With the development of devices and techniques, including complex shape coils, balloons, multiple catheter techniques, and intracranial stents, endosaccular treatment of ruptured or unruptured wide-necked cerebral aneurysms has advanced rapidly with respect to indications and outcomes. In contrast to other options, stent-assisted aneurysm embolization has a distinct feature in which the stent is permanently implanted in the cerebral vessels. Therefore, we must consider the short-term effect of stent-assisted aneurysm embolization in the prevention of coil migration and adverse thrombogenicity, but also the permanent mechanical, hemodynamic, and biological influence on the parent vessels and the aneurysm. We have reviewed the current experimental and clinical data on stent-assisted coil embolization of cerebral aneurysms.

BACKGROUND: Many studies have explained regional disparities in health by socioeconomic status and healthcare resources, focusing on differences between urban and rural area. However some cities in Korea have the highest cardiovascular mortality, even though they have sufficient healthcare resources. So this study aims to confirm three hypotheses: (1) there are also regional health disparities between cities not only between urban and rural area, (2) it has different regional risk factors affecting cardiovascular mortality whether it is urban or rural area, (3) Besides socioeconomic and healthcare resources factors, there are remnant factors that affect regional cardiovascular mortality such as health behavior and physical environment. METHODS: The subject of this study is 227 local authorities (si, gun, gu). They were categorized into city (gu and si consisting of urban area) and non-city (gun consisting of rural area), and the city group was subdivided into 3 parts to reflect relative different city status: city 1 (Seoul, Gyeonggi cities), city 2 (Gwangyeoksi cities), and city 3 (other cities). We compared their mortalities among four groups by using analysis of variance analysis. And we explored what had contributed to it in whole authorities, city and non-city group by using multiple regression analysis. RESULTS: Cardiovascular mortality is highest in city 2 group, lowest in city 1 group and middle in non-city group. Socioeconomic status and current smoking significantly increase mortality regardless of group. Other than those things, in city, there are some factors associated with cardiovascular mortality: walking practice(-), weight control attempt(-), deficiency of sports facilities(+), and high rate of factory lot(+). In non-city, there are other factors different from those of city: obesity prevalence(+), self-perceiving obesity(-), number of public health institutions(-), and road ratio(-). CONCLUSION: To reduce cardiovascular mortality and it's regional disparities, we need to consider differentiated approach, respecting regional character and different risk factors. Also, it is crucial to strengthen local government's capacity for practicing community health policy.

BACKGROUND: Nitric oxide synthase (NOS) has been suggested to have a role in renal injury of aging rats. METHODS: Renal function and histology were compared between 12 month-and 7-9 week-old rats. Proliferating activity and cell death were evaluated by PCNA index and apoptosis. Three isoforms of NOS (eNOS, iNOS, and nNOS) were stained by immunohistochemistry. RESULTS: Serum creatinine level was increased in old rats (1.0 mg/dL vs 0.5 mg/dL, p=0.000). 24 h proteinuria and urinary NO were comparable between the two groups. The percentage of global and segmental glomerulosclerosis increased in old rats. PCNA index decreased in the glomeruli (0.1 vs 0.6/glomerulus, p=0.005) and the tubulointerstitium (10.2 vs 19.2/mm2, p=0.019) of old rats compared to that of young rats. However, no difference was observed in the number of TUNEL positive cells. eNOS was not stained in young and old rat kidney, whereas iNOS was stained in the interstitial inflammatory cells of old rats (0.3 vs 0.0 of young rats/mm2, p=0.188). Macula densa nNOS staining significantly decreased in old rats compared to young rats (5.6 vs 9.5/mm2, p=0.009). CONCLUSIONS: Proliferating activity is more affected than cell death with aging. Decreased nNOS expression without alteration of eNOS and iNOS expressions may implicate nNOS as a marker of renal injury in the early stage of aging.

PURPOSE: We estimated the incidence of retinal lesions at preoperative evaluation of retina for LASIK and investigate its clinical importance. METHODS: We estimated retrospectively the incidence of retinal lesions of the patients who visited our hospital with the purpose of correction of refractive error. The examination of retina upto periphery was done preoperatively. RESULTS: The mean spherical value and axial length were -6.5 +/- 3.4 diopters (range -26.0 ~ +17.0) and 25.4 +/- 2.0mm (range 15.7 ~ 34.5), respectively. The retinal lesions were found in 189 eyes of 139 persons (12.4%). Among them degenerative changes were found in 48 eyes of 34 persons, chorioretinal scars in 7 eyes of 7 persons, pigmented lesions in 28 eyes of 24 persons, and miscellaneous lesions in 106 eyes of 74 persons. There was a significant increase of spherical value and axial length in the group with retinal lesions -7.9 +/- 4.5 diopters (range -26.00 ~ -2.25), 26.5 +/- 2.3mm (range 23.1 ~ 34.5) compared to that without retinal lesions -6.1 +/- 3.4 diopters (range -25.00 ~ 17.00), 25.4 +/- 2.1mm (range 15.7 ~ 35.4). And the incidence of degenerative change and retinal tear was significantly increased in moderate myopes more than -4 diopters. CONCLUSIONS: LASIK is relatively a kind of safe refractive surgery, but to prevent the unexpected postoperative retinal complications, it is essential to thoroughly examine the retina upto periphery before surgery.

