1.Two Cases of Diarrheal Disease Caused by Plesiomonas shigelloides.

Plesiomonas shigelloides is known to cause diarrhea in human. It is a facultatively anaerobic gram-negative rod belonging to the family Vibrionaceae. We isolated P. shigelloides from two patients with diarrhea, a 62-year-old woman with steroid therapy and a 4-year-old boy with no predisposing factor. The organisms were isolated on enteric agars as a nonlactose fermenter and were identified by oxidase, indole, and other biochemical characteristics. The isolates were susceptible to commonly used antimicrobial agents with the exception of ampicillin. P. shigelloides infection is rarely reported in our country, but appears to be a significant cause of diarrhea that responds to antimicrobial therapy. Therefore we suggest the need for correctly identifying P. shigelloides.

2.Xanthoma of the Corneoscleral Limbus: A Case Report.

We describe a case of xanthoma on the corneoscleral limbus in a 21-year-old man. It was characterized by a localized, round, elevated, yellowish mass on the right superonasal limbus with 6mm in size. There was no correlation with systemic or metabolic diseases such as hyperlipidemia and the microscopic examination revealed scanty fibrous element.

4.Effects of an emotional regulation program on maladaptive cognitive emotion regulation, stress, anxiety, and postpartum depression among South Korean mothers of premature infants in the neonatal intensive care unit

Purpose:
This study aimed to evaluate the effects of a rational emotive behavior therapy (REBT)-based emotional regulation program for mothers of premature infants in the neonatal intensive care unit (NICU).
Methods:
The study utilized a nonequivalent control group pretest-posttest design. Seventeen mothers were recruited for the experimental group and 21 mothers for the control group. Data were collected from July 9, 2018, to Oct 12, 2018 from mothers of premature infants in the NICU regarding parental stress, state anxiety, depression, and maladaptive cognitive emotion regulation. The experimental group received eight sessions of the REBT-based emotional regulation program for 3~4 weeks, and the control group only received standard nursing care.
Results:
Significant differences were found between the two groups in maladaptive cognitive emotion regulation (Z=-3.88, p<.001), stress (Z=-2.76, p=.006), state anxiety (Z=-3.72, p<.001), and postpartum depression(Z=-2.62, p=.009) after the intervention in the experimental group.
Conclusion
The REBT-based emotional regulation program for mothers of premature infants was effective for reducing maladaptive cognitive emotion regulation, stress, anxiety, and postpartum depression. Therefore, it may be necessary to provide an REBT-based emotional regulation program for mothers of premature infants in the NICU in clinical practice.

Hydranencehaly is an abnormality consisting of complete or almost absence of the cerebral hemispheres, which are replaced by a large fluid-filled cavity. The midbrain and brain stem are relatively intact, and rudiments of frontal, temporal and occipital cortex may be present. We diagnosed it through the evaluation of clinical features, prenatal US (intrauterine pregnancy, 37 wks) and brain CT. Authors have experienced a case of hydranencephaly combined with twin pregnancy and reported with a brief literature review.

6.The Factors Associated with Changes in the Stage of Breast Cancer Screening Behavior among the Woman who are Eligible for the Korean National Cancer Screening Program.

OBJECTIVES: This study was performed to evaluate the relationships between psychosocial characteristics and changes in the stage of breast cancer screening behavior. METHODS: The 474 study subjects were randomly sampled from 21,459 women (age range, 40-70 years) who were eligible for the Korean National Cancer Screening Program in 2006 in Jinju, Gyeongsangnam-do. The information, including behaviors and sociodemographic characteristics, attitudes, subjective norms and self-efficacy, was collected by trained interviewers via home visits. The breast cancer screening stages were grouped as precontemplation, contemplation, action, maintenance and relapse, according to Rakowski. RESULTS: Of the 474 women, 18.8% were in the precontemplation stage, 23.3% were in the contemplation stage, 13.1% were in the action stage, 36.6% were in the maintenance stage, and 8.2% were in the relapse stage. The distribution of stages was associated with attitude, subjective norms and self-efficacy (p for trend<0.01). To investigate the overall relationship between the variables, we conducted a linear structural equation model analysis based on the theory of planned behavior. The subjective norms and self-efficacy influenced the stage of the women's screening behavior. CONCLUSIONS: We should target on self-efficacy about the screening behavior of women by performing timely, thoughtful interventions. The support from family members, friends and other people is crucial for women to undergo breast cancer screening and to improve the breast cancer screening rate.

