1.An Unusual Hanging Case Mistaken as Homicidal Strangulation in the Scene of a Fire.

Here, we report an autopsy case of an unusual hanging that was misinterpreted as a case of homicidal strangulation, as th body was found on the scene of a house fire. A 29-year-old unmarried woman hanged herself from the gas pipe using a bath towel. Subsequently, the pipe broke and a fire broke out after an explosion. However, as the initial postmortem investigation revealed the absence of cherry-pink lividity and as a burnt bath towel was found around her neck, this case was misunderstood as a case of arson after homicidal strangulation. The cause and manner of death in this case were finally determined based on the autopsy findings of ligature marks that are typically noted in cases of hanging and the presence of a burnt electric lock and a broden gas pipe that were noted at the suicide scene.

5.The Effects of Structured Self-Debriefing Using on the Clinical Competency, Self-Efficacy, and Educational Satisfaction in Nursing Students after Simulation.

PURPOSE: The purpose of this study was to identify the effects of structured self-debriefing using 3S models on the clinical competency, self-efficacy, and educational satisfaction in nursing students after simulation. METHODS: For this study, 76 third-year undergraduate nursing students from S university were invited. They were divided into two groups, which consisted of a self-debriefing (SDG=41) group and an instructor-led debriefing group (ILDG=35). Collected data was analyzed using Chi-square, t-test, and an independent t-test with the PASW statistics 18.0 for Windows Program. RESULTS: Clinical competency was generally high in the SDG, and statistically significant differences between the SDG and the ILDG occurred after simulations 3, 4, and 5. There were no statistically significant differences in self-efficacy and educational satisfaction between the SDG and the ILDG. However, educational satisfaction in the SDG was slightly higher, while self-efficacy was low compared to the ILDG. CONCLUSION: The results indicate that the method of structured self-debriefing using a 3S model can be effective in improving clinical competency. Further studies need to be investigated.

6.Food Habits, Dietary Intake, and Body Composition in Children with Cerebral Palsy.

This study aimed to determine meal-related factors affecting nutritional status, dietary intake, and body composition of children with cerebral palsy (CP). This study was conducted on 16 children with and 16 children without CP, aged 4 to 12 years, through a survey on general characteristics, body composition, eating habits, and nutrient intake. In the case of children with CP, comparisons were made according to classification into types of paralysis (hemiplegia, paraplegia, and quadriplegia). With respect to stature, the percentile of those surveyed was within normal range; however, children with CP were in a significantly lower percentile (p < 0.05) than healthy children. Regarding problems of dietary life, while usually brain-damaged children with CP have an overeating problem, seriously brain-damaged children with CP cannot have a meal by themselves; this was significantly different among the groups (p < 0.01). Regarding average intake of vitamin D and calcium, children with and without CP had a lower intake than required, with no significant difference between the groups. The evaluation of the nutrient status of children with and children without CP showed that children with CP were slow in stature development, and intake of vitamin D and calcium were less than required; therefore, it is necessary to provide education on adequate intake of nutrients. Since CP leads to frequent external intervention to having meals, it is required of parents and teachers to undergo training on adequate eating habits and attitudes.

7.A Long Term Follow-up Study of Prognostic Factors in Infantile Asthma (Report I).

