In case of submucosal tumor, adenoma and atypism, we can't frequently use the snare polypectomy for treatment of them. Adenoma and atypism are premalignant, so other therapeutic modalities are necessary for resection of them. We have perfortned endoscopic mueosectomy such as strip biopsy, and "O"ring ligation on various mucosal lesions of upper GI tract in 23 patients(11 men, l2 women) from June 1993 to December l994. Fourteen patients wbo had adenoma were followed up endoscopically for 6 months to 24 months(mean: 14 months). The results were as follows: 23 patients(27 lesions) were enrolled in this study. Mean age was 50.3(range 32-74 years). The method of mucosectomy was strip biopsy in 19 cases(21 lesions), and "O"ring ligation in 4 cases(6 lesions). The rate of complete resection was 88.9%. If adenomas were resected incompletely, we added a laser therapy. There was no recurrence except a case in which carcinoma was detected on the other site during follow-up. No serious compication occurred such as massive bleeding or perforation. Based on the above results, the endoscopic mucosectomy is considered to be a safe and effective modality in the treatment of the flat polyps of upper GI tract and could be used for prevention of gastric cancer.

The purpose of this study was to evaluate the effects of caffeine and calcium on the activities of the osteoblastic cell from mouse calvaria. The author cultured osteoblastic cells obtained from the mouse calvaria and were divided into three groups: the caffeine-treated, the calcium-treated and the combine-treated group. In caffeine-treated group, the cell toxicity was measured by MTT assay at 1, 2 and 4 days after treatment of caffeine. In all groups, the densities of the mineralized bone nodules were measured by imaging analyzer after Von Kossa staining. The alkaline phosphotase (ALP) activities were measured at 2, 7, 14, 21 and 28 days and the interleukin-1beta activities at 48 hours after treatment of caffeine and calcium. The measurements were statistically executed with ANOVA test and the results were as follows. 1. The cellular toxicity of the caffeine increased with the concentration of caffeine during the incubation period. 2. The maximum densities of mineralization were observed at 0.2 mM caffeine-treated group, 1.2 mM calcium-treated group, 0.1 mM caffeine and 1.8 mM calcium-treated group. 3. The activities of ALP were peaked at 14 days at calcium-treated group as no-treated. But, the activities of ALP increased with concentrations of caffeine at caffeine-treated group. At combine-treated group, the act of ALP were peaked at 24 days at 1.2 mM, 1.8 mM calcium-treated group, But decreased at 2.5 mM calcium-treated group. 4. The activites of the IL-1beta were increased significantly at 0.2 mM caffeine-treated group, 1.8 mM calcium-treated group and 0.1 mM caffeine and 1.8 mM calcium-treated group. But, they were decreased at all groups of high concentration.

PURPOSE: The purpose of this study was to identify relationships between violence response, coping, and burnout among emergency department nurses. METHOD: This descriptive study using self report questionnaires was conducted from October 1 to October 20, 2008. Participants were 147 general nurses working in emergency departments in hospitals having over 500 beds and located in Busan. ANOVA and Pearson's correlation coefficients with SPSS PC+ WIN 14.0. were used to analyze the data. RESULTS: Scores for violence response, coping, and burnout were 3.08 (1-5 scale), 2.57 (1-4 scale), and 3.22 (1-5 scale) respectively. There were significant differences in violence response and burnout by education, clinical careers, clinical careers in ED, and plans to work in the future, and no significant differences in coping by general characteristics. There was a significant and highly positive relationship between violence response and burnout. CONCLUSION: The degree of violence response and burnout was comparatively high, while that of coping was average. Therefore, it is necessary to develop effective programs related to prevention and coping with violence for nurses in emergency departments. Institutional measures by hospital authorities are also needed to improve the nursing quality by providing a safe workplace.

