1.Distribution of the Axis of Astigmatism According to Age in Children .

We performed this study to investigate the distribution of axes of astigmatism, the occurrence and amount of astigma- tism among pediatric population aged from 1 to 15 years who have visited our clinic from January to December 1997. After routine ophthalmologic examination to rule out any organic abnormality, the cycloplegic refraction was performed with 0.5-1.0% atropine on 529 eyes. Out of 529 eyes, astigmatism was found in 356 eyes (67.3%) with the highest incidence in 15-year-old group being 87.0%. Among 356 eyes with astigma- tism, with-the-rule astigmatism was found in 322 eyes (90.4%); against-the-rule, 30 eyes (8.5%); oblique, 4 eyes (1.1%). With-the-rule astigmatism occurred far more frequently found than against-the-rule in all age groups. As to the type of astigmatism classified by the state of refractive errors,hyperopic astigmatism was found in 144 eyes (40.4%); myopic,136 eyes (38.3%); mixed, 76 eyes (21.3%). Of the total of 356 eyes with astigmatism, astigmatism of 1.0 diopter or less was found in 181 eyes (50.8%).

2.Positional Changes of Reattachment Site after Superior Rectus Recession in Rabbit.

In determining the effects of strabismus surgery, it is very important to know where the reattachment site of an extraocular muscle following recession procedure is located. Undercorrections or overcorrections after muscle surgery have been thought to be due to a postoperative positional changes of reattachment site along the surface of the globe. The author performed this experimental study to evaluate the amounts of changes of reattachment site after recession procedure in rabbit with 4 different methods of scleral fixation;direct suture with 6-0 vicryl, hang-back suture, application with Tisseel and Histoacryl. Superior rectus recession was performed in all 40 rabbit eyes, 10 eyes in each group. The distance from limbus to proximal end of recessed superior rectus muscle was measured on day 1, 2, 3, 5 and 7 postoperatively. Further measurements were followed at 2, 3, 4 and 8 weeks after recession procedure. The muscles were found an average of 0.4mm posterior to the intended position in direct suture group and 0.5 mmin hang-back suture group at one week postoperatively. But in the other two groups in which tissue adhesive agents, Tisseel and Histoacryl were used, the amount of displacement of the reattachment site were minimal of within 0.1 mm. From this experimental study, it is suggested that the positional changes of the reattachment site after recession procedure may influence the surgical corrective effects for strabismus.

3.The Association Between Genetic Polymorphisms of the Ethanol-metabolizing Enzymes and Susceptibility to Alcoholic Liver Cirrhosis.

BACKGROUND/AIMS: There is considerable variance in individual susceptibility to hepato-toxic effects of ethanol as evidenced by the finding that only about 10-20% of alcoholics develop alcoholic liver cirrhosis. The aims of this study were, 1) to get the data on the genetic polymorphisms of three major ethanol-metabolizing enzymes (ADH, CYP2E1, ALDH) in normal Korean adults, and to search for the specific genotypes influencing alcohol drinking behavior by the comparison of allele frequencies between healthy control group and heavy drinker group with or without liver disease, 2) to investigate the influence of the genetic polymorphisms of these enzymes on the susceptibility to alcoholic liver disease by the comparison of allele frequencies between heavy drinker group without liver disease and alcoholic liver cirrhosis group. METHODS: Healthy control group included 53 healthy males in military service without evidence of liver disease or alcoholism. Heavy drinker group without liver cirrhosis included 29 males who had been drinking 80g or more of alcohol daily for more than ten years but did not have any clinical evidence of liver disease. Alcoholic cirrhosis group included 43 male patients who had drunk 80g or more of alcohol daily for more than ten years and had clinical evidences of overt cirrhosis. Subjects with hepatitis B surface antigen or anti-hepatitis C antibody were excluded. Genotypes of the three enzymes were determined by PCR (polymerase chain reaction) and restriction fragment length polymorphism (RFLP) with genomic DNAs extracted from peripheral leukocytes. RESULTS: 1) In healthy Korean males, allele frequency of ADH22, ADH31, CYP2E1 c2 and ALDH22 was 81%, 94%, 30% and 14%, respectively. 2) The absence of ALDH22 or CYP2E1 c2 allele were significant risk factors for being a heavy drinker (odds ratio,' 0.09, 0.42, respectively). 3) Although it was not associated with the polymorphism of each ethanol-metabolizing enzymes, the susceptibility to alcoholic liver cirrhosis was significantly associated with combined genotypes of ADH2(22) & ADH3(1+1)& CYP2E1 B or C. COMCLUSION: Genetic polymorphisms of ethanol-metabolizing enzyrnes are significantly associated with the suseptability to alcoholic liver disease as well as alcohol drinking behavior.

4.Effects of Eye Protective Device and Ear Protective Device Application on Sleep Disorder with Coronary Disease Patients in CCU.

