1.A Case of Ocular Rosacea.

Ocular rosacea is the eye manifestations of rosacea, which involves the lids, conjunctiva, and cornea. A recent study revealed that all patients with cutaneous rosacea had some degree of ocular involvement which is often undiagnosed. It is difficult to make a diagnosis of ocular rosacea because it lacks clinical or pathological hallmarks. However, early diagnosis and treatment are emphasized to decrease morbidity and prevent potential blindness. We report a case of ocular rosacea in a 49 year-old man who showed blepharoconjunctivitis, keratitis, and cutaneous manifestations of persistent erythema, telangiectasia, and papules. The ocular symptoms and signs had improved significantly with tetracycline therapy.

2.A Case of Perioral Dermatitis Involving the Eyelids.

Perioral dermatitis is a facial dermatoses affecting the nasolabial folds, the chin, and the upper lip with sparing of a rim of skin at the vermilion border. The classic distribution is of the perioral area, but the glabella, eyelids and even forehead may be affected. In the patients with the periocular variety, the history, course, and response to treatment are exactly the same as in those with classic perioral dermatits. Herein, we report a case of perioral dermatitis involving the eyelids in a 23-year-old man who had used topical corticosteroids but experienced symptom aggravation and improvement after doxycycline treatment.

3.Correlation between Helicobacter pylori Infection and Lymphoid Follicle Formation in Gastrectomy Specimens.

Histopathologic studies for Helicobacter pylori (H. pylori)-associated chronic gastritis have been mostly undertaken with endoscopic biopsy specimens, often leading to an inappropriate evaluation of the gastric mucosal alterations. The purpose of this paper was designed to investigate the actual prevalence of lymphoid follicle formation by H. pylori infection using the resected stomachs. A total of 16 fresh gastrectomy specimens bearing gastric carcinoma were examined under the quick and gentle procedure, with which H. pylori was detected in 12 cases (75%) and lymphoid follicles in 14 cases (87.5%), while the detection rate of H. pylori remained 56.3% in the control group which comprised the same 16 resected stomachs and were examined by routine tissue preparation procedure without any special care. There was a significant correlation between the presence of H. pylori and lymphoid follicle formation (p=0.05), but no correlation was found between the grades of H. pylori and lymphoid follicles. The topographical distribution of H. pylori or lymphoid follicles in antrum and body gave no statistical difference. Similarly, there was no correlation between H. pylori infection and intestinal metaplasia, activity of chronic gastritis or histologic types of accompanying adenocarcinoma. We conclude that studies of the gastric mucosal change by H. pylori infection using the gastrectomy specimens provide a useful information for analysis of lymphoid follicle formation which is a consistent morphological characteristic of H. pylori infection.

5.Long-term Follow-up of Children with Idiopathic Hypercalciuria.

Malignant fibrous histiocytoma is the most common soft tissue sarcoma of late adult life, but it is rare in the pleura. We recently experienced a case of cytology of malignant fibrous histiocytoma in pleural fluid. The smear revealed histiocytic malignant cells, spindle malignant cells and inflammatory cells. The histiocytic cells showed abundant, pale cytoplasm and ovoid, irregular, eccentrically-placed nuclei. The spindle cells showed elongated nuclei. Intercellular collagen was also present.

6.Cytomegalovirus Infection in Idiopathic Inflammatory Bowel Disease: Clinicopathologic Analysis of 6 Cases.

Cytomegalovirus (CMV) infection is an uncommon association with idiopathic inflammatory bowel disease (IBD) often leading to a variety of serious complications. A total of 41 resected cases of IBD were examined to elucidate the pathologic features of intestinal CMV infection which was assessed by histologic examination and confirmed by immunohistochemistry with CMV antibody. Six cases were positve for CMV antibody; five cases in 19 ulcerative colitis (UC, 26.3%) and one case in 22 Crohn's disease (CD, 4.5%). Of 7 cases of the steroid-treated UC group, five cases were superinfected with CMV (71.4%) but none in 12 cases of the steroid-untreated UC group. All of the five CMV-positive cases in UC showed deep ulceration and transmural inflammation, while none of 10 UC cases without above features were CMV positive. Fibrinoid necrosis and thrombi were found in 83.3% of the CMV infected group, while none in the CMV-negative group of UC cases (p=0.01). We conclude that IBD, particularly UC, is susceptible to the CMV infection when steroid hormone is administered, and that deep colonic ulceration, transmural inflammation and fibrinoid necrosis of vasculature may suggest superinfection of CMV in UC patients. It seems that deep colonic ulceration may be the consequence of an ischemic change following vascular luminal occlusion or vasculitis by CMV infection.

