3.Anti-tumor Activity of Saussurea laniceps against Pancreas Adenocarcinoma.

The purpose of this study was to confirm the anti-tumor activity of an ethanol extract of Saussurea laniceps against pancreatic tumor and to isolate the active compound from S.laniceps extract. Treatment with S.laniceps extract and hispidulin inhibited proliferation of pancreatic cell lines, such as Capan-1, Capan-2, Panc-1 and S2-013 in a dose-dependent manner using the hollow fiber assay. Hispidulin showed typical hallmarks of apoptotic cell death a significant anti-tumor activity on Capan-2 cells at a dose of 100 mg/kg and 200 mg/kg. S.laniceps has potential cytotoxic and apoptotic effects on human pancreatic carcinoma cells. Its mechanism of action might be associated with the apoptotic cell death through DNA fragmentation.

Allopurinol is a xanthine oxidase inhibitor widely used to control plasma uric acid levels. Serious and life threatening adverse effects such as the allopurinol hypersensitivity syndrome(AHS) are rare. We describe a 32-year-old female patient who was treated with allopurinol and developed fever, rash, and renal and liver dysfunction.

7.The Role of Intraoperative Frozen Section as a Guide to Sepsis in Hip Arthroplasty for Teatment of painful Hip Disease.

We performed a retrospective analysis of twenty-four consecutive hip arthroplasties (Total Hip Arthroplasty 18, Bipolar Arthroplasty 1, Cup revision 5) during which intraoperative frozen sections were analyzed to identify the occult active infection. We also reviewed the data such as laboratory findings (ESR, CRP), intraoperative culture, hip joint sonographic evaluation and permanent histologic section. Among the twenty-four patients those who received hip arthroplasties, 10 cases received revisional hip arthroplasties due to painful hip prosthesis loosening, 9 cases with infected hip prosthesis, 3 cases with infection after open reduction and internal fixation due to hip fracture and 2 cases with septic hip sequelae. Intraoperative frozen section had been obtained during staged revisional hip arthroplasties to determine the presence of active infection. We considered positive for infection if it shows more than 10 PML/HPF (Polymorphonuclear leukocyte /high power field) in at least 5 distinct microscopic fields from intraoperative tissue frozen section and postponed implantation of prosthesis. If it shows less than 5 PML/HPF, we performed revisional hip prosthesis implantation. We decided the implantation based on patient's condition, laboratory findings and intraoperative tissue conditions if it shows between 5 and 10 PML/HPF from intraoperative frozen section. At last follow up, 20 of 21 patients who had a revisional hip arthroplasties as below 10 PML/HPF from intraoperative frozen section remained free of infection (Specificity; 95%). We concluded that analysis of the intraoperative frozen sections is a reliable predictor as a guide to sepsis for the successful hip joint arthroplasties.

9.The Correlation between Responses of Peripheral Blood Mononuclear Cells and Immediate Hypersensitivity of Skin to Mite Antigen in Atopic Dermatitis.

BACKGROUND: House dust mites have been reported to play an aportant role in the pathogenesis of atopic dermatitis(AD). In fact a high positive rate for house dust mites in the skin prick test was observed in patients of AD. However, it is quite difficult to determine the clinical relevance of positive immediate hypersensitivity to mite antigen by clarical history alone because AD patients are nearly always exposed to these antigens in their daily lives. OBJECTIVE: The aim of this study was to elucidate the clinical. singficance of a positive immediate hypersensitivity reaction by observing the proliferative respons of peripheral blood mononuclear cells(PBMC) to mite antigen in the AD. METHODS: The proliferative response of PBMC separated from Dermatopkagoides farinae(Df) positive AD patients to Df antigen was compared with that from Df negative normal controls, Df positive normal controls and Df negative AD patients. RESULTS: PBMC from Df positive AD patients showed signinc not proliferation in response to Df antigen evaluated by comparing stimulation indices(SI) among different experiment groups. CONCLUSION: Significant correlation between responses of PBMC and immediate hypersensitivity of skin to Df antigen in AD was found, and it was suggested the Df may be a real aggravating factor in the Df positiv AD patients. SI may be used as ar objective criteria to select patients for hyposensitization therapy after doing a skin test.

10.The Risk Factors of Silicone Oil Induced Secondary Glaucoma.

Intravitreal silicone oil injection is useful to manage the complicated retinal detachment. However, many comlications including secondary glaucoma can occur followin gsilicone oil injection. We performed this study to investigate the incidence and risk factors of silicone oil induced secondary glaucoma. The medica records of 213 eyes of 210 consecutive patients who underwent pars plana vitrectomy and silicone oil injection between January 1990 and June 1997, were reviewed retrospectivel. Fifty five of 213 eyes(25.8%) developed secondary glaucoma. The mean follow-up period was 33.4 months. The mean amount of injected silicone oil was 5.0+/-1.0ml, and 33.4 the mean numbers of operation prior to the silicone oil injection was 2.3. 65.5% of secondary glaucoma was developed within 1 year of silicone oil injection. High preoperative intraocular pressure(IOP) (p=0.004), high myopia(p=0.015) and presence of silicone oil in anterior chamber(p=0.004) increased the incidence of secondary glaucoma significantly. In conclusion, patients who have high preoperative IOP, high myopia and silicone oil in anterior chamber must be carefully followed after silicone oil injection due to high incidence of secondary glaucoma, especially during the first 1 year.
