1.Tumor Necrosis Factor-alpha-308G/A Promoter Polymorphism is Associated with the Severity of Gastric Carcinomas.

PURPOSE: The pro-inflammatory cytokine, tumor necrosis factor-alpha (TNF-alpha), is a central mediator of the immune response involved in a wide range of immuno-inflammatory and infectious diseases. There is increasing evidence that TNF-alpha may promote the development and spread of the cancer. Polymorphisms in the TNF-alpha promoter have been related to TNF-alpha production. Therefore, we investigated the potential association of TNF-alpha genotypes with gastric cancer in the Korean population. METHODS: The study included 66 patients with gastric adenoma, 75 patients with gastric carcinoma, and 551 healthy controls. The -308 and -238 polymorphisms in the TNF-alpha promoter were analyzed by PCR- restriction fragment length polymorphism (RFLP). Distributions of TNF-alpha promoter polymorphisms were compared between groups by chi2 test. P values smaller than 0.05 were considered to be significant. RESULTS: The proportion of individuals carrying the TNF-alpha -308A allele was higher in the carcinoma group compared to controls and adenomas, but the differences were not significant (P=0.124). However, the TNF-alpha -308A allele was significantly associated with advanced gastric carcinoma (P=0.026), serosa invasion (P=0.004), neural invasion (P= 0.021), and lymph node metastasis (P=0.005). On the other hand, the TNF-alpha -238G/A polymorphism was not associated with the development of gastric adenoma and carcinoma and the severity of gastric carcinoma. CONCLUSION: These results suggest that the TNF-alpha -308A allele is associated with the severity of gastric carcinoma in terms of invasion and metastasis in the Korean population. Therefore, TNF-alpha promoter polymorphism could be used as a predictive marker of the severity of gastric carcinoma.

2.Relationship of Posterior Decentering of the Humeral Head with Tear Size and Fatty Degeneration in Rotator Cuff Tear

BACKGROUND:
Posterior decentering is not an uncommon finding on rotator cuff tear patients' shoulder magnetic resonance imaging. No previous study has reported on the relationship between posterior decentering and rotator cuff tear.
METHODS:
We assessed patients' rotator cuff tear humeral head positions based on humeral–scapular alignment (HSA). Subjects were classified into centering and decentering groups based on a <2 mm or >2 mm HSA value, respectively. Differences in rotator cuff tear size, degree of tear, and fatty degeneration between the two groups were evaluated.
RESULTS:
One hundred seventy-five patients (80 males, 95 females; mean age: 59.7 ± 6.5 years old) were selected as subjects (case-control study; level of evidence: 3). Tear size, degree of subscapularis tendon tear, and fatty degeneration of the supraspinatus, infraspinatus, and subscapularis muscles were significantly different between the two groups (p<0.001, p<0.001, p<0.001).
CONCLUSIONS
The occurrence of decentering was related to rotator cuff tear size, degree of subscapularis tendon tear, and fatty degeneration of the rotator cuff muscles.
3.Improving Medication Adherence in Isolated Patients With Cognitive Impairment Using Automated Telephone Reminders

Background:
and Purpose: Medication adherence is essential for effective medical treatment.However, it is challenging for cognitively impaired patients. We investigated whether an automated telephone reminder service improves medication adherence and reduces the decline of cognitive function in isolated patients with cognitive impairment.
Methods:
This was a single-center, randomized clinical trial. We enrolled mild cognitive impairment (MCI) or Alzheimer’s disease (AD) patients who lived alone or with a cognitively impaired spouse. We provided an automated telephone reminder service for taking medication to the intervention group for 6 months. The control group was provided with general guidelines for taking the medication every month. The participants underwent neuropsychological assessment at the beginning and end of the study. Statistical significance was tested using nonparametric Wilcoxon rank sum and Wilcoxon matched-pairs signed-rank tests.
Results:
Thirty participants were allocated randomly to groups, and data for 29 participants were analyzed. The mean age was 79.6 (standard deviation, 6.0) years and 79.3% of the participants were female. There was no significant difference in medication adherence between the 2 groups. However, a subgroup analysis among participants with more than 70% response rates showed better medication adherence compared to the control group (intervention: 94.6%; control: 90.2%, p=0.0478). There was no significant difference in the change in cognitive function between the 2 groups.
Conclusions
If a patient’s compliance is good, telephone reminders might be effective in improving medication adherence. It is necessary to develop reminder tools that can improve compliance for cognitively impaired patients.
4.Evaluation of ImmunoproteasomeSpecific Proteolytic Activity Using Fluorogenic Peptide Substrates

The 26S proteasome irreversibly hydrolyzes polyubiquitylated substrates to maintain protein homeostasis; it also regulates immune responses by generating antigenic peptides. An alternative form of the 26S proteasome is the immunoproteasome, which contains substituted catalytic subunits (β1i/PSMB9, β2i/PSMB10, and β5i/PSMB8) instead of constitutively expressed counterparts (β1/PSMB6, β2/PSMB7, and β5/PSMB5). The immunoproteasome expands the peptide repertoire presented on MHC class I molecules. However, how its activity changes in this context is largely elusive, possibly due to the lack of a standardized methodology to evaluate its specific activity. Here, we describe an assay protocol that measures the immunoproteasome activity of whole-cell lysates using commercially available fluorogenic peptide substrates. Our results showed that the most accurate assessment of immunoproteasome activity could be achieved by combining β5itargeting substrate Ac-ANW-AMC and immunoproteasome inhibitor ONX-0914. This simple and reliable protocol may contribute to future studies of immunoproteasomes and their pathophysiological roles during viral infection, inflammation, and tumorigenesis.

