2.MR evaluation of visceroatrial situs abnormality.

Thirteen patients with visceroatrial situs abnormalities were evaluated by magnetic resonance(MR) imaging. Eleven patients were confirmed surgically. Two patitnts were diagnosed by MRI and cardiac catheterization. Right isomerism was found in seven patients, left isomerism in two, and situs inversus in four. For the determination of situs, we evaluated the morphology of atrial appendages and main bronchi, the relationship between abdominal aorta and inferior vena cava(IVC), and the status of upper abdominal viscera. The bilateral atrial morphology was differentiated in 8 of 12 patients. The bronchial situs was determined in 11 of 12 patients. Juxtaposition of abdominal aorta and IVC was found in 6 of 7 with right isomerism. IVC interruption with azygos continuation was found in all two with left isomerism. Incidentally three cases of short pancreas were found. MR imaging showed all structures relevant for the assessment of situs, thus obvrating the need for performing additional diagnostic procedures. MR imaging, therefore, is a valuable tool in the clinical management of patients who are suspected of having a situs abnormality.

4.A Clinical Review of 1420 Burn Cases of Burn Patients.

This is a review and clinical analysis of 1420 cases of burn patients who were treated at the Department of surgery, Hanil Hospital during 6 years from 1991 to 1996. The following results were obtained. 1) The age group distribution showed that the highest incidence of burn patients was in the between 30 and 39 years old.(323 cases, 22.8%). 2) The highest incidence of burn in the seasonal distribution was in the winter(383 cases, 26.9%). 3) The most common cause of the burns was scalding by hot liquids(444 cases, 31.2%). 4) The most frequent period of interval between accident and the arrival at the hospital was within 24 hours or less(1122 cases, 79.0%). 5) The most frequent duration of treatment was within 2 weeks or less(620 cases, 43.7%). 6) Most patients were affected less than 10% of burn(783 cases, 55.1%). 7) Second degree was most frequent burn depth(583 cases, 41.1%). 8) In 336 cases of burn wound culture, the main organism were Pseudomonas aeruginosa 231 cases(68.7%), Staphylococcus aureus 183 cases(41.0%), and others. 9) Operation was performed in 483 cases(34.0%). Skin graft 471cases, escharotomy 132 cases, fasciotomy 26cases, amputation 13 cases, primary repair 29cases. The most frequent period of skin grafting was between 3 to 4 weeks(253 cases). 10) The complication of burn were wound infection 435 cases(25.4%), UTI 96 cases(6.7%), others. 11) The overall mortality rate was 8.7%(124/1420) and the most frequnt cause of death was sepsis in 58 cases(46.8%).

Oral or rectal sodium phosphate(NaP) preparation is commonly used for the colonic cleansing. Since the sole route of excretion of absorbed phosphate is renal, diminished renal function will limit the ability to excrete a sudden phosphate load. To date, about 20 adult cases of hyerphosphatemia associated with oral or rectal sodium phosphate solution have been reported, but not a single case in Korea to our knowledge. We report two cases of hyperphosphatemia and hypocalcemia after the administration of NaP(Colclean(R)) to reemphasizes the potential hazard of sodium phosphate bowel preparation in patients with compromised renal function.

9.Impact of Prostate Volume on Oncological and Functional Outcomes After Radical Prostatectomy: Robot-Assisted Laparoscopic Versus Open Retropubic.

PURPOSE: We compared the impact of prostate volume on oncological and functional outcomes 2 years after robot-assisted laparoscopic prostatectomy (RALP) and open radical retropubic prostatectomy (ORP). MATERIALS AND METHODS: Between 2003 and 2010, 253 consecutive patients who had undergone prostatectomy by a single surgeon were serially followed over 2 years postoperatively. RALP was performed on 77 patients and ORP on 176. The patients were divided into two subgroups according to prostate volume as measured by transrectal ultrasound: less than 40 g and 40 g or larger. Recoveries of potency and continence were checked serially by interview 1, 3, 6, 9, 12, and 24 months postoperatively. RESULTS: RALP was associated with less blood loss (ORP vs. RALP: 910 mL vs. 640 mL, p<0.001) but a longer operation time (150 minutes vs. 220 minutes, p<0.001) than was ORP. No statistically significant differences were found between the two groups for oncological outcomes, such as positive surgical margin (40% vs. 39%, p=0.911) or biochemical recurrence (12% vs. 7%, p=0.155). The overall functional outcomes showed no statistically significant differences at 2 years of follow-up (continence: 97% vs. 94%, p=0.103; potency: 51% vs. 56%, p=0.614). In the results of an inter-subgroup analysis, potency recovery was more rapid in patients who underwent RALP in a small-volume prostate than in those who underwent ORP in a small-volume prostate (3 months: 24% vs. 0%, p=0.005; 6 months: 36% vs. 10%, p=0.024). However, patients who underwent RALP in a large-volume prostate were less likely to recover continence than were patients who underwent ORP in a large-volume prostate (97% vs. 88%, p=0.025). CONCLUSIONS: Patients can be expected to recover erectile function more quickly after RALP than after ORP, especially in cases of a small prostate volume.
