3.A Case of Unilateral Focal Pigmented Paravenous Retinochoroidal Atrophy

PURPOSE: We report a case of unilateral, focal, pigmented paravenous retinochoroidal atrophy (PPRCA). CASE SUMMARY: A 46-year-old female visited our clinic in complaint of a vague problem with her right eye identified during a general medical examination. The visual acuity (without correction) of both eyes was 1.0. Slit-lamp examination of both eyes revealed no specific signs. Fundus examination of the right eye revealed focal, bony-spicule-shaped retinochoroidal atrophy with pigmentation along the course of the superior retinal vein. A fundus autofluorescence examination revealed principally hypofluorescence with some hyperfluorescence at the margin of the atrophic retinochoroidal lesion. Optical coherence tomography revealed mixed clumping and atrophy of the retinal pigment epithelium (RPE) layer and thinning of the choriocapillaris layer. Fluorescence angiography revealed a window defect and blockage at the site of the lesion (the fluorescent material did not enter the lesion). The site of the window defect was in correlation with the atrophic RPE region. The site of the blockage at lesion also matched with the site of the regional pigment clumping. No definite leakage was observed. CONCLUSIONS: To the best of our knowledge, this is the first case of unilateral focal PPRCA reported from Korea.

4.An Ethnography on Fundamental Nursing Practice Class

PURPOSE: Objectives of this study were to identify and describe cultural meaning for nursing students in the class in fundamental nursing practice. METHODS: Data were collected from November 2016 to May 2017 included focus group interviews and participants observation. The key informants in this study included 23 nursing students divided into 3 focus groups who had taken the course in fundamental nursing practice in university U and university S in Kyungpook, and university D in Jeonnam. Interviews continued until no new information could be identified from transcripts. Data were analyzed using the taxonomic analysis method developed by Spradley. RESULTS: Based on the data acquired from the interviews, cultural domains in the class “fundamental nursing practice” were classified as ‘community oriented activities’, ‘learning and playing space’, ‘relationship of difference and discrimination’, ‘time for present and future’. CONCLUSION: The culture in the class “fundamental nursing practice” could be summarized as ‘non-standardized learning with team dynamics’. Also nursing students learned about a small society whose members are becoming nurses. Results indicate that it is critical for professors to understand students' values, beliefs and their attitude in order to aid in adjustment to class.

5.Nursing Student's Experiences on Nunchi in Clinical Practice

PURPOSE: This study aimed to explore and understand nursing students' nunchi experiences in clinical practice. METHODS: Participants were nursing students in the third and fourth years who had clinical experience. A total of 20 nursing students were divided into three focus groups based on their grade levels and gender. Data were collected from December 2017 to March 2018 through focus group interviews and were analyzed via Colaizzi's phenomenological methodology. RESULTS: The study results consisted of 5 theme clusters, 11 themes, and 24 subthemes. These clusters were: confusion about ambiguous behavioral standards, effort in following social norms, concern about getting hurt, physical and psychological exhaustion, and forming social attitudes. CONCLUSION: These results show that nunchi has positive and negative effects on the clinical practice of nursing students. We need to improve the causes of nunchi, which can have a negative impact on clinical practice.

6.Keyword Network Analysis and Topic Modeling of News Articles Related to Artificial Intelligence and Nursing

Purpose:
The purpose of this study was to identify the main keywords, network properties, and main topics of news articles related to artificial intelligence technology in the field of nursing.
Methods:
After collecting artificial intelligence-and nursing-related news articles published between January 1, 1991, and July 24, 2022, keywords were extracted via preprocessing. A total of 3,267 articles were searched, and 2,996 were used for the final analysis. Text network analysis and topic modeling were performed using NetMiner 4.4.
Results:
As a result of analyzing the frequency of appearance, the keywords used most frequently were education, medical robot, telecom, dementia, and the older adults living alone. Keyword network analysis revealed the following results: a density of 0.002, an average degree of 8.79, and an average distance of 2.43; the central keywords identified were ’education,’ ‘medical robot,’ and ‘fourth industry.’ Five topics were derived from news articles related to artificial intelligence and nursing: ‘Artificial intelligence nursing research and development in the health and medical field,’ ‘Education using artificial intelligence for children and youth care,’ ‘Nursing robot for older adults care,’ ‘Community care policy and artificial intelligence,’ and ‘Smart care technology in an aging society.’
Conclusion
The use of artificial intelligence may be helpful among the local community, older adult, children, and adolescents. In particular, health management using artificial intelligence is indispensable now that we are facing a super-aging society. In the future, studies on nursing intervention and development of nursing programs using artificial intelligence should be conducted.
7.Prescribing Patterns of Codeine among Children under Aged 12 in Korea.

