1.Factors Influencing Self-care in Elders with Hypertension Living at Home.

PURPOSE: The purpose of the study was to identify the level of resilience, autonomous motivation, depression and self-care and their correlates in elders with hypertension. METHODS: The participants were 200 elders with hypertension living in J, I, and K city. Data were collected through face-to-face interviews using the Resilience Scale, Treatment Self Regulation Questionnaire, Short Form Geriatric Depression Scale, and Hypertension Self-care Scale. RESULTS: Total scores for resilience, autonomous motivation, depression, and self-care were 109.65± 20.78, 38.18±5.99, 8.33±2.61, and 52.06±6.94, respectively. Autonomous motivation, income, acceptance of self and life, resilience, experience of hypertension education, and gender were verified as factors influencing self-care. These factors accounted for 51.6% of self-care. CONCLUSION: Understanding these factors is essential for developing effective interventions to improve self-care in elders with hypertension.

2.Apolipoprotein E Gene Polymorphism and Plasma Lipid Levels in Male Korean Patients with Chronic Renal Failure.

Accelerated atherosclerosis is not only a frequent complication but also the most common cause of death in patients with chronic renal failure (CRF). Although mechanisms are unclear, disorder of lipid metabolism may be a major factor. Since apolipo-protein (apo) E is known to play a major regulatory role in lipid metabolism, we evaluated apo E genotype in 72 male patients with CRF and compared with that in 194 rnale normal controls. In addition, we measured plasma lipid and apolipoprotein concentrations and evaluated them according to apo E genotype in patients and controls. Apo E genotype was determined with the INNO-LiPA Apo E kit (Innogenetics, Belgium), which is based on reverse hybridization. The results are as follows ; 1) The distribution of the three major apo E alleles in patients with CRF ( e 2: 6.2%, e 3: 80.6%, e 4: 13.2%) was not different from that in controls ( e 2: 4.1%, e 3: 87.6%, e 4: 8.3%). 2) In patients with CRF, total cholesterol, lowdensity lipoprotein (LDL) and high-density lipoprotein (HDL) levels were significantly lower and the triglyceride and lipoprotein (a) levels were significantly higher than those in controls. 3) In controls, E 4/3 group had significantly lower levels of HDL than E 3/3 and E 3/2 groups. In patients with CRF, E 4/3 group had significantly higher levels of total cholesterol and apo B lipoprotein than E3/2 group. In conclusion, although there was no significant difference in the apo E genotype frequencies between male patients with CRF and controls, apo E polymorphism may play an important role in the determination of individual differences in plasma lipids in male patients with CRF.

3.A Study on the Expression of p53 and nm23 Protein in the Colorectal Adenoma and Carcinoma.

The expression of the nuclear phosphoprotein p53, a product of tumor suppressor gene, has been noted in a number of human tumors as a tumor suppressor. nm23 is a gene associated with low tumor metastatic potential and has been proposed to be a metastasis suppressor gene. To assess the role of p53 and nm23 expression in colorectal tumorigenesis and the association with clinicopathological parameters, an immunohistochemical study for mutant p53 and nm23 was done using mouse monoclonal antibodies in 43 colorectal carcinomas, 55 tubular adenomas and corresponding normal mucosa. In the tubular adenomas, p53 expression was significantly correlated with the degree of atypism(p<0.05) but not with other variables as well as with nm23. In the colorectal carcinoma, there were evidence of some correlation between metastasis, laterality and p53; laterality, depth of invasion and nm23 expression, but without statistical significance. Other clinicopathologic features were not significantly correlated. In the aspect of 'adenoma-carcinoma sequence', normal mucosa was totally negative for both p53 and nm23, and they were increasingly expressed through tubular adenoma to carcinoma with statistical significance(p<0.05). Therefore, it is suggested that both p53 and nm23 expressions occur in and around the time of transition to carcinoma from adenoma but are not significantly associated with the infiltrative behavior and metastasis.

4.Trends of Study and Classification of Reference on Occupational Health Management in Korea after Liberation.

