1.Efficacy of Additive Trans-cuff Augmentation Sutures for Proximal Humeral Fractures Stabilized by Locking Plates in Elderly Patients.

BACKGROUND: The purpose of our study was to evaluate the functional and radiologic outcomes of additive augmentation sutures through rotator cuff for proximal humeral fractures stabilized locking plate in elderly patients. METHODS: We enrolled 74 patients over the age of 60 years who received internal fixation using locking plates for proximal humeral fractures. Of these, 50 patients had additive augmentation sutures through rotator cuff. The mean age at the time of surgery was 72.1 years (range, 60-89 years), and the mean follow-up period was 17.5 months (range, 12-62 months). The humeral neck-shaft angle and humeral head height were used as radiological markers to assess the effect of additive augmentation sutures through rotator cuff. We allocated the patients who received additive augmentation sutures into group A and those who did not into group B. RESULTS: At the final follow-up, the mean Korean Showlder Society score and Constant scores were 88.96 +/- 12.1 and 86.6 +/- 11.9, respectively, in group A and 86.21 +/- 11.8 and 85.3 +/- 11.7, respectively, in group B (p=0.368, 0.271). At the final follow-up, the mean loss in humeral neck-shaft angle from the time of immediate postoperative measurement was 1.6degrees in group A and 4.8degrees in group B, whereas the mean loss in humeral head height was 0.82 mm in group A and 0.52 mm in group B (p=0.029, 0.178). CONCLUSIONS: The surgical outcomes of internal fixation using locking plates for proximal humeral fractures were clinically and radiologically good in elderly patients over the age of 60 years without any observable complications. Further, the loss of humeral head shaft angle at the final follow-up from its initial postoperative measurement was significantly smaller in patients who received an additive augmentation suture than in those who did not. Thus, we conclude that augmentation sutures are a beneficial option for elderly patients that clinicians can consider at the time of surgical decision making.

2.Value of the New 5-Tiered Prostate Cancer Grade Group System on Predicting Oncological Outcomes for Radical Prostatectomy Population in Korea

PURPOSE: We evaluated the prognostic value of the 5-tiered grade group in Korean patients who underwent radical prostatectomy. MATERIALS AND METHODS: Between 1996 and 2016, a number of 2,883 consecutive patients who underwent radical prostatectomy were included for the analysis. The impacts of biopsy and pathologic grade group on predicting biochemical recurrence (BCR) were assessed using multivariate analysis. Median follow-up duration was 49.0 months. RESULTS: Mean age was 66.5 years and prostate-specific antigen (PSA) was 11.8 ng/mL. Prostate cancer was locally advanced on magnetic resonance imaging in 13.4%. Biopsy grade group was as follows: 1 (46.8%), 2 (19.8%), 3 (14.2%), 4 (14.1%), and 5 (5.1%). Pathology stage was ≤T2 in 63.6%, T3a in 26.0%, and T3b/T4 in 10.4% patients. Pathologic grade was as follows: 1 (31.3%), 2 (37.9%), 3 (20.2%), 4 (4.7%), and 5 (5.1%). In multivariate analysis using biopsy-related variables, biopsy grade group (1, reference; 2, hazard ratio [HR], 1.771; p=0.001; 3, HR, 2.736; p < 0.001; 4, HR, 2.966; p < 0.001; 5, HR, 3.707; p < 0.001) was associated with BCR-free survival, PSA level and % positive core. In multivariate analysis using pathologic outcomes, pathologic grade group (1, reference; 2, HR, 1.882; p < 0.001; 3, HR, 3.352; p < 0.001; 4, HR, 3.890; p < 0.001; 5, HR: 3.118, p < 0.001) was associated with BCR-free survival in addition to pathologic stage and positive surgical margin. CONCLUSIONS: New 5-tiered grading system could be useful for predicting oncological outcomes in Korean patients although its role for distinguishing outcomes between patients with grade groups 3–5 need to be validated before wide application of this grade system in Korea.

3.Sex Differences in Lifestyle Factors of Metabolic Syndrome in Korean Adults.

BACKGROUND: Metabolic syndrome is a major risk factor for cardiovascular disease. The combination of an aging population with a chronic diseased population is leading to an increase in the mortality rate due to cardiovascular diseases. The purpose of this study was to assess the prevalence of metabolic syndrome in the Korean population and to evaluate its related factors for sex differences. METHODS: From April 2007 to December 2008, a total of 2,729 volunteers aged 40-69 years living in Changwon city underwent a clinical examination at a hospital in Changwon, Korea. Metabolic syndrome was diagnosed following the criteria defined in Circulation 2009. The clinical examination included measuring anthropometric variables and cardiovascular risk factors, while lifestyle factors were assessed through a questionnaire. RESULTS: The prevalence of metabolic syndrome in our sampling pool was 25.2% with a break down by sex showing a rate of 30.2% in men and 22.2% in women. Related factors associated with metabolic syndrome common to both sex were age, self-health assessment, and alcohol consumption. Sleep duration seemed to be a related factor in men, while income, education, and menstruation status played significant roles in women. CONCLUSIONS: To better manage metabolic syndrome, men need to be educated on alcohol use and women of low socioeconomic status require particular attention as do the aging population and postmenopausal women.

