1.Agenesis of the Gallbladder.

Congenital absence of the gallbladder is a rare anomaly and an interesting subject. The overall incidence of gallbladder agenesis is said to approximately 0.035% to 0.065%. It is extremely difficult to make the correct diagnosis of gallbladder agenesis preoperatively in symptomatic patients because its clinical and radiological features are like those of cholecystitis, cholelithiasis, or choledocholithiasis in patients with anatomically normal biliary tracts. Gallbladder agenesis is suspected when the surgeon or the pathologist has failed to identify the gallbladder in its usual position but ectopic location or necrosis of the gallbladder from any causes should be ruled out. Recently, two cases of gallbladder agenesis were encountered in adult patients. We summarize our experience and give a brief review of the literature.

3.The comparison of QTc interval change on the ECG during laparoscopic cholecystectomy under inhalation and total intravenous anesthesia.

BACKGROUND: There can be changes in the cardiac function during laparoscopic cholecystectomy. In this study, QTc interval changes were compared between total intravenous anesthesia (TIVA) group and inhalation anesthesia group during laparoscopic cholecystectomy. METHODS: The study was conducted on adult patients, ages ranging from 20 to 65 years old, and classified as the American Society of Anesthesiologists physical status I or II. At random, the patients were divided into group 1 (TIVA, n = 20) and group 2 (inhalation anesthesia, n = 19). Group 1 patients were induced and maintained with continuous infusion of remifentanil and propofol using a target controlled infusion device. Patients in group 2 were induced with sevoflurane and N2O using mask ventilation, and then anesthesia was maintained with sevoflurane and N2O. The QTc interval, heart rate and mean arterial pressure were measured prior to induction, immediately following intubation, 10 minutes following intubation, following CO2 inflation, immediately following head-up position, 10 minutes following head-up position, following CO2 deflation-supine position respectively. RESULTS: In group 1, the ECG sampling showed no prolongation in the QTc intervals at all measured points. In group 2, QTc interval was significantly longer at all other measured points compared to prior to induction (P < 0.05). Except prior to induction, QTc intervals were significantly longer at all other measure points in group 2 compared to those in group 1 (P < 0.05). CONCLUSIONS: There was no QTc interval prolongation under the TIVA using propofol and remifentanil during laparoscopic cholecystectomy.

4.Thoracic Pedicle Screw Insertion in Scoliosis Using Posteroanterior C-arm rotation Method.

STUDY DESIGN: A prospective study of the accuracy of thoracic pedicle screws inserted in scoliotic patients. OBJECTIVES: To evaluate and present a practical, safe and accurate method for thoracic pedicle screw insertion in the surgical treatment of scoliosis using the posteroanterior c-arm fluoroscopy rotating method. SUMMARY OF LITERATURE REVIEW: Previous studies have emphasized the clinical importance, yet difficulty, of accurate thoracic pedicle screw insertion in scoliotic patients. Three-dimensional alterations in the pedicle orientation of scoliotic patients makes the accurate insertion challenging. No reports exist on the accuracy and benefits of posteroanterior c-arm fluoroscopy, which is rotated to allow visualization from en face, in real patients. MATERIALS AND METHODS: A total of 350 thoracic pedicle screws were inserted in 29 patients, including 24 with idiopathic scoliosis, using the posteroanterior (PA) c-arm rotation method. The smallest patient weighed 14 kg, and the next smallest 17 kg. The average preoperative curve was 60.9 degrees(range, 45 degrees~101 degrees). CT scans were taken, postoperatively, in the transverse and sagittal sections to evaluate the pedicle screw placement. RESULTS: The mean preoperative curve of 60.9 degrees was corrected to 15.4 degrees(range, 3 degrees~45 degrees) in the coronal plane, a correction of 74.7%. A mean of 12.1 thoracic screws were inserted per patient. On analysis of the postoperative CT scans, 39(11.1%) of the 350 screws penetrated the medial or lateral pedicle cortices, 8(2.3%) into the medial cortex and 31(8.9%) into the lateral cortex, by mean distances of 3.3 and 3.6 mm, respectively. No screws penetrated the inferior or superior cortices in the sagittal plane, but 16(4.6%) penetrated the anterior cortex. No neurological or vascular complications were encountered, and none of the screws required subsequent replacement. CONCLUSIONS: Thoracic pedicle screw insertion in scoliotic patients, using a posteroanterior c-arm rotation method, allows the en face visualization of both pedicles by rotating the c-arm to compensate for rotational deformity, which makes it a practical, simple and safe method.

