1.Reviews of Pay-for-Performance and Suggestion for Korean Value Incentive Program.

The effort to measure and improve the quality of healthcare is a common health policy issue worldwide. Korean Value Incentive Programme is one of that effort, but some concerns exist. Compared to pay for performance program in other countries, it measures healthcare quality with relatively narrow performance domain using a small number of clinical indicators. It was designed without involving hospitals and other key stakeholder, and program participation was mandated. Highest and lowest performers get bonus and penalty using relative ranking. As a suggestion for development, the direction for quality management at the national level should be given first. Therefore the philosophy or strategy for quality improvement should be reflected to the program. And various domains and indicators of healthcare quality should be developed with active communication with healthcare providers. The evaluation method is necessary to be changed to provide achievable goal to the healthcare providers and attract quality improvement.

2.Dexmedetomidine Use in Patients with 33degrees C Targeted Temperature Management: Focus on Bradycardia as an Adverse Effect.

BACKGROUND: This study aimed to investigate bradycardia as an adverse effect after administration of dexmedetomidine during 33degrees C target temperature management. METHODS: A retrospective study was conducted on patients who underwent 33degrees C target temperature management in the emergency department during a 49-month study period. We collected data including age, sex, weight, diagnosis, bradycardia occurrence, target temperature management duration, sedative drug, and several clinical and laboratory results. We conducted logistic regression for an analysis of factors associated with bradycardia. RESULTS: A total of 68 patients were selected. Among them, 39 (57.4%) showed bradycardia, and 56 (82.4%) were treated with dexmedetomidine. The odds ratio for bradycardia in the carbon monoxide poisoning group compared to the cardiac arrest group and in patients with higher body weight were 7.448 (95% confidence interval [CI] 1.834-30.244, p = 0.005) and 1.058 (95% CI 1.002-1.123, p = 0.044), respectively. In the bradycardia with dexmedetomidine group, the infusion rate of dexmedetomidine was 0.41 +/- 0.15 microg/kg/h. Decisions of charged doctor's were 1) slowing infusion rate and 2) stopping infusion or administering atropine for bradycardia. No cases required cardiac pacing or worsened to asystole. CONCLUSIONS: Despite the frequent occurrence of bradycardia after administration of dexmedetomidine during 33degrees C target temperature management, bradycardia was completely recovered after reducing infusion rate or stopping infusion. However, reducing the infusion rate of dexmedetomidine lower than the standard maintenance dose could be necessary to prevent bradycardia from developing in patients with higher body weight or carbon monoxide poisoning during 33degrees C targeted temperature management.

3.Dermatophagoides farinae specific IgG-subclass antibody in allergic children.

For determining the distribution of specific IgE and IgG-subclass antibodies and their role in allergic disorders, we measured the serum levels of specific IgG-subclass and IgE antibodies to Dermatophagoides farinae (Df) on 60 atopic children, aged between 8 to 12. All of them had positive skin test to Df, and was grouped into three: children with asthma only (group 1), with rhinitis (group 2), and with them together (group 3). Every data were compared with that of 47 nomal children. ELISA (for IgG and IgG-subclass) and RIA (for IgE) were used. The results are as following. 1) Specific IgE-antibody is significantly higher in all three allergic groups, compared with normal control group. But there was no significant difference between three allergy groups. 2) Specific IgG, IgG1, IgG2 IgG4-antibodies were higher than those in normal control group, but IgG3 was not significant. IgG1 and IgG2 were elevated particularly in patients with asthma and rhintis, but IgG4 was elevated in asthma group. 3) The linear correlation between specific IgE and IgG1-antibody was significant (r=0.286), but IgG4 did not have this correlation with IgE. In conclusion, the levels of specific IgE is essential for the diagnosis of allergies, and IgG1 and IgG4 seem to participate in allergic reactions with a different mechanism.

4.Impact on Marital Stability of Alcohol Use in International Married Korean Men in a Rural Area and the Mediating Effects of Marital Communication and Social Support.

