Fracture of the femoral neck occurred after internal fixation of intertrochanteric fracture of the femur is very rare and have been described previously in terms of stress fracture, stress-riser fracture, Youngs modulus fracture or iatrogenic fracture in the literature. This fracture documented about 20 cases in the English literature and usually occurred in elderly patients with osteoporosis and it always occur in the subcapital region. We report a case of subcapital stress fracture of the femur occurred after internal fixation with compression hip screw of intertrochanteric femur fracture.

3.A Case of Pseudallescheria boydii Mycetoma.

Pseudallescheria (P.) boydii is a true fungus that is ubiquitous in nature, and has a world-wide distribution. It has been isolated from soil, plant debris, polluted water and sewage. Infection may occur via direct inoculation or inhalation, and usually affects the extremities. It is a common cause of mycetoma characterized by tumefaction, draining sinuses and grains. It has polymorphism due to a complicated life cycle which undergoes teleomorph as well as anamorph stages. Cleistothecia, ascocarp and ascospore may be seen during the teleomorph stages, whereas scedosporium and graphium type may be seen during the anamorph stages. We report a case of P. boydii mycetoma occurring in a 69-year old male. He had multiple nodules with a discharging fistula on a dark red plaque at the dorsum of the right forearm. Cultural isolation for definitive diagnosis showed typical P. boydii. The lesion responded well to daily itraconazole.

4.Limited Two Incision Technique in Carpal Tunnel Release.

Open carpal tunnel release surgery has heen the gold standard method of treatment for who did not respond to conservative treatment and whose neurologic symptoms were progressive. However, open carpal tunnel release using a standard incision frequently associated with delayed return of hand function, residual scar tenderness and pillar pain. So, two new alternative methods such as endoscopic carpal tunnel release and limited incision technique have developed. To define the role of the limited two incision technique, we anaiyzed the postoperative functional results of 40 cases in 33 patients. The patients were divided in two groups. Group A was standard incision group (23 cases in 19 patients) and Group B was limited two incision group (l7 cases in 14 patients). The postoperative functional results were analyzed statistically using chi-square test at postoperative 3, 6, 9 and l2 weeks. There were no significant differences in reliet of numbness and paresthesias in both groups. There was a statistically significant difference in relief of scar tenderness and pillar pain at postoperative 3 and 6 weeks in Group B. There was no significant complication in both groups. We can conclude that the limited two incision technique of carpal tunnel release is a safe procedure which allows rapid return of hand functions with reduced incidence of scar tenderness in the early postoperative stages.

5.Efficacy of Haloperidol and Clonidine Combination in the Treatment of Chronic Schizophrenia.

OBJECTIVE: This study was designed to investigate the efficacy in the combination of haloperidol and clonidine with chronic schizophrenia. Moreover, we evaluated the effects of clonidine to the plasma haolperidol and reduced haloperidol concentration 36 patiens were entered on 4 weeks by the double blind trial. METHOD: The patients (male=11, female=25) were randomly divided into two groups of eighteen patients each. Their antipsychotic medication was fixed for 2 weeks before double blind trial with haloperidol and clonidine or placebo. Clonidine was started at 0.15 mg/day and then raised 0.6mg/day during 4 days, and the placebo was applied by same method as clonidine. The clinical assessment of the patients was assessed with BPRS and NOSIE. The plasma concentrations of haloperidol and reduced haloperidol were measured by HPLC (UV detector). RESULTS: 1) There were no statistical differences in sex distribution, age, duration of illness, dosages of haloperidol, plasma concentration of haloperidol and reduced haloperidol of each groups before clonidine or placebo trial. 2) On total and subcluster scores of BPRS and total score of NOSIE, there were decreased tendencies over the time, but there was no statistical significance in both groups. 3) Plasma concentrations of haloperidol and reduced haloperidol after 4 weeks of clonidine trial were revealed statistically significant increasement as compared to baseline. CONCLUSION: The combination of haloperidol and clonidine for 4 weeks did not reveal the improvement. In our opinions the reasons of those results were that our samples were not made of paranoid type of schizophrenia, the duration of clonidine trial was not good enough. So we should suggest that more specifying design will be needed.

