1.Study of cutaneous hydration effect according to the vehicles of moisturizers.

We investigated on how the nature of vehicles influenced on the efficacy of mo isturizer s, cutaneous hydrat,ion, physical property and roughness. Hydration was measured with corneometer, physical property with fermometer, and mughness with image analyser. Used vehicles were as follows : hydrous gel, vater in oil(W/0) emulsion, oil in water(0/W) emulsion. I.Jsed moisturizers were as follows . glycerine, polypeptide and sodium lactat,e, hyaluronic acid-triethanolamine, sodium lactate, pvopylene glycol, The results are summarized as follows : 1. The increment of the hydration effect according to vehicles was in this order. C/ W emulsion, W/0 emulsion, hydrous gel(After 2 hours). 2. The increment of extensibility as physical property was correlated to the in crcment, of hydration effcet(r=0.924). 3. It was shown that, the roughness was decreased after treatment of all mo isturizers except propylene glycol but, there was not significant st.atistically and alsa was no meaningful difference among the vehicles. 4. Among t,he moisturizers, glycerine was superior to other moisturizers on the hydration effect and extensihility. We conclude that 0/W emulsion was suitable vehicle on cutaneous hydration effect and hydration effect is proportion to the increment of flexibility.

2.A case of polymorphism reticulosis that showed eczematoid skin lesion.

A 34-year-old Korean man presented with a 3 week history of a yellowish eczema like patch on the left thigh. Sixteen months prior, he was diagnosed with polymorphie r eticulosis of the left nasal cavity. The patient, refuse<3 conventional radiation therapy and took herbal medication and diet therapy instead. Biopsy of the nasal cavity, hard palate and left thigh lesians showed extensive necrosis and polymorphic infiltrates of atypical lymphocytes with angiodestructive invasion. Irnmunohistochemical study of left thiyh lesion showed strong CD2 positivity con sistent with the diagnosis of peripheral T cell lymphorra.

3.Pigmented Eccrine Poroma on the Scalp.

5.CT Classification of Renal Injury and Its Role in Decision on Operation.

PURPOSE: This study was performed to examine if CT classification of renal blunt injury could aid in expectation of hemodynamic stability and clinical decision of whether to intervene surgically. MATERIALS AND METHODS: Over a 80-month period between July 1987 and March 1994, 41 patients were admitted to our hospital with the diagnosis of renal blunt injury. The renal blunt injuries were classified on Fedede's three-point scale CT classification methods :grade I, contusion, intrarenal hematoma, segmental infarction, and small subcapsular hematoma;grade II, complete or incomplete laceration, large subcapsular hematoma, and renal fracture;grade Ill, shattered kidney and renal pedicle injury. Hemodynamic stability, treatment method and clinical outcome of the patients with different CT grade were analyzed retrospectively. RESULTS: All 34 patients with grade I or II CT findings were hemodynamically stable and were successfully managed with conservative method. Among 7 patients with grade III CT findings, 6 patients were hemodynamically unstable. Out of the 6, One patient with grade IIIb or renal pedicle injury was expired before surgical intervention due to ischemic shock. Four patients were intervened surgically with one failure to thrive. The remaining one patient refused to be intervened surgically, and was discharged against medical advice. Only one out of 7 patients was hemodynamically stable and was managed conservatively. CONCLUSION: The patients with grade I or II CT findings are prone to be hemodynamically stable and to be managed with conservative method. But the patients with grade III CT findings are more likely to be hemodynamically unstable. Therefore patients with grade III CT findings should be closely monitored and be pre- pared for the possibility of immediate surgical intervention

6.Initial Experience of Retroperitoneoscopic Surgery.

PURPOSE: We tried to evaluate the safety, feasibility and the clinical usefulness of the retroperitoneoscopic surgery for the treatment of variable renal diseases. MATERIALS AND METHODS: Between June 1998 and March 1999, a total of 18 patients (9 nonfunctioning kidneys, 1 ureter tumor, 3 simple renal cysts, 1 polycystic kidney, 3 ureter stones, 1 UPJ obstruction) underwent retroperitoneoscopic surgery (9 nephrectomies, 1 nephroureterectomy, 4 cyst marsupialization, 3 ureterolithotomy, 1 pyeloplasty) in our hospital. We analyzed the results of each operations. RESULTS: Out of total 18 cases, 15 retroperitoneoscopic surgeries were performed successfully as planed. Mean operative time was 194 minutes(ranged from 100 to 355 minutes) and mean postoperative hospital stay was 4.1 days(2-7 days). There was no significant intraoperative or postoperative complication in successfully completed cases. Open conversion was needed in three patients, all of them were not in an emergent condition, and there was no significant complication. Transfusion was not required in all cases. CONCLUSIONS: Retroperitoneoscopic surgery is a safe and feasible procedure for benign renal diseases, and there seems to be enough rooms for malignant diseases also.

7.CT Evaluation of Nasal Cavity Masses: Differential Diagnosis between Nasal Polyps and Their Mimics.

PURPOSE: The purposes of this article are to identify CT findings distinguishing nasal polyps, that are the most common cause of the nasal polypiod lesions, from other nasal cavity masses and to identify differential points between benign and malignant masses of nasal cavity. MATERIALS AND METHODS: We classified 567 cases of pathologically proved nasal cavity masses into 4 different groups on CT according to the sites of origin and surrounding bone changes. RESULT: The nasal polyps were the most common cause of nasal cavity masses(515/567). Group I had a high diagnostic specificity for the nasal polyps (513/515, 99.6%) although inverted papilloma (8/522) and malignant melanoma(1/522) showed similar CT appearances. The representitive nasal mass of Group Ila was the inverted papilloma (15/24, 66.5% of the inverted papilloma). Group III pattern was seen in 3 cases of benign minor salivary gland tumor and 2 cases of pyogenic granuloma. Group IV suggested malignancy and thus was mostly observed in malignant tumors except I case of inverted papilloma and 1 case of granulomatous necrosis. CONCLUSION: The differential diagnosis between nasal polyps and the other nasal cavity masses is possible by characteristic bone changes and their sites of origin revealed on CT. In addition, it is possible to differentiate malignant masses from benign in most cases.

