1.Analysis of nucleotides and their derivatives in renal tissue of rat during ischemia by HPLC.

In rat kidney, the changes in concentrations of nucleotides and their derivatives during ischemia induced by renal artery ligation was measured quantitatively with high performance liquid chromatography (HPLC). After the ligation of renal artery for 60minutes, the concentrations of the nucleotides and derivatives to 80.7±18.39 µg (p<0.01); ATP, 307.2±56.68 µg to 47.6±5.95µg (p<0.01); ADP+AMP, 227.1±7.98 µg to 61.4±3.92 µg (P<0.01); NAD+, 217.9±4.49 µg to 126.6±10.44 µg (P<0.01); GTP, 202.5±23.76 µg to 117.7±14.24 µg (P<0.05); GMP, 54.5±9.03µg to 23.7±0.46 µg (p<0.05), and inosine, 16.6±3.45 µg to 7.8±0.87 µg (P<0.05). But hypoxanthine and xanthine were significantly increased from 113.0±15.58µg to 159.7±12.97µg (P<0.05) and from 87.7±6.77µg to 173.1±12.52µg (P<0.01). In ischemic kidney, concentration of ATP was decreased to 39.9% of control at 10 minutes, 19.8% at 30 minutes, and 15.5% at 60 minutes, and ADP+AMP were decreased to 70.3% of control at 10 minutes, 67.3% at 30 minutes, and to 27.0% at 60 minutes, but hypoxanthine and xanthine were increased to 121.5% and 127.1% at 10 minutes, 126.0% and 174.4% at 30 minutes, and 141.4% and 197.3% at 60 minutes. Total adenosine nucleotides were decreased to 20.3% of control during 60 minutes of ischemia, but hypoxanthine and xanthine were increased to 157.5% of control. These results suggest that the changes in the concentration of nucleotides and their metabolic derivatives are useful indices of the extents of tissue ischemia in rat kidney.

4.Accessory Hepatic Nodules: Histopathologic analysis of three cases.

This report deals with three cases of accessory hepatic nodules incidentally noted during operation. Two of them were found on the serosal surface of the gallbladder with no connection to the main body of the liver. The other case was in the greater omentum. All three cases were small oval shaped, measuring less than 1 cm in maximum dimension and were composed of histologically normal hepatic tissue and seemed to receive blood supply from the adjacent tissue through the capsular blood vessels. Presence of the fairly well retained intralobular mesenchymal component may reflect that accessory hepatic nodules develop after conjugation of hepatic diverticulum and septum transversum.
5.Ultrastructural Observations on Human Primary Hepatocellular Carcinomas: Analysis of 35 Lobectomy Specimens.

The etiologic impacts in primary hepatocellular carcinoma among Koreans seem different from those in other countries with its high incidence and close association of hepatitis B virus infection and liver cirrhosis. A series of 35 lobectomy specimens of hepatocellular carcinoma (HCC) was examined by means of electron microscopy to elucidate the general ultrastructural characteristics and to understand the morphogenesis of various histological growth patterns and cytologic features of HCC. 1) General cytological details of HCC were similar to those of non-neoplastic hepatocytes, but characterized by scantiness of subcellular organelles. Degree of cellular differentiation was not correlated with ultrastructural features of HCC. 2) Acinar pattern of HCC seemed to develop by either dilatation of central bile canaliculus or central cystic degeneration of microtrabecular growth, and clear cell group of HCC was expressed in abundance of glycogen particles and lipid droplets. 3) Intranuclear inclusions of HCC proved to be cytoplasmic herniations of tumor cells, and intracytoplasmic tubular arrays appeared to originate from the endoplasmic reticulum. 4) Hyaline globules seen in HCC corresponded to clumps of microfilamentous structures similar or identical to Mallory's hyalin.

6.Promoting Effect of Aflatoxin B1 and D-Galactosamine on Development of Glutathione S-Transferase Positive Foci in Diethylnitrosamine-initiated Rat Liver.

The enhancing potential of anatoxin a (AFB1) and D-galactosamine (DGA) on development of preneoplastic glutathione S-transferase placental form positive (GST-P+) hepatic foci was examined using an in vivo mid-term assay system based on two-stage concept of hepatocarci-nogenesis. Rats were initially given a single dose (200 mg/kg) of diethylnitrosamine (DEN) intraperi-toneally, and thereafter. with an interval of 2 weeks, AFBl at a graded concentration (0.06, 0.012, 0.0024, 0.00048, and 0.000096 mg/kg i.g.) and DGA (100 mg/kg i.p.) were administered for 6 weeks and then sacrificed. All rats were subjected to a two-thirds partial hepatectomy to induce a potent growth stimulus to DEN-altered hepatocytes at the week 3. The modifying potential was scored by comparing the number and the area (mm2) per cm2 of GST-P+ foci in the liver with those of the corresponding control group given DEN alone. AFBl (at a graded concentration between 96 ng/kg and 60 microgram/kg) exerted a strong promoting effect oil induction of GST-P+ foci with both the number and the area. The logarithmic dose of AFBl and the potency to promote hepatocarcinogenesis were in dose-dependent relationship. DGA, a known necrogenic chemical to cause periportal necrosis and stimulate hepatocellular proliferation. also revealed the increase in the area of GST-P+ foci. although its enhancing potentia1 was 1ess profound than that of AFBl. The results suggest that DGA is also a useful proliferative stimulus m improve the medium-termdetection of unknown carcinogens.

7.ene Expression of Enzymes Related to Glutathione Metabolism in Anticancer Drug-resistant L1210 Sublines.

