3.Closed Locked Intramedullary Nailing for Humeral Shaft Fractures

Intramedullary nailing has become the most popular fixation method in the treatment of the shaft fractures of long bones especially of tibia and femur because it affords most rigid fixation of fracture via closed method which makes early rehabilitation possible. Nowadays intramedullary nailing of the humeral shaft is being performed by many surgeons with good results. Newer implants and better surgical techniques are being developed, which suggests application of this method can be widened. Purpose of this study was aimed to define whether intramedullary nailing could be an effective method or not in the treatment of humeral shaft fractures by documenting the practical points in the application of intramedullary nailing of humerus, effectiveness in fracture healing, any complications or obstacles. We managed thirteen humeral shaft fractures with closed locked intramedullary nailing with distal fanning devices(Seidel nail) from March 1993 to April 1994. Average follow-up period was 14 months(12 months-18 months). The results were as follows; 1, Union of the fracture was obtained at average 12.9 weeks(9 weeks-20 weeks) postoperatively except one case of nonunion, in which case, union was obtained at postoperative 10 months finally. 2. Among the 13 cases, painful limitation of shoulder motion remained in 6 cases. This complication was more common in the cases with protruded proximal end of the nail. Above results suggest that locked intramedullary nailing seemed to be one of the useful method in the treatment of the humeral shaft fractures. However, destruction and irritation of shoulder by the nail was found to be a grave problem, and we found that further investigations should solve this problem.

4.Analysis of the Contributions of Original Articles in Major Dermatology Journals in Korea.

BACKGROUND: The Korean Journal of Dermatology and the Annals of Dermatology are representative journals of dermatology in Korea. Evaluation of their content, authors and affiliations are valuable for our understanding of the situation, trends and interests of the Korean dermatologic society. OBJECTIVE: To analyze the original contributions in the major dermatology journals of Korea, and compare these with those of renowned international dermatology journals in terms of original contribution. METHODS: A total of 318 original articles from the last 3 volumes of the Korean Journal of Dermatology (2003, vol. 41 to 2005, vol. 43) were collected and analyzed for their characteristic features, such as categories, authors, and affiliations. We also compared these characteristic features with those of 18 original articles from the last 3 volumes of Annals of Dermatology (2003, vol. 15 to 2005, vol. 17), an English dermatologic journal in Korea. RESULTS: Thirty-two percent of original articles fell into the category of drug evaluation and clinical research, followed by laboratory and experimental research. Most articles had three to six co-authors (78%), and one (55.3%) or two (31.1%) affiliations. The number of articles by a specific corresponding author ranged from one to eight, and the majority of articles by a specific corresponding author were one (43.6%) during 3 years. These situations were similar with those of the Annals of Dermatology. The evaluation of the Korean Journal of Dermatology about charateristics of original articles showed a small number of original articles by a corresponding author and cooperative researches with other afilliations. The main subjects of articles were centralized to 3 major categories in the Korean Journal of Dermatology, whereas the other famous international journals showed a relatively uniform distribution. CONCLUSION: The participation of non-dermatologists was fewer in the Korean journals than the other famous international journals. The Korean dermatologic society should make an effort to involve the specialists of other fields in submitting excellent original articles to the Korean Journal of Dermatology. And for the academic position of the Korean dermatologic society, we should make an effort to register one of our journals to the SCI (science citation index). The Korean Journal of Dermatology should deal with more varied articles from new categories for a more uniform distribution of themes. It should also make more effort to improve the Korean Journal of Dermatology.

7.Risk Factors and Features of Critically Ill Patients with Deep Vein Thrombosis in Lower Extremities.

PURPOSE: The purpose of this study was to identify the features, risk scores and risk factors for deep vein thrombosis in critically ill patients who developed deep vein thrombosis in their lower extremities. METHODS: The participants in this prospective descriptive study were 175 adult patients who did not receive any prophylactic medication or mechanical therapy during their admission in the intensive care unit. RESULTS: The mean age was 62.24 (+/-17.28) years. Men made up 54.9% of the participating patients. There were significant differences in age, body mass index, and leg swelling between patients who developed deep vein thrombosis and those who did not have deep vein thrombosis. The mean risk score was 6.71(+/-2.94) and they had on average 4.01(+/-1.35) risk factors. In the multiple logistic regression, body mass index (odds ratio=1.14) and leg swelling (odds ratio=6.05) were significant predictors of deep vein thrombosis. CONCLUSION: Most critically ill patients are in the potentially high risk group for deep vein thrombosis. However, patients who are elderly, obese or have leg edema should be closely assessed and more than one type of active prophylactic intervention should be provided.

8.Control of Mitochondrial Dynamics by Fas-induced Caspase-8 Activation in Hippocampal Neurons.

Cells undergo apoptosis mainly via two pathways-the mitochondrial pathway and the cytosolic pathway. It has been well documented that activation of the mitochondrial pathway promotes mitochondrial fragmentation and inhibition of mitochondrial fragmentation partly represses cell death. However, the mitochondrial events following activation of the cytosolic pathway are less understood. In this study, we treated Fas-activating antibody and found mitochondrial fragmentation without cell death in hippocampal primary neurons and HT-22 cell lines. Fas antibody treatment, in fact, promoted rapid activation of caspase-8, while executioner caspase-3 activation was not observed. Furthermore, blockage of caspase-8 efficiently prevented Fas antibody-induced mitochondrial fragmentation. These results suggest that the cytosolic pathway induced by death receptor activation promotes caspase-8-dependent mitochondrial fission.

9.Post-transplantation Lymphoproliferative Disorder in Transplanted Liver: A Case Report.

Post-transplantation lymphoproliferative disorder occurs in transplant recipients in whom immunosuppressivetherapy has compromised the immune system. In patients who have undergone transplants, suppressor T lymphocytefunction is suppressed, leading to unbridled B lymphocyte proliferation. We encountered a case withpost-transplantation lymphoproliferative disorder which manifested as a focal hepatic mass in transplanted liver.This mass was isoechoic, with a peripheral hypoechoic halo on ultrasonogram, while on all three phases of helicalCT scan low attenuation without contrast enhancement was seen.

10.The Change of Cognitive Function after Administration of Tolterodine in Brain Disease Patients with Overactive Bladder.

PUROPOSE: It is known that anticholinergics induces cognitive dysfunction and may aggravate the state of it. Tolterodine tartrate (detrusitol(R)) is a widely known selective anticholinergics to bladder, which does not cause a cognitive dysfunction. This study was designed to analyze the change of cognitive function of brain disease patients, whom are taking anticholinesterase inhibitor with tolterodine for overactive bladder (OAB). MATERIAL AND METHODS: From January 2001 to December 2004, with the patients whom have been followed for the brain disease in the department of neurology, we have analyzed 79 patients with tolterodine administered for OAB. We used K-MMSE (Korea minimental status examination) and SNSB (seoul neuropsychological screening battery) to analyze the state of cognition. Mean age of patients was 67.3+/-4.5 (yrs), mean administration period was 4.7+/-9.5 (mon). RESULTS: 7 patients made complaints for the decline of memory, 2 of them with Parkinsonism and 2 with cerebral infarction, 1 with progressive supranuclear palsy and, 2 dementia with lewy body (DLB). Patients with DLB was excluded in this study because the disease had fluctuation of cognitive function. CONCLUSIONS: The result of these studies demonstrates tolterodine tartrate caused the decline of cognitive function in only a few patients with brain disease. We concluded that prospective studies are needed to change the cognitive functions of the brain disease patients with OAB after administration of tolterodine tartrate.
