1.The Parent-Child Relationship and Depressive Symptoms in Korean Adolescents.

OBJECTIVES: The objective of this study was to compare the parent-child relationship of adolescents with depression and those without depression, by using Parent-Child Relationship Instrument (PCRI), Family Relationship Scale (FRS), and Parenting Attitude Test-Youth (PAT-Y). We also investigated the association between the parent-child relationship and depressive symptoms. METHODS: Twenty-five adolescents with depression (age 15.0+/-2.1 years, 8 boys) and 24 adolescents without depression (age 13.7+/-1.3 years, 16 boys) completed the Adolescent-General Behavior Inventory (A-GBI), Adolescent-Mood Disorder Questionnaire (A-MDQ), Beck Depression Inventory (BDI), PCRI, and PAT-Y. The parents of subjects completed the Child Behavior Checklist, Parent-General Behavior Inventory 10-item Mania Scale (P-GBI-10M), Parent-Mood Disorder Questionnaire, and FRS. Independent t-tests, analysis of covariance, and partial correlation analysis were used. RESULTS: The Intimacy (p=.002) and Respect (p=.029) scores of the PCRI were significantly higher in adolescents without depression compared to those with depression. The Intimacy scores of the PCRI showed negative correlation with the BDI and P-GBI-10M and the Strictness scores of the PCRI showed positive correlation with the A-GBI, A-MDQ, and BDI. The Inconsistency, Punishment, and Excessive Expectation scores of the PAT-Y showed positive correlation with the A-GBI and BDI scores. CONCLUSION: Our results suggest a possible association of the parent-children relationship and parenting attitude with adolescents' depressive symptoms.

3.A case of giant cell carcinoma in pancreas

A case of a pleomorphic giant cell tumor of the pnacreas in a 54 year old man is presented. It produced hugecystic mass in the pancreatic area ultrasonographically. Also multiple loculated pancreatic pseudocyst andmultiple low densities in liver were presented in CT. Pathologically this case was a form of giant cell carcinomawhich was called as epulis-osteoid type, but the osteiod component in this case was not conspicuous.

4.Reporting of National Notifiable Infectious Diseases (NNIDs) and Related Characteristics.

The purposes of the study is to estimate reporting proportion of National Notifiable Infectious Diseases(NNIDs) and investigate characteristics related to reporting using KAP(knowledge, attitude, practice) model. METHOD: We surveyed randomly selected 2,185 physicians (speciality: internal medicine, family medicine, pediatrics, dermatology, general physicians) of their knowledge, attitude, and practice of NNIDs reporting through self-administered mail questionnaires. Of them, 231 physicians responded (response proportion: 10.6%). RESULT: The reporting proportion was estimated to 27.0%. Recognition level (knowledge) of NNIDs was relatively high with proportion of 69.4%, and attitude (public health importance) of reporting was 65.8%. Multiple logistic regression analysis showed that knowledge, attitude significantly affected physicians' reporting in a positive direction (O.R. 6.2, 6.2 respectively). Whereas, senior age group, specialty (family medicine, pediatrics, dermatology) showed significantly lower reporting. General (tertiary care) hospital level of care showed significantly higher reporting practice (alpha=0.05). CONCLUSION: The NNIDs reporting proportion, 27.0% is similar with those studied recently. Continuous efforts to increase the performance level of communicable diseases surveillance system. Of those, restructuring surveillance systems considering characteristics of notifiable diseases classes must be stressed. Educational approach of physicians needs to be tailored specially to newly-designated diseases such as Group II, Designated Group NNIDs.

6.Outcomes of an Community-based Intervention Program for Obese Children.

PURPOSE: Childhood obesity is global public health issue and early intervention is essential because the long-term consequences of childhood obesity are well documented. We studied the effects of community-based intervention program for obese children in regard to regulating excess body weight and improving their lifestyle. METHODS: The study population were 101 obese children (age 6-12) who lived in Ansan during year 2003. The Program consists of exercise and nutritional education, and was conducted for 20 weeks (during the first 8 weeks program was performed once every week and once every month after that). We evaluated the changes of weight, body mass index, obesity index, lipid profiles and their lifestyle during the program. RESULTS: After the intensive course during 8 weeks, body mass index, obesity index, LDL-cholesterol decreased significantly. However, those results increased again after 20 weeks program comparable to initial levels. There are no significant differences in total cholesterol, triglyceride and HDL-cholesterol during program. The lifestyle for exercise and eating improved after initial 8 weeks intensive course, but returned to previous state after 20 weeks program finished. CONCLUSION: Our 20 weeks obesity program did not influence body mass index significantly although it showed transient improvement during early intensive course. These results suggest that the importance of long-term intensive programs to maintain educated healthy lifestyle. Hereafter, well-controlled and long term followed clinical researches in larger group will be necessary.

7.Standard or Variant Meso-Rex Shunts for Children with Variceal Bleeding due to Extrahepatic Portal Vein Obstruction: A Report of Two Cases

Extrahepatic portal vein obstruction (EHPVO) is the most common cause of pediatric portal hypertension and can cause life-threatening variceal bleeding. MesoRex shunt (MRS) is a surgical procedure that restores physiological portal venous blood flow to the liver by using a graft to connect the superior mesenteric vein and the left portal vein within the Rex recess, and can relieve variceal bleeding and other complications associated with EHPVO. Although the MRS is regarded as an optimal and potentially curative treatment with good long-term patency, graft thrombosis or failure due to unknown causes is not rare, prompting the need for further research on the risk factors of graft failure or poor patency. Herein, we report two cases of EHPVO in patients with recurrent or uncontrolled variceal bleeding, one treated with the classic Rex shunt and the other with the modified Rex shunt, which resulted in a failure and success, respectively.

Intravascular fasciitis (IVF) is a very rare disease that is difficult to diagnose preoperatively. Frequently, it can be misdiagnosed as a malignancy or deep vein thrombosis. A 26-year-old man presented with a 6-month history of intermittent cramping pain in the right calf. Duplex ultrasonography, computed tomography, magnetic resonance imaging, and positron emission tomography were performed in various hospitals. The work-up revealed a hypermetabolic mass in the femoral vein, suggestive of a malignancy, such as leiomyosarcoma. The tumor was located inside the femoral vein with no invasion, and the mass was resected en bloc with the vein wall. Intraoperative frozen section biopsy revealed no malignancy, and the final pathological diagnosis was IVF. Herein, we report a case of IVF and discuss the role of imaging studies in its preoperative diagnosis, with an extensive literature review.
9.Occurrence of Anthracnose on Highbush Blueberry Caused by Colletotrichum Species in Korea.

A total of 82 isolates of Colletotrichum species were obtained from anthracnose symptoms of highbush blueberry trees grown in the Gochang area of Korea during a disease survey in 2008. Out of the isolates, 75 were identified as Colletotrichum gloeosporioides and the others as C. acutatum based on their morphological and cultural characteristics. Twenty six of C. gloeosporioides isolates produced their teleomorph Glomerella cingulata in PDA culture. Three isolates of each C. gloeosporioides and C. acutatum caused anthracnose symptoms on the leaves by artificial inoculation, which were similar to what was observed in the orchards. Previously in Korea, only C. gloeosporioides has been reported as causing anthracnose in blueberries. This is the first report that C. acutatum causes anthracnose in the highbush blueberry in Korea.