Purpose: This study aims to identify the factors that influence the experience of falls among older adults living in the community. Methods: The study participants were 70,887 65-year-olds who participated in the 2021 Community Health Survey. The study employed the Rao-scott x 2 test to examine the variation in fall experiences based on the characteristics of the older adults. Multiple logistic regression analysis was conducted to investigate these characteristics’ impact on older adults’ fall experiences. Results: The proportion of subjects in fall experience was 16.6%. The factors influencing the subject's fall experience were sex (odds ratio [OR]=1.47, 95% confidence interval [CI]=1.37~1.57), age (OR=1.48, 95% CI=1.34~1.65), family structure (OR=1.23, 95% CI=1.15~1.31), body mass index (OR=1.13, 95% CI=1.06~1.20), diabetes (OR=1.12, 95% CI=1.06~1.20), depression experiences (OR=1.56, 95% CI=1.42~1.70), stress (OR=1.12, 95% CI=1.05~1.19), subjective health status (OR=1.77, 95% CI=1.63~1.92), life satisfaction (OR=1.57, 95% CI=1.41~1.76), and chewing discomfort (OR=1.29, 95% CI=1.21~1.38). Conclusion Efforts should be made to effectively educate and develop various programs aimed at reducing falls among older adults. It is essential to emphasize the importance of continuous and active attention to falls in the older adult population.

Orchids live with mycorrhizal fungi in mutualism. This symbiotic relationship plays an essential role in the overall life cycle of orchids from germination, growth, settlement, and reproduction. Among the 1000 species of the orchid, the Korean lady’s slipper, Cypripedium japonicum, is known as an endangered species. Currently, only five natural habitats of the Korean lady’s slipper remain in South Korea, and the population of Korean lady’s slipper in their natural habitat is not increasing. To prevent extinction, this study was designed to understand the fungal community interacting in the rhizosphere of the Korean lady’s slipper living in the native and artificial habitats. In-depth analyses were performed to discover the vital mycorrhizal fungi contributing to habitat expansion and cultivation of the endangered orchid species. Our results suggested that Lycoperdon nigrescens contributed most to the increase in natural habitats and Russula violeipes as a characteristic of successful cultivation. And the fungi that helped L. nigrescens and R. violeipes to fit into the rhizosphere community in Korean lady’s slipper native place were Paraboeremia selaginellae and Metarhizium anisopliae, respectively. The findings will contribute to restoring and maintaining the endangered orchid population in natural habitats.

Atopic dermatitis and psoriasis are common, chronic inflammatory skin diseases. They are mediated by different subsets of Th cells (Th1 vs. Th2), so they are believed to be mutually exclusive. However, they have many similar features. Both show epidermal hyperplasia, aberrant immunity, and skin barrier anomalies. We describe the case of an 11-year-old girl with concomitant psoriasis and atopic eczema. She had atopic dermatitis since infancy and had developed psoriatic plaques a few years later. She was diagnosed with atopic dermatitis and psoriasis, and was treated with cyclosporine. Here, we present this case, with a literature review.

BACKGROUND: Allergic skin diseases such as atopic dermatitis and urticaria are known to be mediated by IgE. It is important to confirm a causative allergen for diagnosis and treatment. The multiple allergosorbent test (MAST) is a simple method for simultaneously measuring total and allergen-specific IgE. OBJECTIVE: The purpose of this study was to analyze results from the MAST, which measures total and specific IgE, and to compare the different results. METHODS: We reviewed the MAST results of 270 allergic disease patients tested between June 2007 and May 2012. RESULTS: There were statistical differences in total IgE production and in positive sensitization to specific allergens between the disease groups. The level of total IgE and positive rates of specific IgE were highest in atopic dermatitis patients, followed by urticaria, allergic contact dermatitis, and pruritus patients. Atopic dermatitis patients had significantly more allergens than those with other diseases. There were no statistical differences in total IgE level, rates of positive sensitization to specific allergens, and the number of causative allergens between the patients with acute and chronic urticaria. CONCLUSION: Each disease showed a different IgE pattern. Atopic dermatitis showed the highest level of serum IgE. There were no differences in IgE levels between acute and chronic urticaria patients. We identified an increase in IgE level in allergic contact dermatitis patients. Further study is needed to determine whether these patterns could be useful in diagnosis and choice of treatment methods.
