2.Modulatory effects of the fruits of Tribulus terrestris L. on the function of atopic dermatitis-related calcium channels, Orai1 and TRPV3

Objective: To examine the effects of Tribulus terrestris L. (T. terrestris) extract on the modulation of calcium channels to evaluate its use in topical agents for treatment of atopic dermatitis. Methods: The 70% methanol extract of T. terrestris was prepared. Human HEK293T cells with over-expressed calcium release-activated calcium channel protein 1 (Orai1), transient receptor potential vanilloid 1, or transient receptor potential vanilloid 3 (TRPV3) were treated with T. terrestris extract. Modulation of ion channels was measured using a conventional whole-cell patch-clamp technique. Results: T. terrestris extract (100 mg/mL) significantly inhibited Orai1 activity in Orai1-stromal interaction molecule 1 co-overexpressed HEK293T cells. In addition, T. terrestris extract significantly increased the TRPV3 activity compared with 2-Aminoethyl diphe-nylborinate (100 mmol/L), which induces the full activation of TRPV3. Conclusions: Our results suggest that T. terrestris extract may have a therapeutic po-tential for recovery of abnormal skin barrier pathologies in atopic dermatitis through modulating the activities of calcium ion channels, Orai1 and TRPV3. This is the first study to report the modulatory effect of a medicinal plant on the function of ion channels in skin barrier.
3.Computed tomographic evaluation on ossification of posterior longitudinal ligament and ligamentum flavum ofspine

Ossification and calcification of the spinal ligaments frequently cause pressure upon the spinal cord andnerve roots. Authors reviewed 150 cases of C-spine CT, 80 cases of T-spine CT, and 725 cases of L-spine CT whichwere carried out a Pusan Natina University Hospital for 2 years from May 1983 to May 1985. We analysed 34 caseswhich showed ossification of posterior longitudinal ligament (OPLL) and ligamentum flavum (OLF) of these 955cases. The results are follows: 1. The male ot female ratio of spinal ligamentous ossification was 26:8. The 5thdecade group (24.41%) was the most prevalent age group. 4th decade(26.4%), 6th decade(23.52%), 7th decade(11.76),3rd decade (8.8%), and 8th decade(2.94%) in that order. 2. Of 955 cases of spine CT, the incidence of OPLL was 25cases(21.51%) and that of OLF was 10 ases(1.05%). 3. Regional incidence of spinal ligamentous ossification was asfollows. 1) In case of OPLL, cervical area was 19/150(12.67%) and lumbar area was 6/725 (0.83%). 2) In cases ofOLF, Thoracic area was 3/80(3.75%) and lumbar area was 8/725(1.10%). 4. The most frequent length of OPLL was 4body length (32%) and the most frequent locations are C4 and C5(68%). The types fo OPLL were 15 cases(60%) ofsegmental type, 8 cases(32%) of continuous type, and 2 cases(8%) of mixed type in that order. All segmental typeswere degree 1 or 2 and most continuous and mixed type (80%) were degree 2 or 3 dural sac effect. 5. The number ofinvolved interlaminar spaces in OLF was 1 to 5 interspaces and most of OLF were found at low thoracic and lumbararea. 6. There could be noted high correlation between the spinal ligamentous ossification and degenerative discdisease, The incidence of associated disc disease was 18/25(72%) in OPLL and 8/10(80%) in OLF.

6.Evaluation on the Time to Start Parenteral Nutrition in Hospitalized Cancer Patients.