PURPOSE: The purpose of this research was to analyze the effects of an eye protective device and ear protective device application on sleep disorder with coronary disease patients in CCU. METHODS: The research design was set up as a nonequivalent control group non-synchronized design. The subjects were coronary disease patients in the CCU of a tertiary hospital. The subjects were composed of 20 in the group with an eye protective device, 18 in the group with an ear protective device, 17 in the group with an eye protective device and ear protective device and 20 in the control group. The data was analyzed by chi-square-test, ANOVA, Repeated measures ANOVA, Sheffe-test, Simple main effect and Time contrast using SPSS Win 12.0. RESULTS: The hypothesis, 'There are significant differences in sleep quantity among the four groups' was supported (F=1,342.71, p=.000). The hypothesis, 'There are significant differences in the subjective evaluation of the general sleep patterns among the four groups' was supported (F=3,638.73, p=.000). In addition the hypothesis, 'There are significant differences in degree of sleep among the four groups' was supported (F=1,616.61, p=.000). CONCLUSION: It is cost effective and a simple eye protective device and ear protective device should be applied to patients according to their preference and characteristics in the clinical setting.

5.The Relationship between Life Stress and Resilience among Dental Hygiene Students.

The purpose of this study was to investigate life stress and resilience levels, and the relationship between life stress and resilience among dental hygiene students. From July 11, 2016 to July 29, 2016, questionnaires were used to survey dental hygiene students in several areas, after which the data collected from 274 questionnaires were analyzed with IBM SPSS Statistics ver. 21.0 for Windows. The results showed a significant negative correlation between life stress and resilience for dental hygiene students in all factors (p<0.05, p<0.001). This means that higher the resilience, the lower the life stress. Therefore, it is necessary to enhance resilience as a preventive dimension and develop associated program to help dental hygiene students cope with life stress.

6.Experimental Study On The Effect Of Hyperbaric Oxygen Therapy In Postirradiation Sialadenitis Of White Rats.

Main objectives of this study were to produce the experimental postirradiation sialadenitis and to observe the effects of hyperbaric oxygen therapy after the production of postirradiation sialadenitis in the unilateral parotid gland of white rats. To produce the experimental postirradiation sialadenitis, author attempted the various methods of irradiation such as 1000rad weekly for 5 weeks, or single dosage of 2000rad, 3000rad, 5000rad, but those attempts were futile due to the high mortality of experimental animals(used 80rats for pilot study). Finally author had to design fractional irradiation, that is, 1000rad per 2 weeks, total 5000rad for 8 weeks in order to produce postirradiation sialadenitis. The occurrence of postirradiation sialadenitis was proved with serial salivary scan findings. Author used irradiation 1000rats, dividing into the control(50) and experimental group (50) and rendered the hyperbaric oxygen therapy(2 hours/daily at 2.4ATM) on the experimental group for 6 weeks. The effect of hyperbaric oxygen therapy was examined by salivary scan findings and microscopic examination. The obtained results of this study were as follows: 1. The differences of salivary gland function between irradiated parotid glands(right) and nonirradiated parotid glands(left) showed the most remarkable changes at 32 weeks after total 5000rad irradiation. 2. In microscopic examination at 32 weeks after total 5000rad irradiation, irradiated parotid glands(right) showed severe degenerative changes of acinar cells, separations of lobules due to edema, severe intralobular fibrosis and connective tissue fibrosis. 3. After hyperbaric oxygen therapy for 4 weeks, the effect of hyperbaric oxygen therapy showed no distinctive difference findings between the control and the experimental group with salivary scan findings(p>or=0.05), while the experimental group showed progressive intralobular edema, copillary proliferation, hyperemia in connective tissues and intralobular fibrosis in microscopic examination. 4. Tc99m uptake of the experimental group was raised after hyperbaric oxygen therapy for 6 weeks, and showed reduction of Tc99m uptake ratio between irradiated parotid gland and nonirradiated gland up to 8 to 10 weeks(p<0.05), while in the microscopic exemination the experimental group showed aggravated acinar cell degeneration, intercalated duct proliferation, connective tissues fibrosis. In this study, author speculated that though the hyperbaric oxygen therapy improved the capillary impairment, there was a tendency to increase the degenerative changes of parechyma of postirradiated parotid glands, thus the usage of the hyperbaric oxygen therapy should be considered carefully in the aspect of hazardous effect to the parotid parenchymal tissues.

7.In vitro Induction of Cellular Differentiation of Human Fetal Liver Cell Lines with Sodium Butyrate.

