1.Optic Nerve Sheath Meningocele with Optic Disc Pit

Purpose:
Optic nerve sheath meningocele is a very rare condition; if the optic nerve is pressed, visual function can deteriorate. We report a patient with optic nerve sheath meningocele treated with optic nerve sheath fenestration.Case summary: We report a 15-month-old male who visited the outpatient clinic due to an orbital tumor in the left eye found by accident. He had no proptosis or relative afferent pupillary defect, and fundus examination revealed a left optic disc pit. On fat-suppressed magnetic resonance imaging (MRI), there was a well-defined 13.5 × 12.7 mm cystic lesion located at the superotemporal aspect of the left optic nerve within the optic nerve sheath. The optic nerve was compressed and displaced inferomedially. Once the diagnosis had been confirmed as optic nerve sheath meningocele, optic nerve sheath fenestration was performed under general anesthesia to relieve optic nerve compression. The compressed left optic disc returned to normal after surgery and the size of the meningocele shrunk to 11.7 × 10.9 mm on MRI. However, it re-expanded to almost the same size as that preoperatively 10 months later.
Conclusions
We report a case of optic nerve sheath meningocele with an optic disc pit treated with optic nerve sheath fenestration, which has not been reported previously in the Republic of Korea. Because the effect of optic nerve sheath fenestration could be temporary, it is necessary to conduct regular checkups given the possibility of deterioration of visual function.

4.Regulation of TNF - alpha Gene Expression in Human Fetal Astrocytes.

Tumor necrosis factor-n (TNF - alpha) involved in the pathogenesis of multiple sclerosis and contribute to the degeneration of oligodendrocytes as well as neurons. TNF - alpha is produced by miocroglia and astrocytes, which also produce hormones and cytokines that influence its biological activity. Astrocytes, the major glial cells in the CNS, are capable of producing TNF - alpha at both the mRNA and protein levels in response to interleukine-1 (IL-1) or TNF - alpha. Two immunosuppressive cytokines, transforming growth factor - beta (TGF - beta) and IL-10, have been shown to influence glial cell function. TGF - beta can modulate the activity of glial cells by inhibiting interferon-gamma (IFN - gamma) induced expression of class II major histocompatibility complex (MHC) molecules on astrocytes and microglia. To explore the role of astrocytes in the production of TNF - alpha, astrocytes were pretreated with IL-10 or TGF - beta and then stimulated with IL-1p to determine their effects on TNF - alpha production. The secretion of TNF - alpha by human fetal astrocytes was markedly inhibited by TGF - beta at a low concentration. In contrast IL-10 had no effect on TNF - alpha mRNA level. These results show that TGF - beta may regulate the expression of TNF - alpha in activated human fetal astrocytes.

5.Effects of Fatigue and Postpartum Depression on Maternal Perceived Quality of Life (MAPP-QOL) in Early Postpartum Mothers.

PURPOSE: This study was done to identify effects of fatigue and postpartum depression on quality of life in early postpartum mothers. METHODS: The data were collected from 130 mothers at four general hospitals in J and M metropolitan cities. Instruments used to collect the data for the study were the Fatigue Scale developed by Pugh (1993); Postpartum Depression Scale developed by Cox, Holden & Sagovsky (1987), and the Quality of Life Scale developed by Hill, Aldag, Hekel, Riner, G., & Bloomfield (2006). RESULTS: Results showed that the mean for fatigue was 56.74, the mean for postpartum depression was 8.00+/-4.37 and mean for quality of life was 19.78. The quality of life variable showed statistically significant differences for the variable: age (F=3.20, p=.026). The relationship between fatigue and quality of life showed a significant negative correlation (r=-.44, p<.001). The relationship between postpartum depression and quality of life also showed a negative correlation (r=-.42, p<.001). The relationship between postpartum depression and fatigue showed a positive correlation (r=.59, p<.001). These factors explained 23% of the variance in quality of life. CONCLUSION: The results indicate that it is necessary to develop nursing intervention programs to improve quality of life in for early postpartum mothers.

6.A study on the prevalence of malocclusion of Yonsei university students in 1991.

