2.Evaluation of Macular Circulation in Patient with Diabetic Maculopathy using Scanning Laser Ophthalmoscope(SLO).

In the condition of diabetic retinopathy, the vascular changes are localized primarily in the retinal capillaries and are presumed to promote angiogenesis. To investigate the change of retinal blood flow velocities and morphological parameters in diabetic retinopathy, we measured perifoveal capillary blood velocities(v) and the size of foveal avascular zones(FAZ). Thirteen patients with diabetic maculopathy and nine healthy volunteers were included in this study. The scanning laser technique in conjunction with an image analysing system were used to assess the morphological and hemodynamic changes in diabetic retinopathy. Diabetic maculopathy group showed a slower capillary blood velocity than normal group(2.44+/-0.39mm/sec vs2.75+/-0.61 mm/sec, p>0.18). The foveal avascular zone was significantly larger in diabetic maculopathy group than in normal group(313.5+/-64.6micrometervs. 238.9+/-93.8micrometer, p<0.05). This results indicate that the retinal microcirculation is altered in diabetic patients compared with healthy subjects. These alterations may be due to the change of the capillary wall and blood viscosity in diabetic patients. The determination of these parameters can be utilized in monitoring the progress of diabetic maculopathy.

3.Seasonal Variation of Pre-term Births in Korea.

OBJECTIVES: To investigate possible seasonal patterns of pre-term birth in Korea. METHODS: A total number of 2,669,357 single live births reported to the National Statistical Office from 1995 to 1998 were analyzed. Composite monthly cohorts of ongoing pregnancies were constructed for each month of the year and the probability of pre-term birth was estimated. RESULTS: Increases in the probability of a pre-term birth occurred during winter for the birth of first child and during summer for the birth of second or later child. This seasonal variation was similar among groups divided by sex, residency, age of mother, and education of mother. CONCLUSIONS: These findings suggests that some environmental factors related to season may partially explain the incidence of premature births.

Behavioral and psychological symptoms of dementia are major components of Alzheimer’s disease.
In this study, we aimed at investigating the prevalence and severity of behavioral and psychological
symptoms of dementia in very mild to mild Alzheimer’s disease. Forty-four patients with Alzheimer’s
disease who visited the neurology outpatient clinic of Seoul Medical Center were included. A trained
neurologist or a supervised test technician administered the neuropsychological test, the Seoul
Neuropsychological Screening Battery including Mini Mental State Examination and Clinical Dementia
Rating. The instrument used for assessing behavioral and psychological symptoms of dementia was
Neuropsychiatric Inventory-Questionnaire. In order of prevalence, apathy, depression, irritability,
anxiety, and agitation were the most common symptoms occurring in very mild-to-mild Alzheimer’s
disease group. The prevalence and severity of behavioral and psychological symptoms of dementia
were found to be in positive correlation with the Clinical Dementia Rating scores. Behavioral and
psychological symptoms of dementia are found even in the very early stage of Alzheimer’s disease,
apathy and depression being the most common symptoms. Physicians should be aware of this when
managing dementia patients.

Orbital lymphangiomas are benign vascular hamartomas that may be found in the conjunctiva, the eyelids, the orbit, or elsewhere in the head and neck region. Deep lymphangiomatous lesions are classically characterized by acute onset of a fulminant proptosis resulting from spontaneous hemorrhage within the orbit. Complete surgical excision is often difficult because of the infiltrative nature of the tumor.We report one case of lymphangiomas considered to be associated with cavernous hemangioma which could not be removed completely by several surgical excisions and was exenterated because of orbital abscess of unknown origin and septic condition.

7.Three Cases of Orbitofrontal Cholesterol Granuloma.

PURPOSE: To report three cases of orbitofrontal cholesterol granuloma. METHODS: The study subjects were three patients with chief complaints of proptosis and upgaze limitation of ocular movement. Exophthalmometric values were 4~8 mm by Hertel exophthalmometer. Orbital CT and MRI scans were taken in all patients, and regarded as orbitofrontal cholesterol granuloma. In all cases, excisional biopsy was done through the lateral approach. RESULTS: Orbital CT scan showed an isodense to hypodense, well-defined, homogeneous mass and osteolytic lesion in the adjacent superolateral bony orbit. Orbital MRI scan showed a mass with high signal intensity in both T1- and T2-weighted images. Histopathologically, the specimen showed numerous slits like cholesterol clefts surrounded by multinucleated, foreign body type, giant cells with hemosiderin pigments. CONCLUSIONS: Orbitofrontal cholesterol granuloma, although rare, has typical clinical and radiologic features. Surgical excision has a high success rate without recurrence.

8.The effect of exercise on daily minor stress.

BACKGROUND: Stress can be induced from minor daily life event, and it is widely accepted that this daily minor stress is better predictive factor for relationship between disease and stress. In a number of laboratory studies, investigator reported that perception of stress can be reduced by physical exercise. So in this study, we observed relationship between exercise and stress from minor daily life event, and tried to clarify the effect of exercise on daily stress. METHODS: Volunteer was recruited from health promotion center of one university hospital. and they were doing regular exercise in more than three days per week. Participants completed measures of daily stress inventory and amount of daily exercise for seven days. To compare the daily stress amount between exercise day and no exercise day, we used repeated measures analysis of variance. We used the Wilcoxon signed rank test to compare difference of stress in different sex and different trait anxiety group. RESULTS: There was significant difference in appraisal of daily stress between exercise days and no exercise days. The amount of daily stress was significantly reduced in continuous low level of anxiety group and male by exercise. CONCLUSION: Exercise is associated with a reduction of daily stress appraisal from minor life event. And there is different effect of exercise on daily minor stress in the different anxiety level group and different sex group.

9.A case of osteoma in right ovarian fibroma.

Ovarian fibroma is one of the sex cord-stromal tumor, and accounts for about 5 percent of all ovarian tumors surgically removed. This tumor is almost always unilateral and appears to affect the left ovary more often (75%) than the right. Calcification is one of the secondary changes that can take place in a fibroma of ovary. The occurrence of ossification in these calcified fibromas is rare. We experienced a case of an ovarian calcified fibroma with ossification occurring in a 19 year old, unmarried woman with a brief review of literature.