BACKGROUND/AIMS: Imrnortalized human fetal liver cell lines established by transfecting simian virus 40 T gene wae found to lose differentiated liver cell functions in successive long-term culture. Butyrate, known as a differentiation-promoting agent for a variety of cancer cell lines, is produced in the colon by bacterial flora and selectively transported into the liver though the portal blood flow. Therefe, butyrate might play a role in the maintenance of differentiation in hepatocytes in vivo. In thepresent study, the effects of butyrate on cell growth and differentiation in human fetal liver cell lines was investigated. METHODS: Human fetal liver cell lines imrnortalized by SV 40 T antigen were treated with sodium butyrate (1mM), and cell growth rate after butyrate treatment were nmsured by the number of viable cells, determined by trypan blue dye exclusice method. The effects of sodium butyrate on the hepatocyte-specific differentiatian were assessed by albumin and alfa-fetoprotein (AFP) mRNA expression, analyzed using reverse-transcription polymerase chain reaction, and were also by the increment of albumin secretion into culture media, determined by a competitive inhibition ELISA. RESULTS: Treatment with sodium butyrate resulted in a cessation of cellular proliferation and alterations in cellular morphology (increased cell size and polygonal change in shape). The level of albumin mRNA after sodium butyrate treatment was elevated by about two times as compared to that of control. In contrast, AFP mRNA expression were dennstrated neither before nor after sodium butyrate treatment. The average amount of albumin released in the medium was less than 6pghnl/10'cells/2days in the absence of sodium butyrate, and increased to 17 p g/ml/10'cells/2days at day 2, 21ugfml 10'cells/2days at day 4 in the presence of sodium butyrate, and these levels thereafter were over 10 times higher than that in the absence of sodium butyrate until day 10. CONCLUSION: These mults indicate that treatment of immcetalized fetal liver cell lines with butyrate leads to inhibition of cellular proliferation and promotion of adult hepatocyte-specific differentiation.

8.Reviews of Pay-for-Performance and Suggestion for Korean Value Incentive Program.

The effort to measure and improve the quality of healthcare is a common health policy issue worldwide. Korean Value Incentive Programme is one of that effort, but some concerns exist. Compared to pay for performance program in other countries, it measures healthcare quality with relatively narrow performance domain using a small number of clinical indicators. It was designed without involving hospitals and other key stakeholder, and program participation was mandated. Highest and lowest performers get bonus and penalty using relative ranking. As a suggestion for development, the direction for quality management at the national level should be given first. Therefore the philosophy or strategy for quality improvement should be reflected to the program. And various domains and indicators of healthcare quality should be developed with active communication with healthcare providers. The evaluation method is necessary to be changed to provide achievable goal to the healthcare providers and attract quality improvement.

YANG Bong-keun (1897-1982) had lived as a medical doctor and a social and public health reformer during the turbulent period of the port to the western society occupation by and liberation from Japan and the partition of the Korean Peninsular He actively participated in the March First Movement Shinganhoe and other activities for Korea's liberation from Japan. He also founded Bogunwoondongsa an organization for public health movement for Korean people and published Bogeunwoondong. a magazine for introducing and educating new ideas and knowledge of health for Korean people. After the defeat of Japan in the World War II he worked for the protection and repatriation of Korean residents in the Manjoo area as a head of policy division of the Northeastern office of the Korean Provisional Government He also participated in the foundation of Yanbian Hospital and medical school for Korean-Chinese in China His holistic approach of health and public health movement accentuation of preventive medicine and a body under his/her own will public health movement as a part of everyday life movement and minjoong oriented humanism were closely linked with the idea of social medicine that originated from the European society in the 19th century. Those are also valuable ideas to be considered and implemented in this time Moreover his effort of health for Korean people on the way of modernization and liberation of Korea provides an example of being a respectable health reformer and pioneer of social medicine

BACKGROUND/AIMS: Lens culinaris agglutinin-A reactive alpha-fetoprotein (AFP L3) has been reported to be highly specific for the diagnosis of hepatocellular carcinoma (HCC). The present study was to evaluate the clinical usefulness of AFP-L3 for the diagnosis of HCC in the patients either with chronic liver disease or with HCC in complete remission who showed significant increment of serum AFP level and no mass lesion in the liver on ultrasonography. METHODS: A total numer of 34 patients (24 with chronic liver disease, 10 with HCC in complete remission) were enrolled, who showed significant increment of serum AFP level and no mass lesion in the liver on ultrasonography. Serum AFP L3 levels were analysed by AFP differentiation kit L. Abdominal spiral CT or ultrasonogram was performed at 1-3 month intervals and all of the patients were followed up for more than 6 months. RESULTS: Among 24 patients with chronic liver disease, two were positive (higher than 15%) for AFP L3; however, HCC was not detected in these patients, while HCCs were detected in two of 22 patients negative for AFP L3 during followe-up. Eight of the 10 patients with HCC in complete remission were positive for AFP L3; recurrent HCCs were detected in 7 of those 8 patients as well as in the rest 2 patients negative for AFP L3 during follow-up. The overall sensitivity and specificity of AFP L3 measurement for the detection of HCC within 6 months of follow-up were 63.6% and 87.0%, respectively. The positive and negative predictive value for HCC in patients with chronic liver disease were O% and 90.9% and for recurrent HCC in HCC patients in remission were 87.5% and ON, respectively. CONCLUSION: The measurement of AFP L3 is suggested to be useful for the diagnostic strategy in patients either with chronic liver disease or hepatocellular carcinoma in complete remission, who showed progressive increment of serum AFP level and no mass lesion in liver on ultrasonogram.
