BACKGROUND: It is a recent trend that the birth and mortality rates has been decreased and average life span prolonged in proportion to the socioeconomic and scientific advances. The goal of this study is to suggest basic data for comprehensive and continuous management of elderly patients. METHODS: Based upon ICD-10 (International Classification of Disease), the author analysed the outpatient's diseases 19,367 and inpatient's diseases 6,783 beyond the age of 65 who visited outpatient department and were hospitalized to the Kosin University, Kosin Medical Center in Pusan from January to December in 1997 RESULTS: The ratio of elderly patient in outpatient was 4.5% and inpatient was 15.0%. The number of outpatient each disease group were disease of the circulatory system 5,367 (27.7%), neoplasms 4,007 (20.7%), disease of the digestive system 2,635 (13.6%), endocrine, nutritional and metabolic disease 1,798 (9.8%) in order. Frequent diseases in individual system of outpatient were followed; malignant neoplasms of digestive organs 2,642 (13.6%), hypertensive diseases 2,053 (10.6%), diseases of esophagus, stomach and duodenum 1,804 (9.3%), ischemic heart disease 1,101 (5.7%) in order. The number of inpatient each disease group were disease of the neoplasms 2,138 (31.5%), disease of circulatory system 1,191 (17.6%), disease of the digestive system 730 (10.8%), endocrine, nutritional and metabolic disease 489 (7.2%) in order. Frequent diseases in individual system of inpatient were followed; malignant neoplasms of digestive organs 956 (14.1%), malignant neoplasms of respiratory and intrathoracic organs 351 (5.2%), hypertensive diseases 348 (4.2%), diabetes mellitus 325 (4.1%) in older. CONCLUSION: This result indicate that the elderly patients abounded mostly with disease of circulatory, neoplasm, disease of the digestive system and endocrine, nutritional and metabolic disease.

PURPOSE: Choroid Plexus papilloma is a rare intracranial neoplasm that is most commonly found in the trigone of the lateral ventricle in children or in the fourth ventricle in adult. Extraventricular extension of choroid plexus papilloma has been rarely reported within the cerebellopontine angle (CPA) cistern. Authors report two cases of choroid plexus papilloma in the posterior fossa seen on magnetic resonance imaging (MRI). MATERIALS AND METHODS: MRI findings of two cases of choroid plexus papilloma in posterior fossa were retrospectively reviewed. RESULTS: In the first case, the tumor was in the fourth ventricle and extended to the left CPA cistern via the foramen of Luschka. In the second case, the tumor presented as an expansile mass of the of fourth ventricle and right lateral recess. Multiple signal voids of low intensity due to prominent feeding vessels and calcifications within the tumors were noted. CONCLUSION: This report demonstrates MR findings of choroid plexus papilloma in the posterior fossa with the brief review of the literatures.

Oguchi's disease is an unusual form of congenital stationary night blindness that is characterized by a peculiar grey-white discoloration of the retina that gives a metallic sheen. After prolonged dark adaptation, the unusual fundus discoloration disappeared and the retina slowly reverted to its original metallic color after exposure to the light. A 20-year-old man was referred for study because of a complaint of night blindness since childhood. As results of diagnostic work up, especially based on typical ocular fundus finding, dark adaptometry and electrophysiologic studies of the retina the authors have diagnosed as a Oguchi's disease. We report this case with the review of the literature.

Authors studied characteristics of ERG b-wave and oscillatory potential, which can be applied to ERG protocol for clinical purposes. Following conclusions were obtained from the ERG test performed on 20 normal men(40 eyes) during dark adaptation and light adaptation time, under the conditions where other factors were equal. 1. In the scotopic ERG test, 88%(364/414uv) of maximum b-wave amplitude in 30 minutes were obtained in first 1 minute of dark adaptation. By using this shortening scotopic protocol for the acquired retinal disease, sufficient result can be expected without the over 20 minutes dark adaptation. 2. In the photopic ERG test, if over 30 minutes dark adaptation time were given, it would take at least 15 minutes to reach the regular initial photopic value. Therefore, it is recommended that photopic ERG test be done before the scotopic ERG test. 3. In the oscillatory potential test, light adaptation time has little effect on summed photopic oscillatory potential amplitudes and implicit time. Summed scoto pic oscillatory potential amplitude reaches the highest 1 minute after the dark adaptation. Therefore, it is recommended that to obtain higher amplitude and more sensitive test result, oscillatory potential test be done 1 minute after the dark adaptation.

We report CT and MR findings in tow cases of primary malignant melanoma of the vagina, one arising from cervicovaginal junction mimicking squamous cell carcinoma of the cervix and the other one recurring at vagina after resection. Two cases of malignant melanoma had high-attenuation on CT and high signal intensity on T1-weighted MR images and enhanced well after gadopentetate dimeglumine administration.

This study was undertaken to compare by Atomic Force Microscope the effects of various finishing and polishing instruments on surface roughness of filling and veneering composite resins. Seven composite resins were studied : Silux Plus (3M Dental Products, U.S.A.), charisma (Heraeus Kulzer, Germany), Prisma THP (L.D.Caulk, Dentsply, U.S.A.), Photoclearfil (Kuraray, Japan), Cesead (Kuraray, Japan), Thermoresin LC (GC, Japan), Artglass (Heraeus Kulzer, Germany), samples were placed and polymerized in holes (2mm thick and 8.5mm in diameter) machined in Teflon mold under glass plate, ensuring excess of material and moulded to shape with polyester matrix strip, Except control group (polyester mztrix strip), all experimental groups were finished and polishied under manufacturer's instructions. The finishing and polishing procedure were : carbide bur (E.T. carbide set 4159, Komet, Germany), diamond bur (composite resin polishing bur set, GC, Japan), aluminum-oxide disc (Sof-Lex Pop-On, 3M Dental Products, U.S.A.), diamond-particle disc (Dia-Finish, Renfert Germany), white stone bur & rubber point(composite finishing kit, EDENTA, Swiss), respectively. Each specimens were evaluated for the surface roughness with Atomic Force Microscope (AutoProbe CP, Park Scientific Instruments, U.S.A.) under contact mode and constant height mode. The results as follows : 1. Except Thermoresin LC, all experimental composite resin groups showed more rougher than control group after finishing and polishing(p<0.1). 2. A surface as smooth as control group was obtained by Al2O3 disc all filling composite resin groups except Charisma and all veneering composite resin groups except Thermoresin LC(p<0.05). 3. In case of Thermoresin LC, there were no statistically significant differences before and after finishing and polishing(p>0.1). 4. Carbide bur, diamond bur showed rough surfaces in all composite resin groups, so these were inappropriate for the final poslishing instruments.

Three anopheline mosquitoes in Korea were studied for their abilities as vectors for Plasmodium vivax. The female mosquitoes of Anopheles lesteri, An. pullus and An. sinensis were allowed to suck malaria patient blood until fully fed, and they were then bred for 2 weeks to develop from malaria parasites to sporozoites. The result from the above confirmed the sporozoites in one An. lesteri of one individual and five An. sinensis of six individuals. We also reconfirmed that An. sinensis was the main vector to transmit malaria and An. lesteri as well as An. sinensis were able to carry Korean malaria parasites. Therefore, we propose that diversified study is needed to manage malaria projects.
