1.Association of the CYP1B1 Gene Polymorphism with the Risk of Advanced Endometriosis in Korean Women.

OBJECTIVE: To investigate whether polymorphisms of gene encoding CYP1B1 is associated with the risk of endometriosis in Korean women. METHODS: We investigated 199 patients with histopathologically confirmed endometriosis rAFS stage III/IV and 183 control group women who were surgically proven to have no endometriosis. The genetic distribution of four different CYP1B1 polymorphisms at G119-T, G432-C, T449-C, and A453-G were analyzed by polymerase chain reaction (PCR) and restriction fragment length polymorphism of PCR products. RESULTS: We found no overall association between each individual CYP1B1 genotype and the risk of endometriosis. The odds ratio of genotype GG/GC+GG/TC+TT/AA compared to GG/CC/CC/AA (reference) was calculated as 2.06 with a 95% confidence interval of 1.003~4.216. CONCLUSIONS: This results suggest that CYP1B1 genetic polymorphism may be associated with development of endometriosis in Korean women.

3.The Postoperative Analgesic Effect of Transdermal Fentanyl with Patch in Total Abdominal Hysterectomy.

BACKGROUND: Compared with conventional routes of delivering potent analgesics to postoperative patients, transdermal administration of fentanyl offers the advantages of simplicity and noninvasiveness. The analgesic efficacy and safty of transdermal fentanyl patch (TDFP) were evaluated postoperatively. METHOD: TDFP releasing 25 mcg/hr (Group 1) or placebo (Group 2) were applied to 40 women 6 hours before total abdominal hysterectomy under the general anesthesia. Postoperatively, self-administered intravenous fentanyl was maintained with a 20-mcg incremental dose and a 10-min. locking interval. Each group was assessed following 48 hours with respects to vital signs, VAS pain scores, hourly-used fentanyl doses, satisfaction scores and side effects. RESULT: VAS observed 24 hours, 36 hours after operation were significantly lower in group 1 than group 2. Hourly-used fentanyl doses were significantly lower in group 1 than group 2 at 2 hours, 6 hours, 12 hours and 24 hours after operation. The incidence of side effects were similar between group 1 and group 2. CONCLUSION: TDFP-25 mcg applied 6 hours before operation provides supplementary analges-ia after the postoperative period without significant side effects such as respiratory depression.

4.A Systematic Review of the Effects of Aromatherapy on Depression and Anxiety in Postmenopausal Women

Purpose:
This systematic review aimed to investigate the effects of aromatherapy interventions on depression and anxiety in menopausal women.
Methods:
This study adhered to PRISMA (preferred reporting items of systemic reviews and meta-analysis) guidelines. Relevant studies published between 1994 and 2002 were searched in PubMed, Embase, Cochrane Library, CINAHL, Google Scholar, DBPIA, KISS, and RISS databases. Search criteria included the mesh terms 'aromatherapy menopause women depression anxiety.' The review included randomized and nonrandomized studies of women who were menopausal or postmenopausal and received aromatherapy intervention for depression and anxiety associated with symptoms of menopause. The extracted literature was evaluated via quality appraisal checklists of ROB 2 (Risk of Bias 2.0) and ROBINS-1 (Risk Of Bias In Non-Randomized Studies - of Interventions) and visualized using a risk-of-bias visualization tool.
Results:
The review included 6 randomized controlled studies and 2 quasi-experimental studies. The results showed that aromatherapy massage and inhalation therapy were effective in reducing depression and had beneficial effects in reducing anxiety, improving quality of sleep, and menopausal symptoms in menopausal women.
Conclusion
Interventions using aromatic essential oils to massage the hands, arms, back, and scalp or inhalation of aromatic oils from clothing, necklaces, and bedding might be beneficial for the emotional health of menopausal women. Women health professionals should consider applying aromatherapy to menopausal women to improve emotional health, sleep, and menopausal symptoms.
5.Association between Endometriosis and Polymorphisms of N-acetyl Transferase 2 (NAT2), Glutathione S-transferase M1 (GSTM1) and Cytochrome P450 (CYP) 1A1 Genes in Korean Infertile Patients.

OBJECTIVE: To investigate the association between endometriosis and polymorphisms of N-acetyl transferase 2 (NAT2), glutathione S-transferase M1 (GSTM1), and cytochrome P450 (CYP) 1A1 genes in Korean infertile patients. MATERIALS AND METHODS: A total of 303 infertile patients who had undertaken diagnostic laparoscopy during January, 2001 through December, 2003 at Samsung Cheil Hospital enrolled in this study. The patients were grouped according to laparoscopic findings: minimal to mild endometriosis (group I: n=147), moderate to severe endometriosis (group II: n=57), normal pelvic cavity (n=99). Peripheral blood was obtained and genomic DNA was extracted. The genotypes of each genes were analyzed using polymerase chain reaction (PCR) and restriction fragment length polymorphism (RFLP). For NAT2, RFLP was used to detect the wild type (wt) and mutant (mt) alleles, enabling classification into slow (mt/mt) or fast (wt/wt or wt/mt) acetylation genotypes. For GSTM1, PCR was used to distinguish active (+/- or +/+) from null (-/-) genotypes. For CYP1A1, MspI digestion was used to detect the wild type (A1A1), heterozygote (A1A2) or mutant (A2A2) genotypes. RESULTS: The genotype frequencies of NAT2 slow acetylator was 12.8%, 10.9%, 12.8% in group I, group II and control, respectively. The genotype frequencies of GSTM1 null mutation was 55.3%, 41.8%, 53.2% in group I, group II and control, respectively. The genotype frequencies of CYP1A1 MspI polymorphism was 16.3%, 9.1%, 18.1% in group I, group II and control, respectively. No significant difference was observed between endometriosis and normal controls in the genotype frequencies of the NAT2, GSTM1, CYP1A1 MspI polymorphism. CONCLUSION: The NAT2, GSTM1, CYP1A1 gene polymorphism may not be associated with the susceptibility of endometriosis in Korean women.

