1.A Study of Vascular Changes in the Lymph Nodes.

A retrospective morphological study was conducted on 1677 lymph nodes which were removed surgically under the diagnosis of gastric carcinoma, breast carcinoma, colonic carcinoma, chronic gastric ulcer, and superficial lymph nodes nuder the diagnosis of reactive follicular hyperplasia at Chosun University Hospital during a period of 5 years from 1980 to 1984. The hematoxylin and eosin stained sections were examined to evaluate for any vascular changes. In selected cases further levels were prepared from the paraffin blocks for Gomori's reticulin stain. The vascular abnormalities were divided into 3 groups, such as hemangiomatoid, pan-nodal vasodilatation and miscellaneous types. The miscellaneous type included the proliferation of smooth muscle which encircled numerous small hilar arteries and veins. The results obtained were as follows: 1) The hemangiomatoid lesions were localized lesions which appeared to be associated with local malignancy, but infrequent in incidence. 2) A more frequent vascular abnormality was pan-nodal vasodilatation, which tend to affect many nodes in a single group and is relatively associated with chronic gastric ulcer. 3) Miscellaneous lesion was nonspecific and appeared both local malignancy and chronic inflammation. 4) All three vascular reactions were somewhat more frequent in female than male.

3.Evaluation of Dermal Measurement for Workers in Benzidine Dihydrochloride and Benzidine Based Dye Manufacturing Factory.

This study is performed to evaluate usefulness of dermal measurement of benzidine and benzidine based dye as one of the occupational exposure assessment method for these compounds. We selected one benzidine manufacturing factory and one dye manufacturing factory in Incheon area. Eleven workers were for benzidine manufacturing factory and twenty four for dye. We analyzed relationships among air level, amount on skin and concentration of urinary metabolites for these compounds. Airborne levels of benzidine and dye were measured by NIOSH 5509, 5013 methods. Amount of these compounds on skin was measured with skin wipe method. Concentration of benzidine metabolites in urine was measured by High Performance Liquid Chromatography after alkaline hydrolysis. The amount of benzidine on hand skin was 25.05( - 233.2) ng/ur, and the amount of the neck was 2.01 ( - 11.9) ng/cm2 in the benzidine dihydrochloride manufacturing factory. The amount of benzidine on hand and neck skin has positive correlation with concentration of urinary monoacetyl benzidine (r=0.644, p < 0.05) . The amount of benzidine based dye on hand skin was 55.75( - 457.7) ng/cm2, and the amount of the neck skin was 18( - 284.7) ng/cm in benzidine based dye manufacturing factory The amount of dye on hand and neck skin has positive correlation with concentration of urinary benzidine for dye workers (r=0.467, p < 0.05). When assessing the exposure of workers who deal with benzidine, the amount of benzidine on skin should be measured for an accurate exposure assessment.

4.A clinical study of histiocytosis syndrome.

We made a clinical study on 10 cases of histiocytosis syndrome who had been admitted to the pediatric department of Soon Chun Hyang University Hospital from Jan. 1982 to Dec. 1991. The results were obtained as follows 1) The sex incidence revealed male predominance with the ratio 4:1. 2) Among 10 cases, 4 cases were classified as eosinophilic granuloma, 1 case as Letterer-Siwe disease, 1 case as linfection associated hemophagocytic syndrome and 4 cases as malignant histiocytosis. 3) The mean age of symptom onset was 4 and 7/12 years in all disease group. 4) The common clinical symptoms and signs at dignosis were dyspnea, mass, pain on lower extremities and fever. 5) The most common organ involved among 9 organ systems was liver-spleen and the number of organ systems involved were 1 in 3 2 ases(30%), 3 in 2 cases(20%), 8 in 2 cases (20%) and 6, 7 and 10 organs in each 1 case. 6) The abnormal hematologic findings (Hb; 10g/dl and/or WBC; 4,000/mm3 and/or PLT; 100,000/mm3) were found in 6 cases. 7) Low serum albumin was found in 2 cases. Results of blood culture were Pseudomonas aeruginosae and Salmonella typhi in each 1 case. 8) The common findings on tissue biopsy were histiocytic proliferation and infiltration. 9) 4 patients of MH who recieved chemotherapy, a combination of adriamycin, vincristine, cyclophosphamide and prednisone were given in a total of four courses every 2 weeks ad induction therapy. When complete response was attained, a combination of adriamycin, vincristine, prednisone (AOP) and cyclophosphamide, vincristine, prednisone(COP) was administered alternately every 4 weeks as maintenance therapy for 6-64 months. 10) Among 4 patiens of MH who recieved chemotherapy, 1 patient was lost during induction chemotherapy, for 1 day.2 patients expired during induction chemotherapy, for 1 month, 1 patient expired during maintenance chemotherapty, for 8months, Eosinophilic granuloma cases (3) were recieved currettage and no recurrence. IAHS case due to typhoid fever was improved spontaneously.

