1.Right-side Bochdalek Hernia with Unusual Kidney Herniation in an Old Patient.

Bochdalek hernia through Bochdalek foramen, defect at the posterolateral side of the diaphragm is one of the congenital diaphragmatic hernias. It usually occurs in infants, but very rare in old age and also on the right side. We experienced the right-side Bochdalek hernia including kidney herniation in 68 years old man man ifested by hemoptysis. CT scan revealed diaphragmatic defect and herniated liver, colon and kidney. Thoracotomy was performed and the 10 cm-sized large defect was closed with patch. After then, the patient has been well without symptom.

2.Clinical Course of Idiopathic Central Diabetes Insipidus in Adults.

BACKGROUND: Idiopathic Central Diabetes Insipidus in children or adolescents requires a frequent follow-up regimen using serial brain MRI and CSF examinations especially if an isolated pituitary stalk thickening or loss of a hyperintense signal in the posterior pituitary lobe is observed. Although a detailed description has not been made in adults who had Idiopathic Central Diabetes Insipidus, the clinical course of a Central Diabetes Insipidus in children or in adolescents can not be applied to adults because a follow-up study is so invasive and expensive. In this report, we evaluated the clinical course of Idiopathic Central Diabetes Insipidus in adults. METHODS: The diagnosis of Idiopathic Central Diabetes Insipidus was based on the presenting clinical symptoms, the water deprivation test, biochemical studies and a brain MRI. We measured the urine specific gravity, urine and plasma osmolarity, electrolytes, and daily urine amount and we also performed an anterior pituitary evaluation. Patients had contrast-enhanced MRI and biochemical studies every 4 to 12 months. RESULTS: The patients included 8 females and 4 males. Their ages ranged from 20 to 76 years and their mean age was 45+/-17 years. Tumor markers in the CSF were not detected any of the patients. An anterior pituitary evaluation showed that four patients had hyperprolactinemia, and five patients had impaired secretory responses of Growth Hormone to an insulin induced hypoglycemia. Nine of the 12 patients had thickening of the pituitary stalk, seven had lacked the hyperintense signal of a normal neurohypophysis. The abnormalities of MRI disappeared in 3 patients by the 4th, 27th and 36th month follow up periods, respectively. The follow up duration was between 8 months and 11 years 3 months and the mean follow up duration period was 50.6+/-45.5 months. Clinical symptoms were corrected by DDAVP administration. Other symptoms were absent. CONCLUSION: In our study, of Idiopathic Central Diabetes Insipidus in adults there were no observed germinomas or other disease that were observed. Therefore this disorder may have a benign course.

3.Toxic epidermal necrolysis induced by deflazacort.

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are severe cutaneous adverse reactions to drugs, which carry significant morbidities and mortalities. Although SJS and TEN can be treated by corticosteroids or intravenous immunoglobulin, they may be induced by steroids. We experienced a 36-year-old man who developed whole body erythromatous maculopapular rashes and multiple bullae after ingestion of deflazacort. After avoidance of deflazacort and use of dexamethasone, skin lesions resorbed. We reported a case of TEN diagnosed by characteristic clinical findings with a brief review of the literature.

4.Development and Validation Study of the Internet Overuse Screening Questionnaire.

OBJECTIVE: Concerns over behavioral and emotional problems caused by excessive internet usage have been developed. This study intended to develop and a standardize questionnaire that can efficiently identify at-risk internet users through their internet usage habits. METHODS: Participants (n=158) were recruited at six I-will-centers located in Seoul, South Korea. From the initial 36 questionnaire item pool, 28 preliminary items were selected through expert evaluation and panel discussions. The construct validity, internal consistency, and concurrent validity were examined. We also conducted Receiver Operating Curve (ROC) analysis to assess diagnostic ability of the Internet Overuse Screening-Questionnaire (IOS-Q). RESULTS: The exploratory factor analysis yielded a five factor structure. Four factors with 17 items remained after items that had unclear factor loading were removed. The Cronbach’s alpha for the IOS-Q total score was 0.91, and test-retest reliability was 0.72. The correlation between Young’s internet addiction scale and K-scale supported concurrent validity. ROC analysis showed that the IOS-Q has superior diagnostic ability with the Area Under the Curve of 0.87. At the cut-off point of 25.5, the sensitivity was 0.93 and specificity was 0.86. CONCLUSION: Overall, this study supports the use of IOS-Q for internet addiction research and for screening high-risk individuals.

5.A Study to Identify Contents of the Journal through Review of Classification of Sectional Editorship.

PURPOSE: The purpose of this study was to provide data for development of the Journal of Fundamentals of Nursing through reviewing classification of sectional editorship. METHODS: Descriptive statistical analysis was used, to identify content then, the classifications of sectional editorship were reviewed. RESULTS: The categories, "Basic concepts of nursing" (30.2%:17.9%), "Needs for comfort" (15.9%:13.1%), "Nursing education" (12.7%:12.7%), and "Needs for activity and exercise" (7.1%:7.1%) were most frequent through both the 1st and 2nd sectional editorship. CONCLUSION: Classification of sectional editorship was developed to clarify the educational purpose of fundamentals of nursing, and was modified through reflections by researchers. The current classification of sectional editorship is not a permanent formula, so using various viewpoints in further examination is needed.