6.Pathology of Epstein-Barr Virus-Associated Gastric Carcinoma and Its Relationship to Prognosis.

Among Epstein-Barr virus (EBV)-associated neoplasms, EBV-associated gastric carcinoma (EBVaGC) is the most common tumor worldwide. In contrast to the predominant site of occurrence of EBV-negative gastric carcinoma in the antrum, EBVaGC occurs most frequently in the proximal stomach, including the cardia, fundus and body. Microscopically, EBVaGC can be subclassified into three histological subtypes according to the host cellular immune responses: lymphoepithelioma-like carcinoma, carcinoma with Crohn's disease-like lymphoid reaction, and conventional-type adenocarcinoma. Recent studies have shown that patients with the lymphoepithelioma-like carcinoma subtype of EBVaGC have the best overall and disease-free survival, followed by Crohn's disease-like reactions, which in turn have better survival than patients with conventional-type adenocarcinoma. Histologic subclassifications of EBVaGCs are based on the differing degree and pattern of infl ammatory response and the extent of desmoplasia. Because these subclassifications appear to be a powerful prognostic parameter, further research into the underlying mechanisms of the cellular immune reaction in these pathologic subtypes of EBVaGCs may play a key role in understanding the innate immune response of patients with this highly aggressive carcinoma.

8.Effects of Delivery Nursing Care using Essential Oils on Delivery Stress Response, Anxiety during Labor, and Postpartum Status Anxiety.

OBJECTIVES: This study was designed to investigate the effect of delivery nursing care using essential oils on labor stress response, labor anxiety and postpartum status anxiety for primipara. METHODS: This study used nonequivalent control group pretest-posttest design. The subjects of this experiment consisted of forty eight primipara with single gestation, full term, & uncomplicated pregnancies. Twenty four primipra were in the experimental and control group each. Their mean age was 27.9 years old, their mean gestation period 279.9 days. As a treatment, delivery nursing care using essential oils was applied by nurses. Data collected epinephrine, norepinephrine, anxiety during labor. In the 24 hours after birth, the data for the postpartum mother's status anxiety was collected. Data was analyzed by t-test, repeated measures ANOVA, Mann-Whitney U test, & Wilcoxon signed ranks test with SPSS Program. RESULTS: Plasma epinephrine, norepinephrine were significantly low in the experimental group (P=0.001, P=0.033, respectively). There was no significant difference between the two groups in anxiety during labor and postpartum mother's status anxiety. CONCLUSION: These findings indicate that delivery nursing care using essential oils could be effective in decreasing plasma epinephrine, norepinephrine. But, that could not be verified in decreasing mother's anxiety.

9.Immunohistochemical Phenotypes of Phyllodes Tumor of the Breast.

BACKGROUND: The aim of the study is to evaluate the difference of immunophenotypes of stromal cells between the pure phyllodes tumor (PT) type and the co-existent type of phyllodes tumor and fibroadenoma, and between benign and malignant PT. METHODS: Immunohistochemical staining for actin, CD34, CD10, c-kit, bcl-2, p53 and MIB-1 was performed using tissue microarray blocks that contained 25 cases of pure PT (16 benign tumors, 4, borderline malignant tumors, and 5 malignant tumors) and 6 cases of co-existent type. RESULTS: The expression rates of CD34 and MIB-1 in the pure PT type were significantly higher and the expression rate of actin in the pure PT was significantly lower than that of the co-existent type. However, there were no significant differences in the bcl-2, CD10, and p53 expressions between the pure PT type and the co-existent type, and no c-kit expression was observed in both types. No significant differences in the CD34, actin, bcl-2, CD10, and p53 expressions between the benign and borderline/malignant PT cases were found. However, a significant difference of the MIB-1 expression rate was noted. CONCLUSIONS: The stromal cells of the pure PT type are regarded as less mature myofibroblasts, and the CD10 reactivity in some phyllodes tumors suggests a myoepithelial origin. The MIB-1 labeling index would be useful for the grading of phyllodes tumor.
10.Subcutaneous Granuloma Annulare of the Scalp: A case report.

Subcutaneous granuloma annulare (SGA) is a rare, benign noninfectious histiocytic disease of unknown cause, characterized by necrobiosis of the connective tissue surrounded by infiltrates of histiocytes and lymphocytes. We report a case of SGA in a 49/12-year-old boy. The lesions were five nontender subcutaneous nodules in the parieto-occipital scalp, measuring 1.0 cm to 2.0 cm in diameter. Microscopical examination revealed variable stages of multiple necrobiotic areas, which showed central necrobiosis with palisading histiocytes, involving both the lower dermis and subcutaneous fatty tissue. Electron microscopic findings revealed many histiocytes in the necrobiotic areas with degenerating and necrotic collagen fibers as well as regenerating fibroblasts.
