2.Pilot Study for the Assessment of Physical Fitness among Male Workers in a TV Component Manufacturing Factory.

In a TV component manufacturing factory, 102 male workers aged 20~39 years old were participated in testing for physical fitness. At the same time, worker's periodic health examination was done. Test battery for physical fitness include grip strength, trunk flexing, standing long jump, side step, single leg balance with eye close, push ups and Harvard step test. As a result of testing for physical fitness, synthetically, there is no difference between manufacturing workers and officers. By bioelectrical impedance test, it means a declining tendency to all 7 factors in the obese workers, and so, it is important for obese workers not only to promote physical fitness but also to promote health. Excluding grip strength and single leg balance with eye close, 5 fitness factors are negatively associated with degree of diastolic Blood pressure, but it is statistically not significant. And levels of SGOT & SGPT have no association with physical fitness factors.

3.Pilot Study for the Assessment of Work Ability of Manufacturing Workers and Managerial Workers.

This study was carried out to assess work ability of manufacturing workers and managerial workers. Subjects were 446 manufacturing workers and 278 managerial workers employed in Gumi industrial complex and the observation period was for 1 month(February to March), 1995. In this study, as a questionnaire for the assessment of work ability, we used the work Ability Index(WAI) which was developed by Institute of Occupational Health in Finland. Major findings obtained from this study were as follows; WAI was poor in lower economic condition, shift work, irregular diet group(p < 0.05). There were no evident trends of WAI in sex, educational level, obesity index, exercise, smoking, and alcohol drinking. In the groups of sensitive to perception of stress, other industrial classification except textile and electronics, the more than 9 hours work, the evident trends of poor WAl were observed(p<0.01). In the groups of 30-39 years old, 40-49 years old, unmarried, high school, shift work, standard body weight, and regular diet, WAl between manufacturing workers and managerial workers was significantly different(p<0.01) and WAI of managerial worker was worse than that of manufacturing worker.

4.Mortality, prognostic factor and cause of death of acute myocardial infarction in Korean patients: single center experience.

BACKGROUND: The number of patients suffering from acute myocardial infarction is on the increase in Korea due to the westernization of life style. Recent improvement of therapeutic stratigies have shown early mortality benefits in acute myocardial infarction. But we don't have data how many patients died and what's the cause of death in these patients. This study aimed to find out the mortality rate, cause of death and it's relevant prognostic factors of myocardial infarction (MI) patients who admitted alive, and to construct a database which will be used to develop a risk stratification strategy for the implementation of new preventive therapeutic modalities, such as implantable cardioverter-defibrillator (ICD). METHODS: Seven hundred and forty two MI patients admitted to our hospital from March, 1999 to August, 2002 were included in this study. The risk factors and survivals were evaluated by medical record searching and telephone survey in these patients. RESULTS: The average age was 64 years-old and 67% was male. During the mean follow up 20.7+/-15.4 months, total 105 cardiac death (14.2%) was occurred and cumulative mortality rate at 1 year and 2 year was 5.69% and 10.80%, respectively. Of the total 129 death, in-hospital death was 68 (cardiac death 48, non-cardiac death 20) and out of hospital death was 60 (cardiac death 57, non-cardiac death 4). When it comes to cause of death, most common cause of cardiac death was malignant arrhythmia. The proportion of malignant arrhythmia in cardiac death was 81.3% and 72% of in-hospital and out of hospital death, respectively. Multivariate analysis showed that old age, low LV ejection fraction and no percutaneous coronary intervention (PCI) treatment were independent risk factors for cardiac mortality. CONCLUSIONS: Myocardial infarction shows still high mortality rate despite the recent development of therapeutic strategy. As post-MI patients with low LVEF or no PCI shows high mortality, the early reperfusion therapy should be encouraged. Additionally, because malignant arrhythmia was one of the most in cause of cardiac death, ICD therapy to prevent sudden cardiac death should be considered in an active manner.

5.Induction Chemotherapy Followed by Concurrent Chemoradiotherapy in Locoregional Esophageal Cancer.

PURPOSE: The object of this study is to evaluate the efficacy and toxicity of induction chemotherapy followed by concomitant chemoradiotherapy in locoregional esophageal cancer. MATERIALS AND METHODS: Between December 1992 and December 1999, 43 patients with locoregional esophageal cancer were enrolled in this phase II trial. Patients were treated with 2-cycles of induction chemotherapy followed by concomitant chemoradiotherapy. F-P chemotherapy consists of 1,000 mg/m2/Day of 5-FU as continuous infusion on day 1~5 and 80 mg/m2 of cisplatin as an intravenous bolus on day 1 and was repeated every 3~4 weeks. All patients received 60 Gy of external beam radiation concomitantly with F-P chemotherapy; intraluminal brachytherapy was added in 12 patients. A total of 4 cycles of chemotherapy were delivered. No further treatment was planned in patients who achieved complete remission after completion of the treatment. RESULTS: Among the 43 patients entered, 35 patients completed the protocol. Of the 35 evaluable patients, 12 patients (34%) achieved complete response and 13 patients (37%) achieved partial response. In 26 of 33 patients, dysphagia was improved. At a median follow-up of 22 months, the 2-year and 5-year survival rates were 39% and 19%, respectively. The median survival duration of the complete responder group was 69 months (4~100 months) and the 2-year survival rate of the complete responder group was 82%. Toxicities were tolerable, comprised of mucositis and cytopenia. CONCLUSION: Induction chemotherapy followed by concurrent chemoradiotherapy in locoregional esophageal cancer is well tolerated and effective.
