1.A Case of Short Stature with 46, XXp+(Xqter-->Xp22::Xp22-->Xqter).

We experienced a case of Turner variant in 17 year old girl. She showed normal appearance except short stature. The patient was born at term with a birth weight of 2200g and her height was below the 3 percentile at admission. Thelarche appeared at the age of 13 years. FSH and LH levels were elevated, estradiol was within normal range, but progesterone level was decreased, bone age was 14 to 15 year old, and the small sized uterus was detected by abdominal ultrasonogram. Karyotype analysis revealed 46,XXp+(Xqter-->Xp22::Xp22-->Xqter). Her height didn't grow properly despite serial growth hormone therapy, because her epiphyseal growth plate was nearly closed.

2.Comparison of Polymerase Chain Reaction and DNA Hybridization for Detection of the Cholera Toxin Operon of Vibrio cholerae.

Cholera enterotoxin (CT) is a major virulence determinant of Vibrio cholerae 01. CI' is known to be the major virulence factor of Vibrio cholerae 01 and in accordance with the recent report showing which V. cholerae non-01 has ctx gene, we performed the molecular genetic study for the detection of ctx gene related to the production of CT at the subject Vibrio spp. except for V. cholerae non-01 and V. cholerae non-01 stock cultured in the laboratory of microbiology, College of Medicine, Pusan National University and the Vibrio spp. isolated from the marine products of Pusan General Fish Market and the sea water, and then its results are as follows: 1. PCR for the detection of ctx gene at the subject of V. cholerae 01:61H-151 having the ctx gene of which the denaturation is 1 rninute at 95'C, annealing to 1min, 30 sec at 60'C, the extension to be 1min. 30 sec at 72'C and 30 or 40 cycles. ctx gene was detected from 4 strains of V. cholera non-01 derived from the environment isolates. 2. Adjusting the quantity of chromosomal DNA used as template DNA to be from 0.1 pg to 1 ng, in order to know the PCR conditions for the effective search of ctx gene, and the detection limit of the system was 10 pg of chromosomal DNA. 3. The broth culture was used for template DNA, ctx gene of 302 bp was detected from 4 V. cholerae non-01, as in the case of chromosomal DNA, and the cell number was possible to be detected to 3 * 10.4. We attempted the confirmation of ctx gene through Southern blot hybridization, labeling with P and then it was confirmed only from 4 V. cholerae non-01 as like PCR results. 5. As the result of the sensitivity of PCR and Southern blot hybridization, it was shown to be possible which 10 pg was detected in case of chromosomal DNA and in case of cultured broth, the cell number was detected until 10 at PCR and Southern blot hybridization, and thus it was examed which its sensitivity was same.

3.Modulatory effects of the fruits of Tribulus terrestris L. on the function of atopic dermatitis-related calcium channels, Orai1 and TRPV3

Objective: To examine the effects of Tribulus terrestris L. (T. terrestris) extract on the modulation of calcium channels to evaluate its use in topical agents for treatment of atopic dermatitis. Methods: The 70% methanol extract of T. terrestris was prepared. Human HEK293T cells with over-expressed calcium release-activated calcium channel protein 1 (Orai1), transient receptor potential vanilloid 1, or transient receptor potential vanilloid 3 (TRPV3) were treated with T. terrestris extract. Modulation of ion channels was measured using a conventional whole-cell patch-clamp technique. Results: T. terrestris extract (100 mg/mL) significantly inhibited Orai1 activity in Orai1-stromal interaction molecule 1 co-overexpressed HEK293T cells. In addition, T. terrestris extract significantly increased the TRPV3 activity compared with 2-Aminoethyl diphe-nylborinate (100 mmol/L), which induces the full activation of TRPV3. Conclusions: Our results suggest that T. terrestris extract may have a therapeutic po-tential for recovery of abnormal skin barrier pathologies in atopic dermatitis through modulating the activities of calcium ion channels, Orai1 and TRPV3. This is the first study to report the modulatory effect of a medicinal plant on the function of ion channels in skin barrier.
4.An Unusual Hanging Case Mistaken as Homicidal Strangulation in the Scene of a Fire.

Here, we report an autopsy case of an unusual hanging that was misinterpreted as a case of homicidal strangulation, as th body was found on the scene of a house fire. A 29-year-old unmarried woman hanged herself from the gas pipe using a bath towel. Subsequently, the pipe broke and a fire broke out after an explosion. However, as the initial postmortem investigation revealed the absence of cherry-pink lividity and as a burnt bath towel was found around her neck, this case was misunderstood as a case of arson after homicidal strangulation. The cause and manner of death in this case were finally determined based on the autopsy findings of ligature marks that are typically noted in cases of hanging and the presence of a burnt electric lock and a broden gas pipe that were noted at the suicide scene.

5.Identification of Ischemic Myocardium with Simultaneous Dobutamine Stress Echocardiography and 99mTc-MIBI SPECT in Patients with Suspected Coronary Artery Disease.