PURPOSE: This study was designed to identify whether alcohol use predicts marital stability and to investigate the mediating effects of marital communication and social support in international married Korean men who lived in a rural area. METHODS: The participants were 200 of the international married men. The data were collected during June, 2010. The instruments were Alcohol Use Disorders Identification Test (AUDIT), Marital Instability (MI), Marital Communication (MC), Social Support (SS). Data were analyzed using descriptive statistics, Pearson correlation coefficients, and stepwise multiple regression. RESULTS: The mean score for AUDIT was 9.6, and alcohol use was negatively correlated with marital stability, marital communication and social support. We also found that marital communication fully mediated the association between alcohol use and marital stability, while social support did not mediate this association. CONCLUSION: The outcomes of this study indicates a need to develop intervention strategies and education programs that include emphasis on moderate drinking and communication skills for couples in international marriages.

6.The Effects of Emotional Labor and Grit on Job Satisfaction among Pediatric Nurses

Purpose:
This study aimed to identify the relationships among emotional labor, grit, and job satisfaction of pediatric nurses and factors affecting job satisfaction.
Methods:
A cross-sectional study was conducted on 137 pediatric nurses working in a general hospital in Seoul. Data were collected from January 1, 2022 to January 31. The data were analyzed using SPSS/WIN version 28.0.1.1.
Results:
The mean score of job satisfaction was 3.63±0.43 (out of 5), emotional labor was 3.70±0.39 (out of 5), and grit was 2.90±0.30 (out of 4). Job satisfaction had positively correlated with emotional labor (r=.27, p=.002) and grit (r=.65, p<.001). The regression analyses showed subcategories of grit had a significant influence on job satisfaction. The factors significantly affecting job satisfaction were persistence to achieve long-term goals (β=.39, p<.001), patient-oriented intrinsic motivation (β=.30, p<.001), and passion to become a nursing professional (β=.21, p=.004).
Conclusion
These results suggested a need to increase grit for improving the job satisfaction. Furthermore, it would be necessary to develop a diverse program to increase job satisfaction for pediatric nurses by improving grit.
7.Acute megakaryoblastic leukemia in children.

We analyzed the clinical and laboratory features of ten children with acute megakaryoblastic lukemia (M7)and compared the findings with those reported in the literature. The diagnosis was supprted by ultrastructural examination of platelet peroxidase or immunophenotyping for glycoprotein IIb/IIIa. Of the ten children, five were girls and five were boys. The median age at diagnosis was 13 months. Two patients had prominent myelofibrosis and one patient had Down syndrome. Nine patients were treatd with low-dose cytosine arabinoside (10mg/m2)administered intravenously, or subcutaneously, or intramuscularly, twice daily in 21 day courses. Seven patients achieved hematologic response and three patients are alive without evidence of disease. The 4 year event free survival rate was30.0%. It is our impression that the prevalence of acute megakaryoblastic leukemia has been under-estimated, and low-dose cytosine arabinoside treatment may be of value in its management. This approach may be particularily useful in hospitals with scarce well-equipped facilities, since this protocol does not induce profound marrow hypoplasia and intensive supportive measures are not required as they would be with the use of more aggressive drug combination.

8.Echocardiographic Observation in Patients with Mitral Valve Prolapse.

Twenty-one cases of mitral valve prolapse (MVP) diagnosed by M-mode echocardiograms were studied in regard to the underlying or associated conditions, types of MVP, and dimensions of the cardiac structures and parameters reflecting cardiac performance determined by echocardiograms. In 9 cases, MVP was thought to be idiopathic origin. Associated conditions in the remainder were 5 cases of congenital heart disease, 2 cases of rheumatic myocarditis, 1 case each of aortic valve disease, ischemic heart disease and hyperthyroidism, and 2 cases of undiagnosed cardiac conditions. There were 2 cases of buckling type of MVP and in the remainder hammock type. In these 2 patients with hammock type of MVP, mitral valve echoes were seen in the left atrium during systole in 1 case, and diastolic anterior displacement of posterior mitral leaflet in the other. In 9 cases of idiopathic MVP, cardiac dimensions and parameters reflecting cardiac performance as determined from echocardiograms showed no significant differences from those of 27 normal controls.

9.Testicular Involvement in Childhood Acute Lymphoblastic Leukemia.

