1.A case of allergic contact dermatitis due to clindamycin solution.

Contact allergy to clindamycin is rare, especially in relation to its widespread use in topical formulation. Neverthless, when patients using topical clindamycin complain of itching, patch testing with this preparation and its separate ingredients should be considered. We report a case of allergic contact dermatitis due to clindamycin solution for acne vulgaris therapy in a 24-year-old woman.

2.An experimental study on the effect of prostaglandin e1 on the random pattern flap survival in the rats.

The survival of skin flap has vital importance in plastic surgery. Many clinical and experimental researches have been carried out to increase the survival of skin flap. As a result of these researches, many drugs have been used clinically and experimentally for increasing the survival of skin flap, and prostaglandin E1 is known to be used for improving peripheral circulatory disturbance. The pharmacophysiologic mechanisms of the effect of prostaglandin E1 are 1) increasing blood flow by dilatation of smooth muscle of small sized vessels, 2) inhibition of aggregation of platelets, 3) increasing arteriolar blood flow by improving fluidity of erythrocytes, 4) preclusion of free-radical formation, 5) decreasing the endothelial cell damage by down- regulation of sensitivity for eosinophils, 6) making the endothelial cell be refractory to adrenergic stimulation, and 7) promotion of secretion of erythropoietin in the kidney. In this terms, in order to validate effectiveness of preoperative administration of prostaglandin E1 as a pharmacological flap delay, following experimental study was performed. Forty male Sprague-Dawley rats were divided into 4 groups and caudally based 2x8 cmsized flap was elevated on dorsal surface. In group 1, normal saline (0.25 ml/kg) was injected intraperitoneally as control, in group 2, 1 microgram/kg of prostaglandin E1 was injected intraperitoneally for preoperative seven days, in group 3, 1 microgram/kg of prostaglandin E1 was injected intraperitoneally for seven preoperative days, and in group 4, 1 microgram/kg of prostaglandin E1 was injected intraperitoneally for seven days preoperatively and for seven postoperative days. On the seventh postoperative day, the animals were sacrificed and the flap survival area was measured. The following results were obtained. 1. On the seventh postoperative day, the flap survival rates of group 2, 3, 4 were higher than that of group 1 with a statistical significance. 2. On the seventh postoperative day, the flap survival rate of group 3 was higher than group 2, but there was no statistical significance. 3. On the seventh postoperative day, the flap survival rate of group 4 was higher than that of group 2 and 3 with a statistical significance.In conclusion, preoperative administrations of prostaglandin E1 increase flap survival equally as when it is administrated postoperatively, and the improvement in flap survival can be enhanced by continuous administration of prostaglandin E1 in preoperative and postoperative period. So it is expected that surgical flap delay procedure is replaced by pharmacological flap delay by the use of prostaglandin E1 preoperatively.

3.Three Cases of Cutis Marmorata Telangiectatica Congenita.

We report 3 cases of unusual cutis marmorata telangiectatica congenita (CMTC). The ages of the first two cases of CMTC were premature babies, born at intrauterine pregnancy of 31 weeks and 34 weeks. the other patient was a 45-year old female. Compared to the age of other cases reported in the literature our patients ages are unusual. It is not surprising that the first two cases of premature babies had typical signs because CMTC is a congenital disorder, usually seen at birth, but the fact that the skin lesion of CMTC appeared already in the early third trimester may provide some clues to the pathogenesis of CMTC. The third case seems to be very rare because most reported cases show that the skin lesion usually disappear gradually over a period of months to years. The site and size of the lesion in the last case was told to have been fixed for 45 years without any change.

4.The level of residual monomer in injection molded denture base materials.

STATEMENT OF PROBLEM: The residual monomer of denture base materials causes hypersensitivity on oral mucosa and intereferes with the mechanical properties of the cured resin. The amount of residual monomer is influenced by materials, curing cycle, processing method, and etc. PURPOSE: The aim of this study was to investigate the residual methyl methacrylate(MMA) content of injection molded denture base polymer, and to compare this with the self-cured resin and the conventional compression molded heat-cured resin. MATERIALS AND METHODS: Disc shaped test specimens (50mm in diameter and 3mm thick) were prepared in a conventional flasking technique with gypsum molding. One autopolymerized denture base resins (Vertex SC. Dentimex. Netherlands) and two heat-cured denture base resins (Vertex RS. Dentimex. Netherlands, Ivocap. Ivoclar Vivadent, USA) were used. The three types of specimens were processed according to the manufacturer's instruction. After polymerization, all specimens were stored in the dark at room emperature for 7 days. There were 10 specimens in each of the test groups. 3-mm twist drills were used to obtain the resin samples and 650mg of the drilled sample were collected for each estimation. Gas chromatography (Agillent 6890 Plus Gas Chromatograph, Agillent Co, USA) was used to determine the residual MMA content of 10 test specimens of each three types of polymer. RESULTS: The residual monomer content of injection molded denture base resins was 1.057+/- 0.141%. The residual monomer content of injection molded denture base resins was higher than that of compression molded heat cured resin (0.867+/-0.169%). However, there was no statistical significant difference between two groups (p > 0.01). The level of residual monomer in self cured resin(3.675+/-0.791) was higher than those of injection molded and compression molded heat cured resins (p<0.01). CONCLUSION: With respect to ISO specification pass/fail test (2.2% mass fraction) of residual monomer, injection molding technique(1.057+/-0.141%) is a clinicaly useful and safe technique in terms of residual monomer.