5.Normal Values of rNeonatal Periumbilical Skin Length.

Malformations of umbilicus are a feature of many dysmorphic syndromes and the measurement of periumbilical skin length should be considered as the basic step in the description of the umbilical malformations. So, we measured the periumbilical skin length in the 103 normal neonates and obtained the following results. 1) The means (+SD) of the periumbilical skin length were the 11.7mm (+3.0) in the cranial site and the 8.2mm (+2.5) in the caudal site, and the cranial length was 3.5mm longer than the caudal length. 2) No difference was observed between the male and female groups in the periumbilical skin length. 3) Age was the only statistically significant variable associated with the periumbilical skin length and it was observed that the periumbilical skin length decreased according to the advance of age (hour).

6.Diffuse Neonatal Hemangiomatosis Successfully Treated with High Dose Corticosteroid.

Diffuse neonatal hemangiomatosis is a fatal disorder characterired by multiple cutaneous and visceral hemangiomas. The organs most commonly affected are the gastrointestinal tract, brain, liver and lung. The complications are high output cardiac failure, gastrointestinal bleeding and hydrocephalus. We present a patient with diHuse neonatal hemangiomatosis associated with massive hepatic involvement and high output cardiac failure, which was successfully treated with high dose corticosteroid.

8.Nasal Bone Fractures : Evaluation with Thin-section CP.

PURPOSE: To determine the value of thin-section CT in the diagnosis of nasal bone fractures. MATERIALS AND METHODS: We evaluated the thin-section CT scans of 40 patients with nasal bone fracture. CT scans were obtained with both axial and coronal planes, 1.5mm collimation with 2mm interval, and 9.6cm field-of-view. The axial scan plane was kept parallel to the orbitomeatal line from the nasion to the lower limit of the nose and the coronal plane was kept perpendicular to the axial plane. The data were reconstructed with bone algorithm. Nasal bone fracture was classified into 1 of 3 types on thin section CT:(I) simple fracture;(ll) simple fracture with displacement;(III) comminuted fracture. Associated facial bone injuries were also evaluated Simple radiographs of nasal bone were reviewed for comparison. RESULTS: Six patients had simple fracture, 10 patients had simple fracture with displacement, and 24 patients had comminuted fracture. Twenty-six patients had associated facial bone injuries which included fracture of nasal septum (n=15), fracture of frontal process of maxilla (n=9), fracture of ethmoid (n=6), widening of nasofrontal suture (n=5), and fracture of nasolacrimal duct (n=2). In 15 of 40 patients, CT could identify nasal bone fractures not detected on simple radiographs. CONCLUSION: Thin-section CT is a valuable aid in the evaluation of nasal bone fracture for accurate identification, nature, and combined facial injury.

10.Maximum pubertal spurts in cranial base and mandible.

The purpose of this study was to investigate not only the variability in the timing and amount of the maximum pubertal spurt in cranial base and mandible, but also its interrelationship with the timing of peak height velocity. This study was carried out by analysing biannual serial lateral cephalometric radiographs of twenty-six males and twenty-one females who were taken from 8.5years to 16.5years old of mean age, according to the established land-marks and linear measurements. The results of this study were summarized as follows. 1. Prevalance of the maximum growth spurt more than 80 percentage was occurred in all measurements of cranial base and mandible, except posterior cranial base length in the female. 2. In all measurements of cranial base and mandible, the maximum spurt was occurred earlier in the female than the male while it was greater in male in all measurements except ramal height. 3. In body height measurement, the peak height velocity was occurred 2 years earlier for the female(11.0 years old) than the male(13.0 years old). 4. The timing of maximum spurt in anterior cranial base length and total mandibular length in both sexes and ramal height in the female were coincided with PHV. The maximum spurt was occurred in both sexes in all measurements 2 years before or after PHV, except mandibular body length and posterior cranial base length the in female. 5. In all ages, there was significant correlation between th etotal mandibular length and ramal height, and was also correlation between total cranial base length and anterior cranial base length(P<0.05). However, there was no significant correlation between the ramal height and mandibular body length. In addition, there was also no any correlation between the anterior cranial base length and posterior cranial base length in all ages(P>0.05).