The purposes of this study are to define the scope of occupational health management and to classify occupational management by review of related journals from 1945 to 1994 in Korea. The steps of this study were as follows: (l)Search of secondary reference; (2) Collection and review of primary reference; (3) Survey; and (4) Analysis and discussion. The results were as follows; 1. Most of the respondents majored in occupational health(71.6%), and were working in university (68.3%), males and over the age 40. Seventy percent of the respondents agreed with the idea that classification of occupational health management is necessary, and 10% disagreed. 2. After integration of the idea of respondents, we reclassified the scope of occupational health management. It was defined 3parts, that is, occupational health system, occupational health service and others (such as assessment, epidemiology, cost-effectiveness analysis and so on). 3. The number of journals on occupational health management was 510. It was slightly increased from 1986 and abruptly increased after 1991. The kinds of journals related to occupational health management were The Korean Journal of occupational Medicine(18.2%), several kinds of Medical College Journal(17.0%), The Korean Journal occupational Health(15.l%), The Korean Journal of preventive Medicine(15.l%) and others(34.6%). As for the contents, the number of journals on occupational health management systems was 33(6.5%) and occupational health services 477(93.5%). Of the journals on occupational health management systems, the number of journals on the occupational health resource system was 15(45.5%), occupational finance system 8(24.2%), occupational health management system 6(l8.2%), occupational organization 3(9.1%) and occupational health delivery system l(3.0%). of the journals on occupational health services, the number of journals on disease management was 269(57.2%), health management 116(24.7%), working environmental management 85(18.1%). As for the subjects, the number of journals on general workers was l85(71.l%), followed by women worker, white color workers and so on. 4. Respondents made occupational health service(such as health management, working environmental management and health education) the first priority of occupational health management. Tied for the second are quality analysis(such as education, training and job contents of occupational health manager) and occupational health systems(such as the recommendation of systems of occupational and general disease and occupational health organization). 5. Thirty seven respondents suggested 48 ideas about the future research of occupational health management. The results were as follows: (l) study of occupational health service 40.5%; (2) Study of organization system 27.1%; (3) Study of occupational health system (e.g. information network) 8.3%; (4) Study of working condition 6.2%; and (5) Study of occupational health service analysis 4.2%.

5.A Study on the Menopausal Symptoms and Quality of Life in Middle Aged Women.

PURPOSE: The purpose of this study is to investigate the differences Quality of Life(QOL) according to menopausal symptoms in middle aged women. METHOD: The subjects consisted of 578 women(45-60 years old) residing in urban area by convenience sampling from March 15 to May 30, 2002. The data were collected by structured questionnaire that included general characteristics, menopausal symptoms and QOL scale. the collected data were analyzed by the SPSS 11.0 program that included descriptive statistics, t-test and ANOVA. RESULT: The averaged age of menopause of subjects was 48.29 4.66 years and 74.8% in the subjects complained menopausal symptoms. As for the orders of complained menopausal symptom, it was fatigue, hot flashes, benumbed hands and feet, and irritability. The mean score of the QOL scale was showing above average level of quality of life. Religion was significantly different to the score for activity of QOL, monthly income was different to the score for activity and physical well-being of QOL. Menopausal status was significantly different to the QOL. CONCLUSION: This study suggests that a replicate study is needed. The results are also useful in developing various programs for health promotion of middle aged women.
6.The Actual Condition and an Alternative of Students in the Department of Dental Hygiene about Dental Instrument Injuries during Clinical Practice

Background:
Students in the department of dental hygiene are exposed to injections and sharp instruments during clinical practice. Therefore, it is necessary to develop measures for a safe practice environment. This study aimed to investigate dental stabbing accidents caused by dental instruments during clinical practice among students in the department of dental hygiene and suggest appropriate preventative measures.
Methods:
This study was conducted from May 1 to June 30, 2016, with students in the department of dental hygiene located at several universities in Seoul and Gyeonggi area. The study included 339 participants. A frequency analysis was performed to determine the general characteristics of dental infection control. A cross-analysis was conducted to identify the relationship between dental infection control education, stabbing accident prevention education, and treatment after stabbing accidents with a dental instrument.
Results:
Among the participants, 81.1% received dental infection control education and 66.4% received stabbing accident prevention education. Only 50.9% received hepatitis B vaccinations.
Conclusion
Dental infection control education and stabbing accident prevention education were shown to be effective in preventing dental instrument stabbing in students. However, post-accident processing, such as reporting to upper management and medical treatment after the accident, was insufficient. Therefore, it is necessary to present a treatment flowchart for dealing with stab accidents in clinical practice for students in the department of dental hygiene and strengthen education.
7.Comparison of Clinical Characteristics of Fluorescence in Quantitative Light-Induced Fluorescence Images according to the Maturation Level of Dental Plaque

