2.Cardiac Fibroma of the Ventricular Septum: A case report.

This is an autopsy case of a 6 month old girl who suddenly died of respiratory distress during sleep. She had suffered from mild but frequent episodes of common cold and was treated for eczema for several days. At autopsy, the heart was enlarged and weighed 100 gm. A firm and gray-white tumor, measuring 4.5 x 3.8 x 2.8 cm, was located in the interventricular septum and encroached upon the wall of left ventricle. The mass was well demarcated but was not encapsulated. Neither necrosis nor calcification was present. Microscopically the tumor was composed of haphazardly arranged bundles of collagen fibers and fibroblasts. Myocardial cells are intermingled with the fibroblasts at the margin of the tumor. Massive edema of the lung and congestion of the liver and spleen were pronounced.
3.Relationship between Physical Illness and Depression in North Korean Defectors.

OBJECTIVES: The present study aims to investigate the effects of physical illness on depression in North Korean Defectors. METHODS: One hundred forty-four North Korean Defectors(20 males, 124 females) and 376 South Koreans 133 males, 243 females) in Incheon Metropolitan areas participated the present study. Face-to-face interview was conducted for demographic information including presence of physical illnesses. To investigate depressive symptoms, all participants were required to complete the Center for Epidemiologic Studies-Depression scale (CES-D). RESULTS: After controlling gender, age, marital status, educational year, employment status and physical illness, North Korean Defectors showed higher CES-D score than South Koreans(beta=0.449, p<0.001). Within North Korean Defectors, the presence of physical illnesses independently was related to higher CES-D score(beta=0.243, p<0.001). However, within South Koreans, the presence of physical illnesses did not significantly predict CES-D score. North Korean Defectors with physical illness have higher CES-D score than North Korean Defectors without physical illness(26.8+/-13.8 versus 19.7+/-12.7). However, there was no significant differences of CES-D score between South Koreans with physical illness and South Koreans without physical illness(10.3+/-9.8 versus 9.3+/-8.8). CONCLUSION: Compared to South Koreans, North Korean Defector showed higher depressive symptoms independently from gender, age, education, employment, marriage. In addition, only North Korean Defectors showed the relationship between depression and physical illness. Our study suggests that depression should be assessed when North Korean Defectors have physical illness.

7.Frequency of Irritable Bowel Syndrome, Perceived Stress, and Mental Health Among Women.

PURPOSE: This study was performed to examine the frequency of irritable bowel syndrome(IBS), perceived stress, and mental health among community-dwelling women. METHODS: This study conducted a survey to collect data on 869 women in Seoul and Gyeonggi-do. The measurements included the Rome III criteria(to diagnose IBS), the Global Assessment Recent Stress(GARS scale to evaluate perceived stress), and symptom check list 90R(SCL-90R to evaluate psychological distress). Data were analyzed using SPSS 15.0 program by frequency, chi-square -tests, t-test, and analysis of variance(ANOVA). RESULTS: The frequency of IBS in women was 15.1%. Eighty-six women(65.6%) had mixed constipation and diarrhea subtype of IBS. Compared to the women without IBS, those with IBS reported significantly higher scores on GARS, all subscales of GARS, SCL-90R, all subscales of SCL-90R, and the global severity index(GSI). No significant differences were observed among stress, mental health, and the IBS subtypes. CONCLUSION: Nursing interventions targeting women with IBS should be developed in order to reduce the associated stress and psychological distress.

8.MR Findings of Knee Injuries in Skiing: Relation with the Mechanism of Injury.

PURPOSE: To evaluate the MR findings of knee injuries in skiing and to explain the mechanism of injury with MR findings. MATERIALS AND METHODS: We reviewed MR findings of 18 patients with history of knee injuries in skiing. The MR images were evaluated retrospectively to identify the ligament injuries, bone lesions and meniscal injuries. RESULTS: Ligament injuries were seen in 16 patients, bone contusions in 16 patients, meniscal lesions in two patients. The most common group of injury was anterior cruciate ligament (ACL) and roedial collateral ligament (MCL) injuries with bone contusion on posterior lip of the lateral tibial plateau (LTP). The second common group of injury was isolated ACL injury with bone contusions on the lateral fernoral condyle (LFC) and posterior lip of the LTP. We considered that the mechanism of injury of the former group may be correlated with the valgus torque with secondary anterior displacement of the tibia and the latter group may be correlated with the pivot shift phenomenon. CONCLUSION: MR may play an important role in the diagnosis of knee injuries in skiing and its findings may explain the mechanism of injury.

9.Diagnostic Efficacy of Fine Needle Aspiration Cytology, Mammography, and Ultrasonography for a Palpable Breast Mass.

PURPOSE: This study was done to determine the diagnostic efficacy of clinical examination, fine needle aspiration (FNA) cytology, mammography, and ultrasonography in palpable breast mass. METHODS: We performed 248 FNA cytologies for a palpable breast mass, among which 106 cases were histologically examined during the period from Jan. 1994 to Dec. 1997 at the Department of Surgery, Chonbuk National University Hospital. Among the 106 patients, mammography was done for 96 patients, and ultrasono graphy for 73 patients. RESULTS: The main clinical symptom was a palpable mass in the breast at the time of visitation. Clinical diagnosis based on symptoms and physical examination had a sensitivity of 96.9% and a specificity of 57.5%. Based on a definite histologic diagnosis, the sensitivity of FNA cytology was 96.9%, its specificity was 91.7%, and its diagnostic accuracy 93.5%. 86.1% of the cytologically malignant cases proved to be malignant histologically, and 100% of the cytologically benign cases turned out histologically benign. Mammography had a sensitivity of 89.3%, a specificity of 84%, and a diagnostic accuracy of 86.8%. Ultrasonography showed a 93.5% sensitivity, a 51.4% specificity, and a 74.6% diagnostic accuracy. CONCLUSION: FNA cytology was a reliable and safe diagnostic method compared to physical examination, mammography, and ultrasonography. FNA cytology of a palpable breast mass should be the diagnostic procedure of choice for those patients classified clinically as probably benign or malignant.

10.The Distributional Changes in the First-Visit Psychiatric Child and Adolescent Outpatients at a University Hospital over a Ten-Year Period.

OBJECTIVES: The purpose of this study was to investigate changes in child and adolescent outpatients at a university hospital in Daegu from 2004 to 2013. METHODS: The subjects were first-visit patients under 18 years old, who visited Dongsan Medical Center, Keimyung University, from January 1, 2004 to December 31, 2013. Computerized medical records of 2,738 patients (male 1,906, female 832) were reviewed. RESULTS: The ratio of male to female was 2.3 : 1 in the period of study. The most prevalent age category was 7-9 years. The mean age was 10.12+/-4.68 years (9.84+/-4.59 years in males, 10.76+/-4.81 years in females). The ratio of child and adolescent patients to total outpatients was 27.0%. The most common diagnostic category was the attention-deficit hyperactivity disorder (ADHD) group, followed by the mental retardation (MR) group. In males, the ADHD, MR, communication disorder groups were more prevalent, but in females, the MR, ADHD, depressive disorder groups were more prevalent. CONCLUSION: The female to male ratio and total mean age were on the rise. The ADHD group was the most prevalent and the depressive disorder group was also on the rise in this period.
