1.Fibro-osseous Pseudotumor of the Digits: A case report .

Fibro-osseous pseudotumor of the digits is a heterotopic ossification closely related to myositis ossificans and occurs in the subcutaneous tissue of the digits. This lesion is considered a reactive fibroblastic proliferation with metaplastic bone formation. We report a case of fibro-osseous pseudotumor of left index finger in a 28-year-old woman. She had had an ovoid smooth subcutaneous mass with tenderness on the left index finger for one month. In gross, the specimen consisted of a relatively circumscribed, rubbery soft mass with grayish white cut surface measuring 2.0 1.7 1.5 cm. Upon microscopic examination the lesion showed irregular multinodular growth with considerably variable cellularity. Because of the focal hypercellularity, cellular atypia, and increased mitotic activity this lesion may be confused with extraskeletal osteosarcoma or parosteal osteosarcoma. This rare lesion is curable by complete local excision.

2.Detection of Mycobacterium tuberculosis DNA in Ethanol-Fixed and Papanicolaou Stained Archival Materials.

Granuloma is a chronic inflammatory process associated with non-infectious agents or infectious diseases such as tuberculosis. It is well known that AFB staining, which has been used to determine the etiology of the granulomatous inflammation, lacks both sensitivity and specificity. Due to the slow growth rate of most pathogenic mycobacteria, culturing of organisms can take up to eight weeks. It is not uncommon for specific therapy to be delayed, or for an inappropriate treatment be given to patients without mycobacterial infections or with infections caused by atypical mycobacteria. Determination of the causative agent in Papanicolaou stained cytology specimens gives pathologists even more difficulties when only necrotic material has been aspirated from the center of the granuloma. In recent years, the use of a polymerase chain reaction for the amplification of DNA has appeared promising in terms of speed, efficiency, sensitivity, and specificity. Since a polymerase chain reaction permits the sensitive genetic analysis of small amounts of tissue, it is ideally suited to the genetic analysis of cytologic specimens. A polymerase chain reaction is easily performed on unfixed and unstained cells, however, an analysis of ethanol fixed and Papanicolaou-stained archival smears has also been described. We have recently established a method to detect Mycobacterium tuberculosis organism by a nested polymerase chain reaction with primers in the insertion sequence IS 6110, using cellular digests of ethanol-fixed and Papanicolaou-stained archival specimens aspirated from the lymph nodes, lungs, thyroid, etc. Inhibitors present in Papanicolaou stained material was removed by destaining the slides with 0.5% HCl solution for 10-30 minutes. Eight out of ten cases which have shown the epithelioid granulomas revealed a positive reaction and four out of ten cases which have shown lymphohistiocytic cells in a necrotic background without any evidence of granuloma revealed a positive reaction. This study showed that it was possible to employ a polymerase chain reaction to detect Mycobacterium tuberculosis in Papanicolaou stained archival cytology specimens.

3.Radiologic findings of male breast cancer: two cases report.

Male breast cancer is very rare, with the incidence of 0.15-1% of all breast cancers and less than 1% of all cancers in men. The prognosis of male breast cancer is poorer than that of female because the median age of detection of the disease is in more late stage. It usually involves higher axillary lymph nodes. We report two cases of male breast cancer to describe characteristic mammographic and ultrasonographic findings and to compare with the findings of gynecomastia.

4.Development and Effects' Analysis of Nutrition Education Pamphlet for the Lower Grades Elementary Students -Focused on Individual Daily Needed Food Exchange Units-.

The purpose of this study was to investigate effects of nutrition education program and pamphlet for the lower grades elementary students focused on individual daily needed food exchange units using Food Exchange System. Program consisted of four lessons (40 min/lesson), "5 major nutrients & function", "6 food group and sources", "daily needed food exchange units for normal body weight", and "smart snack choice and exercise". Pamphlet as activity book was developed for the program. The subjects were 3rd grade elementary students (educated group, 31 vs. non-educated group, 31). Educated group were lessoned as group and/or individual. We examined the differences in nutrition knowledge, dietary attitudes, dietary intakes and satisfaction of the program and pamphlet. In educated group, there were positive improvements on nutrition knowledge score "function and foods of 5 nutrients" and on dietary attitudes "type of breakfast and snacks". In the evaluation of dietary intakes according to KDRI, there were positive improvements on intakes level of riboflavin, vit. C, folate, Ca, P, Fe and Zn in educated group. In satisfaction with the program and pamphlet, contents, font size, visual, figure, difficulty and program curriculum were over 2.90/3.0. It showed that the developed nutrition education program and pamphlet focused on individual daily needed food exchange units using Food Exchange System improved nutrition knowledge, dietary attitudes and nutrients intake level in the lower grades elementary students.

5.Incidence of Hypothermia and Factors Associated with Body Temperature Changes during Surgery in Burned Patients.