7.Congenital Anomaly of Urinary Tract in Children.

Giant cell tumor (GCT) occurs very unusually in the rib (less than 1% of GCT). We present the cytologic features of GCT of the rib. It showed multiple cellular clusters composed of characteristic, benign looking osteoclast-like multinucleated giant cells and fibroblast-like mononuclear cells. The multinucleated giant cells contained numerous nuclei (average, 30 to 40 per cell, which were closely packed. The nuclei in giant cells were remarkably uniform and round to oval. The mononuclear, neoplastic stromal cells were elongated and spindle-shaped, There was no cytologically malignant portion in the tumor.

8.A Case of Atypical Hemolytic Uremic Syndrome Associated with Invasive Streptococcus pneumoniae Infection.

The recently proposed Bethesda system for cervical/vaginal cytology has made a standardization related to "atypia". In cellular changes due to inflammation or repair, the word "benign cellular change" has been suggested as a substitute for atypia. Terminology related to atypical cells may become standardized, but the cytologic criteria has not been well defined yet. We evaluated 160 cases of atypical squamous cells of undetermined significance(ASCUS) by the Bethesda System(TBS). Among 30,428 cases screened, a cytologic diagnosis of ASCUS was made in 498 cases(1.6%) and 160 cases were histologically verified. ASCUS was diagnosed based on nuclear enlargement and nonclassical signs of condyloma. The results are as follows: One hundred and twenty three cases(76.9%) revealed chronic cervicitis. Thirty seven cases(23.1%) demonstrated squamous intraepithelial lesion. Among intraepithelial lesions, condyloma and mild dysplasia were 28 cases(75.7%). Moderate and severe dysplasia were 5 cases(13.5%) and 4 cases(10.8%), respectively. It is concluded that patients with ASCUS should be colposcopically examined.

10.Effects of Nail Lacquer on Transonychial Water Loss.

BACKGROUND: Nail lacquer strengthens and protects the nail plate by preventing excess loss of moisture and providing a hard surface. On the other hand, it may produce allergic contact and irritant dermatitis, paronychial and nail bed inflammation and infection, dystrophic nails and onycholysis. OBJECTIVE: The purpose of study was to investigate the effect of the nail lacquer on the barrier function of the nail. METHODS: The study populatian consisted of 25 healthy young subjects who had not applied the nail lacquer to the nail for the last 3 months. Nail lacquer was applied to the left thumb and middle fingernail for 6 weeks and then removed by acetone. Transonychial water loss(TOWL) of the treated left fingernails and the untreated corresponding right fingernails was measured with an Evaporimeter. Lipids of the nails were extracted from distal nail cuttings and analysed by thin layer chromatography. RESULTS: 1. Before application of the nail lacquer, there was no difference in TOWL between the left and right fingernails while the TOWL of the thumb nail was higher than that of the middle fingernail with a statistical significance(p<0.05). 2. The TOWL of the left fingernails, measured 1 day, 2, 4 and 6 weeks after application of the nail lacquer, was lower than that of the untreated right fingernails with a statistical significance( p< 0.05). 3. The TOWL of the left fingernails, measured 1, 3 and 7 days after removal of the nail lacquer by acetone, was not different statistically from that of the untreated right fingernails. 4. Lipid analysis demonstratcd that the cholesterol content was higher in the left fingernail which had been covered with nail lacquer than in the untreated right fingernail. 5. There was no clinical adverse effect observed during application of the nail lacquer or after removal of it. CONCLUSION: According to the results of the study, nail lacquer has some occlusive effects on transonychial water evaporatior . Because increased cholesterol content may be due to perturbations of barrier function , further studies of the effect of the nail lacquer on the barrier function of the nail will be needed.