BACKGROUND: Nutrition label reading is helpful in attaining a healthy diet. Nutrition label use and its related factors have been studied in many Korean articles, but their research samples were the general population and not chronic disease patients. This study aimed to explore the use of nutrition labels and its relation to dietary intake among chronic disease patients in Korea. METHODS: A total of 3160 respondents aged> or =20 years from the 2008-2009 Fourth Korean National Health and Nutrition Examination Survey (KNHANES-IV) participated in the study. Their chronic medical conditions included hypertension, hyperlipidemia, coronary artery disease, diabetes and/or stroke and they were interviewed regarding their use of nutrition labels. Nutrition intake was also estimated by 24-hour dietary recalls. RESULTS: Overall, 10.4% of the chronic disease patients reported to using nutrition labels. The nutrients they looked at first were total calories, fat and cholesterol. Factors related to reading nutrition labels were relatively higher education, high body mass index and lower frequency of alcohol intake in males and relatively younger age and higher education in females. Nutrition label users consumed lower total calories, protein and fat in the male CAD group, lower total calories and carbohydrate in both diabetes and total groups and lower total calories and fiber in the female stroke group. CONCLUSIONS: Our study found that, in Korea, a significantly lower rate of patients with chronic disease read nutrition labels. Strategies need to be developed to improve clinical application of nutrition labels in this population.

PURPOSE: This study was done to investigate the factors that influence non-combat soldiers' suicidal ideation. METHODS: The participants of survey were 302 of soldiers in Y and H city. The data were collected from Aug. 5 to 8, 2008 in H city and Oct. 25 to 30, 2009 in Y city. The instruments were the SRI (Stress Response Inventory) developed by Ko, Park, & Kim (2000), SDS (Self-Rating Depression Scale), developed by Zung (1965) and SSI (Scale for Suicidal Ideation) developed by Beck (1979). The data were analyzed using SPSS 12.0. RESULTS: The mean score for the SRI, SDS and SSI were 15.71+/-19.81, 32.02+/-9.67 and 1.34+/-2.74 respectively. There were positive correlations between the SRI and SDS (r=.763), SRI and SSI (r=.583), SDS and SSI (r=.575). Stress, depression, age, and educational level accounted for 41% of the variance in suicidal ideation. CONCLUSION: The results of this study indicates that soldiers' suicidal ideation is related to stress, depression, age, and educational level and that there is a need to develop programs which focus on stress and depression management.

BACKGROUND: Helicobacter pylori (H. pylori) infection may cause systemic inflammation and increase the production of tumor necrosis factor-alpha, interleukin-1, and interleukin-6. Unfortunately, bone mineral density also may be affected by these cytokines. This study aimed to evaluate the association between bone mineral density and H. pylori infection. METHODS: A cross-sectional study evaluated 1,126 men undergoing a comprehensive health screening in a private Korean screening center. Subjects' sera were tested for H. pylori antibodies (immunoglobulin G) using an enzyme-linked immunosorbent assay, and bone mineral densities (g/cm2) of the lumbar spine, femoral neck, and total femur were obtained using dual-energy X-ray absorptiometry. To evaluate the difference in bone mineral density according to H. pylori infection status, the adjusted mean bone mineral densities at each site were compared after adjusting for potential confounders, including age, sex, body mass index, smoking, alcohol consumption, and exercise. RESULTS: H. pylori infection was associated with a significant decrease in mean lumbar bone mineral density (H. pylori-positive, 1.190 g/cm2; H. pylori-negative, 1.219 g/cm2; P=0.006), which was greatest among men who were > or =50 years old (H. pylori-positive, 1.193 g/cm2; H. pylori-negative, 1.233 g/cm2; P=0.006). However, no significant association was observed in the bone mineral densities of the total femur and femoral neck. CONCLUSION: In men, H. pylori infection was negatively associated with lumbar bone mineral density. This association may be useful in the early detection, prevention, and management of male osteoporosis.

We report a case with an isolated single coronary artery originating from the ascending aorta, which to our knowledge has never been described before. A 51-year-old man presented for evaluation of atypical chest pain, suffering from a short resting chest pain for 3 days. However, serial electrocardiogram showed no change during chest pain. Cardiac enzymes such as CK-MB, Troponin-I, and myoglobin were within normal limit. Aortography and coronary angiography revealed a single coronary artery originating from the postero-lateral wall of the ascending aorta above the left coronary siuns. However, this anomaly was not of clinical significance based on the atypical nature of the chest pain, negative exercise echocardiography and an absence of obstruction of the coronary artery.