5.Scoliosis Correction with Thoracic Pedicle Screws: Posteroanterior C-arm Rotation Method.

Purpose: This study evaluated a practical, safe and accurate method of thoracic pedicle screw insertion for the surgical treatment of scoliosis using the posteroanterior C-arm fluoroscopy rotation method. Materials and Methods: A total of 611 thoracic pedicle screws were inserted in 45 patients using the posteroanterior (PA) C-arm rotation method. CT scans were taken postoperatively in the transverse and sagittal sections to evaluate the pedicle screw placement. Results: A mean preoperative curve of 57.7o was corrected to 17.1o (range, 3o-45o) in the coronal plane. The postoperative CT scans revealed that 10 screws (1.6%) had penetrated the medial cortex by a mean distance of 3.0 mm and 56 screws (9.2%) penetrated the lateral cortex by a mean distance of 3.5 mm. No screw penetrated the inferior or superior cortex in the sagittal plane. However, 21 screws (3.4%) penetrated the anterior cortex. No neurological or vascular complications were encountered, and none of the screws required replacement. Conclusion: Thoracic pedicle screw insertion in scoliosis patients using the posteroanterior C-arm rotation method is a practical, simple and safe technique that allows the en face visualization of both pedicles by rotating the C-arm to compensate for the rotational deformity.

6.Comparison of the Utility of dnaJ and 16S rDNA Sequences for Identification of Clinical Isolates of Vibrio Species

BACKGROUND: Among the many Vibrio species that can cause infections in humans, several species can cause a fatal outcome. Therefore, accurate identification of Vibrio species is very important. Since some species show atypical phenotypic features, selecting an appropriate molecular method is necessary to avoid misdiagnosis. METHODS: Vibrio clinical isolates (N=53) and reference strains (N=8) were used in this study. We analyzed the following sequences for identification: dnaJ gene, 16S rDNA, gyrase B (gyrB) V. vulnificus-specific sequence, gyrB V. navarrensis-specific sequence, and V. vulnificus hemolysin gene PCR (Vvh PCR). We performed phylogenetic analysis of the 16S rDNA, dnaJ, and gyrB sequences. Final identification was based on the combined results of all tests described above. Concordance of the 16S rDNA and dnaJ sequence analysis was measured using the Chi-square test. RESULTS: The 61 Vibrio strains were identified as follows, in descending order: V. vulnificus (78.69%), V. parahaemolyticus (6.56%), V. navarrensis (4.92%), V. mimicus (1.64%), V. cholera (1.64%), V. furnissii (1.64%), V. alginolyticus (1.64%), and Grimontia hollisae (1.64%). The accuracy rates of the dnaJ gene and 16S rDNA sequence for identification were 91.80% and 86.89%, respectively. The 16S rDNA and dnaJ sequences showed a concordance rate of 0.45, which indicates moderate agreement. CONCLUSIONS: Our results suggest that analysis of the dnaJ sequence may be a useful method for the identification of clinical isolates of Vibrio species, especially for distinguishing between closely related Vibrio species.