BACKGROUND: The testes are one of the most common extramedullary sites of relapse in boys with acute lymphoblastic leukemia(ALL). The reported incidence of isolated testicular relapse varies from 3 to 40%. If these patients are treated exclusively with testicular irradialion, a systemic relapse occurs within a few months. Recently, the use of intensive chemotherapy and testicular irradiation improved the survival rate for boys with testicular leukemia. So, we performed this study to identify clinical manifestations, disease free survival and prognostic factors of testicular leukemia in children. METHODS: We reviewed 33 patients of testicular leukemia among total 410 boys with ALL diagnosed at the Department of Pediatrics, Seoul National University Children's Hospital from Jan. 1970 to Aug. 1996. Testicular leukemia was confirmed by testicular biopsy in all 33 patients. These patients were treated with combined local testicular irradiation(2,400~2,500 cGy/8~12fractions) and systemic chemotherapy. Two patients, in whom testicular relapse was diagnosed before 1979, unilateral orchiectomy of the involved site and testicular irradiation of the opposite site were performed. Probability estimates of disease free survival (DFS) were calculated by the method of Kaplan and Meier, and the relationship of prognostic factors to DFS was compared using the chi-square test in survival analysis. RESULTS: In 410 boys with ALL, testicular leukemia occurred in 33 patients(8%). Of 33 patients, 6 patients presented with testicular involvement at initial diagnosis, 16 patients had testicular relapse while still receiving chemotherapy and 11 patients had testicular relapse 3 to 57 months(median : 15 months) after cessation of chemotherapy. The median age of 33 patients was 7.4 yrs(9 months~18 yrs) and median WBC count 7,600/ L(2,700~270,000/L). All patients presented with painless testicular enlargement and testicular leukemia was confirmed by testicular biopsy. Among 33 patients, 2 had prior CNS relapse and 11 had concomitant bone marrow and/or CNS relapse. Twenty nine patients were treated with combined local testicular irradiation and systemic chemotherapy. Eleven had second relapse(6 bone marrow, 3 CNS, 2 opposite testis). Seventeen have been followed until now: 6 patients on chemotherapy and 11 patients(37.9%) in complete remission for 48.5+/-22.3 months(19~86 months). The 3 year DFS for 29 patients was 55.3%+/-10.1%. The following prognostic factors showed no significant association with DFS in testicular relapse : age and WBC count at initial diagnosis, age at testicular relapse, and concomitant relapse. Whether testicular relapse occurred on initial therapy or off initial therapy has prognostic value in predicting DFS. The 3 year DFS for boys with testicular relapse on and off initial therapy were 40.0%+/-12.9% and 78.8%+/-13.4%, respectively(P: 0.046). CONCLUSION: With the use of chemotherapy and testicular irradiation, prolonged second re mission can be achieved in many patients with testicular leukemia. The patients with testicular relapse off initial therapy fared significantly better than patients on therapy. So, to improve the DFS for boys with testicular leukemia, a better understanding of its biology and prognostic factors is needed.

10.Neural Substrates of Aphasia in Acute Left Hemispheric Stroke Using VoxelBased Lesion-symptom Brain Mapping

It is unclear how these brain lesions fit into the language processing in acute stroke. In this study, we aimed to investigate the neuroanatomical lesion related to language processing in acute stage of stroke patients using voxel-based lesion-symptom mapping (VLSM). 73 acute first-ever post-stroke patients were enrolled in this retrospective study, who had undertaken brain magnetic resonance imaging (MRI) and Korean version of the Western Aphasia Test within 1 month from onset. Each voxel was compared with aphasia quotient and subtest scores as dependent variables using VLSM. The aphasia group showed significantly much more involvement of extra-nuclear area, insula, inferior frontal gyrus and superior temporal gyrus compared to non-aphasia group. The deficit of spontaneous speech domain was associated with the inferior parietal lobule, inferior and middle frontal gyrus and insula.The insular cortex, inferior parietal lobule, inferior frontal gyrus, middle frontal gyrus and superior temporal gyrus were related to deficit of comprehension. The inferior parietal lobule, insula, precentral gyrus, inferior frontal gyrus were related to the deficit of repetition. The deficit of naming was related to inferior parietal lobule, insula and inferior frontal gyrus. In conclusion, VLSM from early MRI imaging study after stroke may be useful to understand the language process network and establish early rehabilitation strategies after stroke.