4.Sex- and Age-Related Changes in Connexin 43 Expression in Normal Rat Bladder.

PURPOSE: Gap junctions are intercellular channels to facilitate electrical and metabolic communication between adjacent cells. Connexin 43 is the most predominant type of connexin expressed on rat detrusor muscle cells. We investigated the connexin 43 expressions in various age groups of either sex in normal rats. METHODS: Eighty Sprague-Dawley rats were used for analysis. Each group was quantified by 8 rats at 1 week, 2 weeks, 1 month, 3 months, and 6 months of age in either sex. In each animal, bladder was removed without any kind of intervention and fresh-frozen in liquid nitrogen. Total RNA extraction was done with easy-BLUE total RNA extraction kit. Reverse transcription polymerase chain reaction was done for connexin 43 and glyceraldehyde-3-phosphate dehydrogenase as an internal control using ImProm-II Reverse Transcription System. RESULTS: In female rats, no age-related change was detected in connexin 43 expressions. In male rats, connexin expression at 3 months of age showed significant decrease compared with 1 week, 2 weeks, and 6 months of age (P<0.05). When connexin expression at the same age in male and female were compared, only 3 months group in male showed significant decrease than the same age group in female. CONCLUSIONS: Our data suggest that the expressions of connexin 43 mRNA in normal detrusor muscle cell showed age-related changes especially in male rats. Although it is difficult to interpret these findings at this stage, age should be considered as a possible compounding factor affecting connexin 43 expressions in male rats.

5.Gastric Cancer Susceptibility in the P53 Codon 72 Polymorphism.

Purpose: The P53 codon 72 polymorphism results in either arginine or proline, there are many studies to clear the relationship between P53 codon 72 genotypes and specific cancer risk and susceptibility. The purpose of this study was to investigate the association of the genotype distribution of the P53 codon 72 polymorphism and gastric cancer susceptibility via in comparison of gastric cancer group and normal control genotypes. We also studied the relation between the distribution of P53 codon 72 genotypes and the state of P53 immunohistochemical staining, infectivity of Helicobacter pylori (H.pylori) and the clinicopathologic findings in gastric cancer patients. METHODS: In our study, the samples consisted of 145 gastric cancer patients and 77 normal controls. The analysis was performed by polymerase chain reaction (PCR), restriction fragment length polymorphism (RFLP) method using DNA extracted from gastric cancer patients blood and normal controls blood. RESULTS: The frequency of three genotypes arg/arg, arg/ pro and pro/pro in gastric cancer patients was 41.4%, 38.6% and 20.0%. In controls, it was 36.3%, 53.2% and 10.3%. There was no statistical significance (P=0.312, 0.665). There was no correlation between the frequency of the three genotypes and the state of P53 immunohistochemical staining, infectivity of H. pylori. The pro/pro homozygote was more frequent in lymph node metastasis (25.6% vs 7.3%, P= 0.026). Conclusion: The P53 codon 72 polymorphism does not contribute to gastric cancer susceptibility. The P53 codon 72 polymorphism is not associated with the state of P53 immunohistochemical staining and the infectivity of H. pylori but pro/pro genotype is associated with the lymph node metastasis in gastric cancer patients.

Constipation is a prevalent, often chronic, gastrointestinal motility disorder. Bisacodyl, a stimulant laxative, is widely used to treat constipation in adults and children. This drug is usually safe, but it has some side effects including diarrhea, abdominal pain, colitis, and proctitis. There have been reports that rectal administration of bisacodyl could cause injury to the rectal mucosa by mechanical and chemical mechanisms. However, there has been no report of severe proctitis with rectal ulcers in patients taking oral bisacodyl. In this report, we describe the case of an 80-year-old woman with severe rectal ulcers and proctitis after taking oral bisacodyl for several days, and review the literature.

7.Health Care Behavior of People 60 Years and Older in Korea According to Family Type and Sociodemographic Factors - The 5th Korea National Health and Nutrition Examination Survey.

