1.Risk Factors of Colorectal Polyps in Jeju Island.

BACKGROUND: The incidence of colorectal polyps have been reported to be increasing in those aged 30-40 years who consume fast food, have low physical activity, and use alcohol and cigarettes. We analyzed the characteristics and risk factors of colorectal polyps in asymptomatic adults in Jeju Island. METHODS: Data on 1700 individuals who underwent a screening colonoscopy from 1 July, 2010 to 30 June, 2012 were analyzed based on age, gender, body mass index (BMI), blood pressure, blood tests, and behavioral habits including alcohol use, smoking, and exercise. RESULTS: The prevalence of colorectal polyps, including adenomatous and hyperplastic, is 43.1% and is higher in males (P<0.001). The prevalence according to age group shows increasing by age (P for trend<0.001). The most common histologic type of polyp is adenomatous in the <50 years and >50 years age groups, 27.3% and 45.3% respectively. And the percentage of adenomatous polyps has shown an increasing trend along with increasing number and size of polyps (P for trend<0.001). BMI, waist circumference (WC), fasting blood sugar, and triglyceride were higher in males with polyps than males in the control group. BMI and WC were higher and HDL-cholesterol was lower in females with polyps. The incidence of colorectal polyps was higher in male and female smokers (P<0.001 and P=0.035 respectively). CONCLUSIONS: Our results showed that older age, male gender, obesity, and smoking are risk factors for colorectal polyps. The prevalence of colorectal adenomatous polyps is 27.3% in <50 yrs, 33.4% in the 40s. For early detection and treatment, we recommend a screening colonoscopy for those with risk factors.

2.Criterion Development of 4 Activity Levels for Estimated Energy Requirement Calculation for Primary School Students.

The purpose of this study is finding the easy way of 4 categories activity level confirmation for estimated energy requirement calculation. Total of 386, 5th and 6th grade primary school students participated. The time spent on 7 kinds of activity were collected for 1 day by the internet program developed. Judged by the activity coefficient, sedentary were 6.7% and 5.1%, low active 33.2% and 40.4%, active 43.8%, and45.5%, and very active 16.3% and 9.0% for boy and girl, respectively. The highest and significant correlation coefficient between activity coefficient and time spent on activities shown were 0.339 in commute activity for boys, and 0.466 in leisure for girls. The sensitivity of the sedentary conformation by commute hour for boys was 0.79, and that of very active was 0.56. The sensitivity of the sedentary conformation by leisure hour for girls was 0.67, and that of very active was 0.63. The sensitivity of low active and active by 7 different types of activity was quite low, 0.04~0.37. The exact agreement of activity level conformed by easy way developed was 30.8% and 33.7%, for boys and girls, respectively. More accurate way to identify 4 categories activity level needs to be developed, especially sensitive to conformation of low active and active levels.

3.The Analysis of Activity Energy, Total Energy, and Estimated Energy Expenditures in 5th and 6th Grade Primary School Students.

This study was conducted to investigate the time spent, activity coefficient and energy expenditure on 7 different types of activity. Total of 386 5th and 6th grade primary school boys and girls in Seoul participated in this study. The participants spent 8 hrs for rest, 4 hrs for out of school study, 3 hrs for leisure, 1 hr 45 mins. for hygiene, 1 hr for commute, and 15 mins. for house choir activity in the day of 6 class hours. The average activity coefficient of 1 day for boys and girls were 1.69 and 1.64, respectively. The TEEs were 2,382 kcal and 2,050 kcal for boys and girls, respectively. The energy cost for study related activity (in school and study activities) was 40% of the TEE, and for rest 20%, for commute 9%, and for hygiene 10% of the TEE. The energy cost for house choir was only about 2% of TEE. The 91.2% participant's EER was higher than the EER shown in the table of 2010 DRI for Koreans. When the participant's EER was compared with the energy allowance calculated by the method in 2000 RDA for Korean, 81.6% was in the range of +/- 5%, and the correlation coefficients between the 2 values were 0.981 for boys and 0.978 for girls, which means high agreements.

4.Reliability and Validity of the Korean Version of the Perinatal Post-Traumatic Stress Disorder Questionnaire.