7.Superior Vena Caval and Airway Obstruction on Induction of Anesthesia in a Patient with Anterior Mediastinal Mass: A Case Report.

Mediastinal masses may cause life-threatening complications such as major airways obstruction, superior vena caval obstruction, and cardiac compression during general anesthesia. We present a case in which superior vena caval and airway obstruction developed at induction of anesthesia in a patient with an anterior mediastinal mass. Shortly after induction of anesthesia in the supine position, the patient became difficult to ventilate and blood began to flow upward into the IV line. The patient was immediately turned to the right lateral position and allowed to breathe spontaneously. The ventilation of patient was improved significantly and cyanosis of the face and upper extremities disappeared. We think that general anesthesia should be avoided if at all possible in patients with mediastinal mass and alternative methods of diagnosis and management are discussed.

8.Clinical impact of the treatment modality on small, solitary, recurrent intrahepatic hepatocellular carcinomas after primary liver resection

Purpose:
The aim of this study was to determine the survival benefit based on different treatment strategies in patients with small, solitary, recurring intrahepatic hepatocellular carcinomas (HCCs) that were defined as recurred Barcelona Clinic Liver Cancer stage O (reBCLC-O).
Methods:
Among the 917 patients with HCC recurrence after primary hepatic resection, 394 patients with reBCLC-O were selected. Of these, 150 patients underwent curative treatment (re-resection, radiofrequency ablation, and liver transplantation) and 203 underwent transarterial chemoembolization (TACE) group for recurrent HCC. After propensity score matching (PSM), both the groups were well balanced (89 patients in each group).
Results:
Before PSM, the 1-, 3-, and 5-year overall survival (OS) rates of patients in the curative treatment group (96.7%, 78.6%, and 70.5%, respectively) were significantly better than those in the TACE treatment group (95.6%, 53.7%, and 44.2%, respectively) (P < 0.001). After PSM, the 1-, 3-, and 5-year OS rates also differed significantly (92.0%, 79.6%, and 71.1% in the curative treatment group vs. 88.8%, 65.6%, and 57.9% in the TACE group) (P = 0.005). The independent predictors of worse OS were tumor number at the time of resection and treatment modality for the recurrence, time interval to recurrence, and prothrombin time international normalized ratio and alpha-fetoprotein levels at the time of recurrence.
Conclusion
The OS of patients in the curative treatment group was better than that in the non-curative treatment group after PSM. Based on our results, curative treatment should be strongly recommended in the patients with reBCLC-O recurrence for better survival.

10.Expression of Brain-Derived Neurotrophic Factor and its mRNA in Catecholaminergic Neurons of the Rat Lower Brainstem after Colohicine Treatment or Hemorrhage.

Widespread brain-derived neurotrophic factor (BDNF) mRNA expression has been detected in the region of catecholamine groups of the rat lower brainstem, while few BDNF-immunoreactive cells were found in this area. In the present study, a double-color immunofluorescence (IF) technique for BDNF and tyrosine hydroxylase (TH) after colchicine treatment was employed to evaluate the possible presence of BDNF immunoreactivity in the catecholamin-ergic cells of rat lower brainstem. Additionally, a double-color IF technique for BNDF and TH and in situ hybridiza-tion for BDNF mRNA were performed to see effects of hemorrhage on the expression of BDNF and its mRNA. We detected many new BDNF-immunoreactive cells in the A1, A2, A4, A6-A10 and C1-C3 cell groups and in the other lower brainstem nuclei where, without colchicine treatment, BDNF mRNA was expressed, but not BDNF immunoreactivity. In addition, the catecholaminergic neurons were found to express BDNF immunoreactivity with the co-existence being greatest, in percentage terms, in medullary catecholaminergic cell groups. Hypotensive hemorrhage, which activates medullary catecholaminergic neurons, induced the expression of BDNF immunoreactivity in catecholaminergic neurons (A1/C1 and C2) and increased the number of BDNF mRNA-containing neurons in the area. These results demonstrate that BDNF is regulated by activity in medullary catecholaminergic cell groups involved in central cardiovascular regulation.
