2.The Effects of Gait Training with Partial Weight Bearing Harness on Treadmill in Early Stroke Patients.

OBJECTIVE: To assess the effects of partial weight bearing (PWB) harness with comparison of traditional exercise program by gait analysis in non-ambulatory acute stroke patients. METHOD: Nine patients participated with PWB harness on the treadmill, and nine patients with traditional exercise. In harness group, the gait training with 30% weight reduction was offered on treadmill, they were treated daily and gait training was done for 20 minutes. In no-harness group, traditional gait training was done by physical therapists. Variables were gait parameters assessed by VICON 370 Gait Analyzer, other function tested by Motricity Index and Functional Ambulatory Category. RESULTS: In linear parameter, the harness group had faster gait speed, longer stride length, and more decreased double support time compared to the no-harness group. In kinematic parameters, the maximal flexion of knee was significantly increased in harness group, but no significant differences was in hip and ankle motion. In kinetic parameters, hip flexion moment of normal side was increased in harness group at heel strike phase, knee flexion moment of normal side was decreased in harness group at heel strike phase. CONCLUSION: The gait training with partial weight bearing harness was more effective in acute stroke patients than traditional therapy, and was influenced in gait pattern, functional ability.

5.Clinical Characteristics of Traumatic Epidural Hematoma: A Comparison between Children and Adults.

Seventy-nine children and 433 adults were treated for epidural hematoma at our hospital during the last 10 years. The authors have tried to compare the various clinical characteristics and significant prognostic factors of epidural hematoma in children and adults. The result confirmed that children with traumatic epidural hematoma are less likely to have direct relationship with various causes of head trauma, associated extracranial injuries, skull fractures, temporal region hematomas, and immediate surgeries, but tend to have high preoperative GCS scores, prompt pupil responses, require only conservative treatments, have short durations of coma, and have good outcome. The cause of injury, amount of hematoma and focus of bleeding on surgery are found to be significant prognostic factors in adults, but are not in children. In cases with same scores of preoperative Glasgow Coma Scale(GCS) and durations of coma, very severe neurologic status(below 7 of GCS, both dilated fixed pupil) had a worse outcome in children as well as in adults without significant difference between them. The clinical index on preoperative GCS and pupillary response used when deciding the operation time were different in children compared to adults. Operations performed in delayed fashion in children under close observation for changes in GCS and pupillary response showed good outcome. However, adults require operations as early as possible because of possible rapid progression.

6.Clinical Analysis of Traumatic Intracerebral Hematoma Associated with Other Traumatic Intracranial Lesions.

One hundreds eighty cases of traumatic intracerebral hematoma(TICH) among 1,633 head injury patients diagnosed with computerized tomography in neurosurgical department from Jan. 1990 to Jun. 1996. were classified into two TICH groups; TICH group associated with other traumatic intracranial lesions and TICH group without other traumatic intracranial lesions. The incidence of TICH was 11.0% and percentage of TICH with other traumatic intracranial lesions among 180 cases of TICH was 66.7%. TICH group with other traumatic intracranial lesions showed worse initial level of consciousness, more likely to have abnormal pupillary reflex, higher incidence of delayed traumatic intracerebral hematomas(DTICH), greater amount of hematoma, increased chance of surgery and worse outcome. In TICH group with other traumatic intracranial lesions, the factors affecting prognosis were initial Glasgow coma scale(GCS) score, pupillary reflex, amount of hematoma and treatment modality. In TICH only group, the factors affecting prognosis were initial GCS score, pupillary reflex, amount of hematoma and DTICH. The patients with 12-15 of GCS score, normal pupillary reflex, absence of DTICH, and amount of hematoma below 10cc, the presence of other traumatic intracranial lesions were found to be important prognostic factor. The overall mortality was 27.7% but in TICH group with other traumatic intracranial lesions, the mortality was 35.0% compared to 13.3% in TICH only group. It is concluded from this study that in the case with other traumatic intracranial lesions, TICH showed worse initial neurological status and prognosis compared to those without other lesions. Thus, these patients, although in conditions of good general clinical index at admission, should be considered to provide intensive care and treatment because these associated lesions will play as a bad prognostic factor.

