2.Acute Renal Failure Associated with Gross Hematuria in a Patient with Focal Glomerulonephritis.

A 58-year-old female with an episode of gross hematuria two months before and fever and chill for the past three days presented oliguric acute renal failure. She has taken NSAID intermittently for 18 years due to rheumatoid arthritis, and herb medicine for one week two months ago when gross hematuria developed. Physical examination revealed mild tenderness on costovertebral angles. Her blood pressure was 170/100 mmHg, the urinalysis showed >300 mg protein with many RBCs and 10-20 WBCs and the serum creatinine was 5.8 mg/dl. A renal biopsy performed on the 4th hospital day showed that it was overwhelmed by severe tubular lesions which reveal intratubular obstruction by massive erythrocyte casts and tubular necrosis. The glomeruli showed focal minimal crescents with many red blood cells entrapped in the crescents and in the capillaries. Immune deposits were not present. A renal failure resolved spontaneously and the patient was discharged three weeks later with creatinine of 2.4 mg/dl. In this patient, acute renal failure was considered to be due to a tubular lesion related to the glomerular bleeding from focal glomerulonephritis revealing minimal crescents.

4.The Effects of Self-efficacy and Social Support on Depression in Elementary School Children.

PURPOSE: The purpose of this study was to investigate the effects of self-efficacy, social support, and depression in elementary school children. METHODS: The participants of this study were 619 students from seven elementary schools located in Jollabuk-do Province, Korea. The methods included data analysis, descriptive statistics, t-test, ANOVA, Scheffe test, Pearson's correlation coefficients and stepwise multiple regression using SPSS/WIN version 15.0. RESULTS: The mean scores of self-efficacy, social support, and depression were 2.9+/-0.58, 3.2+/-0.44 and 14.0+/-7.32, respectively. There was a significant positive correlation between self-efficacy and social support (r=.517, p<.001). Significant negative correlations were observed between self-efficacy and depression (r=-.557, p<.001) and between social support and depression (r=-.571, p<.001). The variables of depression in elementary school children were self-efficacy and social support, which explained 42.7% of the cases. CONCLUSION: The findings from this study suggest that current intervention programs are not sufficient to curb depression in elementary school children and therefore it is needed to develop nursing intervention programs to promote self-efficacy and social support.

6.Autogeous Bone-Articular Cartilage stored within Abdominal Wall

Articular cartilage can be damaged by trauma or by progressive degradation caused by disease. Unfortunately, it has a extremely limited capacity for repair and regeneration. The use of joint prosthesis has revolutionized but remains limited in its use. To know the possibility of clinical application of clinical application of autogenous bone-articular graft in open complicated intraarticular fractures we studied the change of bone-articular cartilage defect in medial femoral condyle of rabbits and viability of autogenous bone-articular graft stored within abdominal wall of rabbits. The results were obtained as follows: 1. There was over 90% viability of subchondral bone after 10 days storage in the adbominal wall but the articular cartilage showed degenerative change about 40% after 5 days passed. 2. The bone-articular defect was replaced by undifferentiated mesenchymal tissue and fibrous tissue, postoperative 6 weeks later. 3. In implanted bone-articular cartilage graft at 3 days, the chondrocyte of basal cell layer in articular cartilage was relatively well preserved but in 7 days, transplanted articular cartilage graft showed almost necrotic chage. Most of subchondral bones were well healed with host bone without regard to duration of implantation.

7.Effect of Cyclosporine A on Plasma and Urine Levels of Endothelin-1 in Steroid Dependent Minimal Change Nephrotic Syndrome.

A fine needle aspiration biopsy specimen of a retroperitoneal mass in a 26-year-old man who had had an orchiectomy for seminoma was submitted for cytologic evaluation. Cytologic features of the specimen included uniform neoplastic cells found singly or in groups of several cells intermingled with lymphocytes in a characteristic foamy, lacelike background. These cells varied from 10 to 20 m in diameter. The nuclei were round to ovoid with fine or reticular chromatin and one or more prominent nucleoli. The poorly defined cytoplasm stained pale-blue or blue with cytoplasmic vacuoles. The cytologic appearance was consistent with seminoma. Documented reports of the cytological appearance of seminoma are rare. The diagnosis of primary gonadal seminoma by fine needle aspiration biopsy is probably not indicated since the treatment of a primary gonadal tumor, regardless of its histogenesis, requires surgical resection. However, fine needle aspiration biopsy is extremely valuable in the diagnosis of extragonadal as well as metastic and recurrent seminoma.

8.Factors Predicting Medication Compliance among Elderly Visitors of Public Health Centers.

PURPOSE: The purpose of this study is to investigate compliance with prescribed medication in the elderly visiting public health centers. METHOD: Data were collected from 665 elders living in Seoul and the Gyeonggi Province during the period from February 21 to June 30 in 2006. The data were collected through individual interviews and were analyzed using correlation and multiple regression analysis with the SAS 9.1 program. RESULTS: The mean of medication compliance was 2.97(+/-.68) on a 5-point Likert scale. Specifically, compliance 3.14(+/-.70) for medication dose, 2.94(+/-.77) for medication frequency, and 2.84(+/-.79) for medication time. The elderly with a higher level of education (beta=.095, p<.001), with health insurance (beta=.208, p=.0009) and with a higher level of family support (beta=.040, p=.0306) showed a higher level of mediation compliance. Female elders (beta=.142, p<.001) kept higher medication compliance than male ones. CONCLUSION: These findings suggest that people with low education, low socioeconomic status and less family support need more education before medication. The evaluation of medication compliance needs to be encouraged before starting medication to distinguish those who may not comply with medical prescription. Predictive factors identified in this study must be considered when designing interventions, program development and education for appropriate medication management for the elderly.

10.Microsurgical Anatomy of Circle of Willis in Korean Cadavaric Specimen.

The authors studied and analyzed microscopically the structure, length and diameter of each part of Willis circle in 264 cerebral hemispheres of 182 autopsied patients in the Taegu area from 1984 to 1990. The mean length of each A1 was 14.1mm and diameter at distal A1 was 2.25mm. The most common anatomical anomaly of the ACA part was A1 that of hypoplasia which appeared in 7.5% of the cases. The origin of the Heuner a. was accounted for the highest incidence of AcoA(82.5%). The length of AcoA. Noted 3.5mm with the incidence of duplication and triplication in 38.6%. The inner diameter of ICA noted 3.9mm at the communicating segment in ICA, and the average length of C4 was 12.5m. Except 5 hemispheres of undetectable origin at ICA, all the anterior choroidal a. originated from ICA, and the double stump of which noted 15.5%. Fetal type PcoA. Showed 6.6% of all and average length of the a. was 13.5mm. The inner diameter of MCA was 3.1mm and the mean length of M1 noted 15.5mm. The length of each P1 noted 6.95mm and the inner diameter of basilar a. at 1cm below the bifurcation was 3.3mm. The mean circumference of willis circle including both side of A1, P1, PcoA. AcoA. And choroidal and communicating segment of ICA was 88.6mm. Incidental finding of unruptured aneurysm noted 5.3% and junctional dilatation of PcoA. Showed 11.4% of all the cases.
