1.Efficacy of Endoscopic Variceal Ligation on Bleeding Esophageal Varices.

Endoscopic injection sclerotheraphy(EIS) was an effective method for treatment of bleeding esophageal varices. However, it might be associated with some undesirable complications. Endoscopic variceal ligation(EVL) is a recently developed method for control of active variceal bleeding and eradicating esophageal varices with similar efficacy and minimal risk of complications compare to EIS. We performed EVL in 40 patients who had recently bled from esophageal varices. Total 198 variceal ligations were performed during 64 separate EVL session. Control rate of acute bleeding was 90%(36 of 40patients) and 4 patients died after EVL because of failure of bleeding control. Rebleeding following initiation of EVL occured in 5 patients-three patients were successfully controlled by EVL, one patient was controlled but eradication was impossible and one patient died. Varices were eradicated or reduced to grade I in 31(86.1%) of 36 survivors by 1-9 ligation(mean 5.0) in l-4 EVL sessions(mean 1.6). After EVL, there were mild complications-mild substernal discomfort in 4 patients, mild dysphagia in 2 patients and fever in 2 patients. These results suggest that EVL is a safe and effective method for treatment of bleeding and eradication of esophageal varices with less complication.

2.Improved Bioethanol Production Using Activated Carbon-treated Acid Hydrolysate from Corn Hull in Pachysolen tannophilus.

To optimally convert corn hull, a byproduct from corn processing, into bioethanol using Pachysolen tannophlius, we investigated the optimal conditions for hydrolysis and removal of toxic substances in the hydrolysate via activated carbon treatment as well as the effects of this detoxification process on the kinetic parameters of bioethanol production. Maximum monosaccharide concentrations were obtained in hydrolysates in which 20 g of corn hull was hydrolyzed in 4% (v/v) H2SO4. Activated carbon treatment removed 92.3% of phenolic compounds from the hydrolysate. When untreated hydrolysate was used, the monosaccharides were not completely consumed, even at 480 h of culture. When activated carbon-treated hydrolysate was used, the monosaccharides were mostly consumed at 192 h of culture. In particular, when activated carbon-treated hydrolysate was used, bioethanol productivity (P) and specific bioethanol production rate (Qp) were 2.4 times and 3.4 times greater, respectively, compared to untreated hydrolysate. This was due to sustained bioethanol production during the period of xylose/arabinose utilization, which occurred only when activated carbon-treated hydrolysate was used.

5.A Clinical Study of Malignant Germ Cell Tumors of the Ovary.

Malignant germ cell tumore occur in children and young women in reproductive age, of all the germ cell maligaanci orily pure dysgerminiomas had a high cure rate prior to 1970. This was due to the exquisite wdioseneitivity of these tumors. Multiple agent chemotherapy has dramatieally improved the pr nosis af patients with malignant ovarian germ cell tumors. The purpose of this study is to report the experience at Aaan Medical Center, department of Obstetrics and Gynecology, in 16 patients withmalignant ovarian germ cell tumors treated between July, l989 and June, l994. We analyzed the effect of age, histolagic subtype, FIGO stage, surgical pmcedurse and regimens of chemotherapy, on the prognosis of thwe tumors. The results obtained were as follows: 1. In histologic subtypes, dysgenninoma(38.0%), endodermal sinus tumor(25.0%), squamous cell carcinoma arising in mature cystic teratoma(19.0%), mixed cell tumor(6.0%), immature teratoma(6.0%), malignant ectodermal tumor in mature cystic teratoma(6.0%), were counted in order. 2. No site predilection was identifed. 3. Main initial symptoms were abdominal distension(31.0%), abdominal pain(31.0%), abdominal mass palpation(25.0%), amenorrhea(6.0%) in order. 4. Multiple tumor markers were considered to be useful in diagnosis and follow up of malignant germ cell tumors of ovary. 5. The mean age of malignant ovarian germ cell tumors was 29.5 years, and 11 cases(69.3%) of tumors under the age of 30.0 years. 6. The survival rate seemed to be decreased with advancing FIGO stage.

6.Malignant Fibrous Histiocytoma in Spine.

Malignant fibrous histiocytoma is a rare malignant tumor of probably histiocytic origin. It is more common in soft tissue than bone. Usually it involves metaphysis or diaphysis of long bone. Primary spinal malignant fibrous histiocytoma is exceedingly rare. Authors report a case of primary malignant fibrous histiocytoma which showed rapid spinal cord compression in thoracic spine with review of literatures.

8.A Study of Factors Related to Exercise Behavior in Civil Airmen.

The main purpose of this study was to examine factors related to exercise behavior of civil airmen. The subjects for the study were 200 civil airman including pilots, flight officers and flight engineers working for "A" airline in South Korea. The data was collected from April, 1 to 30, 1996 using a questionnaire that was developed for this study, and analyzed using SPSS(Statistical Package for Social Science). The subjects consisted of 200 males. The average age was 42 years old and the majority(82.5%) were married. The large proportion of subjects had received four years of college education or more(86%), and reported having a religion(71%). The average height and weight were 171.7cm and 69.4kg respectively. The average working period and total flight time were 8 years and 6002 hours respectively. Ninety-five percent of sample exercised regularly. As for the frequency of the exercise, 4% of the sample never exercised and 26% of the sample exercised less than once per week, while 44.5% and 24.5% of the sample exercised once or two times, and more than three times per week respectively. The type of exercise was strengthening (30.0%) and aerobic exercise such as jogging(54%), mountain climbing(37.5%) and golf(33.0%). Nearly three fourths of the sample were involved in more than the "light heavy" level of intensity(72.5%) and the average duration was 74.9 minutes. The following factors are related to the exercise behavior. 1) Age and marriage were positively related to exercise behavior(p<.05). 2) The cognitive/perceptual factor is related to exercise behavior. Those who perceived that their health was poor, that health was out of their control exercised less frequently. Also those who showed low self efficacy, and those who valued their health less exercised less frequently. 3) Among environmental factors, social support such as peers, spouse and media influenced exercise behavior. Also the use of a health center was related to a higher level of exercise behavior compared to self-regulated home exercise. Irregular duty was the higher barrier to exercise. 4) The type and intensity of exercise were related to exercise behavior, but not duration. Those who have been jogging or mountain climbing regularly, and those who exercised rather intensely showed a higher frequency of exercise. However, there was no difference in duration 5) The results of stepwise multiple regression analysis showed that the variables which were related to exercise behavior were exercise intensity, exercise place and whether the subject was married. These three factors accounted for 37% of the variance.

10.A comparative study on the standard and individually corrected radiographs in TMJ transcranial radiography.

Among the radiographs for temporomandibular joint, transcranial radiograph is widely used for screening and diagnosing temporomandibular disorder because it has sharp image and it is easy to take. This study was performed to compare condylar position and image sharpness in standard and individually corrected transcranial radiographs using Accurad-200 headholder. Submentovertex view, Reverse-Towne view, Standard and individually corrected transcranial radiographs of 45 university students who were randomly selected were traced, measured and analyzed. The results were as follows: 1. The means of condylar axes and lateral slopes were 16.8degrees and 22.5degrees respectively. There were no differences between male and female or right and left side. 2. Individually corrected radiographs showed smaller posterior joint space and larger anterior joint space than standard radiographs, but superior joint space did not show a statistical difference between standard and individually corrected radiographs. 3. While a large number(42.2%) of the standard radiographs showed concentric condylar position, lots of(57.8) condylar positions were retropositioned in the individually corrected radiographs. 4. The image sharpness was inferior in the individually corrected radiographs to that of the standard radiographs.
