1.A Comparative Study between Rheumatoid Arthritis Patients and Healthy Persons on Activities of Daily Living, Fatigue and Depression.

This study was conducted to compare activities of daily living, fatigue and depression between rheumatoid arthritis patients and healthy persons. The subjects consisted of 53 rheumatoid arthritis patients and 53 healthy persons at a university hospital in Daegu City. Data were collected by means of structured interviews with questionnaires from July 20, 1999 to August 25, 1999. The instrument used in this study were the activities of daily living scale developed by Katz et al. (1970) and Barthel(1973), Multidimensional Assessment of Fatigue by Belza et al.(1995) and CES-D(Center for Epidemiologic Studies-Depression) scale. Analysis of data was done by use of descriptive statistics, Pearson Correlation, Chi-square test, t-test, ANOVA, MANCOVA and Duncan with the SPSS program. The major findings are summarized as follows: 1. The first hypothesis that the rheumatoid arthritis patients will have a lower degree of activities of daily living than the healthy persons was supported (F=4.584, p=.035). 2. The second hypothesis that the rheumatoid arthritis patient will have a higher degree of fatigue than the healthy persons was supported (F=7.799, p=.006). 3. The third hypothesis that the rheumatoid arthritis patients will have a higher degree of depression than the healthy persons was supported (F=4.768, p=.031). With the above results, it can be concluded that rheumatoid arthritis patients had a lower degree of activities of daily living and a higher degree of fatigue and depression than the healthy persons. Therefore, by providing appropriate nursing intervention, activities of daily living would be much better and fatigue and depression would be alleviated.

2.Weight Gain Associated with Atypical Antipsychotic Drugs.

The purpose of this study is to compare weight gain by risperidone, clozapine and olanzapine which are atypical antipsychotic drugs with that by haloperidol which is typical antipsychotic drug. METHODS: We performed a retrospective chart review of 278 schizophrenic patients who were treated longer than 12 weeks with 4 different drugs. We measure weight gain weekly until 12th week and weight change curve over 12 weeks was drawn. Additionally, we examined any correlations of weight change with some factors. RESULTS: 1) The highest weight gains were observed in the olanzapine and clozapine groups, followed by the risperidone and haloperidol groups. 2) The haloperidol group showed a main weight gain within first 3 weeks, and patients ingested 3 atypical drugs gained weight steadily during 12 weeks. 3) The initial underweight group showed the largest weight gain. CONCLUSION: This study shows that the atypical antipsychotic drugs induced more weight gain than haloperidol. And we can expect that long-term treatment with clozapine and olanzapine would induce more severe weight gain, particularly in underweight patients.

3.Factors Influencing Life Satisfaction Among the Senior Welfare Center Elderly.

PURPOSE: This study was conducted to evaluate life satisfaction(LS) and to assess the factors that influence LS in senior center elderly people. METHODS: The subjects were 253 elders who visited the Y city Senior Welfare Center between July 2006 and August 2006. Data were collected using structured questionnaires. The instruments were composed of Kang's Family Support Scale, Depression Scale by Sheikh & Yesavage, Laffery's Health Concept Scale, and LS scale by Choi. T-test, ANOVA, Duncan test, Pearson coefficients correlation and stepwise multiple regression were used to analyze the data using the SPSS Win 12.0 program. RESULTS: The LS level showed a maximum score of 38 with a mean score of 23.23. The mean scores for depression, health perception and family support were 5.52(maximum score, 15), 77.46(maximum score, 112), 43.45(maximum score, 55) respectively. The LS were significantly correlated with health perception, depression and family support. Depression was the most powerful predictor of and it accounted for 42.0% of the total variance in LS. A combination of significance of eudaimonistic health perception and satisfaction with pocket money accounted for 47.2% in LS. CONCLUSION: Depression and health perception were identified as affecting variables for the LS. Based upon these results, nurses should help the aged to enhance their LS.

4.Needle Depth to Lumbar Plexus in Lumbar Plexus Block by Posterior Approach and Its Relation with Body Indices in Korean.

BACKGROUND: Lumbar plexus block by posterior approach has been used for thigh and hip surgery and unilateral low back pain and/or low extremity pain control. We measured the needle depth from the skin to the lumbar plexus and its relation with body indices in Korean. METHODS: Forty-eight (male 35, female 13) patients with unilateral low back pain and/or low extremity pain received lumbar plexus block by the posterior approach known as "psoas compartment block". We measured the distance from the skin to the lumbar plexus with the aid of a nerve stimulator and evaluated its relation to body mass indices such as weight, height, abdominal circumference and body mass index (BMI; kg/m2). RESULTS: The distance described above was 7.42 +/- 0.82 cm (range 6.0-9.5 cm) in males and 7.18 +/- 1.24 cm (range 5.3-10.0 cm) in females. Its relation to body weight was greater than with other body indices. The Pearson's coefficient between the distance and body weight was 0.745 in males and 0.842 in females. The predicted distance (cm) was 4.23 + 0.0471 x weight (kg) in males (adjusted R2 = 0.532) and 1.25 + 0.0975 x weight in females (adjusted R2 = 0.682). CONCLUSIONS: Body weight is the most important factor for prediction of the distance from the skin to the lumbar plexus. The distance was about 6-10 cm in korean patients.

5.Effects of Lifestyle and Depression on the Nutritional Status of Elderly People.

