3.Dental Co-Cr alloys fabricated by selective laser melting:A review article

Cobalt-chromium alloys are used to fabricate various dental prostheses, and have advantages of low cost and excellent mechanical properties compared to other alloys. Recently, selective laser melting, which is an additive manufacturing method, has been used to overcome the disadvantages of the conventional fabrication method. A local rapid heating and cooling process of selective laser melting induces fine microstructures, grain refinement, and reduction of porosities of the alloys. Therefore, it can improve mechanical properties compared to the alloys fabricated by the conventional method. On the other hand, layering process and rapid heating and cooling cause accumulation of a large amount of residual stresses that can adversely affect the mechanical properties. A heat treatment for removing residual stresses through recovery and recrystallization process caused complicated changes in mechanical properties induced by phase transformation, precipitate and homogenization of the microstructures. The purpose of this review was to compare the manufacturing methods of Co-Cr alloys and to investigate the characteristics of Co-Cr alloys fabricated by selective laser melting.

5.A case of inflammatory myofibroblastic tumor originated from the greater omentum in young adult.

Inflammatory myofibroblastic (IMF) tumor is a rare solid tumor that often affects children. IMF tumors occur primarily in the lung, but the tumor may affect any organ system with protean manifestations. A 22-year-old woman was evaluated for palpable low abdominal mass that had been increasing in size since two months prior. Abdominal computed tomography showed a lobulated, heterogeneous contrast enhancing soft tissue mass, 6.5 x 5.7 cm in size in the ileal mesentery. At surgery, the mass originated from the greater omentum laying in the pelvic cavity and was completely excised without tumor spillage. Histologically, the mass was a spindle cell lesion with severe atypism and some mitosis. Immunohistochemistry for anaplastic lymphoma kinase-1 revealed that the lesion was an IMF tumor. Because of its local invasiveness and its tendency to recur, this tumor can be confused with a soft tissue sarcoma. Increasing physician awareness of this entity should facilitate recognition of its clinical characteristics and laboratory findings.

6.Metastases to Ureteral Stump and Bladder from Renal Cell Carcinoma.

Although metastatic renal cell carcinoma is not uncommon, metastases to the ureteral stump and bladder are very rare. A 69-year-old woman who had undergone radical nephrectomy because of right renal cell carcinoma about 8 years ago, visited our hospital for evaluation of painless gross hematuria. Histologic examination of the removed ureteral stump and bladder specimen demonstrated metastatic renal cell carcinoma. We report a case of renal cell carcinoma with metastases to the ureteral stump 5 years after radical nephrectomy and bladder 8 years after radical nephrectomy.

7.Estimation of Creatinine Clearance with Serum Creatinine in Korean Patients.

Cockcroft and Gault's formula is frequently used to estimate creatinine (Ccr) in clinical practice. To determine the accuracy of such estimation in Korean patients, we measured simultaneously, serum creatinine and 24-hour urinary creatinine excretion in 696 Korean patients (male:350, female:346). Measured Ccr was significantly different from estimated Ccr in several age groups and the decrease of creatinine excretion with age is less than Cockcroft and Gault's estimation. We assumed that this difference can be due to difference of the body habitus and difference of urinary creatinine excretion per body weight between different races. So we divided the sample population into two groups and derived the new formula in one group with regression analysis between age and 24 hour urinary creatinine excretion per body weight for estimation of Ccr as Cockcroft and Gault derived their formula and applied it to another group to compare the new formula with Cockcroft and Gault's formula in Korean patients. The new formula was Ccr (mL/min)=[ (260-age)x weight (kg)]/[160 x serumCr (mg/dL)] for male and Ccr (mL/min)-[ (236-age) x weight (kg)]/[180 x serum Cr (mg/dL)] for female. Predictive accuracy of the new formula was significantly better than the Cockcroft and Gault's formula in the other sample population and also in subgroup of the patients with azotemia.

8.A Case of Epididymal Metastasis from Mucinous Adenocarcinoma of the prostate.

Mucinous adenocarcinoma of the prostate is very rare tumor and comprises about 0.4% of all prostatic carcinomas. The metastatic form of mucinous adenocarcinoma of the prostate is similar to that of the adenocarcinoma of the prostate. Common sites of metastasis are pelvic lymph node and bone. At presentation, half of patients have evidence of metastasis to pelvic lymph nodes, bone, brain, liver, bladder and seminal vesicles. Especially, epididymal metastasis from mucinous adenocarcinoma of the prostate is uncommon and only 15 cases was reported in the literature. We report a case of mucinous adenocarcinoma of the prostate with metastasis to the epididymis and discuss the clinicopathologic features of this lesion.

9.Rehabilitation of a patient with non-syndromic partial oligodontia.

Oligodontia is defined as a congenital tooth agenesis with the absence of six or more permanent teeth. This clinical report describes a patient with non-syndromic partial oligodontia, with retained deciduous teeth and the absence of 16 permanent teeth. Anterior esthetic problems were caused by interarch tooth size discrepancy, interdental space, aberrant tooth dimensions, and the absence of centric contacts of the anterior teeth. Prosthetic restoration after orthodontic and implant treatment was performed with a multi-disciplinary team approach. Favorable functional and esthetic results were obtained using a definitive prosthesis.

10.The Etiological Role of Legionella Pneumophila in Patients with Community-Acquired Pneumonia in Korea.

BACKGROUND: Legionella pneumophila has been recognized as an important cause of community-acquired pneumonia(CAP) requiring hospitalization. However, epidemiological data on the occurrence of legionella-related pneumonia is unavailable in Korea. The purpose of this study was to evaluated the etiological imprtance of legionella pneumophila serogroup 1 in patients hospitalized with CAP. METHOD: The CAP patient over 16 year-old were recruited from July 1999 to June 2000 at the Chunchon Sacred Heart Hospital. Fifty four patients (male 29, female 25, average age 63.8±15.3) were included in this study. A diagnosis of a legionella pneumophila infection was based on a urinary antigen test using the Binax Company enzyme immunoassay. The severity of pneumonia was assessed using the Fine's PORT scoring system. RESULT: The average Fine's PORT score was 99.7(±44.9). According to the risk classification proposed by the Infectious Disease Society of America, the number of patients in each class(from class I to class V) were 6(11.1%), 13(24.1%), 9(16.7%), 14(25.8%), and 12(22.2%), respectively. Thirty two patients(59.3%) were initially admitted to the intensive care unit. The mortality rate was 16.7%(9 in 54). In all patients, urinary antigens to Legionella pneumophila serogroup 1 were not detected. CONCLUSION: Legionella pneumophila may play little role in causing adult CAP in Korea. Therefore, the routine use of macrolide in the empirical treatment of the CAP patients based upon the ATS guidelines(1993) in Korea should be reevaluated.