BACKGROUND: To compare the diagnostic usefulness of dobutamine stress echocardiography(DSE) and 99mTc-methoxyisobutyl isonitrile SPECT (MIBI SPECT), two studies were performed simultaneously. METHOD: Fifty-six consecutive patients undergoing coronary angiogram and MIBI SPECT for clinical indications without clincal evidence of myocardial infarction were studied prospectively. During the DSE, MIBI was injected at peak stress, and post-stress images of MIBI SPECT were required on hour later. Both echocardiographic and MIBI SPECT images were visually analysed in a blind fashion. RESULTS: On the basis of coronary angiographic findings, the sensitivites of the DSE and MIBI SPECT (n=36) were 89% and 86%, respectively. The specificities of those (n=20) were 90% and 85%, respectively. Among 33 patients without resting perfusion defect on MIBI SPECT, resting regional wall motion abnormalities on DSE were found in only one patient, whereas, resting perfusion defect on MIBI SPECT were found in 9 patients among 41 patients without resting regional wall motion abnormalities on DSE. Among 17 patients who had resting perfusion defects with partial reversibility on MIBI SPECT, resting wall motion abnormalities were present in 11 patients and five of them showed improvement in the regional wall motion during low dose dobutamine infusion. CONCLUSION: Both dobutamine stress echocardiography and MIBI SPECT are useful methods in the detection of the coronary artery disease, however, MIBI SPECT seems to overestimate the regional ischemic myocardium with contractile reserve that can hardly be evaluated with MIBI SPECT.

7.Comparison of the depression and anxiety between the elderly in the home for the aged and those in the community.

This survey was conducted from January 1991 to May 1991. Two hundred and eight residents in 4 government supported homes for the aged and hundred and eleven living in the community in Taegu, Korea, were examined to evaluate the depression and the anxiety of the aged with combined anxiety and depression scale (CADS) and somatization symptom check list (SCL). There are no significant differences between residents in the home for the aged and those living in the community on the total scores of CADS and SCL. However, in the case of the total score of CADS of the female subjects in the home for the aged were significantly higher than those of the community residents. The elderly in the home for the aged tended to have pure depression, while community residents were likely to have anxiety and depression. Fifty-two subjects of home for the aged and sixty-nine of the community scored over 50 points of CADS, which indicates considerable depression or anxiety. In psychosocial factors, the subjects who in the following situations had statistically significant higher scores than others. The results were as follow. Poor health, unhappiness, unsatisfaction to the past occupation, pessimistic thought in future view for both group and unsatisfaction of the relationship with familiar people for the elderly in the community. The SCL scores of two groups subgrouped by under 49 and 50 on CADS showed significant differences between each subgroup on all of the SCL items. It could be suggested that somatic symptoms for the aged is a sign of depression.

8.Comparative Study on Self-leadership, Team Efficacy, Problem Solving Process and Task Satisfaction of Nursing Students in Response to Clinical Training.

PURPOSE: This research compares self-leadership, team efficacy, problem solving processes and task satisfaction in response to teaching methods applied to nursing students, and determines whether variations exist. METHODS: This research experiments before and after the training of a nonequivalent group. The subjects were 36 learners of action learning methods and 39 learners of nursing course methods, and the research took place from October through December 2012. RESULTS: Prior to the training, the general features and measurable variables of the two groups of subjects were similar, and self-leadership, team efficacy, problem solving process and task satisfaction in both groups were elevated compared to pre-training. In particular, in comparison with the nursing course, there was a notable difference in scores, the action learning method receiving high scores in the problem solving process (t=2.92, p=.005) and task satisfaction (t=2.54, p=.013) CONCLUSION: It is recommended that educators not only conduct the practice training course for teaching methods, but also incorporate action learning.

10.Determinants of Health-Promoting Behavior in the Elderly.

This study was undertaken to grasp health-promoting behavior of the elderly and to identify variables related to them in order to facilitate nursing intervention for health promotion of this population. The subjects for this study were 291 old persons obtained by cluster sampling from twenty general social welfare centers located in Tague. Data were collected by self-reported questionnaires from August 13 to September 13, 1996. Questionnaires were developed based on Sherer and others Self-Efficacy scale, Rosenberg's Multidimensional Health Locus of Control scale, Northern Illinois University's Health Self Rating Scale, Walker and others' Health Promotion Lifestyles Profile. Analysis of the data was done by use of descriptive statistics, stepwise multiple regression, Pearson Correlation Coefficient, MANOVA, t-test, and ANOVA. The results were summarized as follows : 1. For the practice of health-promoting behavior, the mean score was 2.89 and range was 3.59 to 2.09. The factor of the highest mean score was regular diet(M=3.42) and factor of the lowest mean score was stress management(M=2.27). 2. The combination of self-efficacy, internal health locus of control, family number, and average monthly pocket money explained 30.0% of the variance of health-promoting behavior. 3. With regard to the relationship between health-promoting behavior and cognitive perceptual factor, self-efficacy correlated positively with health-promoting behavior(r=.4951, P=.0001), self-esteem correlated positively(r=.3263, P=.0001), internal health locus of control correlated positively(r=.3244, P=.0001), perceived health status correlated positively(r=.1355, P=.0274). 4. According to age(F=2.50, P=.0431), sex(t=2.14, P=.0332), marital status(F=7.85, P=.0005), education(F=5.44, P=.0003), family number(F=11.18, P=.0001), people living together(F=7.21, P=.0009), previous occupation(F=5.83, P=.0001), average monthly pocket money(F=7.27, P=.0001), there were differences of health-promoting behavior. The above findings show that health-promoting behavior are related to demographic characteristics, four cognitive perceptual factors(self-efficacy, self-esteem, internal health locus of control, perceived health status). On the basis of the above findings the following recommendations are made; 1. Nursing interventions enhancing exercise or activity, accountability for health, stress management of the elderly must be provided. 2. Nursing strategies enhancing self-efficacy which is most significant effect on health-promoting behavior must be developed.