Over recent 20 years, with socioeconomic development and change of recognition of the population, more people are concerned about their health and appearance. To obtain the change of frequency of malocclusion and the demand for orthodontic treatment, with this trend, 2460 freshmen and students of Yonsei Univ. in 1991, aged from 18 to 21 were examined excluding 187 students who have history of orthodontic treatment, 86 students who are undergoing treat?ment, 39 students who have too much missing teeth to classify. After analize the frequency of malocclusion and the demand for orthodontic treatment, following results was obtained. 1. Sex ratio in the prevalence of malocclusion was 91.7%/90.8%, male to female, so there was no sex predilection. 2. With regard to Angle's Classification, each percentage of Class I, Class II div. 1, Class II div. 2 and Class III was 61.6%.11.3%,1.9% and 16.7%. 3. Of Class I malocclusion, percentage of crowding was 53.2, this occupied the largest part of single findings and prevalent findings in combinations were also crowding - crossbite and crowding - Protrusion. 4. Of Class I malocclusion, over the portion of 95%, Bialveolar Protrusion have arised alone. 5. The distribution of Demands for orthodontic treatment of malocclusion were 41.3% in males, and 51.3% in females.

8.Analysis of the virtual simulation practice and high fidelity simulation practice training experience of nursing students: A mixed-methods study

This study used an exploratory sequential approach (mixed methods) design to explore essential meaning through comparing and analyzing the experiences of nursing students in virtual simulation practice and high fidelity simulation practice education in parallel.
Methods:
The study participants were 20 nursing students, and data were collected through focus group meetings from July 17 to August 5, 2020, and via online quantitative data from November 10 to November 15, 2020. The qualitative data were analyzed using Giorgi's phenomenological method, and the quantitative data were analyzed using descriptive statistics, the Mann-Whitney U test, Kruskal–Wallis H test analysis of variance and Spearman’s ρ correlation.
Results:
The comparison between the two simulation training experiences was shown in five contextual structures, as follows: (1) reflection of the clinical field, (2) thinking theorem vs. thinking expansion, (3) individual-centered learning vs. team-centered learning, (4) attitudes toward participating in practical training, (5) metacognition of personal competency as a prospective nurse, and (6) revisiting the method of practice training. There was a positive correlation between satisfaction with the practice and the clinical judgment ability of high fidelity simulation, which was statistically significant (r=.47, p=.036).
Conclusion
Comparing the experiences between virtual simulation practice training and high fidelity simulation practice training, which has increased in demand due to the Coronavirus Disease-2019 pandemic, is meaningful as it provides practical data for introspection and reflection on in-campus clinical education.

10.Bone Marrow Progenitors and IL-2 Signaling Contribute to the Strain Differences of Kidney Innate Lymphoid Cells

Innate lymphoid cells (ILCs) are critical immune-response mediators. Although they largely reside in mucosal tissues, the kidney also bears substantial numbers. Nevertheless, kidney ILC biology is poorly understood. BALB/c and C57BL/6 mice are known to display type-2 and type-1 skewed immune responses, respectively, but it is unclear whether this extends to ILCs. We show here that indeed, BALB/c mice have higher total ILCs in the kidney than C57BL/6 mice. This difference was particularly pronounced for ILC2s. We then showed that three factors contributed to the higher ILC2s in the BALB/c kidney. First, BALB/c mice demonstrated higher numbers of ILC precursors in the bone marrow. Second, transcriptome analysis showed that compared to C57BL/6 kidneys, the BALB/c kidneys associated with significantly higher IL-2 responses. Quantitative RT-PCR also showed that compared to C57BL/6 kidneys, the BALB/c kidneys expressed higher levels of IL-2 and other cytokines known to promote ILC2 proliferation and/or survival (IL-7, IL-33, and thymic stromal lymphopoietin). Third, the BALB/c kidney ILC2s may be more sensitive to the environmental signals than C57BL/6 kidney ILC2s since they expressed their transcription factor GATA-3 and the IL-2, IL-7, and IL-25 receptors at higher levels. Indeed, they also demonstrated greater responsiveness to IL-2 than C57BL/6 kidney ILC2s, as shown by their greater STAT5 phosphorylation levels after culture with IL-2. Thus, this study demonstrates previously unknown properties of kidney ILC2s. It also shows the impact of mouse strain background on ILC2 behavior, which should be considered when conducting research on immune diseases with experimental mouse models.