Glutathione(GSH) has a very important role in detoxification of cells and is closely related to antitumor drug-resistance of cancer cells. In order to evaluate the importance of glutathione metabolism in the drug-resistant cancer cells, the concentration of celluar GSH and activities of y-glutamylcysteine synthetase(GCS), y-glutamyl transpeptidase (GGT) and glutathione S-transferases(GST) in the adriamycin, vincristine, or cisplatin resistant L1210 (L1210AdR; L1210VcR, or L12100s) sublines were measured. Expression and amplification of GCS, GGT, and GST-i7 genes were also observed in the parent L1210 and the drug-resistant L1210 sublines. The concentration of GSH was increased 5.34 fold in L12100s, 2.83 fold in L1210VcR, and 1.78 fo-d in L1210AdR, compared to L1210. The activities of GCS and GGT were -increased in drug-resistant L1210 sublines. The GST activity was increased in L1210VcR and L1210Cis but decreased in L1210AdR compared to L1210. Expression of GCS, GGT, and GST-rr genes were increased in the resistant L1210 sublines compare to the parent L1210 in northern blot analyses. Overexpression of GCS, GGT, and GST-77 were observed in the resistant sublines, and the increases of the concentration of glutathione and the activities of GCS and GGT in the resistant sublines may be involved in a part of the drug-resistance in the resistant sublines.

8.Endometrial Carcinoma Associated with Stein-Leventhal Syndrome: Two cases report.

Stein-Leventhal syndrome is a clinicopathological entity characterized by chronic anovulation and sclerotic ovaries. Clinically, the affected patients typically present their third decade with a history of premenarchal obesity and postmenarchal onset of oligomenorrhea or amenorrhea, infertility, and clinical evidence of increased androgen production. In some patients, there are estrogenic phenomena such as menometrorrhagia with or without endometrial hyperplasia or carcinoma. Several studies have indicated that there may be an increased incidence of ovarian neoplasia in polycystic ovaries. Recently, we experienced two cases of polycystic ovaries assotiated with endometrial carcinoma in premenopausal women, who had clinical characteristics of Stein-Leventhal syndrome; one of the cases had additional serous cystandenofibroma.

9.Decubitoma: A Pseudosarcoma in Decubitus: Report of a case.

Decubitus ulcer is often seen in the skin and underlying tissue of debilitated or immobilized patients as the result of prolonged pressure and impaired circulation. It manifests chiefly as an ulcer over bony prominences, but tumefaction is an extremely unusual presentation. A 53-year old male, a paralytic of the lower extremity for 18 years, developed a recurrent decubital ulcer despite repeated surgical repair, from which a rapidly growing, large fungating mass grew within a month. The last resected mass was bosselated and measured 15x9x3 cm with a major area of cicatrix-like induration, interdigitated with skeletal muscle bundles at the central area. Microscopically, the mass was composed of an upper half of active granulation tissue layer and a deeper half of dense, poorly cellular, fibrocollagenous bundles admixed with florid proliferation of atypical fibroblasts, but the absence of mitosis and the multifocal admixture of active inflammatory process-granulation tissue formation seemed to help exclude genuine fibromatosis, nodular fasciitis or proliferative myositis. We assume that this rapidly growing pseudofibromatosis is an additional manifestation of a prolonged decubitus ulcer, possibly related to the modified reparative process of decubitus ulcer following repeated excisions, for which we propose a term of decubitoma.
10.An Application of Immunohistochemical Study of Cytokeratin in Tumor Diagnosis.

Cytokeratins are a family of polypeptides of intermediate filaments which in diverse epithelia are expressed in diffeent, yet specific combinations. To evaluate the diagnostic value of keratin, immunohistochemical staining was done in formalin-fixed, paraffin-embedded normal and neoplastic tissues by PAP and StreptABC methods. The antiserum for cytokeratin in monoclonal antibody which gives the specificity for 40, 46, 50, 52, 56, 58, and 65-67 Kd keratin classes. The results are as follows: 1) The staining was positive for cytokeratin in all of the squamous epithelium, ductular epithelial cells of various glands, respiratory and urinary tract epithelium, and mesothelial cells. 2) No staining for cytokeratin was ovserved in respiratory alveolar epithelium, acinar cells of various glands, renal glomeruli, hepatocytes, and many mesoderm-derived tissues such as muscle, hematopoieitc and lymphoid tissues, nerve, bone, cartilage, and fibroblasts. 3) Squamous cell carcinomas, transitional cell carcinomas, mesotheliomas, and some of the adenocarcinomas (stomach, colon, uterine cervix, biliary tract and breast) exhibited positive staining for cytokeratin. Epithelial cells of thymoma, adenomatoid tumor, plemorphic adenoma of salivary gland, papillary carcinoma of thyroid, lymphoepithelioma, and craniopharyngioma were also positive. 4) Some of the adenocarcinomas (prostate and pancreas), renal cell carcinoma, ovarian stromal and germ cell tumors, hepatocellular carcinoma, malignant melanoma, and mesoderm-derived tumors including malignant lymphoma were uniformly negative for staining. 5) From the above results, the immunohistochemical study in paraffin-embedded tissues using monoclonal antibody for cyto keratin may be useful to differentiate various tumors, especially in differential of hepatocellular carcinoma from bile duct adenocarcinoma, lymphoepithelioma and other undifferentiated carcinomas from lymphoma, thymoma from lymphoma, and squamous cell carcinoma from melanoma. It will be helpful in the diagnosis of transitional cell carcinoma in which the differentiation from renal cell carcinoma and prostatic adenocarcinoma be difficult.