PURPOSE: The study aimed to evaluate the changes of body temperature and to identify the factors related to changes during surgery in burned patients. METHODS: A retrospective study was conducted by reviewing the medical records of 439 adult burned patients who had a surgery under general anesthesia at the Burn Center of a university hospital. RESULTS: After surgery, body temperature of the burned patients declined from 36.6℃ to 35.2℃; 52.2% were hypothermia. There were significant differences in the changes of body temperature according to the participants' characteristics including American society of anesthesiologists physical status, type of burn injury, total burn surface area, range of exposure, operation time, anesthesia time, amount of fluid, blood transfusion, use of tourniquet, and the method of warming therapy. Factors that influence the temperature changes were total burn surface area (β=0.26), operation time (β=0.25), amount of fluid (0.20), and warming therapy including ‘Room temperature setting + Heated circuit + Hot line’(β=0.09) and ‘Room temperature setting+one of others’(β=0.08). CONCLUSION: Burned patients experienced a decrease of their body temperature during surgery despite of warming therapy. A nursing protocol is needed to provide an appropriate warming therapy based on their characteristics in burned patients.

6.Cost-Effectiveness of Denosumab for Post-Menopausal Osteoporosis in South Korea.

BACKGROUND: In South Korea, 22.3% of women ≥50 years of age and 37% of women ≥70 years of age visit the doctor to obtain treatment for osteoporosis. According to the analysis of the National Health Insurance Services claim data between 2008 and 2012, the number and incidence of hip and vertebral fractures increased during the same period. Denosumab, a newly marketed medicine in Korea, is the first RANK inhibitor. METHODS: A cost-utility analysis was conducted from a societal perspective to prove the superiority of denosumab to alendronate. A Markov cohort model was used to investigate the cost-effectiveness of denosumab. A 6-month cycle length was used in the model, and all patients were individually followed up through the model, from their age at treatment initiation to their time of death or until 100 years of age. The model consisted of eight health states: well; hip fracture; vertebral fracture; wrist fracture; other osteoporotic fracture; post-hip fracture; post-vertebral fracture; and dead. All patients began in the well-health state. In this model, 5% discounted rate, two-year maximum offset time, and persistence were adopted. RESULTS: The total lifetime costs for alendronate and denosumab were USD 5,587 and USD 6,534, respectively. The incremental costeffectiveness ratio (ICER) for denosumab versus alendronate was USD 20,600/QALY. Given the ICER threshold in Korea, the results indicated that denosumab was remarkably superior to alendronate. CONCLUSION: Denosumab is a cost-effective alternative to the oral anti-osteoporotic treatment, alendronate, in South Korea.

Purpose:
A case of optic tract injury caused by subarachnoid hemorrhage in the perimesencephalic cistern is reported.Case summary: A 26-year-old female with a history of traumatic subdural hemorrhage along the bilateral frontal convexity presented with a right visual field defect. Her visual acuity was 1.0 in both eyes. Slit-lamp examination and fundus examination were not remarkable. There was a relative afferent pupillary defect in the right eye. A Humphrey visual field test revealed right homonymous hemianopsia. Optical coherence tomography showed left homonymous hemiatrophy of the ganglion cell-inner plexiform layer in both eyes. Magnetic resonance imaging was performed. Susceptibility-weighted images showed minimal subarachnoid hemorrhage in the left perimesencephalic cistern, and the patient was confirmed to have optic tract injury due to subarachnoid hemorrhage.
Conclusions
We report a case of optic tract injury in a 26-year-old patient caused by subarachnoid hemorrhage in the perimesencephalic cistern, which was confirmed by susceptibility-weighted images.

9.SLC9A6 -related developmental and epileptic encephalopathy with spike-and-wave activation in sleep: A case report

The gene encoding solute carrier family 9 member 6 (SLC9A6) on Xq26.3 is associated with Christianson syndrome (CS) mimicking Angelman syndrome. In CS, developmental and epileptic encephalopathy (DEE) appears in about 20%, and DEE with spike-and-wave activation in sleep (SWAS) is reported only in several cases. A 10-year-old boy with DEE showed multidrug resistant focal seizures from 6 months of age. He had progressive microcephaly, regression, global developmental delay without speech, hyperkinesia, and truncal ataxia; he had a long thin face, esotropia, and happy demeanor. Brain magnetic resonance imaging demonstrated cerebellar atrophy. Electroencephalogram at 7.5 years of age showed nearly continuous diffuse paroxysms in slow wave sleep. The seizures were responsive to corticosteroids for a while. Trio whole exome sequencing exhibited a likely pathogenic variant of SLC9A6 in the proband and his asymptomatic mother: c.1194dup (p.Leu399AlafsTer12).This is a rare case report of CS with DEE-SWAS in a Korean patient.
10.Mediating effect of moral sensitivity on the relationship between nursing professionalism and ethical decision-making confidence in nursing students

Purpose:
The purpose of this study is to confirm the relationship between nursing students’ nursing professionalism and ethical decision-making confidence, and to clarify the mediating effect of moral sensitivity which has a mutual influence on nursing professionalism and ethical decision-making confidence.
Methods:
The data collection for this study was conducted from January 23 to February 1, 2021 with junior and senior nursing students enrolled in five nursing departments in two regions. The data collected in this study were analyzed using the IBM SPSS statistics version 23.0 program.
Results:
Through this study, it was found that nursing professionalism (β=.47) and moral sensitivity (β=.32) acted as important influencing factors on the ethical decision-making confidence of nursing students, with an explanatory power of 31.2%. In addition, it was confirmed that there was a partial mediating effect of moral sensitivity in the relationship between the nursing professionalism and ethical decision-making confidence of nursing students, and the mediating effect coefficient of moral sensitivity was .16.
Conclusion
Based on the results of the study, it is suggested to prepare a plan to increase ethical decision-making confidence and moral sensitivity when planning interventions to improve nursing professionalism in nursing students.