7.A Symptomatic Choroid Plexus Cyst in the Lateral Ventricle: Case Report.

The authors report a case of symptomatic choroid plexus cyst, located in the trigone of the left lateral ventricle in a 18-year-old man who presented with headache and seizure attack. The cyst was diagnosed by magnetic resonance image(MRI), and was confirmed with surgery. The cyst had no communication with the ventricular system orsubarachnoid space. Total removal of cyst adhering to the choroid plexus was accomplished, with subsequent disappearance of the seizure and headache. A brief review of the literature is included.

8.A Clinical Analysis of Surgically Managed Tuberculous Spondylitis.

Fourty three patients with tuberculous spondylitis were surgically treated through the anterior approach at our hospital from January, 1989 to December, 1994. Among them, 32 cases were followed up more than 18 months postoperatively, and were included in this study. The most prevalent location was lumbar region(50%). Paraparesis was frequently seen in patients with middle and lower thoracic spinal lesions and all patients with neurologic deficits improved after decompression of spinal cord. Autogenous rib and/or iliac strut bone grafting was performed, followed by spinal instrumentation. Solid bone fusion was obtained in all patients. There was no need for prolongation of duration of antituberculous drug therapy and no increased incidence of secondary infection due to spinal instrumentation.

9.Correlation between Heart Rate Variability and Sleep Structure in Primary Insomnia.

OBJECTIVES: It is well established that primary insomnia affects the activity of autonomic nervous system. We tried to know how the activity of autonomic nervous system during night sleep changes by analyzing correlation between heart rate variability (HRV) index and the variables related with sleep structure in primary insomnia. METHODS: Thirty three subjects (mean age:36.2+/-14.2 years, male:female=15:18) who were diagnosed with primary insomnia were selected for the study. Nocturnal polysomnography (NPSG) was carried out on each subject and correlation was analyzed between high frequency/low frequency ratio (LF/HF ratio), one of HRV indices and the variables related with sleep structure which were calculated from NPSG. RESULTS: When age and sex were controlled, LF/HF ratio showed negative correlations with slow wave sleep and stage 2 sleep, respectively (r(p)=-0.43, p=0.01;r(p)=-0.37, p=0.04). On the other hands LF/HF ratio showed a positive correlation with arousal index (r(p)=0.65, p<0.001). The activity of autonomic nervous system responded differentially depending on the change of sleep structure in primary insomnia. Especially the increase of arousal index and the decrease of slow wave sleep and stage 2 sleep which are the components of non-REM sleep provoked hyperactivity of sympathetic nervous system. CONCLUSION: This study suggests that the typical change of sleep structure in primary insomnia can negatively impact on cardiovascular system.

10.A comparative study of guiding methods for natural head posture in cephalometrics.

The purpose of this study was to compare the degree of vertical head rotation and to evaluate the reproducibility of natural head posture (NHP) according to two guiding methods, the head posture aligner (HPA) method and the self balance posture (SBP) method. The subjects consisted of 30 adults. On the first day, lateral and frontal cephalometric radiographs were obtained through the two guiding methods. One month later, lateral and frontal cephalometric radiographs were obtained again through both guiding methods. The degrees of vertical head rotation of both guiding methods were compared and the reproducibility was evaluated for each guiding method. A comparison of the degrees of vertical head rotation for the two methods revealed that the vertical head posture was lower in the SBP method than in the HPA method by an average of 2.79 degrees. All measurements obtained using the HPA and SBP methods with a time interval of one month did not show any significant difference in lateral and frontal cephalometric radiographs. The results of the present study suggest that the SBP method may be used as an alternative to the HPA method in case the HPA method can not be applied.