OBJECTIVE: Codeine may result in death or respiratory depression in children, particularly who are rapid metabolizer of CYP2D6, therefore it should be used cautiously among children under 12 years of age. This study was to investigate the prescribing pattern of codeine among children according to the age group, prescribed diagnosis, type of medical service and medical specialties. METHOD: We used Korea Health Insurance Review and Assessment Service-National Patient Sample (HIRA-NPS) database. Study subjects included inpatients or outpatients, who were prescribed codeine between January, 1, 2011 and December, 31, 2011. Contraindicated use of codeine was defined as the use of codeine at least one times under aged 12. Age groups were sub-classified according to the <2 years, 2-4 years, 5-8 years, and 9-11 years. Frequently prescribed diagnosis (ICD-10), type of medical service, and medical specialties were also described among codeine users under aged 12. RESULTS: Codeine users were 6,411 inpatients (9,958 prescriptions), and 3,397 outpatients (6,258 prescriptions), respectively. Codeine prescription under 12 years of age were 2.1% (210 prescriptions) among inpatients, and 12.3% (776 prescriptions) among outpatients (p-value<0.05). Outpatient prescriptions of codeine under 12 aged were issued mostly from primary care clinics and frequent diagnosis were unspecified bronchopneumonia (51.6%), and vasomotor rhinitis (23.7%). CONCLUSION: This study found prescribing of codeine under 12 aged is common in outpatient and primary clinics. Nationwide and community-based efforts should be needed to reduce inappropriate prescribing among children.

8.Analysis of stromal Cells Developed from Cord Blood CD34+ Cells.

BACKGROUND: Cytokine-mediated ex vivo expansion has been proposed as a means of increasing the number of cord blood (CB) hematopoietic stem cells for transplantation. As well as stem cell number, stromal cells are necessary for functional maturation of hematopoiesis. The purpose of this study was to analyze the development of stromal cells during ex vivo expansion of CB CD34+ cells. METHODS: CD34+ cells were purified from CB by magnetic bead selection. The levels of of interleukin-3, interleukin-1 beta, interleukin-6, granulocyte macrophage-colony stimulating factor and tumor necrosis factor-alpha were measured in culture supernatants on 0, 1, 2, and 3 weeks, using ELISA techniques. CB CD34+ cells were expanded in Iscoves modified Dulbeccos medium in the presence of several cytokines. The expression of E-selectin, vascular cell adhesion molecule- 1, intercellular adhesion molecule- 1, platelet/endothelial cell adhesion molecule-1, von Willebrand factor, vimentin, and CD14 in newly developed stromal cells was examined by immunocytochemical method. Relevant extracellular matrix (ECM) proteins and proper cytokines were also assayed for the most suitable condition for expansion of stromal cells. RESULTS: Several cytokines were found to have been produced by CB CD34+ cells as well as bone marrow-derived CD34+ cells. During ex vivo expansion of CB CD34+ cells, stromal cells appeared in the culture by day 4 and expanded over the following 7- 10 days before being confluent by day 2 1. These cells expressed surface markers characteristic of cells of endothelial lineage. Furthermore, these stroaml cells also expanded effectively when treated with thrombopoietin+flt-3 ligand+stem cell factor+leukemia inhibitory factor or 0. 1% poly-L-lysine-coated wells. CONCLUSION: Stromal cells were developed during ex vivo expansion of CB CD34+ cells and that this development could be enhanced further by treating the stromal cells with cytokines or ECM.

9.Antibody Levels and Opsonophagocytic Function to S. pneumoniae 19F Capsular Polysaccharide.

PURPOSE: The increasing prominence of pneumococcal infections and of antimicrobial resistance has emphasized the need for a effective vaccine to reduce the risk of pneumococcal disease in childhood. To study the antibody response of pneumococcal vaccine, we compared the concentration and opsonophagocytic capacity of antibodies to the capsular polysaccharide of S. pneumoniae serotype 19F in sera from healthy adults after immunization with 23-valent pneumococcal polysaccharide(PS) vaccine. METHODS: Thirty healthy adults were immunized with pneumococcal 23-valent PS vaccine. One month after vaccination, the amounts of total, IgG, IgG1, and IgG2 antibody were determined by enzyme-linked immunosorbent assay(ELISA). To investigate the function of anti-19F antibodies, the opsonization titer was determined by opsonophagocytic assay. RESULTS: The mean antibody levels of total immunoglobulin and IgG specific to 19F serotype were 21.9mg/mL and 16.2mg/mL, respectively. Most of the IgG anti-PS antibodies were IgG2 isotype. The mean value of opsonization titer was 229.1. The opsonization titer against the 19F serotype was strongly correlated with total immunoglobulin and IgG anti-19F antibody concentration. And IgG2 concentration was correlated more with opsonization titer than with IgG1. CONCLUSION: Antibody level may be of general use in predicting vaccine-induced protection among adults for 19F pneumococcus. This study should be extended to other serotypes and to assess the antibody response to new protein conjugate pneumococcal vaccine in young children.

10.Clinical Approach of Ultrasonography in the Diagnosis of Intussusception in Infant and Children.

Thirty consecutive patient seen in a pediatric out patient and emergency department, in whom the diagnosis of intussusception was considered, had an ultrasound examination of the abdomen before the barium enema. The peak age was 5~12 month (range 4~34 month). Intussusception was detected by ultrasonography in all 25 cases proved by barium enema (sensitivity 100%). Normal findings on ultrasonography correlated with a negative barium enema results in 5 of 5 cases (negative predictive value=100%). No intussusception was missed by ultrasonography. To determine which patient would most benefit from ultrasonography, we divided patients into either a high risk group (100% intussusception) or a low risk group (37.5% intussusception) on the basis of clinical symptoms. We conclude that ultrasonography can be used as a rapid sensitive screening procedure in the diagnosis or exclusion of childhood intussusception. Probability of surgical reduction was associated with size of of total thickness and external rim thickness.