PURPOSE: Asthma has been known as a chronic inflammatory disease to cause airway obstruction by allergens, viral infections and other factors. Nowdays infantile asthma is increasing markedly. However, to date the enough study for predicting its long term prognosis was not performed. This study aimed to estimate the long term prognosis of infantile asthma. METHODS: In pediatric allergy clinic of Hanyang University Hospital, 84 asmatics younger than 24 months of age were included from January 1986 to December 1992, who had no abnormality in the airway. Infantile asthma was diagnosed on the basis of chronic cough, episodic wheezing more than three times and reversibility of clinical symptoms by using inhaled bronchodilators. Forty four asthmatics could have been followed and divided into two groups, symptomatic and resolved by analyzinng their medical records and performing follow- up examination with blood and skin prick tests to common allergens. RESULTS: 1) The two study groups included resolved 28 children (63.6%) and symptomatic 16 children (36.4%) with the ratio of male 3.9 to female 1. 2) There were no significant differences the age of first wheezing episode (9.6+/-5.6 mo. vs. 9.9+/-3.5 mo.) as well as diagnosis (14.3+/-5.7 mo. vs. 16.2+/-5.1 mo.) between two study groups, respectively (P>0.05). 3) For history of atopic dermatitis, there was significant difference between the resolved group 6 of 28 children and the symptomatic group 12 of 16 (P<0.05). 4) For laboratory examination, no significant difference was observed, between the levels of PB eosinophil count (310.4+/-282.2/mm3 vs. 368.1+/-465.9/mm3 at diagnosis; 294.3+/-205.3/mm3 vs. 457.7+/-511.2/mm3 at follow-up) and serum IgE (107.2+/-126.5 IU/ml vs. 157.6+/-246.3IU/ml at diagnosis; 267.3+/-700.5IU/ml vs. 442.6+/-546.0IU/ml at follow-up). 5) The frequency of asthmatic attack per year was 3.3+/-1.6 vs. 3.7+/-1.7; the duration of clinical symptom was 2.6+/-1.4 vs. 5.7+/-2.0 years, respectively. 6) The results of skin prick test showed that more frequent positivity to common inhalants in symptomatic group than in resolved was observed. CONCLUSIONS: In the long term follow-up for infantile asthma, the resolved 28 of 44 asthmatics were observed. The positive history of atopic dermatitis might be one of the important predictors for their persistent clinical symptoms.

9.The Comparison of Videofluoroscopic Findings between the Patients with Lateral Medullary Infarct and Middle Cerebral Artery Territorial Infarct.

OBJECTIVE: To compare the videofluoroscopic findings between the patients with lateral medullary infarct and middle cerebral artery infarct and to investigate specific findings relevant to lateral medullary infarct. METHOD: Among patients with stroke taking videofluoroscopic study for swallowing problems, thirteen patients had a lesion in lateral medulla in imaging study and twenty-six patients in middle cerebral arterial territory. The findings of videofluoroscopic study on two groups were analyzed and compared. RESULTS: In oral phase, the ability of mastication and bolus formation were better in lateral medullary group. In pharyngeal phase, lateral medullary group revealed significantly impaired triggering of pharyngeal reflex, impaired laryngeal elevation, larger amount of residual materials, repeated swallow, delayed pharyngeal transit time, weaker pharyngeal muscle contraction, and poorer upper esophageal sphincter relaxation. Inadequate relaxation of upper esophageal sphincter was most significant factor in indicating the possibility of lateral medullary infarct (positive predictability 90.0%). Inadequate triggering of pharyngeal swallow indicated least possibility of lateral medullary infarct (negative predictability 92.3%). CONCLUSION: Lateral medullary group has the characteristics of more impaired pharyngeal function and better oral function during swallowing than middle cerebral artery group in videofluoroscopic study. Inadequate upper esophageal relaxation and triggering of pharyngeal swallow are the most predictive for lateral medullary infarct.

10.Minimal Change Disease Associated with Gastric Adenocarcinoma and Multiple Liver Metastasis.

A 35-years-old man was hospitalized with gene-ralized edema. 6 months ago, the patient was operated by radical subtotal gastrectomy for advanced gastric adenocarcinoma. The patient presented with multiple liver metastasis and the massive proteinuria. The patients renal biopsy revealed minimal change nephrotic syndrome and treated with prednisolone and diuretics. The patient was improved clinical symptome and decreased 24 hours urine protein. In literature reviewed, gastric carcinoma was not associated with minimal change nephrotic syndrome. We experienced advanced gastric carcinoma associated with minimal change nephrotic syndrome, thus we report it.
