1.The Change in Regional Cerebral Oxygen Saturation after Stellate Ganglion Block.

BACKGROUND: Stellate ganglion block (SGB) is known to increase blood flow to the innervations area of the stellate ganglion. Near infrared spectroscopy reflects an increased blood volume and allows continuous, non-invasive, and bedside monitoring of regional cerebral oxygen saturation (rSO2). We investigated the influence of SGB on bilateral cerebral oxygenation using a near infrared spectroscopy. METHODS: SGB was performed on 30 patients with 1% lidocaine 10 ml using a paratracheal technique at the C6 level and confirmed by the presence of Horner's syndrome. The blood pressure (BP), heart rate (HR) and rSO2 were measured before SGB and 5, 10, 15 and 20 minutes after SGB. Tympanic temperature of each ear was measured prior to SGB and 20 minutes after SGB. RESULTS: The increments of the rSO2 on the block side from the baseline were statistically significant at 5, 10, 15 and 20 minutes. The rSO2 on the non-block side compared with the baseline, however, decreased at 15 and 20 minutes. The difference between the block and the non-block sides was significant at 15 and 20 minutes. The BP at 10, 15 and 20 minutes was increased and the HR was increased at 10 and 15 minutes. CONCLUSIONS: We observed an increment of the rSO2 on the block side from the baseline; however, the rSO2 on the non-block side decreased.

4.A Case of Intracranial Mycotic Aneurysm due to Aspergillus species.

Intracranial mycotic aneurysms due to Aspergillus species are extremely uncommon but fatal. A medium-sized ruptured intracranial aneurysm at the middle cerebral artery bifurcation was identified in a 50-year-old female patient. Proper microsurgical clipping was not feasible due to the aneurysm's friable nature. Microsuture and wrapping were done instead. Histological findings confirmed a mycotic aneurysm caused by Aspergillus. Herein, we report on the clinical course and histopathological findings with a relevant literature review.

5.Short-term Clinical Outcomes after Flanged Intrascleral Fixation of Intraocular Lenses Using Oblique Intrascleral Tunnels

Purpose:
To report the short-term clinical outcomes after intrascleral fixation of intraocular lenses (IOLs) using oblique intrascleral tunnels.
Methods:
We retrospectively studied 17 patients (18 eyes) who underwent flanged intrascleral IOL fixation from October 2019 to October 2021. The patients were divided into those who underwent fixation using horizontal (group A) and oblique (group B) intrascleral tunnels. We compared the best-corrected visual acuities (BCVAs), cylindrical powers, refractive errors (the differences between the targeted spherical equivalents [SEs] and postoperative SEs) before and 3 months after surgery, and operating times.
Results:
At 3 months vs. preoperatively, there were no significant differences in BCVA (-0.83 ± 0.43 vs. -0.48 ± 0.59), refractive error (-0.06 ± 0.97 diopter [D] vs. -0.05 ± 0.80 D), cylindrical power (-0.42 ± 3.81 D vs. -0.33 ± 1.20 D), or operating time (83.33 ± 28.05 minutes [min] vs. 66.33 ± 20.57 min) between groups A and B, respectively.
Conclusions
In terms of the short-term clinical outcomes after use of horizontal and oblique intrascleral tunnels, we found no significant differences in any parameters studied. However, use of an oblique intrascleral tunnel may shorten the operating time.
6.A Case of Rheumatoid Arthritis Associated with Renal Amyloidosis and Crescentic Glomerulonephritis.

A case of renal amyloidosis with crescentic glomerulonephritis associated with rheumatoid arthritis is described. A 60-year-old female with 15 years' history of rheumatoid arthritis developed nephrotic syndrome followed by rapid deterioration of renal function. Glomerular amyloid deposition and sclerotic change was present in kidney biopsy specimen and crescentic change was found in 85% of the glomerulus. Electron microscopic finding of glomerulus showed randomly oriented, rigid-appearing, long nonbranching fibrils. The patient was treated with intravenous high-dose methylprednisolone pulses combined with intravenous cyclophosphamide followed by oral corticosteroids. Three months after the above treatment, renal function gradually improved, reaching serum creatinine level to 2.5 mg/dL. But the nephrotic range proteinuria persisted.

Oral or rectal sodium phosphate(NaP) preparation is commonly used for the colonic cleansing. Since the sole route of excretion of absorbed phosphate is renal, diminished renal function will limit the ability to excrete a sudden phosphate load. To date, about 20 adult cases of hyerphosphatemia associated with oral or rectal sodium phosphate solution have been reported, but not a single case in Korea to our knowledge. We report two cases of hyperphosphatemia and hypocalcemia after the administration of NaP(Colclean(R)) to reemphasizes the potential hazard of sodium phosphate bowel preparation in patients with compromised renal function.