PURPOSE: This study was conducted to evaluate nutritional status, and the effects of lifestyles and depression on nutritional status of elderly people (NSE). METHOD: The participants were 280 elders who visited the Y city Senior Welfare Center between August 2006 and October 2006. Data were collected using a structured questionnaire that included general characteristics, lifestyle checklist, depression scale, and nutritional risk index. Also, Body Mass Index (BMI) was calculated from height and weight. T-test, ANOVA, Duncan test, Pearson coefficients correlation and stepwise multiple regression were used to analyze the data using the SPSS Win 12.0 program. RESULTS: The NSE mean score was mid level at 5.03. BMI was 23.42 which indicates overweight. Depression was the factor that influenced the nutritional risk index most strongly, accounting for 17.3% of the total variance in the NSE. A combination of significance of health, concerns about health, housing, condition of teeth, activity, and smoking pattern accounted for 29.8% of the variance in the NSE. Condition of teeth accounted for only 3.8% of the variance in BMI. CONCLUSION: It is necessary to be aware that variance in the NSE can be affected by several factors including depression and lifestyle and that depression is strongly related to poorer NSE.

6.The Effect of Back Pain Prevention Intervention Program on Back Pain Relief in Patients Following Percutaneous Coronary Intervention.

PURPOSE: The purpose of this study was to identify the effectiveness of back pain prevention intervention program on reducing back pain of patients undergoing percutaneous coronary intervention (PCI). METHODS: The patients were divided into two groups as the experimental group and control group. In the experimental group, the participants took bedrest for 4 hours after PCI and then received back pain prevention intervention program (BPPIP). Total of 5 times BPPIP with 1 hour interval for 5-10 minutes per each intervention was administered to the patients taking bedrest for 4 hours after PCI. In the control group, total of 5 times routine nursing care with 1 hour interval was administered to the patients taking bedrest for 4 hours after PCI. The data were collected on admission in the ICU and after the 5 exercise sessions. The collected data were analyzed using SPSS/WIN program. RESULTS: No significant differences in the occurrence of hemorrhage after the BPPIP were observed between the experimental group and the control group. After the BPPIP, back pain outcomes were significantly low in the experimental group. However, urination disorder and cortisol level did not show a statistically significant differences between two groups. CONCLUSION: It is clear that BPPIP is a useful nursing intervention for reducing back pain of patients undergoing PCI.

7.ST-Segment Elevation Myocardial Infarction as a Result of Coronary Artery Ectasia-Related Intracoronary Thrombus in a Patient with Liver Cirrhosis.

Coronary artery ectasia (CAE) is a rare condition defined as the dilatation of coronary artery to at least 1.5 times larger than the normal adjacent coronary artery. Clinical manifestations of CAE vary, ranging from asymptomatic to ST-segment elevation myocardial infarction (STEMI). Because of its rarity and clinical diversity, the best treatment strategy and prognosis for CAE remain unclear. We describe a case of STEMI caused by intracoronary thrombus formation within an ectatic area in a patient with liver cirrhosis (LC). The patient was successfully managed by thrombus aspiration only, without balloon angioplasty or stent implantation, and maintained by dual antiplatelet therapy with aspirin and ticagrelor, a potent new P2Y12 inhibitor.

8.Growth changes in external nose of the Korean people from 8 to 16 years old.

This study was conducted to examine the growth change in external nose according to age which plays an important role in determining the facial profile, by analysing the biennial serial lateral cephalograms of 20 males and 20 females of mean 8.5 years old to 16.5 years old who have a normal occlusion. The results of this study were summarized as follows. Both mean and standard deviation were obtained according to each item, sex and age for the male and female. Nose heights of the male and female increased with increase in their age. The upper to lower nose height ratio somewhat decreased for both male and female with increase of age. The mean ratio was 3.69 : 1 for the male, respectively, and 3.83 : 1 for the female during this study. The nose depth indicated the most significant increase of all items according to the increase of age, while the sagittal depth indicated higher value for the female than that of the male in the most age groups. The nose inclination did not show a remarkable change during this study. The proportions of growth completed at the earliest age showed the highest percentage for sagittal depth(male : 87.14% & female: 91.29%). Also, the male indicated the lowest percentage in lower nose height (73.04%), while the female did the lowest in nose depth(79.09%). There was shown the variable of the greatest correlation between the upper nose inclination and nose depth. From this study, the whole external nasal growth was occurred downward and forward which was more frequent for forward growth than vertical growth.

We report our experience of unexpected low expiratory concentration of sevoflurane 0.8 vol%, low inspired fraction of oxygen 31%, and high value of bispctral index (BIS) 62 with sevoflurlane vaporizer setting 2.0 vol% and the inspired fraction of oxygen 50% during general anesthesia of a 56-year-old man undergoing elective laparoscopic hemicolectomy. The anesthesia machine (Cato edition, Drager, Lubeck, Germany) with a piston-based, electronically controlled ventilator was used for anesthesia. We checked thoroughly all the possible causes of the discrepancy of anesthetic gas concentration, and then detected the partial disconnection of fresh gas outlet. But there were no alarm signs of anesthetic monitor with adequate ventilator function. This malfunction of anesthetic gas supply without alarm signs may lead to hypoxia and awareness of patients. The intraoperative monitoring of anesthetic gas analyzer and BIS may be essential to detect the malfunction of anesthesia machine.