6.Development of Nursing Intervention Standards for Home Hospice Patients.

PURPOSE: The purpose of this study is to develop a set of standardized nursing interventions and their associated nursing activities according to the NIC system to guide home care nurses in performing nursing intervention activities for hospice patients. METHOD: This study was a descriptive survey that first identified frequently used & specialized hospice nursing interventions based on the NIC systems and next validated a set of standardized nursing interventions and their associated nursing activities. One hundred chart records of home hospice patients were used to identify nursing interventions. Also, thirty-nine hospice specialized nurses participated twice using the Delphi technique to test the content validity of the standards. RESULTS: Among the nursing interventions, 19 important nursing interventions and 418 associated nursing activities were selected after two rounds of Delphi technique by hospice specialized nurses. The mean content validity of the final nursing activities was 0.82. In this paper, only the 3 most frequently used nursing interventions and 3 hospice specialized nursing interventions with their associated nursing activities are presented since space is limited. CONCLUSION: The nursing intervention standards will be a basis for home hospice nurses to improve quality of hospice care for hospice patients.

7.Comparative Study between Gastric Cancer with Metastasis to Paraaortic Node and Stage IV Gastric Cancer with Distant Metastasis.

BACKGROUND: Extended radical dissection of gastric cancer offers a more accurate stage, and complete removal of the metastatic lymph nodes raises the rate of curability. It has been thought for a long time that gastric cancer with paraaortic lymph-node metastasis is impossible to cure. However, with several reports of long-term survival of cases with paraaortic lymph-node metastasis, recently the procedure of paraaortic node dissection has become prevalent in Japan. METHODS: The study group was comprised with 348 patients of stage IV gastric cancer from among total of 2,015 patients with gastric cancer who had undergone gastric resection between 1979 and 1997 at the Department of Surgery, Chonnam University Hospital. Thirty-eight (38) of the 348 patients were paraaortic lymph-node positive (group I), 233 presented peritoneal metastasis (group II), and the remaining 77 presented liver metastasis (group III). The following factors were taken into consideration: age, sex, size of tumor, histopathologic classification, Borrmann type, location of tumor, DNA ploidy pattern, and survival rate. RESULTS: 1) In sex distribution, the third group (III) showed a statistically significant male predominence compared with the other groups (I=63.2%, II=67.4%, III=87.0%) (p<0.01). 2) In histopathologic classifications, well-differentiated tubular adenocarcinomas were more common in group III (I=10.5%, II=6.4%, III=22.0%). Poorly differentiated tubular adenocarcinomas were more common in group II (I=44.7%, II=59.2%, III=27.3%) (p<0.01). 3) In Borrmann types, type IV was more common in group II (I=7.9%, II=27.8%, III=9.1%) (p<0.01). 4) In tumor location, antral involvement of the tumor was presented in 55.3% of group I, 54.7% of group II, and 67.5% of group III whereas whole involvement of stomach was presented in 5.3% of group I, 15.0% of group II, and 5.2% of group III (p<0.01). 5)Group I had a significantly higher (37.9%) two-year survival rate than the other groups (II=10.9%, III=12.8%) (p<0.01). CONCLUSIONS: This study suggests a greater prognostic benefit for radical surgery in cases of paraaortic lymph-node metastsis than in cases involving other stage IV gastric cancers.

8.The Effect of Coculture with Human Oviductal cells on In Vitro Development of ICR Mouse Embryo.