PURPOSE: The Perinatal Post-Traumatic Stress Disorder Questionnaire (PPQ) was designed to measure post-traumatic symptoms related to childbirth and symptoms during postnatal period. The purpose of this study was to develop a translated Korean version of the PPQ and to evaluate reliability and validity of the Korean PPQ. METHODS: Participants were 196 mothers at one to 18 months after giving childbirth and data were collected through e-mails. The PPQ was translated into Korean using translation guideline from World Health Organization. For this study Cronbach's alpha and split-half reliability were used to evaluate the reliability of the PPQ. Exploratory Factor Analysis (EFA), Confirmatory Factor Analysis (CFA), and known-group validity were conducted to examine construct validity. Correlations of the PPQ with Impact of Event Scale (IES), Beck Depression Inventory II (BDI-II), and Beck Anxiety Inventory (BAI) were used to test a criterion validity of the PPQ. RESULTS: Cronbach's alpha and Spearman-Brown split-half correlation coefficient were 0.91 and 0.77, respectively. EFA identified a 3-factor solution including arousal, avoidance, and intrusion factors and CFA revealed the strongest support for the 3-factor model. The correlations of the PPQ with IES, BDI-II, and BAI were .99, .60, and .72, respectively, pointing to criterion validity of a high level. CONCLUSION: The Korean version PPQ is a useful tool for screening and assessing mothers' experiencing emotional distress related to child birth and during the postnatal period. The PPQ also reflects Post Traumatic Stress Disorder's diagnostic standards well.

5.Comparison of the Attitudes of Nurses and Physicians toward Palliative Care in Neonatal Intensive Care Units

Purpose:
This study aimed to compare the attitudes of nurses and physicians toward neonatal palliative care and identify the barriers to and facilitators of neonatal palliative care, with the goal of improving palliative care for infants in neonatal intensive care units (NICUs).
Methods:
This cross-sectional study analyzed data from the NICUs of seven general hospitals with 112 nurses and 52 physicians participating. Data were collected using the Neonatal Palliative Care Attitude Scale questionnaire.
Results:
Only 12.5% of nurses and 11.5% of physicians reported that they had sufficient education in neonatal palliative care. In contrast, 89.3% of the nurses and 84.6% of the physicians reported that they needed further education. The common facilitators for both nurses and physicians were: 1) agreement by all members of the department regarding the provision of palliative care and 2) informing parents about palliative care options. The common barriers for both nurses and physicians were: 1) policies or guidelines supporting palliative care were not available, 2) counseling was not available, 3) technological imperatives, and 4) parental demands for continuing life support. Insufficient resources, staff, and time were also identified as barriers for nurses, whereas these were not identified as barriers for physicians.
Conclusion
It is necessary to develop hospital or national guidelines and educational programs on neonatal palliative care, and it is equally necessary to spread social awareness of the importance of neonatal palliative care.

7.The Estimation of the Daily Energy Expenditure of Korean Adolescents.

Middle and high school students in Korea spend a lot of time at school. In other words, many Korean teenagers spend most of their time studying, and have little time for other activities, such as physical workouts. Fewer physical activities, and consumption of many snacks, etc. may lead to overweight or obesity in some teenagers. This study was conducted to find out the activity levels of teenagers' in order to calculate their daily energy expenditures. A total of 1,192 students (461 middle school students and 731 high school students) who lives in either a city or a country area were recruited for this study. One-day Activity Diaries were collected using questionnaires and analyzed by means of SPSS/win. The participating students spent about 7 hours 24 minutes resting including sleeping; 6 hours 28 minutes studying, 4 hours 4 minutes in leisure activities, 1 hour 32 minutes in personal hygiene activities such as washing, dressing and undressing, etc., 50 minutes on transportation : and 23 minutes on household chores such as cleaning, doing the laundry, etc. The average activity factor was 1.49. That of middle school female students was the lowest at 1.47, followed by middle school male students at 1.48, high school female students at 1.49, and high school male students at 1.51. The daily energy expenditure of middle and high school male students was 2,289 kcal and 2,600 kcal, respectively. That of female students was 1,959 kcal and 2,067 kcal, respectively.