Background:
Conventional diagnostic approaches for adrenal tumors require multi-step processes, including imaging studies and dynamic hormone tests. Therefore, this study aimed to discriminate adrenal tumors from a single blood sample based on the combination of liquid chromatography-mass spectrometry (LC-MS) and machine learning algorithms in serum profiling of adrenal steroids.
Methods:
The LC-MS-based steroid profiling was applied to serum samples obtained from patients with nonfunctioning adenoma (NFA, n=73), Cushing’s syndrome (CS, n=30), and primary aldosteronism (PA, n=40) in a prospective multicenter study of adrenal disease. The decision tree (DT), random forest (RF), and extreme gradient boost (XGBoost) were performed to categorize the subtypes of adrenal tumors.
Results:
The CS group showed higher serum levels of 11-deoxycortisol than the NFA group, and increased levels of tetrahydrocortisone (THE), 20α-dihydrocortisol, and 6β-hydroxycortisol were found in the PA group. However, the CS group showed lower levels of dehydroepiandrosterone (DHEA) and its sulfate derivative (DHEA-S) than both the NFA and PA groups. Patients with PA expressed higher serum 18-hydroxycortisol and DHEA but lower THE than NFA patients. The balanced accuracies of DT, RF, and XGBoost for classifying each type were 78%, 96%, and 97%, respectively. In receiver operating characteristics (ROC) analysis for CS, XGBoost, and RF showed a significantly greater diagnostic power than the DT. However, in ROC analysis for PA, only RF exhibited better diagnostic performance than DT.
Conclusion
The combination of LC-MS-based steroid profiling with machine learning algorithms could be a promising one-step diagnostic approach for the classification of adrenal tumor subtypes.

Korean Society of Thyroid-Head and Neck Surgery appointed a Task Force to develop clinical practice guidelines for the surgical treatment of laryngeal cancer. This Task Force conducted a systematic search of the EMBASE, MEDLINE, Cochrane Library, and KoreaMed databases to identify relevant articles, using search terms selected according to the key questions. Evidence-based recommendations were then created on the basis of these articles. An external expert review and Delphi questionnaire were applied to reach consensus regarding the recommendations. The resulting guidelines focus on the surgical treatment of laryngeal cancer with the assumption that surgery is the selected treatment modality after a multidisciplinary discussion in any context. These guidelines do not, therefore, address non-surgical treatment such as radiation therapy or chemotherapy. The committee developed 62 evidence-based recommendations in 32 categories intended to assist clinicians during management of patients with laryngeal cancer and patients with laryngeal cancer, and counselors and health policy-makers.

The aim of this study was to investigate the efficacy of simvastatin to improved lipid profiles in hypercholesterolemic Korean patients. METHODS: From 25 hospitals in Korea, 478 hypercholesterolemic patients were enrolled from November 1996 to April 1998. The inclusion criteria was hypercholesterolemia over 240 mg/dl after diet therapy for 1 month or hypercholesterolemia over 220 mg/dl in patients with definite evidence of ischemic heart disease. Simvastatin 10mg was started and doubled up to 40mg if total cholesterol level remained higher than 200 mg/dl at monthly check. Of 478 subjects, 344 patients in whom study protocol was not violated were analyzed. RESULTS: Male to female ratio was 27:73 and 47% of the subjects were in 6th decade. Hypertension, coronary artery disease, and diabetes mellitus were present in 30, 10, and 4% of the subjects. Baseline lipid profile (mean of total cholesterol-LDL-HDL-triglyceride mg/dl) was 274-185-52-188. The dose of simvastatin for 3 months was 10/10/10mg in 61% of subjects, 10/20/20mg in 21%, 10/10/20mg in 7%, and 10/20/40mg in 12%. The change of total cholesterol level(before-4wk-8wk-12wk-withdrawal 4wk) was 274-209- 205-198-250, and the maximal reduction rate was 27%. The change of LDL-cholesterol was 185-123-116-110-159, with maximal reduction rate 39%. The change of HDL-cholesterol was 52-54-56-55-54, with maximal increase rate 9%. The change of tryglyceride was 188-161- 164-162-189, with maximal reduction rate 15%. The value before/after treatment of ApoA1, ApoB, and Lp(a) was 129/129, 138/83, and 9.3/10.7, respectively. The level of LDL-cholesterol at the end of treatment was below 100mg/dl in 36% of subjects, 100-130 in 45%, 130-160 in 16%, and over 160mg/dl in 4%. The reduction rate of LDL-cholesterol was different between subjects whose LDL decreased below 100 and those whose LDL did not decrease below 130mg/dl, which suggests the existence of the individual difference of responsiveness to simvastatin. There were only 3 subjects (0.9%) who showed increase of liver enzyme over 3 times as the upper normal limit. Conclusion: Simvastatin is effective in improving lipid profiles in hypercholesterolemic Korean patients without serious side effects.