6.Study on the Recognition and Purchase Status of Eco-Friendly Oral Hygiene Products

Background:
Plastic waste generates pollutants in the process of incineration or landfilling, and accumulates in water or marine organisms, causing adverse effects on the environment and the human body. Recently, various eco-friendly oral hygiene products (Eco-OHPs) such as bamboo toothbrushes and biodegradable plastic toothbrushes have been developed. Therefore, this study aimed to investigate the current level of awareness and purchasing status of eco-OHPs among adults who are interested in eco-friendly products.
Methods:
This study included adults aged >19 years who regularly visited eco-friendly shops and online sites; the online survey links were distributed during their visits to eco-friendly Internet cafés and companies. Of the 22 questions, seven assessed the participants’ general characteristics, three assessed the general oral hygiene care products used, six assessed the level of awareness of Eco-OHPs, and six assessed the purchasing status of Eco-OHPs. Frequency analysis, chi-square test, and regression analysis were performed using SPSS software.
Results:
Among the respondents, 108 (51.4%) were aware of Eco-OHPs, and 79 (37.6%) had experience purchasing Eco-OHPs. The most common reason for not purchasing was the lack of information about related brands or products (74, 56.5%). The most common platform used in obtaining information was the Internet (general: 31.5%, eco-friendly: 46.3%), such as Social Network Service, Internet cafes, and blogs. The experience in purchasing Eco-OHPs was affected by whether the respondents recognized the possibility of contributing to environmental preservation, availability of vendors, product safety, and the number of eco-friendly products purchased.
Conclusion
In order to expand the use of Eco-OHPs, various efforts such as promotion of eco-friendly characteristics, determination of related vendors, reliable analysis of product safety, and expansion of product experience opportunities are required.

BACKGROUND AND OBJECTIVES: There are no data comparing clinical outcomes of complex percutaneous coronary intervention (PCI) between biodegradable polymer-biolimus-eluting stents (BP-BES) and durable polymer-everolimus-eluting stents (DP-EES). We sought to evaluate the safety and efficacy of BP-BES compared with DP-EES in patients undergoing complex PCI. METHODS: Patients enrolled in the SMART-DESK registry were stratified into 2 categories based on the complexity of PCI. Complex PCI was defined as having at least one of the following features: unprotected left main lesion, ≥2 lesions treated, total stent length >40 mm, minimal stent diameter ≤2.5 mm, or bifurcation as target lesion. The primary outcome was target lesion failure (TLF), defined as a composite of cardiac death, target vessel-related myocardial infarction (TV-MI), or target lesion revascularization (TLR) at 2 years of follow-up. RESULTS: Of 1,999 patients, 1,145 (57.3%) underwent complex PCI: 521 patients were treated with BP-BES and 624 with DP-EES. In propensity-score matching analysis (481 pairs), the risks of TLF (3.8% vs. 5.2%, adjusted hazard ratio [HR], 0.578; 95% confidence interval [CI], 0.246–1.359; p=0.209), cardiac death (2.5% vs. 2.5%, adjusted HR, 0.787; 95% CI, 0.244–2.539; p=0.689), TV-MI (0.5% vs. 0.4%, adjusted HR, 1.128; 95% CI, 0.157–8.093; p=0.905), and TLR (1.1% vs. 2.9%, adjusted HR, 0.390; 95% CI, 0.139–1.095; p=0.074) did not differ between 2 stent groups after complex PCI. CONCLUSIONS: Clinical outcomes of BP-BES were comparable to those of DP-EES at 2 years after complex PCI. Our data suggest that use of BP-BES is acceptable, even for complex PCI.

The genus Populus (poplar) belonging to the Salicaceae family has been used in traditional medicine, and its several species show various pharmacological properties including antioxidant and anti-inflammatory effects. No study regarding protective effects of Populus species against cerebral ischemia has been reported. Therefore, in the present study, we examined neuroprotective effects of ethanol extract from Populus tomentiglandulosa (Korea poplar) in the hippocampal cornu ammonis (CA1) area of gerbils subjected to 5 minutes of transient global cerebral ischemia. Pretreatment with 200 mg/kg of P. tomentiglandulosa extract effectively protected CA1 pyramidal neurons from transient global cerebral ischemia. In addition, glial fibrillary acidic protein immunoreactive astrocytes and ionized calcium binding adapter molecule 1 immunoreactive microglia were significantly diminished in the ischemic CA1 area by pretreatment with 200 mg/kg of P. tomentiglandulosa extract. Briefly, our results indicate that pretreatment with P. tomentiglandulosa extract protects neurons from transient cerebral ischemic injury and diminish cerebral ischemia-induced reactive gliosis in ischemic CA1 area. Based on these results, we suggest that P. tomentiglandulosa can be used as a potential candidate for prevention of ischemic injury.

10.Age-dependent differences in myelin basic protein expression in the hippocampus of young, adult and aged gerbils.

Myelin degeneration is one of the characteristics of aging and degenerative diseases. This study investigated age-related alterations in expression of myelin basic protein (MBP) in the hippocampal subregions (dentate gyrus, CA2/3 and CA1 areas) of gerbils of various ages; young (1 month), adult (6 months) and aged (24 months), using western blot and immunohistochemistry. Western blot results showed tendencies of age-related reductions of MBP levels. MBP immunoreactivity was significantly decreased with age in synaptic sites of trisynaptic loops, perforant paths, mossy fibers, and Schaffer collaterals. In particular, MBP immunoreactive fibers in the dentate molecular cell layer (perforant path) was significantly reduced in adult and aged subjects. In addition, MBP immunoreactive mossy fibers in the dentate polymorphic layer and in the CA3 striatum radiatum was significantly decreased in the aged group. Furthermore, we observed similar age-related alterations in the CA1 stratum radiatum (Schaffer collaterals). However, the density of MBP immunoreactive fibers in the dentate granular cell layer and CA stratum pyramidale was decreased with aging. These findings indicate that expression of MBP is age-dependent and tissue specific according to hippocampal layers.