5.Quantification of Nucleolar Organizer Regions in Skin Tumors.

BACKGROUND: Nucleolar organizer regions (NORs) have recently attracted much attention because of claims that their frequency within nuclei is significantly higher in malignant cells than in normal, reactive, or benign neoplastic cells. OBJECTIVE: The purpose of this paper is to analyze a method allowing selection of the best morphometric criterion for quantifying AgNOR proteins under conventional observation conditions by light microscopy. METHOD: We tried to investigate the various parameters including NORs counting in cutaneous tumors using image analysis system. RESULTS: There were significant differences in mean nucleus area per a AgNOR, nucleus area between the benign and potentially malignant group. But the conventional counting of AgNORs is not able to differentiate between the two groups. We could discriminate squamous cell carcinoma from Bowen's disease using parameters of mean ratio of AgNORs area per nucleus area, mean ratio of greatest AgNORs area per nucleus area, coefficient of variation (C V) of nucleus area, and mean area of largest AgNORs. In squamous cell carcinoma and keratoacanthoma, C V of nucleus area has shown a significant difference. CONCLUSION: Study of AgNORs using image analysis system is a useful tool for the diagnosis of cutaneous tumors.

6.A case of Ki-1 positive large-cell lymphoma.

In 1985, Stein et al, indentified an anaplastic large-cell malignant lymphoma that was distinctive because of the diffuse dermal infiltration of pleomorphic large lymphocytes, sinus growth pattern, and reactivity with the monoclonal antibody Ki-1(CD30). The clinical feature; distinguishing anaplastic large-cell Ki-1 lymphoma include a young median age, frequent peripheral adenopathy with sparing of the mediastinum, and extranodal disease with skin the most common sitc of involvement. We report a case of Ki-1 postive large-cell lymphoma in a 74-year-old male patient who presented with multiple ulcerated nodules on the right shin area.

7.Total Knee Replacement Arthroplasty In Varus Deformity.

Vnrus deformity is a common finding in patients who are candidates for toial knee replacement arthropiasty. To obtain excellent clinical results and to maintain well-aligned and stable prosthetic components in patients who have such a deformity, adequate ligament balancing as well as accurate cone.ction of bony alignment is required. But many technical problems are encountered. Especially the bony defect on the medial aspect of the tibia after adequate bone cutting and ligament imbalance with laxity in the lateral side and contracture in the medial side. Total knee replacement arthroplasty with 43 patients(56 cases) to be mcre than 10 degrees of varus of tibio-femoral angle was done by a single surgeon and reviewed retrospectively. The diagnosis was osteoarthritis in 46 cases, rheumatoid arthritis in 9 cases. The average follow up period was average 33 months(l-7 years). Preoperative varus deformity of average 13.5 degree., was corrected to valgus 6.69 degrees at last follow-up. Postoperative range of motion was 122.5 degree. And the mean knee society score improved from 24.8 to 87.2 postoperatively and the function score improved from 23.6 to 83.9 postoperatively. At self-assements, 88.2% of the patients was very satisfied or somewhat satisfied. Total knee replacement ;u1hroplasty in the patients with vaus deformity is effective procedure to corri.ct deformity and improve clini- c;1 and functional status, but long-term follow up period is required for changing alignment.

8.Total Knee Replacement Arthroplasty Without Patellar Resurfacing.

We analysed the results of total knee replacement arthroplasty without patellar resurfacing in 20 patients(28 knees) retrospectively. The patella was not resurfaced when it maintained relatively healthy cartilage in osteoarthritis or the patella was too thin to be replaced in rheumatoid arthritis. The diagnoses were osteoarthritis in 11 and rheumatoid arthritis in 17 cases. The follow up period was 30.3 months in average(12-70 months). A lateral retinacular release was performed in 22 knees(78.6%). The mean knee society score improved from 45.4 preoperatively to 84.1 postoperatively and the functional score improved from 21.4 preoperatively to 63.3 postoperatively. The modified Kujala score was 43.2 points in osteoarthritis and 43.6 points in rheumatoid arthritis. We conclude the total knee replacement arthroplasty without patellar resurfacing shows good results, without high risk of postoperative peripatellar complication.

10.Serum CA 125 Levels in Patients with Impaired Renal Function.

Measurement of the serum CA 125 level as a tumor marker in patients with epithelial ovarian cancer has been widely used to monitor disease status and predict survival of patient. While a number of benign gynecologic as well as benign or malignant non-gynecologic conditions are associated with elevations of serum CA 125 levels, the established normal range describes a healthy population of women. Because the metabolism and clearance of CA 125 is not well understood and mild or moderate degrees of renal impairment frequently occurs in ovarian cancer patients during treatment or course of disease, it is valuable to investigate the effect of impaired renal function on serum level of CA 125. Eighty-nine women on hemodialysis who had no other definite cause to elevate serum CA 125 level were selected at random. The age of the patients ranged from 19 to 83 and renal disease was secondary in most cases to diabetes mellitus, hypertension or glomeru-lonephritis. The creatinine clearance was less than 10cc/min for all patients. (continue)