6.Estimating Age Using Nationwide Survey Data on the Number of Residual Teeth

Given that tooth loss is a degenerative change, the number of residual teeth may be used to specify a particular age range as a marker for age estimation. This study examined changes in the number of teeth with age using a nationwide oral survey database and derived the age distribution of the Korean population according to the number of teeth. Data on the number of teeth and age were extracted from the oral examination data of the Korean National Health and Nutrition Examination Survey (KNHANES) from 2016 to 2018. Statistical analyses of a complex sample survey were performed using weighted values. The distribution range of the number of teeth by age was broad. The proportion of young people decreased progressively as the number of remaining teeth decreased. In contrast, the proportion of those from the older age group decreased slightly as the number of teeth increased. The number of teeth was subdivided into groups of four, age was categorized into 5-year intervals, and the distribution of age groups by the number of teeth was analyzed. We attempted to determine the age group threshold at approximately 95th percentile for age. In summary, we found that if there were ≤4, 5-12, and 13-20 residual teeth, the estimated age was ≥60, ≥55, and ≥50 years, respectively, with an approximately 95% probability. When many teeth are lost and it is difficult to apply conventional dental age estimation methods, our method may assist in narrowing the age range, although it is not an accurate age determination method.
7.Ultrasonographic Analysis of Normal Thymus in Infants.

PURPOSE: To analyse the ultrasonographic findings of normal thymus in infants and determine the pathologic findings. MATERIALS AND METHODS: Forty two infants without a history of chronic disease, tumor or steroid therapy were scanned in the axial planes with a 7.0 MHz sector transducer. Bilaterality, homogeneity, and the shape and intensity of thymic echogenicity were analysed; shapes were classified as one of three types, namely bilobate, quadrangular or triangular. The intensities of thymic echogenicity were compared with those of the liver and spleen under the same conditions (time-gain compensation, depth and gain). RESULTS: Bilaterality was seen in 36 cases, homogeneity in 35, homogeneity and bilaterality in 29, inhomogeneity and bilaterality in seven, and homogeneity and unilaterality in six. No case showed inhomogeneity and unilaterality. The shape was bilobate in 23 case, quadrangular in 13 and triangular in six, while the outer border was convex in 29 cases and straight in 13. In no case was echogenicity of the thymus greater than that of the liver or spleen. Transverse diameters were 21-47mm (mean, 37.0mm; SD, 9.4mm) and depths were 10-37.5mm (mean, 25.8mm; SD, 5.9mm). CONCLUSION: The usual findings of normal thymus in infants are bilaterality and homogeneity, convex or shaight outer border, and echointensity less than or equal to that of the liver and spleen. The possibility of thymic pathology is suggested when ultrasonography shows unilaterality, inhomogeneous echogenicity, and echogenicity greater than that of the liver and spleen.

8.A Case of Toxocariasis in a Patient with Cecal Cancer Mimicking Hepatic Metastasis.

Toxocariasis is a common helminth infection and the most common cause of peripheral blood eosinophilia. The clinical presentation of liver toxocariasis is often confused with primary or metastatic hepatic tumors. Here, we report on a 52-year-old man admitted with cecal cancer and eosinophilia. Computerized tomography (CT) scans revealed multiple lesions with peripheral enhancement in the liver, and T1- and T2-weighted images revealed isointensity. The patient underwent right hemicolectomy and an exploratory laparotomy with a liver biopsy. Resection of the cecal specimen revealed an adenocarcinoma, while the liver biopsy revealed an eosinophilic abscess with no evidence of malignancy. A serologic test showed a marked increase in specific immunoglobulin G antibody concentrations against Toxocara canis and daily antiparasitic treatment with albendazole (800 mg per day for 7 days) was initiated. Follow-up CT scans indicated that all liver masses and eosinophilia were resolved.

9.A Case of Alveolar Soft Part Sarcoma of the Pleura.

Alveolar soft part sarcoma (ASPS) is a rare malignant soft-tissue neoplasm of unknown histogenesis. The two main sites of occurrence are the lower extremities in adults and the head and neck in children. We report the first case of pleural ASPS occurring in a 58-yr-old man who presented with progressive dyspnea. A computed tomographic scan of the thorax revealed a large enhancing pleural mass with pleural effusion in the left hemithorax. Wide excision of the pleural mass was performed. Histologically, the tumor consisted of organoid nests of large polygonal cells, the cytoplasm of which had eosinophilic and D-PAS positive granules. Immunohistochemical staining showed that the tumor cell nuclei were positive for transcription factor 3 (TFE3). The pleural ASPS with multiple bone metastases recurred 1 yr after surgery and the patient died of acute pulmonary embolism 1.5 yr after diagnosis.

10.Dose-Volume Analysis of Lung and Heart according to Respiration in Breast Cancer Patients Treated with Breast Conserving Surgery.

PURPOSE: Adjuvant radiotherapy of breast cancer using a photon tangential field incurs a risk of late heart and lung toxicity. The use of free breathing (FB), expiration breath hold (EBH), and deep inspiration breath hold (DIBH) during tangential breast radiotherapy as a means of reducing irradiated lung and heart volume was evaluated. METHODS: In 10 women with left-sided breast cancer (mean age, 44 years) post-operative computed tomography (CT) scanning was done under different respiratory conditions using FB, EBH, and DIBH in 3 CT scans. For each scan, an optimized radiotherapy plan was designed with 6 MV photon tangential fields encompassing the clinical target volume after breast-conserving surgery. RESULTS: The results of dose-volume histograms were compared using three breathing pattern techniques for the irradiated volume and dose to the heart. A significant reduction dose to the irradiated heart volume for the DIBH breathing technique was compared to FB and EBH breathing techniques (p<0.05). CONCLUSION: This study demonstrated that the irradiated heart volume can be significantly reduced in patients with left-sided breast cancer using the DIBH breathing technique for tangential radiotherapy.