8.Performance and Knowledge of Nurses on the Practice of Pediatric Sedation

Purpose:
This study aimed to examine the pediatric sedation knowledge and performance of nurses in tertiary hospitals.
Methods:
Altogether 120 nurses working in the Pediatric Departments of 4 tertiary hospitals in Busan and Kyungnam participated in this descriptive survey. The questionnaire was developed based on domestic and international guidelines and consisted of 40 questions including 18 questions on knowledge and 22 on the performance level of sedation nursing.
Results:
The mean score regarding the knowledge of pediatric sedation care was 3.14 points and the mean performance level score was 3.44 points. Questions that received low scores in terms of pediatric sedation care knowledge included items regarding the latest policies, level of sedation, and fasting before sedation. Questions that received low scores in the performance level included items regarding psychological preparation before sedation, sedation education considering developmental level and disease, and items related to blood pressure measurement. Pediatric sedation knowledge scores were higher in the groups with higher age, longer clinical experience, higher educational experience, and awareness of sedation guidelines. Performance scores were significantly higher in the groups having educational experience in pediatric sedation and in those exhibiting awareness of sedation guidelines.
Conclusion
Sedation education was significantly associated with both knowledge and performance of sedation nursing, conducting educational programs on pediatric sedation nursing.
9.A Case of Acute Interstitial Nephritis and Myoglobinuria after Alcohol Drinking.

Rhabdomyolysis is defined as skeletal muscle injury with release of muscle cell constituents into the plasma and may lead to acute renal failure secondary to myoglobinuria. The causes of rhabdomyolysis is diverse:alcohol abuse, primary muscle disease, disturbance of muscle metabolism, sustained seizure, infection, drugs, tox ins, trauma, severe exercise, CO intoxication etc. Rhabdomyolysis may cause acute derangement in electrolyte balance and death. It should be diagnosed earlier and managed properly. We experienced a 49 year-old woman developed acute renal failure and myoglobinuria after alcohol drinking. A kidney biopsy revealed acute interstitial nephritis. In the presence of otherwise unexplained acute renal failure in alcoholic patients, rhabdomyolysis should be considered in the differential diagnosis.

10.Clinical Experiences of 52 Cases Cadaveric Donor Kidney Transplantation.

PURPOSE: The organ procurement organization (OPO) and transplant coordinator were established in Chonnam National University Hospital in 1993 to promote organ donation and to manage organ procurement. At the same time, the first protocol and planning transplant was performed simultaneously. We performed this study to know the predicting factors and survival rate of cadaveric kidney transplantation. METHODS: First cadaveric donor kidney transplantation was performed at May 1993 in Chonnam National University Hospital. Thereafter 52 cases of cadaveric kidney transplantation were performed using 28 cadaver donors till December 1999. The most frequent cause of brain death was head injury by traffic accident. Male to female donor ratio was 1.8 : 1. 52 recipients (29 males, 23 females), aged 20 to 65 years (median age 36 years) were the subjects of this study. The immunosuppressive regimens consisted of cyclosporin, mycophenolate mofetil, and prednisone. Acute rejection was treated with three consecutive bolus of 1.0 gram methylprednisolone or 5 mg/day for 10 days of OKT3. Three HLA mismatchs were 7 cases (13.5%) and six mismatches were 6 cases (11.5%). There were 2 cases of multiple renal arteries. RESULTS: There was no primary non functioning graft. In the first 7 post operative days, urine amount less than 4,000 mL per day was noted in 29 patients (55.8%) and serum creatinine over 1.5mg/dL was noted in 13 patients (25%). No significant proteinuria and hematuria was observed. Postoperative medical complications were occured in 12 patients (23.1%) and minor surgical complications in 3 patients (5.8 %). One patient was performed reoperation because of urinary leakage. CMV infections were noted in 15 patients (28.8%). Acute rejection episodes were 17 cases (32.7%). We lost 4 grafts within 1 year. The major cause of graft loss was patient death. Recipient age was significant risk variable for graft and patient survival in multivariate analysis (P=0.012). one and five year graft survival rates were 92.15%. CONCLUSION: To achive better results, continued attention should be paid to the cadaveric donor organ procurement.