Background:
Proper detection and management of dental plaque are essential for individual oral health. We aimed to evaluatethe maturation level of dental plaque using a two-tone disclosing agent and to compare it with the fluorescence of dental plaque on the quantitative light-induced fluorescence (QLF) image to obtain primary data for the development of a new dental plaque scoring system.
Methods:
Twenty-eight subjects who consented to participate after understanding the purpose of the study were screened. Theimages of the anterior teeth were obtained using the QLF device. Subsequently, dental plaque was stained with a two-tone disclosing solution and a photograph was obtained with a digital single-lens reflex (DSLR) camera. The staining scores were assigned as follows: 0 for no staining, 1 for pink staining, and 2 for blue staining. The marked points on the DSLR images were selected for RGB color analysis. The relationship between dental plaque maturation and the red/green (R/G) ratio was evaluated using Spearman’s rank correlation. Additionally, different red fluorescence values according to dental plaque accumulation were assessed using one-way analysis of variance followed by Scheffe’s post-hoc test to identify statistically significant differences between the groups.
Results:
A comparison of the intensity of red fluorescence according to the maturation of the two-tone stained dental plaque confirmed that R/G ratio was higher in the QLF images with dental plaque maturation (p<0.001). Correlation analysis between the stained dental plaque and the red fluorescence intensity in the QLF image confirmed an excellent positive correlation (p<0.001).
Conclusion:
A new plaque scoring system can be developed based on the results of the present study. In addition, these study
results
may also help in dental plaque management in the clinical setting.
8.Minimal Change Disease and Focal Segmental Glomerulosclerosis in Identical Twin Brothers.

The simultaneous occurrence of primary glomerulonephritis in identical twins has been rarely reported previously. It has suggested that genetic factors may play an important role in the pathogenesis of primary glomerulonephritis. We describe a pair of 17-year-old identical twin brothers with asymptomatic proteinuria, one with histologically proven minimal change disease and the other with focal segmental glomerulosclerosis. HLA typing in twin brothers revealed an identical phenotype consisting of A25, A33, B44, B54, Cwl, Cw7, DR7 and DRB1. To our knowledge, this is the first case of glomerulonephritis in identical twins in Korea.

9.Clinical features of children with carbon monoxide intoxication: a single center study

Purpose:
To investigate the effect of lifestyle changes on patterns of carbon monoxide (CO) exposure and the association between neurologic symptoms and outcomes in Korean children with CO intoxication.
Methods:
We reviewed the medical records of patients (< 18 years) with CO intoxication who visited the emergency department of Pusan National University Hospital between February 2012 and January 2020. We collected clinical findings, including age and sex, transfer from other hospitals, source, time and duration of exposure, manifestations with neurologic symptoms (syncope, seizure, and altered mental status), intensive care unit hospitalization, hospital length of stay, implementation of hyperbaric oxygen therapy, and findings of neuroimaging. These variables were compared between children with and without neurologic symptoms. In addition, levels of carboxyhemoglobin and lactate were compared between patients with and without specific manifestations.
Results:
The enrolled 47 patients’ median age was 10 years (interquartile range, 4.5-14.0). The most common source of exposure was fire (46.8%), followed by camping (23.4%). The most common times of exposure were night (44.7%) and winter (44.7%). The patients with neurologic symptoms (14 [29.8%]) showed longer duration of exposure and hospital length of stay (P < 0.001 and P = 0.007, respectively). Of the 14 patients, 2 were hospitalized to the intensive care unit without an in-hospital mortality. A significant association was found between dyspnea and lactate level (P = 0.049), also between syncope or presyncope and carboxy hemoglobin level (P = 0.017).
Conclusion
CO intoxication in Korean children is most often caused by fire and camping, and at night and in winter. There is a correlation between neurologic symptoms and duration of exposure to CO.
10.Characteristics of children with trauma compared to those with disease in the emergency department: a Korean single regional emergency medical center study

Purpose:
Trauma is the leading cause of death and disability in children. We aimed to compare the clinical characteristics of children with trauma and disease.
Methods:
We reviewed the medical records of children (< 19 years) who visited the emergency department of Pusan National University Hospital from 2016 through 2018. Data on the age, age group, sex, details of trauma or disease, severe trauma or disease (Korean Triage Acuity Scale 1-2), hospitalization rate (overall and intensive care unit [ICU]), hospital length of stay, in-hospital mortality, and the Injury Severity Score were compared between the children with trauma and those with disease.
Results:
In a total of 10,205 children, 3,028 (29.7%) had trauma. The children with trauma were older than those with disease (median age, 78.5 months [interquartile range, 35.0-165.0] vs. 49.0 [16.0-120.0]; P < 0.001). Boys were more common in the former group than the latter (63.7% vs. 56.3%; P < 0.001). The most common injury mechanism was traffic accident (16.0%), followed by fall and foreign body. The overall hospitalization rate was higher in the children with disease (17.1% vs. 35.9%; P < 0.001). However, the children with trauma underwent more frequent ICU hospitalization, and showed higher in-hospital mortality rate and longer hospital length of stay than those with disease (all P < 0.001). The children with severe trauma showed higher median age, percentage of boys, in-hospital mortality, and ICU hospitalization rate, and longer hospital length of stay than those with severe disease (all P < 0.001).
Conclusion
Children with trauma tend to be older, and their condition may be more critical in severity than those with disease. This difference is more prominent in those with severe trauma or disease.
