1.Nationwide Survey on the Current Status of Infection Control in Oriental Medical Hospitals.

BACKGROUND: Oriental medicine is a part of medical service in Korea. However, there are no information about the status of infection control program in oriental medical hospitals. The purpose of this study was to investigate the current status of infection control activities in oriental medical hospitals and to provide a data to establish infection control policy in oriental medical hospitals. METHODS: This study was based on the survey questionnaires about infection control program and activities with total 13 oriental medical university hospitals in Korea. Questionnaires were collected from February 1 to March 31, 2011. RESULTS: Nine hospitals (69.2%) had infection control practitioner (ICP). Only 4 hospitals (30.8%) had full-time ICP and infection control office. The 6 hospitals (46.2%) had a position for infection control doctor and half of them were oriental medical doctors. Eight hospitals (61.5%) had an infection control committee and they had a meeting for average 2.5 times a year. Six hospitals (46.2%) performed surveillance and most of them were monitoring site directed infections. Only 4 hospitals have microbiology laboratory. All hospitals were using disposable acupuncture needles. Six hospitals were only using disposable wet cupping. Two hospitals (15.4%) were reusing wet cupping after disinfection. CONCLUSION: Through this study it was able to find out the status of infection control activities in oriental hospitals. There were also many limits on infection control activities in oriental medical hospitals. Based on this study it is needed to develop the infection control guideline and education for associates in oriental hospitals.

3.Low-Grade Endometrial Stromal Sarcoma with Inferior Vena Cava Extension: First Report in Korea.

Low-grade endometrial stromal sarcoma (LGESS) with intravascular extension is very rare, with only 26 cases having been reported. We experienced a case of LGESS with inferior vena cava (IVC) extension. A 60-year-old female presented with left leg edema. She had a history of total hysterectomy, and was diagnosed of leiomyoma at that time. On imaging study, tumor masses were located around both common iliac veins (CIV), and within the CIV and IVC. The pelvic masses on both side and IVC mass were resected, and then the patient received adjuvant hormonal therapy and radiotherapy over the remnant pelvic masses. LGESS with IVC extension is difficult to distinguish from intravascular leiomyomatosis. LGESS is a malignant disease and commonly recurs, even in early stages. Accurate diagnosis, complete resection, proper adjuvant therapy and close follow-up are very important.

4.Validity and Reliability of Professional Identity of Dental Hygienists.

This study aimed to develop a measure of professional identity for Korean dental hygienists and to analyze the factors and characteristics of professional identity of these dental hygienists. In this study, 890 dental hygienists completed a self-administered survey. The final analysis was conducted with a total of 880 responses, excluding 10 questionnaires with unreliable responses and partial responses. A systematic literature review was conducted to establish the definition of professional identity, while content validity, exploratory factor, confirmatory factor, and reliability analysis were conducted to establish the constructive factors. Descriptive statistics, independent sample t-test, and one-way ANOVA were used to identify the level of each factor. Authors conceptualized the professional identity for Korean dental hygienists and item generation, item reduction, and questionnaire formatting. The developed measure of professional identity for dental hygienists consisted of five factors, with 15 items. “Sense of calling,”“need for academic capacity building,”“performance of delegated authority,”“compliance of code of ethics,” and “usage of professional organizations” were the constituent factors. Based on these factors, the level of professional identity was the highest in the “performance of delegated authority” and lowest in the “usage of professional organizations.” These finding indicated that dental hygienists with higher education levels have higher professional identity levels. The dental hygienists working at the general/university hospital had the highest level of professional identity. Thus, further research is needed to establish a model that consists of factors affecting and influencing the professional identity of dental hygienists.

5.The Effect of Preceptorship on OJT(On the Job Training)of New Nurses.

The purpose of this study was to examine the effect of preceptorship on On the Job Training of new nurses. The research design was a retrospective survey and one group pre-post test design. The subjects were 49 entry nurses and 49 preceptors. The data were collected from Feburary 1, 1998 to December 1, 1998. One questionnair applied in this study consisted of general characteristics, the responses of new entry nurses and their preceptors and their qualities. The other questionnair consisted of job satisfaction of new entry nurses and preceptors and their competencies. The data were analyzed for frequencies, mean score, standard deviation, t-test, and ANOVA. The results of the study were as follows: 1) Preceptees's responses were that the nurses' affiliation of unit, cooperation of other nurses, and competencies of nursing skills improved after OJT applied preceptorship. 2) Preceptors's answers were that preceptor- ship had a positive effect on their continuing education. 3) While preceptees's job satisfaction scores were high in interaction, administration, and organization, they were low in autonomy and pay. 4) Pre-competency scores were the same as post-competency scores. 5) After preceptorship, preceptors responded that their nursing skill & communication competency increased, but their nursing administration competency decreased. 6) Finally, preceptorship's other effects were as follows: 1. Checklists for preceptee education at the nursing unit were developed separately. 2. It was proved that 'preceptor interview note' was effective on manage preceptorship. 3. We obtained duty overlap periods; medical- surgical units-1 month, 5 ICU-2 months, operation room-3 months. 4. We prepared a preceptor program based on the study results.