PURPOSE: Malnutrition is quite prevalent in hospitalized cancer patients, with a 40%~80% rate. Malnutrition in cancer patients can result in an increase in the number of complications, length of stay, mortality, and morbidity. Therefore, cancer patients with malnutrition must have the appropriate nutritional support to improve the prognosis of cancer. This study evaluated the appropriate time point to start parenteral nutrition (PN) after admission according to the nutrition support guidance in Samsung Medical Center. METHODS: This study enrolled patients diagnosed with the Korean standard classification of disease 6 (KCD6) code C00-C97 and discharged from March 1st to 31st, 2016. The following data were collected: patients'age, gender, diagnosis, length of stay, body mass index, nutritional status, and whether to consult nutrition support team (NST). RESULTS: Among a total of 2,944 patients, 381 patients (12.9%) were in a malnourished status upon admission. In the malnourished patients, 139 patients were prescribed PN for a median of 6 days (range, 1∼49) and moderate to severe malnourished patients were started on PN within 2 days after admission. The proportion of patients with a poor nutritional status was lower in the NST group than in the non-NST group (50.0% vs. 66.7%) on the 28th day after admission. Among the nourished patients, 229 patients were prescribed PN. Of them, 183 patients (79.9%) were started on PN within 7 days after admission. CONCLUSION: In moderate to severe malnourished cancer patients, the initiation of PN on the day after admission is appropriate.

7.Recognition and Buying Practices of Street-vended Foods among College Students.

The purpose of this study was to investigate recognition and buying practices of street-vended foods among college students in Chungbuk Area. A general dietary practices including snacks, related factors of buying street-vended foods were analyzed. The results showed that many students had irregular dietary habit, especially more than eighty percent of them skipped breakfast. More than sixty percent of students had frequent snacks. It was investigated that college students ate street-vended foods two or three times per week. The order of frequent choice of street-vended foods was rice cake with pasted red pepper(51.9%), sweet and sour pork(19.7%), sundae(5.7%). In microbiological examination, MPN of Coliform and CFU of standard plate count was over a standard value respectively, and acid value of frying oil revealed over value for maximum limitation point. Therefore, food and equipment in street-vended food store was evaluated as poor sanitation status. Most of college students have opinion that street-vended food store must be kept the line. In conclusion, nutrition education for college students should be required to protect them from unsafe street-vended foods.

9.External pH effects on delayed rectifier K+ currents of small dorsal root ganglion neuron of rat.

Under certain pathophysiological conditions, such as inflammation and ischemia, the concentration of H+ ion in the tissue surrounding neurons is changed. Variations in H+ concentration are known to alter the conduction and/of the gating properties of several types of ion channels. Several types of K+ channels are modulated by pH. In this study, the whole cell configuration of the patch clamp technique has been applied to the recording of the responses of change of external pH on the delayed rectifier K+ current of cultured DRG neurons of rat. Outward K+ currents were examined in DRG cells, and the Charybdotoxin and Mn2+ could eliminate Ca2+-dependent K+ currents from outward K+ currents. This outward K+ current was activated around -60 mV by step depolarizing pulses from holding potential -70 mV. Outward K+ currents were decreased by low external pH. Activation and steady-state inactivation curve were shifted to the right by acidification, while there was small change by alkalization. These results suggest that H+ could be alter the sensory modality by changing and modifying voltage-dependent K+ currents, which participated in repolarization.

10.A case of diacerein-induced DRESS (drug reaction with eosinophilia and systemic symptoms) syndrome with thromboembolic complications

Diacerein is a safe drug for osteoarthritis due to its inhibitory effect on proinflammatory cytokines. Its common side effects, such as diarrhea, abdominal pain and skin rash, are relatively mild. There have been a few cases of drug reactions with eosinophilia and systemic symptoms (DRESS) syndrome accompanied by thromboembolic complications. We report the first case of diacerein-induced DRESS syndrome with thromboembolic complications. A 64-year-old man developed fever, skin rash, increase in liver enzymes, eosinophilia, deep vein thrombosis (DVT) and pulmonary thromboembolism (PTE) after taking diacerein. DVT and PTE were improved after anticoagulation therapy; however, fever, skin rash, eosinophilia and increase in liver enzymes were aggravated. A high-dose systemic corticosteroid (methylprednisolone 1 mg/kg/day) was administered, after which his clinical conditions improved.
