Anti-E and anti-c is one of the clinical significant irregular antibodies developing a delayed hemolytic transfusion reaction and hemolytic disease of the newborn. Since anti-c occurs frequently with anti-E in immunized people whosoe cells are E-and c-, it has been recommended to select blood of the patient's own R1 phenotype for transfusion, even when the presence of anti-c cannot be demonstrated in his/her serum. To determine the utility of this approach, we reviewed the blood bank laboratory records of patients identified anti-E and anti-c in his/her serum in Severance hospital over a 12 year period (1985-1996). During the 12-year period of study, 53 patients were identified with anti-E and/or anti-c; 30(56.6%) patients had anti-E alone, 6(11.3%) had anti-c, and 17(32.1%) had both. Thirty eight of forty two patients whose Rh-hr phenotypes were tested were R1R1. Of these 38 R1R1 patients, 16 patients (42.1%) presented with anti-c concomitant with anti-E. Ouru study showed that the incidence of antni-c in R1R1 patients with anti-E is similar to that of studies reported in Caucasian groups. We highly suggest the transfusion protocol of prophylactic use of c negative blood for R1R1 patients with anti-E. Furthermore, because anti-c may be present in concentrations too low to be detected, the enzyme technique is recommended in parallel with standard serologic methods for the identification of irregular antibodies.

2.Analysis of the Contributions of Original Articles in Major Dermatology Journals in Korea.

BACKGROUND: The Korean Journal of Dermatology and the Annals of Dermatology are representative journals of dermatology in Korea. Evaluation of their content, authors and affiliations are valuable for our understanding of the situation, trends and interests of the Korean dermatologic society. OBJECTIVE: To analyze the original contributions in the major dermatology journals of Korea, and compare these with those of renowned international dermatology journals in terms of original contribution. METHODS: A total of 318 original articles from the last 3 volumes of the Korean Journal of Dermatology (2003, vol. 41 to 2005, vol. 43) were collected and analyzed for their characteristic features, such as categories, authors, and affiliations. We also compared these characteristic features with those of 18 original articles from the last 3 volumes of Annals of Dermatology (2003, vol. 15 to 2005, vol. 17), an English dermatologic journal in Korea. RESULTS: Thirty-two percent of original articles fell into the category of drug evaluation and clinical research, followed by laboratory and experimental research. Most articles had three to six co-authors (78%), and one (55.3%) or two (31.1%) affiliations. The number of articles by a specific corresponding author ranged from one to eight, and the majority of articles by a specific corresponding author were one (43.6%) during 3 years. These situations were similar with those of the Annals of Dermatology. The evaluation of the Korean Journal of Dermatology about charateristics of original articles showed a small number of original articles by a corresponding author and cooperative researches with other afilliations. The main subjects of articles were centralized to 3 major categories in the Korean Journal of Dermatology, whereas the other famous international journals showed a relatively uniform distribution. CONCLUSION: The participation of non-dermatologists was fewer in the Korean journals than the other famous international journals. The Korean dermatologic society should make an effort to involve the specialists of other fields in submitting excellent original articles to the Korean Journal of Dermatology. And for the academic position of the Korean dermatologic society, we should make an effort to register one of our journals to the SCI (science citation index). The Korean Journal of Dermatology should deal with more varied articles from new categories for a more uniform distribution of themes. It should also make more effort to improve the Korean Journal of Dermatology.

6.Comparision of Patient Characteristics in Graves’ Ophthalmopathy with and without Strabismus Surgery

Purpose:
To evaluate the clinical and radiological differences between the strabismus surgery and non-surgery groups in patients with Graves' ophthalmopathy.
Methods:
From January 2009 to January 2019, 20 patients who underwent strabismus surgery and 225 patients who did not undergo strabismus surgery were retrospectively analyzed using medical records. The mean age of the patients, the presence of smoking, the presence of hypertension and diabetes mellitus, anti-thyroid peroxidase antibody levels, anti-thyroglobulin antibody levels, thyroid stimulating hormone (TSH) receptor antibody levels, the presence of steroid therapy, the presence of radiation therapy, soft tissue scores, clinical activity scores (CASs), using methimazole, the presence of exophthalmos, and the maximal cross-sectional areas of the inferior and medial rectus muscles using computed tomography (CT) were compared between the two groups.
Results:
There was no statistically significant difference in sex, smoking, the presence of hypertension and diabetic mellitus, anti- thyroid peroxidase antibody levels, anti-thyroglobulin antibody levels, steroid therapy, radiation therapy, soft tissue scores, CASs, using methimazole, and the presence of exophthalmos between the strabimus surgery and non-surgery groups (p = 0.472, p = 0.523, p = 0.537, p = 0.751, p = 0.107, p = 0.195, p = 0.320, p = 0.071, p = 0.562, p = 0.144, p = 0.663, and p = 0.423, respectively). The mean age of the patients, TSH receptor antibody levels, and the averages of the maximal cross-sectional areas in the medial and inferior rectus muscles were significantly different between the two groups (p = 0.034, p = 0.042, p = 0.001, and p = 0.038, respectively).
Conclusions
In patients with Graves’ ophthalmopathy, the risk factors for ocular motility disorders requiring strabismus surgery were not significant except for age, TSH receptor antibody levels, and the average of the maximal cross-sectional areas in inferior and medial rectus muscles using a CT scan.
7.Anxiety Sensitivity and Psychological Distress in Patients with Rosacea:A Questionnaire-Based Study