6.Ovarian Large Cell Neuroendocrine Carcinoma Associated with Endocervical-like Mucinous Borderline Tumor: A Case Report and Literature Review.

Ovarian large cell neuroendocrine carcinoma is a rare tumor that is usually associated with surface epithelial tumors. Mucinous tumors are most common surface epithelial component identified in reported cases. Ovarian mucinous tumor associated with large cell neuroendocrine carcinoma is almost always an intestinal type. However, large cell neuroendocrine carcinoma associated with pure mucinous borderline tumor of endocervical-like type has not been described previously. The present case report describes a large cell neuroendocrine carcinoma associated with endocervical-like mucinous borderline tumor of the ovary in a 35-year-old woman. The tumor was confirmed by histopathology and immunohistochemistry. A review of the pertinent literature is included.

7.A Clinical and Statistical Study on Pregnancy of Elderly Primipara.

OBJECTIVE: Recently, the incidence of elderly primipara is increasing. Thus, impact of old age on pregnancy outcomes became important. Our purpose was to assess the effect of maternal age on pregnancy outcomes. METHODS: 73 cases of the elderly primipara over 35 years at the department of obstetrics and gynecology, Chon-ju presbyterian medical center, from Jan. 1, 1995 to Dec. 31, 1998, were compared with 73 cases randomly chosen young primiparas during same period. RESULTS: Elderly primipara had higher rate of infertility and spontaneous abortion history, antenatal complications, cesarean section, poorer neonatal outcome than young primipara. CONCLUSION: The pregnancy outcome of elderly primipara is considered to be less favorable than those of young primipara. Careful antenatal and intrapartal care should be performed for the elderly primipara. We think that many clinical study of congenital anomaly in elderly primipara will be necessary.

8.Full-mouth rehabilitation of a patient with reduced occlusal vertical dimension: a clinical report.

It is a great challenge for dentists to do prosthetic restorations of a patient with reduced occlusal vertical dimension (OVD). Proper determination of the OVD is significant. An 85-year-old male patient came in with missing right maxillary posterior teeth. After diagnostic wax-up, reversible provisional prostheses were used to evaluate the new OVD in the maxilla and mandible. After that, the patient was evaluated clinically based on the criteria of esthetic, phonetic, and OVD. After 8 weeks, teeth with excessive wear were treated with root canal therapy and fiber post and core and irreversible provisional prostheses were done. After 8 weeks of observation period, final restorations with fixed partial dentures and removable partial dentures were made. Throughout the follow-up period of 6 months, no symptoms related with increased OVD and mechanical complications were observed.

9.Foreign Body in the Gastrointestinal Tract in Children.

BACKGROUND: The accidental swallowing of foreign body is a common problem in the children. Ingested foreign bodies may be managed by endoscopy, observation, or surgery. So we analyzed the methods of removal, type, location and complications of foreign bodies. MATERIALS AND METHODS: This report reviewed 37 cases of ingested foreign body in the gastrointestinal tract at the Department of Pediatrics, Yeungnam University Hospital between January 1997 and April 2001. RESULTS: The age ranged from 8 months to 8 years. The most prevalent age group was between 1 year and 2 years of age(19%). The male to female ratio was 2.1:1 with 25 male and 12 female patients. The type of foreign bodies were coins in 20 cases(54%), nail in 4 cases(11%), key in 4 cases(11%), pin in 2 cases(5.5%), necklace in 2 cases(5.5%) and others. The locations of foreign bodies were upper esophagus in 12 cases(32.5%), lower esophagus in 4 cases(10.8%), stomach in 16 cases(43.2%), small bowel in 5 cases(13.5%). 4. Presenting symptoms were variable with asymptomatic (59. 4%) , vomiting (19. 0%) , epigastric pain (8.1%), dysphagia (5. 4%) and others. The methods for removal of foreign bodies included 20 cases of endoscopic removal(54.0%), 3 cases of spontaneous removal(8.1%) and there was no surgical removal. 14 cases(37.9%) did not confirmed removal of foreign body because of no revisit of our hospital. Endoscopic finding of patients were normal(15 cases), ulceration(2 cases), erosion(1 case), inflammation(l case), mucosal scratch(l case). CONCLUSION: It appears that the endoscopic approach is the preferable method for the removal of upper gastrointestinal foreign bodies in the children.

10.Concomitant Glomus Tumor with CRPS in the Hand.

Glomus tumors are benign tumors that account for 1% to 5% of all soft tissue tumors of the hand and are characterized by a triad of sensitivity to cold, localized tenderness and severe paroxysmal pain. Paroxysmal pain is a symptom common not only in glomus tumors but also in CRPS, and the hand is one of the commonly affected sites in patients with both glomus tumors and CRPS. Therefore, it is not easy to clinically diagnose glomus tumors superimposed on already affected region of CRPS patients. We report a case of glomus tumor concomitantly originating with CRPS at the hand.