BACKGROUND: This study evaluated the effects of family type and sociodemographic factors on medical treatment, health behavior, and subjective health status in Koreans 60 years and older. METHODS: This study was based on data from the 5th Korea National Health and Nutrition Examination Survey (KNHANES), a nationwide health interview survey using a rolling sampling design involving complex, stratified, multistage, probability cluster. Our subjects included those > or =60 years of age (n=1,946) participating in the KNHANES 2010. Family type was categorized into 7 groups according to cohabitation: alone, couple cohabitation (living with a spouse), parent(s) cohabitation (couple living with parent(s)), offspring cohabitation (no spouse, living with offspring), couple offspring cohabitation (couple living with offspring), couple offspring parents cohabitation (couple living with parent(s) and offspring), and others. Logistic regression analyses were used to examine the association of family type and sociodemographic factors with health care. RESULTS: The most common family type was couple cohabitation (38.26+/-1.8%). In this group, the percentage of non-receipt of needed medical care (odds ratio [OR], 0.60; 95% confidence interval [CI], 0.42 to 0.87) and current smoking rate (OR, 0.32; 95% CI, 0.20 to 0.50) were significantly lower than those living alone. Among the sociodemographic charicteristics, male gender, apartment dwelling, high middle household income, 2 member family, education higher than middle school, and possessing own house were associated with good health care behavior. CONCLUSION: This study shows that couples have better health care behavior than other family types, in particular, limited smoking, appropriate medical treatment, and positive subjective health status. Those living alone visit medical clinics less often and have a negative subjective health status.

9.The Role of Bone Marrow Mononuclear Cells in Angiogenesis in Mouse Hind Limb Ischemic Model.

BACKGROUND: Angiogenesis is enhanced in the ischemic tissues after the injection of bone marrow cells (BMCs). However the exact mechanisms for this are not yet fully understood. METHODS: A unilateral ischemic limb was surgically induced in mice and then BMCs were injected into the ischemic area. We measured the capillary/muscle ratio. Fluorescence-labeled BMCs were injected into the ischemic tissues and then the locations of the cells were examined by using a confocal microscope. Recruitment of bone marrow-derived cells into the ischemic tissue was examined in a sex-mismatched bone marrow transplantation (BMT) setting by identifying the Y chromosome with using the FISH technique. The expressions of VEGF, MMP-9, SDF-1 and CXCR-4 were measured by Western blot analysis. RESULTS: The capillary/muscle ratio was more increased in the BMC-injected group than in the control group (P<0.05). Florescence-labeled BMCs, which had been directly injected into ischemic tissue, were not detected in the tissue. In the sex-mismatched bone marrow transplantation models, the ischemic tissues of the BMC-injected group recruited a much greater number of Y chromosome-positive bone marrow- derived cells, as compared to the control group. The expressions of VEGF and MMP-9 were increased after injection of BMCs. SDF-1 was expressed on the seventh day in the BMC-injected group and CXCR-4 was highly expressed until 12 weeks in the BMC-injected group. CONCLUSION: We suggest that the injection of BMCs into ischemic tissue recruits CXCR-4-positvie cells from the bone marrow via the up-regulation of VEGF, MMP-9 and SDF-1, and these CXCR-4-positive cells may play a role in neovascularization.

10.Differential Control Rate of Systolic and Diastolic Blood Pressure among Korean Adults with Hypertension: the Sixth Korean National Health and Nutrition Examination Survey, 2013–2015 (KNHANES VI)

BACKGROUND AND OBJECTIVES: Discrepancy in blood pressure (BP) control rate may be owed to different risk factors associated with elevated systolic and diastolic BP (SBP and DBP). METHODS: In a nationally representative survey of Korean population with BP measurements, a total of 5,100 participants with hypertension was included in the analyses. Three separate types of control rates were calculated: SBP, DBP, and (combined) S&DBP among all participants with hypertension and participants with treated hypertension, separately. Control rates were compared across sex and age groups. Multivariable logistic regression was used to identify demographic factors associated with SBP, DBP, and S&DBP control rates, separately. RESULTS: In total, SBP, DBP, and S&DBP control rates were 63.0%, 63.7%, and 42.5% among all hypertension patients, and 77.5%, 87.4%, and 71.6% among treated hypertension patients. Men aged 40–49 years with hypertension and 30–39 years treated for hypertension had the highest SBP control rate (74.6% and 96.2%), which decreased by older age. Inversely, DBP control rate progressively increased with older age. SBP control rate among women with hypertension was the highest in 40–49 years (67.0%) but without linear trend by age group. Interestingly, both combined and DBP control rates were noticeably low among women aged 50–59 years. Women with body mass index ≥25 were at higher odds of having controlled SBP. Lower DBP control rate was observed in men with lower education level, higher household income, and heavy drinkers. CONCLUSIONS: Separate examination of control rates demonstrated different sex- and age-differential trends, which would have been overlooked in combined control rates.
