cases were for allogeneic BMT with ABO incompatibility. Initial bone marrow and final bone marrow concentration product were analyzed for volume, hematocrit, WBC and mononuclear cell count by manual differential count. RESULTS: Within 60-120 min (mean 81 min), the initial marrow volume 1212 mL (+/-205 mL) was processed. A mean of 31.9% (+/-13.4%) of the initial nucleated cells and 70.1% (+/-27.0%) of initial mononuclear cells was recovered in a mean volume of 214 mL (+/-17 mL). This procedure effectively removed 97% of the red cells and 81% of the granulocytes. CONCLUSIONS: We concluded that automated bone marrow processing with the blood cell separator was an effective method for collecting mononuclear cells while removing red blood cells, granulocytes, and plasma.

9.Establishment of an HLA-matched Platelet Donor Registry for the Management of Patients with Platelet Refractoriness.

BACKGROUND: Heavily transfused patients commonly become refractory to platelet transfusion. Patients with platelet refractoriness due to HLA alloimmunization need HLA-matched platelet transfusion. We have established an hospital-based donor registry of HLA-typed platelet donors for the first time in Korea and evaluated the possibility of HLA-matched platelet supplies. METHODS: A total of 450 donors were registered and typed for HLA class I (A, B, C) antigens. A computer program was developed and used for the donor registry and HLA-matched donor search. A simulation study was performed on the availability of HLA-matched platelets for 100 patients from a pool of 450 donors. The availability of HLA-matched and cross reactive epitope group (CREG)-matched donors were analysed for HLA-A, B antigens. The CREGs defined by Fuller, Rodey, and UCLA criteria were used. RESLUTS: Among 100 patients, 63% had HLA-identical or HLA-matched (match grade: A, B1U, B2U) donors with only a low number of donors (mean 1.4) available per patient. Including CREG-matches (match grade: B1X, B2UX, B2X), majority (98%) of the patients had HLA- or CREG-matched (match grade: A~B2X) platelet donors with a higher number of donors (mean 26.6~40.5 by different CREG criteria) available per patient. Majority (86-98%) of the patients had 20 or more A~B4X matched donors, and about two thirds (61-74%) of the patients had 20 or more A~B2X matched donors. However, the number of ABO-identical matched donors was less than 30% of the total HLA- or CREG-matched donors. CONCLUSION: We established an HLA-matched platelet donor registry and using a donor pool size of < 500 donors, attainable in an hospital-based donor registry, the possibility of HLA-matched platelet supplies was confirmed in this study. However, for more satisfactory and ABO-matched platelet supplies a larger pool size is needed.

10.A Case of Papulopustular Lesions in Behcet's Disease.

Cutaneous lesions of Behcet's disease are varied and include papulopustular lesions, erythema nodosum-like lesions, superficial thrombophlebitis, erythema multiforme-like lesions, Sweet's syndrome-like lesions, furuncles, pyoderma gangrenosum and subungal infarctions. Papulopustular lesions, the most common presentation of skin lesions in Behcet's disese, are cutaneous, sterile folliculitis or acne-like lesions on an erythematous base. Disagreement exists as to the exact nature of these acneiform and folliculitis-like lesions and whether they should be included as a major criteria. We report a case of papulopustular lesions in Behcet's disease which developed in a 41-year-old male.