8.NCAM140 and pCREB Expression after Tianeptine Treatment of SH-SY5Y Cells.

OBJECTIVE: Antidepressants Modulate Neuronal Plasticity. Tianeptine, An Atypical Antidepressant, Might Be Involved In The Restoration Of Neuronal Plasticity; It Primarily Enhances The Synaptic Reuptake Of Serotonin. Ncam140 Is Involved In Neuronal Development Processes, Synaptogenesis And Synaptic Plasticity. We Investigated The Effect Of Tianeptine On The Expression Of Ncam140 And Its Downstream Signaling Molecule In The Human Neuroblastoma Cell Line Sh-sy5y. METHODS: NCAM protein expression was measured in human neuroblastoma SH-SY5Y cells that were cultivated in serum-free media and treated with 0, 10, or 20 microM tianeptine for 6, 24, or 72 hours. NCAM140 expression in the tianeptine treatment group was confirmed by Western blot, and quantified through measurement of band intensity by absorbance. CREB and pCREB expression was identified after treatment with 20 microM tianeptine for 6, 24, and 72 hours by Western blot. RESULTS: Compared to cells treated for 6 hours, cells treated with 0 or 10 microM tianeptine for 72 hours showed a significant increase in NCAM140 expression and cells treated with 20 microM tianeptine showed a significant increase after 24 and 72 hours. The pCREB level in cells treated with 20 microM tianeptine increased in time-dependent manner. CONCLUSION: Our findings indicated that the tianeptine antidepressant effect may occur by induction of NCAM140 expression and CREB phosphorylation.

9.Transient Total Occlusive Attack of the Ophthalmic Artery with Asymptomatic Patent Foramen Ovale

Purpose:
To report a case of transient total occlusive attack of the ophthalmic artery with asymptomatic patent foramen ovale.Case summary: A 31‐year‐old female presented with worsening of intermittent visual loss in her right eye from the previous day. The visual acuity in the right eye was reduced to 0.04 at her first visual examination. In the doctor’s room, her visual acuity in the right eye improved to 1.0 at first; however, after several minutes, it decreased to no light perception. A relative afferent pupillary defect was observed. Following that, fluorescein angiography was performed, and visual acuity improved and worsened repeatedly; corresponding intermittent perfusion and occlusion of the ophthalmic artery were observed during the examination. Further evaluation was performed to determine the cause of intermittent occlusion of the ophthalmic artery; patent foramen ovale was diagnosed. Transient ophthalmic artery occlusion was presumed to have occurred owing to embolism by the patent foramen ovale. Aspirin was used as a prophylaxis. Since then, there have been no recurring symptoms of visual impairment.
Conclusions
Patients with ophthalmic diseases such as ophthalmic artery occlusion, retinal artery occlusion, and amaurosis fugax, especially young people, require a thorough assessment to identify potential causes of embolism.
10.Ocular Manifestations Associated with Cerebral Vein Sinus Thrombosis Developing after SARS-CoV-2 Vaccination

Purpose:
We report a case of neuroretinitis combined with external ophthalmoplegia in a patient who developed a cerebral venous sinus thrombosis after administration of a SARS-CoV-2 vaccine.Case summary: A 26-year-old woman who was on oral contraceptives was diagnosed with a cerebral venous sinus thrombosis 1 month after the first injection of a SARS-CoV-2 vaccine (BNT162b2, Pfizer-BioNTech). The levels of factors 8 and 9 were elevated on the blood test. The corrected visual acuities were 0.3 in both eyes. A complete limitation of abduction and esotropia were evident in the left eye. Both eyes exhibited optic disc swelling and hemorrhage and retinal nerve fiber layer swelling. Subretinal fluid was apparent in the right eye. Three weeks later, the optic disc swellings and hemorrhages had worsened and both eyes evidenced macular stars. After 10 months, the corrected visual acuities improved to 0.9 in both eyes. Ocular motor function and the esotropia also improved. However, the overall contraction of the visual field did not.
Conclusions
SARS-CoV-2 vaccination can trigger cerebral venous sinus thrombosis, neuroretinitis, and external ophthalmoplegia. In patients with risk factors for such thrombosis, the possibility of ophthalmic complications should be considered after administration of a SARS-CoV-2 vaccine.