Background:
Rosacea is a common chronic inflammatory disorder that affects the skin of the central face. Anxiety sensitivity (AS) refers to the fear of sensations and symptoms arising from an expected experience of anxiety. High AS can cause anxiety disorders. Several studies have evaluated depression in patients with rosacea; however, AS has not yet been investigated.
Objective:
We aimed to investigate AS, depression, and quality of life in patients with rosacea.
Methods:
Eighty-five patients with rosacea and 85 controls (aged>18 years) were enrolled in the study. The severity of rosacea was evaluated according to the standard guidelines of the National Rosacea Society. Patients with rosacea and controls were asked to complete the Korean version of the anxiety sensitivity index-revised (ASI-R), beck depression inventory (BDI), and dermatology life quality index (DLQI).
Results:
The ASI, BDI, and DLQI scores were significantly higher in patients with rosacea than in controls (p< 0.001). In particular, the cardiovascular subscale of the ASI was significantly increased in patients with rosacea. The ASI and BDI scores were significantly correlated. DLQI scores were weakly correlated with ASI compared with BDI scores in patients with rosacea.
Conclusion
Our study is the first to report that AS is elevated in patients with rosacea. It is necessary to consider the anxiety-related symptoms of patients with rosacea, as they may benefit from psychological intervention to alleviate distress.
8.Late Hemorrhagic Disease of Infancy.

A hemorrhagic disease due to prothrombin complex deficiency os described in 4 infants from 1 to 11 months of age. This disease is different from the hemorrhagic disease of the newborn. The main differences are that it is found in an older age group and that intracranial hemorrhages are frequent. Rapid clinical and laboratory response after vitamin K therapy are observed. Severe prothrmbin complex deficiency due to vitamin K deficiency is a accounted for the pathogenesis of bleeding. Possible causes of vitaimn K deficiency are reviewed.

9.Plasma Lactate and Base Deficit: Early Predictors of Morbidity in Burn Patients.

PURPOSE: Plasma lactate and base deficit has been used as a marker to determine the status of tissue perfusion in trauma and clinically ill patients and also to predict the outcome of these patients. This study was performed to investigate the effect of plasma lactate and base deficit in predicting burn patients outcome. METHODS: A retrospective review was performed on 102 patients from January 2012 to December 2013 who were admitted as severe burn patients to our burn care unit. Plasma lactate and base deficit were measured upon admission to the hospital and SIRS score, hospital day, ABSI and TBSA were collected after admission. RESULTS: 102 patients were enrolled. Initial base deficit, hospital day, burn surface area and ABSI score showed statistical differences between low SIRS group and high SIRS group. The SIRS score, hospital day and ABSI score showed statistical differences between high base deficit group and low base deficit group (P<0.05). But there were no statistical differences in plasma lactate between two groups (P>0.05). CONCLUSION: In this study, initial base deficit but not plasma lactate, was predictor of morbidity following burn injury.

10.Diagnostic Performance of 2018 KLCA-NCC Practice Guideline for Hepatocellular Carcinoma on Gadoxetic Acid-Enhanced MRI in Patients with Chronic Hepatitis B or Cirrhosis: Comparison with LI-RADS Version 2018

Objective:
To evaluate the performance of the 2018 Korean Liver Cancer Association-National Cancer Center (KLCA-NCC) Practice Guidelines (hereafter, PG) for the diagnosis of hepatocellular carcinoma (HCC) using gadoxetic acid-enhanced MRI, compared to the Liver Imaging-Reporting and Data System (LI-RADS) version 2018 (hereafter, v2018).
Materials and Methods:
From January 2013 to October 2015, treatment-naïve hepatic lesions (≥ 1 cm) on gadoxetic acidenhanced MRI in consecutive patients with chronic hepatitis B or cirrhosis were retrospectively evaluated. For each lesion, three radiologists independently analyzed the imaging features and classified the lesions into categories according to the 2018 KLCA-NCC PG and LI-RADS v2018. The imaging features and categories were determined by consensus. Generalized estimating equation (GEE) models were used to compare the per-lesion diagnostic performance of the 2018 KLCA-NCC PG and LI-RADS v2018 using the consensus data.
Results:
In total, 422 lesions (234 HCCs, 45 non-HCC malignancies, and 143 benign lesions) from 387 patients (79% male; mean age, 59 years) were included. In all lesions, the definite HCC (2018 KLCA-NCC PG) had a higher sensitivity and lower specificity than LR-5 (LI-RADS v2018) (87.2% [204/234] vs. 80.8% [189/234], p < 0.001; 86.2% [162/188] vs. 91.0% [171/188], p = 0.002). However, in lesions of size ≥ 2 cm, the definite HCC had a higher sensitivity than the LR-5 (86.8% [164/189] vs. 82.0 (155/189), p = 0.002) without a reduction in the specificity (80.0% [48/60] vs. 83.3% [50/60], p = 0.15).In all lesions, the sensitivity and specificity of the definite/probable HCC (2018 KLCA-NCC PG) and LR-5/4 did not differ significantly (89.7% [210/234] vs. 91.5% [214/234], p = 0.204; 83.5% [157/188] vs. 79.3% [149/188], p = 0.071).
Conclusion
For the diagnosis of HCC of size ≥ 2 cm, the definite HCC (2018 KLCA-NCC PG) had a higher sensitivity than LR-5, without a reduction in specificity. The definite/probable HCC (2018 KLCA-NCC PG) had a similar sensitivity and specificity to that those of the LR-5/4.