OBJECTIVE: To examine the effects of coculture with human oviductal cells regarding the development of 1-cell stage ICR mouse embryos and to investigate the effects of duration and start time of coculture. MATERIALS AND METHODS: ICR mice were superovulated with PMSG/hCG and 1-cell stage mouse embryos were recruited. 1-cell mouse embryos were cocultured on human oviductal cells in a CO2 incubator(coculture group) and were cultured on 0.4 % BSA+HTF media(control group)(Experiment 1). 1-cell mouse embryos were cocultured on human oviductal cells for 36, 44, 52, 60 hours after hCG IP respectively, and then were transferred to 0.4 % BSA+HTF media(Experiment 2). In comparison, 1-cell mouse embryos were cultured by using 0.4 % BSA+HTF media, and then were transferred to human oviductal cell coculture system using the same schedule(Experiment 3). Afterward, they were examined regarding the development to 2-cell, 4~8 cell stage mouse embryos, morulas and blastocysts. RESULTS: In experiment 1, the developmental rates to 2-cell embryos of coculture group and control group were 97.3 % and 98.7 %, respectively. After 2-cell embryos, coculture group showed significantly higher developmental rate than control group (p<0.05). In experiment 2, the developmental rates after 2-cell embryos showed the significant differences. The groups with coculture effects removed before post-hCG 60 hours showed significantly lower developmental rates (p<0.05). In experiment 3, the developmental rates after 2-cell embryos were higher when the coculture started at an earlier stage. Furthermore, the groups which were cocultured from post-hCG 52 hours exhibited significant lower developmental rate than the groups which were cocultured continuously (p<0.05). CONCLUSION: The coculture with human oviductal cell could improve the development of the embryos in vitro and might mimic the natural physiological condition better than media environment. The degree of improvement was more pronounced when the coculture started at an earlier stage and the duration of coculture was longer. More importantly, the changes of culture condition at post-hCG 52 hours in which secondary mitosis occurs, have significant detrimental effects on growth and development of mouse embryos.

9.The Utility of Non-Invasive Nasal Positive Pressure Ventilation for Acute Respiratory Distress Syndrome in Near Drowning Patients

PURPOSE:
Near drowning refers to immediate survival after asphyxia due to submersion or immersion in water, which is a crucial public safety problem worldwide. Acute lung injury or acute respiratory distress syndrome (ARDS) is a common complication of near drowning. The purpose of this study was to investigate the feasibility and effectiveness of noninvasive nasal positive pressure ventilation (NINPPV).
METHODS:
This retrospective study was conducted at a tertiary emergency department. NINPPV was administered for moderate ARDS caused by submersion or immersion in patients who were older than 18 years, from January 2015 to December 2018. We collected the demographic (age, sex, length of hospital stay, and outcome), laboratory (arterial blood gas, lactate, oxygen saturation, partial pressure of oxygen divided by the fraction of inspired oxygen, complete blood count, blood urea nitrogen, and creatinine), and clinical data (acute lung injury index and ventilator failure) of the patients. A statistical analysis was performed using Statistical Package for the Social Sciences version 20.0 for Windows.
RESULTS:
NINPPV treatment was provided to 57 patients for near drowning, 45 of whom (78.9%) were successfully treated without complications; in 12 (21.1%), treatment was changed to invasive mechanical ventilation within 48 hours due to ARDS or acute kidney injury. NINPPV treatment was successful in 31 (75.6%) out of 41 sea-water near drowning patients. They were more difficult to treat with NINPPV compared with the fresh-water near drowning patients (p<0.05).
CONCLUSIONS
NINPPV would be useful and feasible as the initial treatment of moderate ARDS caused by near drowning.
10.The Clinical Feasibility of Transradial Coronary Intervention in Selective Patients Undergoing Left Main Coronary Intervention.

BACKGROUND AND OBJECTIVES : The transradial approach has been increasingly used as an alternative to conventional transfemoral intervention. However, there is little data on the efficacy of transradial coronary intervention (TRI) in left main coronary arterial (LMCA) disease. The purpose of this study was to evaluate the feasibility of TRI in selective patients undergoing percutaneous coronary intervention (PCI) for LMCA disease. SUBJECTS AND METHODS : Between Jan 2003 and May 2005, 83 patients with LMCA stenosis were treated with PCI. Of these, 40 selected patients having undergone TRI were included in this study. RESULTS : The patients included 30 males and 10 females, with a mean age of 61+/-12 years. There were 28 cases (70%) involving coronary arteries other than the LMCA and 24 cases (60%) with bifurcation lesion involvement. In 28 cases (70%), a 6 Fr sized guiding catheter was used. No case required crossover from TRI to TFI due to procedural failure. During hospitalization, 2 patients who underwent primary PCI in the setting of STEMI died, but there were no other clinical events. CONCLUSION : In selected patients with LMCA disease, TRI can be a feasible and safe approach for PCI.