8.The Effect of Adult ADHD Tendency on Psychological Characteristics in Young Male Adults with Depression

OBJECTIVES: This study investigated psychological factors in patients with adult ADHD tendency and the relationship of adult ADHD tendency with depression and anxiety.METHODS: Ninety patients with depression(38 with adult ADHD and 52 with non-adult ADHD) were recruited in this study. All patients were subjected to the following tests : Korean Wechsler Adult Intelligence Scale-IV(K-WAIS-IV), Adult Attention-Deficit/Hyperactivity Disorder Self-Report Scale(ASRS-v1.1), Lee Ji-Yeon's adult ADHD scale, Beck's Depression Inventory II(BDI-II), Beck's Anxiety Inventory(BAI), and Temperament Character Inventory-Revised Short version(TCI-RS).RESULTS: The adult ADHD group had higher scores for BDI-II and BAI, novelty-seeking, harm-avoidance, and self-transcendence than the non-adult ADHD group. Conversely, the adult ADHD group showed lower scores for self-directedness and cooperativeness than the non-adult ADHD group. Depression scores were positively correlated with ASRS(r=0.524, p=0.000), Lee Ji-Yeon's adult ADHD scale(r=0.598, p=0.000), anxiety scores(r=0.650, p=0.000), novelty-seeking(r=0.411, p=0.000), harm-avoidance(r=0.517, p=0.000), and self-transcendence(r=0.234, p=0.026). However, they were negatively correlated with persistence(r=−0.231, p=0.029), and self-directedness(r=−0.594, p=0.000). Higher Lee Ji-Yeon's adult ADHD scale scores and lower self-directedness scores among TCI, resulted in higher depression scores. In addition, anxiety scores increased in parallel with the the Lee Ji-Yeon's adult ADHD scale and harm-avoidance TCI scores.CONCLUSION: In this study, we found a difference in the severity of depression and anxiety between the adult and non-adult ADHD groups. In addition, it was confirmed that the difference in temperament and character between the two groups was related to adult ADHD tendency and severity of depression and anxiety.

9.Injury of Anterior Cruciate Ligament with Associated Bony Lesions: MR Image.

PURPOSE: To evaluate the characteristic MR findings in injury of the anterior cruciate ligament (ACL) with associated bony lesions. MATERIALS AND METHODS: We reviewed MR findings and the corresponding arthroscopic or operative results of 48 patients with ACL injuries, and evaluated ACL signal intensity and contour. In associated bony lesions, we determined the location of avulsion fracture and bony bruise. RESULTS: Cmplete ACL tears were seen in 27 cases, partial tears in 13, and avulsion injury in eight. Complete tears showed heterogeneously increased signal intensity with contour bulging in ten cases (37 %), and combined bony lesion in 14 (52 %). ACL with a thin continous low signal intensity band surrounding heterogeneously-increased signal intensity suggested partial tears, and was seen in three of 13 proven cases (23 %) of partial ACL tears ; combined bony lesion was seen in four such cases (31 %). There were eight cases of avulsion fracture ; the most frequent site was the anterolateral portion of the tibial spine (n=6). The most frequent sites of bony lesion were at the midportion of the lateral femoral condyle (n=6), and the posterior portion of the lateral tibial plateau (n=6) ; the next most frequent site was the anterior portion of the lateral tibial plateau (n=5). CONCLUSION: Tearing of the ACL was seen on MRI as ligament discontinuity, and heterogeneously increased signal intensity with ACL contour bulging. The most frequent sites of associated bony lesions were the midportion of the lateral femoral condyle, and the posterior portion of the lateral tibial plateau. In associated bony lesions, bony contusion suggested ACL tearing, but avulsion fracture suggested ligament avulsion injury without tear.

10.A Randomized Prospective Trial Comparing Oral Sodium Phosphate with Magnesium citrate in Preparing of Patients for Double Contrast Barium Enema.

PURPOSE: The purpose of this study was to compare two bowel preparation agents, sodium phosphate solution with magnesium citrate solution. MATERIALS AND METHODS: A total of 94 subjects that underwent a double-contrast barium enema were included in this study. Bowel preparation before performing the barium study was done by using a sodium phosphate solution in 47 subjects and by using a magnesium citrate solution in the other 47subjects. We evaluated the presence or absence of side effects when using these bowel preparation agents. Two radiologist who were blinded to the type of bowel preparation evaluated the quality of bowel preparation at the colonic segments (ascending, descending, and sigmoid colon) on the radiographs obtained by double-contrast barium enema, with regard to stool cleansing, water retention, barium coating and bubble formation. RESULTS: The side effects, such as abdominal clamping pain, nausea, hunger pain and chill occurred more frequently in the sodium phosphate group than in the magnesium citrate group (p<0.001). Stool retention was more frequently found in the magnesium citrate group (p<0.001). However, no statistical difference was noted on the status of water retention and barium coating between two groups. Gas bubble formation was more commonly seen in the sodium phosphate group (p<0.001). The sodium phosphate solution appeared to be more effective in cleansing the right colon (p=0.001). CONCLUSION: Sodium phosphate solution appears to be more effective for colonic cleansing, with a lower incidence of side effects, than when using magnesium citrate solution.
